Cervical Medial Branch Radiofrequency Ablation Using Bipolar Multi-Tined Cannulae in a Patient with an Implanted Cervical Spinal Cord Stimulator: A Case Report.

BACKGROUND: Cervical medial branch radiofrequency ablation (RFA) is an established treatment for chronic cervical facet joint pain. However, RFA procedures generate electromagnetic interference (EMI) that can pose a significant safety risk, potentially leading to spontaneous activation or damage of implanted electronic devices, such as spinal cord stimulators (spinal cord stimulation [SCS]). Previous reports have highlighted the successful use of conventional bipolar RFA, but data are limited regarding large-volume lesioning techniques in patients with SCS implants. CASE REPORT: A 70-year-old

Accelerating diagnosis of degenerative cervical myelopathy through improved education: a mixed-methods study protocol from Myelopathy.org RECODE-DCM to define stakeholders, knowledge requirements and an optimal intervention strategy.

INTRODUCTION: Outcomes for degenerative cervical myelopathy (DCM) patients are limited by delayed and missed diagnoses, driven in part by poor professional awareness. Despite DCM being the most common cause of adult spinal cord injury, it remains under-recognised and undertaught in clinical education. Lessons from other common pathology like stroke and acute myocardial infarction highlight the potential of education to improve early diagnosis. This study will develop a professional education strategy to improve early DCM diagnosis. It will define key audiences and identify an effective deliver

Finite element biomechanical evaluation of a novel double-door screw-plate system for posterior cervical laminoplasty.

BACKGROUND: To address the surgical challenges and postoperative implant displacement associated with conventional internal fixation techniques in posterior cervical double-door laminoplasty, we developed the double-door screw-plate system (DDSPS). PURPOSE: This study aimed to evaluate the biomechanical effects of DDSPS using finite element (FE) analysis. METHODS: An intact adult cervical spine FE model and three postoperative fixation models representing the DDSPS, the LA lamina staple, and the hydroxyapatite (HA) spacer construct were developed. Under a 75 N follower load and a 2.0 N·m mom

Return to Duty Rates in Active-Duty Military: Comparing Minimally Invasive and Open Cervical Spine Surgery.

BACKGROUND: Military medical readiness plays a significant role in global military operations. While the severity of combat-related trauma is evident, musculoskeletal injuries (MSK's), are the largest medical threat to deployment readiness, and accounted for more than 60% of limited duty days in 2019. Across MSK's, the spine (lumbosacral: 30%; cervical: 22%; thoracic: 10%) accounts for 62% of MSKs. In the military population, there is a tremendous economic impact in training, retaining, and deploying a service member. Return-to-duty (RTD) rates and length of time for the treatment protocols fo

Spontaneous vocalisation as a presenting clinical sign in dogs: An association with cervical localisation and intervertebral disc disease.

BACKGROUND: Spontaneous vocalisation is a common reason for dogs to be presented to veterinary practices. However, the cause can be unclear, making prioritisation of investigations challenging. Although vocalisation has been associated with syringomyelia, and anecdotally with cervical disc disease, the most common causes are not reported. METHODS: The medical records of dogs presented at a single institution with spontaneous vocalisation between 2016 and 2022 were retrospectively analysed. RESULTS: A total of 115 dogs were included. The most common breeds were crossbreeds (n = 23), Cavalier

Improvement in vestibular symptoms following cervical medial branch radiofrequency neurotomy: a case report suggesting cervicogenic mechanisms.

This case report describes a patient with chronic neck pain and concurrent vestibular symptoms (dizziness, vertigo, tinnitus, and nausea) whose symptoms improved following cervical medial branch radiofrequency neurotomy. While radiofrequency neurotomy is an established treatment for cervical zygapophyseal joint-mediated pain, this case illustrates a temporal association between treatment of cervical pain and concurrent improvement in vestibular symptoms. These findings suggest a potential link between cervical facet pathology and vestibular dysfunction that warrants further investigation.

Total en bloc resection of a giant cell tumour of the cervical spine involving the vertebral artery.

A woman in her 20s presented with neck pain and was diagnosed with a primary giant cell tumour (GCT) involving the C5 vertebral body and encasing the left vertebral artery (VA). Denosumab therapy was initiated to reduce tumour vascularity and facilitate resection. After preoperative embolisation of the left VA, total en bloc resection of the tumour was performed. Postoperatively, the patient experienced transient left upper extremity weakness, which improved significantly over time. Follow-up imaging showed successful bony fusion and no recurrence. This case highlights the importance of multid
