Paroxysmal sympathetic hyperactivity associated with posterior reversible encephalopathy syndrome.

Paroxysmal sympathetic hyperactivity (PSH) causes paroxysms of increased sympathetic hyperactivity in the setting of acquired brain injury. Posterior reversible encephalopathy syndrome (PRES) is a clinico-radiological syndrome characterised by cortical/subcortical vasogenic oedema in multiple brain areas. Patients present with seizures, encephalopathy and headaches. In this article, we report a patient who presented in status epilepticus with a hypertensive peak and decreased level of consciousness. Brain imaging showed diffuse hyperintensities with beading intracranial vessels. Then, the pati

Brain Shift Patterns: Upward, Lateral and Downward Herniation, Its Correlation with Clinical Patterns in Acute Intracranial Pathologies and Neurosurgical Management.

Brain shift is a critical radiological indicator of intracranial mass effect, resulting from haematoma, tumour, infarction, or cerebral oedema. Depending on the direction of displacement, herniation may occur as downward transtentorial, upward (reverse transtentorial), or lateral midline shift (MLS). This study aims to classify brain shift patterns and correlate radiological findings with clinical parameters, including level of consciousness, Glasgow Coma Scale (GCS), pupillary size and reactivity, and predicted intracranial pressure (ICP). Radiological data from CT and MRI were analysed along

Pathophysiology and aetiologies of hypernatremia.

Hypernatremia is a disorder of water balance defined by a serum sodium concentration above 145 mEq/L. It reflects a relative deficiency of free water rather than sodium excess. Under physiological conditions, hypothalamic osmoreceptors maintain plasma osmolality through stimulation of arginine vasopressin release- which promotes renal water conservation- and stimulation of thirst, which drives fluid intake. Hypernatremia develops when these defences fail due to impaired vasopressin secretion or action, diminished thirst, or inability to access water. The resultant hyperosmolality causes osmoti

Management of drop attacks in Ménière's disease: a systematic literature review.

OBJECTIVES: Drop attacks are described as an instantaneous fall to the ground, occurring without warning and without loss of consciousness, a consequence of abrupt deformation of the otolithic membrane due to high endolymphatic pressure. Drop attacks present significant injury risk, therefore effective treatment is imperative. This review is the first to examine all evidence for the management of drop attacks in Ménière's disease, and make recommendations. METHODS: We conducted a systematic literature review. Inclusion criteria consisted of all published English language examining treatments

A Case of Multiple Intracranial Hemorrhages Due to Biopsy-Proven Varicella Zoster Virus Vasculopathy Without Rash.

Varicella zoster virus (VZV) vasculopathy is a rare complication of VZV infection that can lead to severe neurological manifestations, including multiple intracranial hemorrhages. We report the case of an 80-year-old male patient with a history of Parkinson's disease and hypertension who presented with altered consciousness and motor difficulties. Imaging studies revealed recurrent cortical-subcortical hemorrhages in the right frontal lobe. Laboratory tests showed elevated cerebrospinal fluid (CSF) protein, increased CSF cell count, and a significantly elevated VZV IgG antibody index, while CS

Two Cases of Severe Food-Borne Botulism: Could High-Dose Methylcobalamin Accelerate Recovery?

Botulinum neurotoxins (BoNTs) are zinc-dependent endopeptidases that cleave presynaptic SNARE proteins and arrest acetylcholine release, producing flaccid paralysis with protracted recovery. We report two cases of a mother and son who developed severe food-borne botulism after eating Fesikh (traditional salted fish) with rapidly progressive descending paralysis requiring ventilation, prominent dysautonomia (hypotension responsive to fluids, bradycardia, dry mucosae, constipation with abdominal distension, as well as urinary retention), and intact consciousness. Both received trivalent antitoxi

Clinical features and outcomes of posterior reversible encephalopathy syndrome after heart transplantation: a case series.

Posterior reversible encephalopathy syndrome (PRES) is a rare neurological disease that may be associated with hypertension, autoregulatory failure, and the use of calcineurin inhibitors following heart transplantation (HT). In this article, we present a case series of PRES, discussing its potential causes and management strategies. Among the 126 HT recipients at our hospital, four were diagnosed with PRES. Three of these patients developed PRES within 7 days after HT. Prior to the onset of PRES, all patients experienced sustained hypertension, and strict blood pressure (BP) control was mainta

Post-hypnotic suggestion improves confidence and speed of memory access with long-lasting effects.

In our study, we use the post-hypnotic suggestion of easy remembering to improve memory with long-lasting effects. We tested 24 highly suggestible participants in an online study. Participants learned word lists and recalled them later in a recognition memory task. At the beginning of the study, participants were hypnotized and the post-hypnotic suggestion to remember easily was associated with a cue that participants used during the recognition memory task. In a control condition, the same participants used a neutral cue. One week later, participants repeated both conditions with new word lis

To be aware or not aware: Do intrusions with and without meta-awareness differ?

People sometimes re-experience traumatic events via intrusive memories that spontaneously and unintentionally intrude into consciousness (i.e., intrusions). Such intrusions can be experienced without explicit awareness (i.e., meta-awareness). However, we do not know whether intrusions with and without meta-awareness differ in how people experience them (i.e., characteristics) or react to them via maladaptive responses (i.e., suppression, negative interpretations), and therefore whether they are important to differentiate. To investigate this issue, we asked participants to watch a trauma analo

Hemodynamics and arrhythmia disorder caused by lithium poisoning: A case report.

Bipolar affective disorder refers to a category of mood disorders characterized clinically by the presence of both manic or hypomanic episodes and depressive episodes. Lithium stands out as the primary pharmacological intervention for managing bipolar affective disorder. However, its therapeutic dosage closely approaches toxic levels. Toxic symptoms appear when the blood lithium concentration surpasses 1.4 mmol/L, typically giving rise to gastrointestinal and central nervous system reactions. Cardiac toxicity is rare but serious in cases of lithium poisoning. The study reports a case of a pati

Pseudohypoxic brain swelling following cerebrospinal fluid leakage: a case report on rapid identification and multidisciplinary management.

This report delineates the intricate diagnostic journey and therapeutic conundrum presented by a 61-year-old male who exhibited atypical neurological deterioration shortly after lumbar fusion surgery, manifesting clinical and radiological features suggestive of pseudohypoxic encephalopathy, an entity characterized by symptoms mimicking cerebral hypoxia in the absence of a discernible hypoxic insult. Following an initially unremarkable recovery from an elaborate spinal surgery, the patient's postoperative condition was confounded by a perplexing decline in consciousness, unresponsive to convent
