Risk of Orthostatic Hypotension in Chronic Kidney Disease: A Community-Based Cohort Study.

INTRODUCTION: Patients with chronic kidney disease (CKD) often experience symptoms consistent with orthostatic hypotension (OH); however, not much is known about OH in this population. Therefore, study analyzed OH prevalence and risk according to renal function decline. METHODS: A community-based cohort study was conducted using data from the Korean Genome and Epidemiology Study (KoGES). OH prevalence was assessed at baseline and at a 2-year follow-up across three estimated glomerular filtration rate (eGFR) categories: high (≥90 mL/min/1.73 m2, G1), moderate (60-89 mL/min/1.73 m2, G2), and lo

Comparison of reconstruction techniques after pancreatoduodenectomy: A single-center retrospective cohort study.

BACKGROUND: There is controversy regarding which is the best reconstruction technique after the pancreatoduodenectomy. Currently, there are no studies comparing the three most frequent reconstruction techniques: Whipple + Roux-en-Y gastrojejunostomy (WRYGJ), pyloric-preserving + Billroth II (PPBII), and Whipple + BII (WBII). METHODS: Between 2012 and March 2023, 246 patients underwent pancreaticoduodenectomy with the following type of reconstruction techniques: (1) WRYGJ: 40 patients; (2) PPBII: 118 patients; and (3) WBII: 88 patients. A retrospective comparative study among these groups was

Association of walking volume and intensity with incident gout: a population-based cohort study.

Previous studies have reported that walking is associated with a lower risk of several metabolic diseases; however, evidence of its association with gout, a metabolic condition, is lacking. We conducted cohort studies using data from the UK Biobank (2013-2021) to examine the relationship of walking volume and intensity with the risk of incident gout. We included 92 066 participants who were free of gout and had valid baseline seven-day accelerometer data, which allowed us to derive walking volume (total steps), walking intensity (peak 30-minute cadence), and their combination (purposeful steps

Timing of endoscopy in cirrhotic patients with acute variceal bleeding: a multicenter cohort study.

BACKGROUND: The optimal timing of endoscopy in cirrhotic patients with acute variceal bleeding (AVB) remains uncertain. The study aimed to determine whether the timing of endoscopy affects short-term outcomes in patients with AVB. METHODS: Cirrhotic patients with AVB who underwent endoscopy within 24 hours of admission between 2016-2022 were evaluated. Patients were categorized into two group: the urgent endoscopy group (<12 hours) and the early endoscopy group (12-24 hours). The primary outcome was 30-day mortality. Secondary outcomes included the incidence of treatment failure, the need for

Specific focus on the basal dura for improving Cushing's disease surgery: a cohort study.

BACKGROUND AND OBJECTIVES: In Cushing's disease, the rate of endocrine remission after surgery reaches 80% in expert centers. However, due to the tumor's capacity to invade surrounding dural structures, hypercortisolism persists in 20% of patients or recurs in 15%. To improve the likelihood of remission, tumor resection can be extended to the dura in patients who show basal dura invasion. The objective was to evaluate the benefit of a surgical strategy based on a systematic focus on the basal dura. METHODS: Endoscopic surgery was performed in 89 adult patients with no obvious cavernous invasi

Incidental Adrenal Nodules and Growth Rates: A Single Centre Retrospective Cohort Study.

BACKGROUND: Incidental adrenal nodules are increasingly being detected due to the widespread use of computed tomography (CT) and advances in imaging technology. Current guidelines often recommend follow-up imaging around one year, but, given the poor prognosis of malignant adrenal lesions, the utility of this approach is unclear. Therefore, we investigated whether growth rates of incidental adrenal nodules are consistent with this guidance. METHODS: We conducted a retrospective cohort study at Te Toka Tumai Auckland, New Zealand. We identified all CT adrenal examinations performed between Jan

Critical Events in Patients With Alternating Hemiplegia of Childhood: A Cohort Study Subgroup Analysis.

People with alternating hemiplegia of childhood undergoing general anesthesia are at risk for severe complications including critical bradycardia and hemiplegic incidents. We performed a 10-year historical cohort study and completed a subgroup analysis of patients who specifically underwent general anesthesia. Patients with ATP1A3 positive variants were compared with those without such variants. The 14 patients reviewed underwent general anesthesia a combined total of 39 times. Two patients had a short QTc documented prior to anesthesia and presented with bradycardia in the recovery room. Two

Learnings from a registry-based cohort study for spinal muscular atrophy disease.

Spinal muscular atrophy is a rare recessive progressive neurodegenerative disorder. To better understand the progression of spinal muscular atrophy the European Medicines Agency launched a study based on registry data. This manuscript describes some lessons learnt and considerations how to improve future registry-based studies that aim to inform regulatory decision-making. The study started with a feasibility assessment to select appropriate registries from the TREAT-NMD network. The feasibility assessment is key to understand upfront the quality, capability and capacity of registries to addre

Autoimmune cerebellopyramidal syndrome as a complex form of autoimmune cerebellar ataxia: a cohort study.

BACKGROUND: Autoimmune cerebellar ataxia (ACA) is a significant and treatable cause of sporadic cerebellar syndrome. Pyramidal tract damage affects approximately 30% of ACA patients, profoundly impacts motor function and prognosis, yet this complex form remains under-investigated. METHODS: We retrospectively analyzed patients with pyramidal signs from our institutional ACA cohort, excluding those with intrinsic upper motor neuron impairment due to specific ACA subtypes. Clinical and paraclinical data were compared between patients with and without pyramidal signs. RESULTS: Among the 48 ACA p

Assign a systematic traumatological status in orbital fracture patients - A prospective cohort study.

The study aims to clarify the injury profile and the causes leading to missed injuries in isolated orbital fracture patients. The hypothesis was that orbital fracture patients are prone to underdiagnosis, especially regarding injuries to other body parts. Both injury profiles and causes leading to missed injuries in isolated orbital fracture patients were evaluated in a prospective cohort study. The patients were examined systematically by the oral and maxillofacial surgeon and an on-call orthopaedic traumatologist. The patients were also referred to ophthalmology for consultation. The study i
