In-vivo measurement of brain relaxation after lobectomies.

Very little data exists on the visco-elastic properties of the living human brain tissue and collection of such data is vital for mechanical modelling. The purpose of this study was to measure the rate of relaxation of brain that has been compressed by tumour after surgical decompression. Seven patients who underwent a lobectomy for an intracranial space occupying lesion were included in the study. All underwent two CT scans within the first 24 h after the surgery. Volumes of the residual void on the serial CT scan were calculated using tools in the image acquisition software. There was a rapi

Conus medullaris syndrome without lower-extremity involvement in L-1 burst fractures: report of four cases.

Conus medullaris injury from burst fractures is known to occur in conjunction with other neurological deficits, including lower-extremity motor weakness or sensory changes. Rarely does an isolated conus medullaris injury occur from an extradural cause without other neurological deficits. The authors report four cases of L-1 burst fractures in which conus medullaris dysfunction was the sole neurological injury in the absence of lower-extremity involvement.

Arthroscopic-assisted core decompression of the humeral head.

Humeral head osteonecrosis is a progressive disease that requires prompt diagnosis and treatment. Core decompression is a viable treatment option for early-stage cases. Most surgeons perform core decompression by arthroscopically visualizing the necrotic area of bone and using a cannulated drill to take a core. Several attempts are frequently needed to reach the proper location. In the hip multiple passes are associated with complications. We describe the use of an anterior cruciate ligament (ACL) tibial drill guide to precisely localize the area of necrotic bone. Diagnostic arthroscopy is per

Multifocal Langerhans cell histiocytosis of the pediatric spine: a case report and literature review.

CASE REPORT: An 11-month-old boy presented with a 3-month history of lower extremity weakness. CT and MRI of the spine revealed an enhancing epidural mass, extending from T1 through T5 and exiting through multiple foramina. The largest extraspinal extent was located at the T3 vertebral level and was accompanied by complete T3 vertebral collapse. A second lytic lesion at the L2 vertebral body without an obvious enhancing mass was also noted. Open biopsy and decompression of the spinal cord were performed, and histopathological analysis revealed a mixed inflammatory lesion with abundant S-100 an

Endoscopic assisted lumbar microdecompressive spinal surgery with a new SMART endoscopic spine system.

In response to the rapid development and demand of outpatient endoscopic minimally invasive lumbar surgical technique, the SMART endoscopic spine system was developed for neurodecompression. This lumbar spine surgery is performed with a small skin incision, dilatation surgical technology, and an endoscopic-assisted spinal surgical system with progressive serial tubular retractors providing superior lighting and better visualization of the operative field for performing minimally invasive spinal surgery (MISS). The SMART system incorporates the advantages of posterior paramedian endoscopic assi

[Anterior cervical corpectomy and fusion with preserved posterior vertebral wall for multi-level cervical spondylosis: a prospective random study of 84 cases].

OBJECTIVE: To compare the results of anterior cervical corpectomy and fusion with preserved posterior vertebral wall (PWCF) with the classical anterior cervical corpectomy with fusion (ACF). METHODS: Eighty-four patients with multi-level cervical spondylosis were randomized to ACF (n = 42) or PWCF (n = 42). Postoperative JOA score were evaluated by an independent observer. Fusion rate, segmental lordosis, and disc height were assessed by radiographs at postoperative 3 months, 6 months, and 12 months. Fusion evidence was observed by CT scan. RESULTS: JOA score improvement was similar for both

Tension gastrothorax.

BACKGROUND: Tension gastrothorax develops when the stomach herniated through a congenital diaphragmatic defect into the thorax is massively distended by trapped air. The authors present 5 cases and discuss the diagnostic and therapeutic management. CASE REPORTS: Four children, aged 3, 4, 6, and 13 months, presented with progressive respiratory distress. In only 1 child was the diagnosis of tension gastrothorax established initially, and immediate insertion of a nasogastric tube led to complete resolution of respiratory distress symptoms. In the remaining 3 children, the initial chest radiogra

Anterolateral approach to the V2 segment of the vertebral artery.

OBJECTIVE: We describe our surgical technique of exposure and control of the second segment of the vertebral artery (VA V2 segment). Our basic principle is that working in the VA vicinity is more confident under visual control. METHODS: The VA V2 segment extends classically in and between the transverse processes from C6 to C2. This segment can be exposed through an anterolateral approach, passing medially to the sternocleidomastoid muscle and laterally to the internal jugular vein. Except in case of anatomic variation, the VA V2 segment is protected by the transverse processes bone, even if

Chiari malformation type I associated with familial spastic paraplegia: report of a surgically treated case.

CASE REPORT: We describe the case of a 7-year-old girl with Chiari malformation type I (CMI) and concomitant familial spastic paraplegia (FSP), who exhibited spastic gait disturbance. She also showed poor results on the standing tolerance test using thermography, which evaluates autonomic nerve function. The patient was subjected to posterior fossa decompression and cervical 1 (C1) laminectomy for CMI based on the result of the standing tolerance test. After the operation her gait disturbance and autonomic nerve dysfunction improved. DISCUSSION: Although no cases of CMI and concomitant FSP ha

Microvascular decompression of cranial nerves using sheets of a dural substitute--technical note.

Several types of prosthesis are used for microvascular decompression (MVD) surgery for neurovascular compression syndrome. However, most prostheses adhere to the surrounding neuronal structures and occasionally cause granulomas. The present study evaluated a dural substitute made of expanded polytetrafluoroethylene, the Gore-Tex EPTFE patch, as a prosthesis for MVD. Twelve patients with trigeminal neuralgia, 19 patients with hemifacial spasm (HFS), and two patients with glossopharyngeal neuralgia underwent MVD using the dural substitute. In most cases, one or two sheets of the dural substitute

Effect of wrist position on power grip and key pinch strength following carpal tunnel decompression.

Power grip and thumb key pinch strength were measured pre- and immediately postoperatively in 30 patients with carpal tunnel syndrome while the wrist was in flexion and extension. The carpal tunnel decompression was performed under local infiltration with 1% lignocaine. Grip strength decreased more in wrist flexion than in wrist extension. No difference was found in thumb pinch strength. The authors conclude that some of the immediate postoperative loss of grip strength in wrist flexion can be attributed to prolapse of flexor tendons out of the carpal tunnel in this position.

Ogilvie's syndrome treatment.

INTRODUCTION: Ogilvie's Syndrome (OS) is a rare condition caused by parasympathetic dysfunction of large bowel characterized by acute and massive colon distension without mechanical obstruction. Rarely this disease has to be treated by the surgeon but operations may be indicated in case of medical treatment failure. METHODS: A retrospective analysis was carried out at the Emergency Surgery DPT of St Orsola-Malpighi University Hospital Bologna Italy. From 1995 to 2002 11 patients were treated for severe OS: they were 8 males and 3 females and the mean age was 68 yrs. All these subjects had lar

Evaluation of surgical procedures for trigeminal neuralgia.

Trigeminal neuralgia is a type of facial pain that is difficult to treat. The pain can be excruciating and debilitating. The wide range of treatments currently used for trigeminal neuralgia is ample evidence that there is no simple answer to how it should be managed. This review will evaluate the current surgical procedures used for the treatment of trigeminal neuralgia. A critical analysis of the evidence-based studies to date was done to evaluate and compare the efficacy of the different surgical procedures. Arguments for and against the use of surgery for trigeminal neuralgia are presented.

Osteonecrosis after powered core decompression.

A 44-year-old man with alcohol-related osteonecrosis of his left femoral head, Ficat Stage 2, was treated by femoral head decompression. During this procedure the (1/4)-inch trephine driven by a power reamer became lodged in the femoral head and became hot to the touch. The trephine eventually was removed with difficulty after the decompression was completed. Four and a half years later, the patient continued to have progressive pain and difficulty ambulating. A radiograph at the time revealed a wide zone of symmetric increased density about the core decompression track. A total hip arthroplas

Kinesthetic sense of the shoulder in patients with impingement syndrome.

A proprioceptive deficit is an important determinant of disability in various shoulder disorders, such as instability and osteoarthrosis. In 15 patients with impingement syndrome stage II (Neer 1983), who were treated by arthroscopic subacromial decompression, we measured movement sense by determining threshold levels for the perception of motion of the shoulder. The patients were placed in a specially designed chair allowing continuous passive motion of the shoulder joint, while avoiding cutaneous, auditory and visual stimuli. To assess movement detection thresholds, passive abduction movemen

Lateral approach for resection of the C3 corpus: technical case report.

OBJECTIVE AND IMPORTANCE: The C3 level is the transition zone between the upper and lower cervical spine. Because of its high position and anatomic relationships to significant structures, exposing C3 is challenging, and the surgical approach is controversial. CLINICAL PRESENTATION: A 16-year-old girl was admitted to our institution with a 3-year history of neck pain and progressive quadriparesis. Neuroradiological examination revealed severe spinal cord compression from kyphosis at the C3 level. TECHNIQUE: We used the lateral approach to resect the C3 corpus and realign the cervical spine.
