[Implanting haemodialysis catheters in the brachiocephalic vein: a little-used approach].

We report three patients with chronic renal failure in hemodialysis therapy, which had exhausted conventional venous access in the upper hemitórax (internal jugular, subclavian, axillary) for hemodialysis, primarily due to thrombosis of these veins by previous catheterization. We chose the technique of Rao et al. and cols for puncture of innominate vein, resulting in the implantation of catheters and their subsequent subcutaneous tunneling. Permanent catheters3 work properly and are permeable to the date after a period averaging 8 months.

Effective evidence based catheter management.

People who have long-term urinary catheters commonly experience problems with bypassing and leakage. Changing the catheter is not necessarily the most appropriate solution. Catheter related problems provide nurses with the opportunity to evaluate the clinical indications for continued catheterization and to remove catheters that are not clinically indicated. When catheters are clinically indicated nurses who use an evidence based approach to diagnose and treat problems provide effective care and reduce the risk of patient discomfort and recurrent problems.

Addressing the hemodynamic dilemma of combined mitral and aortic stenosis.

Accurate assessment of valvular hemodynamics is essential in the decision-making process when referring a patient for cardiac surgery who has suspected coexistent aortic and mitral valve disease. Often times, one valvular lesion is worse than the other, and the question of "prophylactically" replacing the less severe valve at the time of surgery is controversial. In these situations and because of uncertainty regarding noninvasive assessment, hemodynamic evaluation in the cardiac catheterization laboratory provides critical information in guiding therapy.

Imaging of extralobar pulmonary sequestration in a patient with tetralogy of Fallot with pulmonary atresia, ventricular septal defect and multiple aorto-pulmonary collateral arteries with multidetector computed tomography.

Complex pulmonary vascular blood supply is common in patients with tetralogy of Fallot with pulmonary atresia, major systemic to pulmonary collateral arteries and hypoplastic or deficient central pulmonary arteries. An extralobar lung sequestration, which has not been described previously in these patients, was imaged in a 6-week-old infant with multidetector computed tomography with sub-millimeter resolution. Arterial and venous vessels were analyzed using three-dimensional vascular exploration tools and results were confirmed with cardiac catheterization.

Inadvertent aortic cannulation with a pulmonary artery catheter detected by transesophageal echocardiography.

Attempted pulmonary artery catheterization via the left internal jugular vein resulted in a misdiagnosis of pulmonary hypertension before transesophageal echocardiography revealed the catheter positioned in the ascending aorta. Inadvertent aortic cannulation through an unusual type of partial anomalous pulmonary venous connection was confirmed with transcatheter fluoroscopy and later at autopsy. Partial anomalous pulmonary venous connection describes one or more of the pulmonary veins draining into the right atrium or its tributaries instead of the left atrium.

[Transcatheter treatment of combined congenital heart anomalies in an adult cyanotic patient].

Contemporary catheterization techniques allow effective percutaneous transcatheter treatment of congenital heart defects as an attractive alternative to open heart surgery. This case report describes transcatheter treatment of congenital pulmonary valve stenosis and secundum atrial septal defect which were diagnosed in a 31 year old woman who presented with cyanosis. This illustrates difficulties in the diagnosis of congenital heart disease in adult patients and demonstrates that transcatheter techniques in the treatment of these congenital anomalies are effective.

Bilateral lumbosacral plexopathy after femoral vein dialysis: synopsis of a case.

Femoral vein dialysis is a technique applied in many clinics. Hemorrhagic complications following the procedure either directly due to the femoral catheterization itself in the early period or less with concomitant late neurological impairments may pose serious challenges to the clinician. Likewise in this report, we are presenting a dialysis patient with bilateral retroperitoneal hematomas causing bilateral lumbosacral plexopathies-to our best knowledge the first in the literature. We have also touched upon its prompt diagnosis and treatment.

Traumatic ascending aortic transection with pseudo-aneurysm.

We present a 64-year-old woman who was brought to the emergency department after suffering a motor vehicle accident, involving multiple soft tissue and bone trauma. Even though she was free from cardiovascular symptoms for the last three years, her past medical history of aorto-coronary bypass surgery prompted preoperative consultation. The diagnostic images showed a unique visualization of an ascending aortic traumatic pseudoaneurysm by transthoracic and transoesophageal echocardiogram as well as with cardiac catheterization.

The VB-1 catheter: an improved catheter for difficult-to-engage internal mammary artery grafts.

As patients with prior coronary artery bypass grafting age, internal mammary artery grafts are more frequently encountered in the cardiac catheterization laboratory. Angiography and interventions involving these grafts are often difficult, particularly in patients with tortuous subclavian anatomy and/or proximal internal mammary artery origins. We describe a new catheter shape, the Cordis Behar Internal Mammary, VB-1, which allows selective intubations of both right and left internal mammary arteries that are not possible with conventional catheters.

Aortic thrombus after coil occlusion of a type E patent ductus arteriosus.

A 5-year-old girl was referred to our institution for closure of a silent patent ductus arteriosus. Cardiac catheterization revealed a tiny-to-small patent ductus arteriosus of the elongated, conical type (Type E). Coil occlusion was performed with a Gianturco coil, 0.035-4 cm(-3) mm. A follow-up echocardiogram showed a very small residual ductal shunt and a moderate-sized thrombus at the aortic end of the ductus arteriosus. The patient remained asymptomatic.

[Congenital stenosis of pulmonary veins: long-term efficacy after intraluminal dilatation].

We present a 7-month-old infant diagnosed with congenital pulmonary vein stenosis. Cardiac catheterization showed two sequential stenoses of the superior left pulmonary vein and balloon angioplasty was performed. The gradient decreased from 20 to 4 mmHg and pulmonary artery pressure from 75/35 mmHg to 44/28 mmHg. The diameter of the stenoses increased from 2 and 3.5 mm, respectively, to 6 mm. Fourteen years after the procedure the patient is asymptomatic with normal cardiac examination.

Bladder dysfunction due to Behçet's disease.

Behçet's disease is a multiple disorder in which urogenital involvement mainly consists of genital aphthous ulcers, epididymitis, urethritis and recurrent cystitis. We report 2 cases of neurogenic bladder due to Behçet's disease. Both cases showed detrusor hyperreflexia and detrusor-sphincter dyssynergia. One case has been managed by twice-daily intravesical oxybutynin treatment (5 mg/10 ml) and clean intermittent catheterization (CIC) 7 times/day, and the other case has been managed by CIC 6 times/day.

[Biventricular diverticulosis associated with rheumatic heart disease: apropos of a case].

We report a case of a 58-year-old woman with rheumatic mitral stenosis scheduled for percutaneous valvuloplasty. Prior left and right ventricular angiograms showed multiple diverticula at left ventricular apical and diaphragmatic walls and right ventricular diaphragmatic wall. Chest x-ray and echocardiogram were normal. Magnetic resonance imaging was concordant with catheterization findings and ruled out other cardiac malformations. The risk of ventricular perforation changed our indication of percutaneous valvuloplasty in favor of open heart commissurotomy.

[Mechanisms of cardiac involvement in systemic scleroderma. Apropos of 3 cases].

To investigate myocardial manifestations in progressive systemic sclerosis, we studied three patients with transthoracic echocardiography, cardiac catheterization with coronary angiography, thallium scans with dipyridamol completed by thoracic cold exposure and endomyocardial biopsy with immunohistology. Two patients were symptomatic. In the three cases, abnormalities of myocardial perfusion were detected without coronary stenosis. Two patients had myocardial fibrosis with a coronary spasm in one case. The immunohistological study always showed an unusual expression of HLA class II antigen on

Technical report: fallopian tube recanalization--a simplified technique.

A simplified technique of fallopian tube catheterization is described in which the tube is recanalized with a guidewire alone. The technique has been used to treat 13 infertile women, age range 27-39 years (mean 32 years), with a diagnosis of proximal tubal obstruction. One tube was perforated; all other tubes were successfully recanalized. Three women conceived within 10 months of the procedure. The technique has results similar to the standard coaxial catheter method but is simple and quick.

[Central cyanosis in a newborn infant due to a prominence of the eustachian valve].

A 3-day-old neonate with severe central cyanosis was found to have a right to left shunt across a secundum atrial septal defect, due to prominence of the Eustachian valve. Two-dimensional echocardiography with Doppler and color Doppler allowed a precise morphological and pathophysiological diagnosis. Cardiac catheterization and angiocardiography confirmed the diagnosis. The patient is asymptomatic 12 months after surgical excision of the Eustachian valve and closure of the atrial septal defect, performed at two months of age.

[Diverticulum of the right ventricle].

We report the case of a 38-year-old woman who underwent cardiac catheterization because of cyanosis from birth. This study revealed a right ventricular cavity hypoplasia, an atrial septal defect, and a diverticulum originated from the free right ventricular wall. Ventricular diverticuli, especially those in the right ventricle, are very rare malformations, usually associated with other cardiac defects. The right ventricular diverticulum is generally non-symptomatic, diagnosed by chance, and does not usually require surgical treatment.

Stiff left atrial syndrome.

A 67-year-old female presented seven years after insertion of a mitral prosthesis with mild to moderate pulmonary edema and severe right heart failure with pulmonary and tricuspid insufficiency. Clinical examination and noninvasive tests did not demonstrate prosthetic valve dysfunction. Cardiac catheterization revealed a marked V wave but no mitral regurgitation and no significant mitral diastolic gradient. A clinical diagnosis of a stiff left atrium was made and confirmed at autopsy. The pathophysiology of the syndrome is discussed.

Percutaneous sclerotherapy of the internal spermatic vein for varicocele through an open ended guide wire.

Percutaneous sclerotherapy of the internal spermatic vein through an open ended guide wire was performed in 50 patients with left sided varicocele. The procedure was successful in 47 patients, without major complications. Percutaneous sclerotherapy through an open ended guide wire offers the advantage of very distal and superselective catheterization with complete embolization of the internal spermatic vein, its side branches and parallel collateral channels. Further follow-up hopefully will confirm the reduction of recurrence rate in our patients.

[Is protein C deficiency responsible for portal thrombosis after neonatal exchange transfusion?].

The authors report the case of a 12-year old boy admitted to hospital for cerebral ischaemic accident and in whom laboratory examinations revealed a 40% deficiency of protein C. Other investigations showed that the child, who had undergone exchange transfusion by the umbilical route at birth, had portal vein thrombosis. It is suggested that protein C deficiency might be responsible for some cases of portal vein thrombosis occurring after umbilical catheterization performed in the neonatal period.
