Dissolution of the Capitellum After Lateral Condylar Fracture of the Humerus in a Child: A Case Report.

Lateral condyle fractures of the humerus are a common elbow injury sustained by pediatric patients. Complications from surgical intervention can include malunion, fishtail deformity, osteonecrosis, and avascular necrosis (AVN). AVN of the capitellum is a rare complication of lateral condyle fractures with very few cases reported in the current literature. Here we report the rare case of dissolution of the capitellum following closed reduction and percutaneous pinning for a lateral condylar fracture of the humerus of a six-year-old child performed at an outside hospital that was subsequently ma

Variable expressivity in a family with an aggrecanopathy.

BACKGROUND: Osteochondritis dissecans is a condition wherein there is a subchondral bone lesion that causes pain, inflammation, and cartilage damage. Dominant Familial Osteochondritis Dissecans is a rare and severe form of osteochondritis dissecans (OCD). It is caused by heterozygous pathogenic variants in the gene encoding Aggrecan; ACAN. Aggrecan, a proteoglycan, is an essential component of the articular and growth plate cartilage. METHODS: Herein, we report three individuals from one family; the proband who presented with short stature, a lower limb bone exostosis, and bilateral knee and

Management of the Ulnar Nerve in Throwing Athletes.

Purpose of reviewDuring overhead throwing, there are tremendous forces placed on the medial elbow that can result in injury to the ulnar nerve. The purpose of this article is to review the anatomy, biomechanics, and appropriate clinical care of an overhead throwing athlete suffering from injury to their ulnar nerve. The authors preferred clinical work-up; non-operative and operative care are included in this review.Recent findingsMore recent findings support the use of subcutaneous transposition instead of the submuscular transpositions and only to perform a transposition in the setting of spe

Designing instrumented walker to measure upper-extremity's efforts: A case study.

The high prevalence of shoulder pain in using walkers in patients who have spinal cord injury (SCI). Also, the limited options available to economically measure grip forces in walkers, which drove the need to create one. This article describes a method to obtain upper-extremities' forces and moments in a person with SCI by designing an appropriate instrumented walker. First, since the commercial multidirectional loadcells are too expensive, custom loadcells are fabricated. Ultimately, a complete gait analysis by means of VICON motion analysis and using inverse dynamic method has been held to m

A Case Report of Tuberculosis of Isolated Radial Head A Rare Location with Diagnostic Dilemma.

INTRODUCTION: Tuberculosis (TB) of isolated radial head is scantily reported in the literature. Nonspecific symptoms and difficulty in interpreting initial screening radiographs often lead to misdiagnosis. CASE REPORT: We present a case of 42-year-old male elsewhere diagnosed as pyogenic arthritis of left elbow and treated by incisional drainage and broad-spectrum antibiotics, who presented to us 6 months later with multiple non-healing actively discharging sinuses. The repeat radiographs and Magnetic Resonance Imaging (MRI) were reported as chronic osteomyelitis of proximal radius without th

Acute Shortening Versus Bridging Plate for Highly Comminuted Olecranon Fractures.

Severely comminuted olecranon fractures, for which elbow stability becomes the main goal of surgery, remain a challenge for surgeons. We conducted a study to determine the percentage loss of articular surface at which a bridge plating (BP) construct becomes too unstable and an acute shortening (AS) construct is required. The olecranon process of 8 fresh-frozen cadaveric upper limbs was serially resected. At each resection, the simulated fracture was fixed first with BP and then with AS. Stability was tested by performing valgus and varus stress tests at various angles under fluoroscopy. As man

The effect of capitellar impaction fractures on radiocapitellar stability.

PURPOSE: To determine the effect of capitellar impaction fractures on radiocapitellar stability in a model that simulated a terrible triad injury. METHODS: Six cadaveric elbows were dissected free of skin and muscles. Tendons were preserved. The lateral collateral ligament was released and repaired (surgical control). Two sizes of capitellar impaction defects were created. After lateral collateral ligament release and repair, we then sequentially created osseous components of a terrible triad injury (partial radial head resection and coronoid fracture) through an olecranon osteotomy that was

A 1-year prospective analysis of injuries in amateur and elite fistball.

The purpose of this study was to examine the incidence and mechanisms of acute injuries in the sport of fistball. No scientific studies on injury characteristics have yet been conducted in this traditional sport game. The study was conducted prospectively over the course of 12 months. During a total of 40.308 h of sport-specific exposure, 240 players reported 492 injuries, representing an overall injury rate of 12.2 injuries/1000 h of exposure. Most injuries were classified as bagatelle injuries (67.8%). The majority of the injuries were located in the knee (23.5%) followed by the elbow (11.9%

Clinical expression in Pfeiffer syndrome type 2 and 3: surveillance in Japan.

Pfeiffer syndrome (PS) is a classic type of craniosynostosis syndrome. Severe cases usually require emergency care at birth. However, early diagnosis is often precluded by the rarity and consequent low awareness of this disease. This study aimed to clarify phenotypic expressions useful for the diagnosis of PS. We reviewed all cases of PS type 2 or 3 according to Cohen's classification that were reported between 1980 and 2011 in Japan. Clinical and genetic information were extracted from the patients' medical records. A total of 23 patients with PS type 2 or 3 were identified. All 23 patients p

Traumatic separation of the distal epiphysis of the humerus sustained at birth.

We reviewed the clinical and radiological results of six patients who had sustained traumatic separation of the distal epiphysis of the humerus at birth. The correct diagnosis was made from plain radiographs and often supplemented with ultrasonography, MRI and arthrography. An orthopaedic surgeon saw two patients within two days of birth, and the other four were seen at between nine and 30 days. The two neonates underwent unsuccessful attempts at closed reduction. In the remaining patients, seen after the age of eight days, no attempt at reduction was made. All six displaced fractures were imm

On-table reconstruction of comminuted fractures of the radial head.

The most widely accepted treatment for comminuted fractures of the radial head is either the excision or open reduction and internal fixation. The purpose of the present study is to evaluate the value of an 'on-table' reconstruction technique in severely comminuted fractures of the radial head. In this study, two patients with a Mason type-III and four patients with a Mason type-IV radial-head fracture were treated with 'on-table' reconstruction and fixation using low-profile mini-plates. After a mean follow-up of 112 months (47-154 months), the mean elbow motion was 0-6-141 degrees extension

Nodular lymphangitic subcutaneous dissemination after intralesional antimonial treatment for localized cutaneous leishmaniasis.

Human leishmaniases are protozoan diseases with diverse clinical features. They are transmitted by the bites of the Phlebotomus sand fly, and the reservoirs are usually wild and domestic animals, particularly dogs. In Spain, Leishmania infantum is the causative agent of both cutaneous and visceral leishmaniasis. Cutaneous leishmaniasis is a parasitic disease occurring throughout the Americas and in the Old World, particularly the Middle East and North Africa. It is spread by the female sandfly. We report a 33-year-old man who presented with a solitary plaque on the right elbow, which was found

Disorders of the distal biceps brachii tendon.

Pathologic conditions of the distal biceps brachii tendon are of clinical interest, with partial and complete tears being the most common. However, the anatomy of the distal biceps brachii tendon makes imaging of the distal tendon somewhat difficult. An innovation in patient positioning for magnetic resonance (MR) imaging of the distal biceps tendon was recently described in which the patient lies prone with the arm overhead, the elbow flexed to 90 degrees , and the forearm supinated, so that the thumb points superiorly. The acronym FABS (f lexed elbow, abducted shoulder, forearm supinated) ha
