'It hurts, but I don't have a choice, I'm not working and I'm sick': decisions and experiences regarding abortion of women living with HIV in Cape Town, South Africa.

Research was conducted with 36 women living with HIV in Cape Town, South Africa, regarding their decision-making about, and experiences with, abortion of unwanted pregnancies in the public health sector. Abortion intentions and decisions were explored by investigating influencing factors; knowledge of abortion policy and public health sector services; and abortion perceptions and experiences. Findings reveal that many women face censure both for becoming pregnant and terminating a pregnancy, including by family, partners, community members and healthcare providers. Data suggest that abortion m

Association of Women Empowerment with Intimate Partner Violence in Saudi Arabia.

BACKGROUND: The prevalence of intimate partner violence (IPV) is higher among women with lower social and economic status. Moreover, empowerment-focused interventions might not protect them from domestic abuse. This study assessed Saudi women's empowerment and its usefulness as a stand-alone IPV predictor. METHODS: 400 married women, ages 19 to 65, who visited the outpatient clinics of PHC centers in Riyadh were interviewed using the Women's Empowerment module and the previously Arabic-validated version of the WHO multi-country instrument on Violence Against Women (VAW) to learn more about th

Atrial fibrillation in stroke patients: facts, numbers and current challenges.

Atrial fibrillation (AF) is a leading, modifiable risk factor for ischaemic stroke, with 20-30% of stroke patients harbouring underlying AF. Management of AF, however, differs substantially depending on clinical context. In ischaemic stroke patients with pre-known AF, direct oral anticoagulants (DOACs) are the cornerstone of secondary prevention, with initiation timing guided by infarct severity following a risk-adapted approach. In patients with no history of AF, prolonged cardiac rhythm monitoring after stroke or transient ischaemic attack improves AF detection rates. However, it remains unc

"What really works for us?" exploring therapeutic strategies for non-small cell lung cancer: an umbrella review of systematic reviews and meta-analyses.

BACKGROUND: Common non-surgical therapeutic strategies for non-small cell lung cancer (NSCLC) and their efficacy have been reported in many systematic reviews (SRs) and meta-analyses (MAs). However, there is no systematic comparative analysis regarding the relative efficacy of different treatments. Thus, we conducted an umbrella review to provide higher-level evidence for clinical decision-making. METHODS: A systematic search of PubMed and Web of Science was conducted to identify SRs/MAs focusing on different therapeutic strategies for NSCLC. In total, 34 eligible studies were included. Trial

Preoperative Risk Factors for Poor Hernia-Related Quality of Life One Year After Hernia Repair.

INTRODUCTION: Long-term patient-reported outcomes (PROs) after abdominal wall hernia repair (AWHR) are essential for evaluating recovery and informing shared decision-making, yet population-level data and predictors of hernia-related quality of life (QOL) are limited. We measured 1-y postoperative QOL after AWHR and identified preoperative predictors of PROs. METHODS: We performed a cohort study using prospectively collected clinical data and 1-y postoperative surveys from the Michigan Surgical Quality Collaborative Core Optimization in Hernia Registry. Adults ≥18 y undergoing AWHR from Janua

Maternal health in women with spina bifida: A scoping review.

BACKGROUND: An increasing number of women with spina bifida are reaching reproductive age and may consider pregnancy. However, limited evidence on how spina bifida affects maternal health can hinder tailored counseling and guidance, contributing to uncertainty during pregnancy-related decision-making. OBJECTIVE: This scoping review aimed to synthesize existing literature on maternal health among women with spina bifida, focusing on pregnancy and childbirth outcomes, women's health-related outcomes, care needs, and health-related decision-making across pregnancy, childbirth, and parenting cont

Development and Clinical Application of a Deep Learning-Based AI Support Model for Endometrial Cancer Cytology.

BACKGROUND: The global increase in endometrial cancer, including in Japan, and a shortage of pathologists and cytotechnologists have increased the diagnostic burden, emphasizing the need for an AI-based diagnostic support model that uses deep learning. We evaluated the clinical application of an improved AI-supported endometrial cytology model. METHODS: Using YOLOv5x and YOLOv7 models evaluated by mean average precision (mAP), we compared two datasets-one annotated for both benign and malignant cell clusters, and one for malignant only. In addition, using the Two One-Sided Tests (TOST) proced

Incidence of and Risk Factors for Repeat Adverse Drug Events: A Systematic Review.

Adverse drug events (ADEs) are unintended harm resulting from appropriate or inappropriate medication use. Many ADE-related emergency department visits are repeat events that occur after re-exposure to a medication that previously caused harm. Identifying risks and risk factors for repeat ADEs could inform clinical decision-making. We conducted a systematic review of literature on repeat ADEs published over a 20-year period. We applied a predefined search strategy in MEDLINE, Embase, and Scopus and extracted data on risk factors (e.g., age, sex, drug type, re-exposure type) and ADE outcomes. W

Enteropathy-associated T-cell lymphoma: molecular clonal evolution and its clinical consequences.

Refractory celiac disease (RCD) is a rare complication of celiac disease and bears a high risk for lifethreatening enteropathy-associated T-cell lymphoma (EATL). While RCD is generally considered a direct EATL precursor, supporting evidence is limited and the predictive value for EATL development of molecular aberrations in RCD as risk factors unclear. We aim to dissect the clonal evolution of RCD and EATL to support molecular diagnostics-based clinical decision making. RCD specimens of patients with (n=6) and without (n=21) development of EATL within 5 years after diagnosis, and EATL specimen

Predictors of mortality in individuals with disseminated histoplasmosis and advanced HIV disease.

Disseminated histoplasmosis remains a major cause of morbidity and mortality among people living with advanced HIV disease (AHD), particularly in Latin America, where delayed diagnosis and limited access to optimal antifungal therapy persist. Accurate tools to assess disease severity and predict mortality are essential to guide clinical decision-making, including hospitalization, intensive care unit admission, and treatment strategies. We conducted a retrospective observational cohort study of hospitalized adults with AHD (CD4 ≤ 200 cells/mm³) and a first episode of probable or proven dissemin

Recommendations for generating real-world evidence with regulatory-grade relevance and reliability.

Real-world data (RWD) and real-world evidence (RWE) are increasingly incorporated into regulatory decision-making to complement randomized controlled trials (RCTs), particularly in settings where conventional trial designs are infeasible, such as rare diseases, rare molecular subtypes, and post-marketing evaluation. Across regulatory authorities, the use of RWD/RWE is consistently framed by the fit-for-purpose principle, centered on two foundational concepts: relevance and reliability. However, the absence of operational guidance has created uncertainty regarding how academic registries and he

Imaging for molecular and pathological subtyping of hepatocellular carcinoma-a critical appraisal and future directions.

OBJECTIVES: Hepatocellular carcinoma (HCC) is characterized by distinct molecular and pathological subtypes, each with unique prognostic implications. This review aims to synthesize the imaging features associated with these HCC subtypes and discuss their potential to guide therapeutic decision-making. MATERIALS AND METHODS: We searched PubMed and Embase for articles published from September 2004 to December 2024. The search strategy combined terms for imaging modalities ("CT," "MRI"), the primary disease ("hepatocellular carcinoma"), and various molecular and pathological subtypes (e.g., "ma

Exploring the impact of antibody-dependent cellular phagocytosis-related genes on the prognosis of metastatic melanoma.

BACKGROUND: Metastatic melanoma is a challenging clinical condition with poor prognosis. Recent research has highlighted the role of antibody-dependent cellular phagocytosis (ADCP) in tumor immunity, suggesting prognostic implications for ADCP-related genes (ARGs). This study develops a prognostic model for metastatic melanoma using ARGs to enhance clinical decision-making and therapeutic strategies. METHODS: Prognostic ARGs were identified from the GSE46517 and GSE7553 datasets. A prognostic model was constructed using LASSO-Cox regression and validated across multiple cohorts, including TCG
