Syncope, seizure-induced bradycardia and asystole: two cases and review of clinical and pathophysiological features.

Episodes of transient loss of consciousness (TLOC) are commonly due to syncope or epileptic seizures. The distinction between both entities on clinical grounds and eyewitness accounts can be challenging and is often hampered by similar clinical features. We briefly summarize syncope-related symptoms and present the case of a female patient who suffered from TLOC episodes due to both reflex syncope and epileptic seizures. Seizure-induced syncope is a rare complication particularly of non-generalized temporal lobe seizures and may be suspected in people with epilepsy who report new semiological

True zero-training brain-computer interfacing--an online study.

Despite several approaches to realize subject-to-subject transfer of pre-trained classifiers, the full performance of a Brain-Computer Interface (BCI) for a novel user can only be reached by presenting the BCI system with data from the novel user. In typical state-of-the-art BCI systems with a supervised classifier, the labeled data is collected during a calibration recording, in which the user is asked to perform a specific task. Based on the known labels of this recording, the BCI's classifier can learn to decode the individual's brain signals. Unfortunately, this calibration recording consu

Successful Treatment of Refractory Seizures With Rufinamide in Children With Schizencephaly: Report of 3 Cases.

Schizencephaly is an uncommon malformation of cortical development. Patients with schizencephaly present with a broad range of severe neurologic symptoms including pharmacoresistant epilepsy. Rufinamide is a new antiepileptic drug approved for use as adjunctive therapy of seizures associated with Lennox-Gastaut syndrome and it is also effective for refractory partial seizures. We report 3 cases of pediatric patients aged 7.2, 8.1, and 10.1 years, respectively, with intractable epilepsy associated with bilateral open-lip schizencephaly and septo-optic dysplasia. The follow-up ranged from 3.8 to

Towards a gaze-independent hybrid-BCI based on SSVEPs, alpha-band modulations and the P300.

In recent years it has been shown to be possible to create a Brain Computer Interface (BCI) using non-invasive electroencephalographic (EEG) measurements of covert visual spatial attention. For example, that both Steady-State Visual Evoked Potentials (SSVEP) and parieto-occipital alpha band activity have been shown to be sensitive to covert attention and this has been exploited to provide simple communication control without the need for any physical movement. In this study, potential improvements in the speed and accuracy of such a BCI are investigated by exploring the possibility of incorpor

Encephalopathy of infection and systemic inflammation.

This review will discuss several intracranial infections and sepsis-associated encephalopathy. Intracranial infections and inflammation of interest to the neurologist and EEG technicians include viral and autoimmune encephalitides; bacterial, fungal, and other meningitides; cerebritis; and brain abscess and subdural empyema. Sepsis-associated encephalopathy refers to a diffuse brain dysfunction secondary to infection that is principally located outside of the central nervous system. It is much more common than all of the intracranial infections put together, at least for adults in Western soci

Effect of prolonged wakefulness on electroencephalographic oscillatory activity during sleep.

The human sleep electroencephalogram (EEG) is characterized by the occurrence of distinct oscillatory events such as delta waves, sleep spindles and alpha activity. We applied a previously proposed algorithm for the detection of such events and investigated their incidence and frequency in baseline and recovery sleep after 40 h of sustained wakefulness in 27 healthy young subjects. The changes in oscillatory events induced by sleep deprivation were compared to the corresponding spectral changes. Both approaches revealed, on average, an increase in low frequency activity and a decrease in spind

Malignant migrating partial seizures in infancy.

The syndrome of malignant migrating partial seizures in infancy (MMPSI) was first reported in 1995, and is now included among the childhood epileptic syndromes in the revision proposal of the ILAE Commission on classification and terminology. The main clinical features are seizure onset in the first 6 months of life, occurrence of almost continuous migrating polymorphous focal seizures, associated with multifocal ictal EEG discharges, progressive deterioration of psychomotor development combined with frequent evolution of acquired microcephaly, and lack of a significant familial and etiologica

Epilepsy surgery for hemispheric syndromes in infants: hemimegalencepahly and hemispheric cortical dysplasia.

OBJECTIVE: Hemimegalencepahly (HME) and Hemispheric Cortical Dysplasia (HCD) are rare congenital diseases that occur with intractable epilepsy. They manifest by early epilepsy, mental retardation, hemianopsia and contralateral hemiplegia. Hemispheric disconnection (mainly anatomical hemispherectomy, peri-insular hemispherotomy, modified lateral hemispherotomy and vertical parasagittal hemispherotomy) have been reported to be efficient on seizures and also to prevent additional cognitive injury and developmental delay. METHOD: We reviewed literature about clinical presentation, predictors of o

The electroencephalogram of the full-term newborn: review of normal features and hypoxic-ischemic encephalopathy patterns.

OBJECTIVE: The objective of this study is to specify, by reference to the normal newborn, the current contribution of the electroencephalogram (EEG) to the diagnosis and prognosis of hypoxic-ischemic encephalopathy (HIE) in the full-term newborn. Both digitized traditional EEG and cerebral function monitoring will be considered. DISCUSSION: A good knowledge of the EEG features of the sleep-wake cycle (SWC) is a prerequisite. We first describe the main features of normal and pathological EEGs. Very early recordings (before 6hours of life) are needed to indicate any required neuroprotective (hy

Prolonged ictal monoparesis with parietal Periodic Lateralised Epileptiform Discharges (PLEDs).

We report a patient with prolonged monoparesis and parietal periodic lateralised epileptiform discharges (PLEDs). The patient was a 73-year-old man with chronic myelomonocytic leukaemia who developed persisting monoparesis of the right arm, sensory aphasia, and finger agnosia, initially associated with focal clonic seizures. These neurological deficits remained for seven days without subsequent focal clonic seizures. The EEG showed left-sided PLEDs, maximal in the left occipito-parietal area. Ten days later, following phenytoin treatment, these symptoms suddenly improved and parietal PLEDs dis

[Differences in the EEG synchrony between healthy children and adolescents and patients with schizophrenia].

In this paper, we have used a method for EEG synchrony estimation (an analysis of correlation synchrony of EEG. EEG recording was performed in a group of children and adolescents, aged 8-15 years, normal group (n=40) and schizophrenic group (n=30). One of the basic features of the integrated EEG picture is the presence of a pathology of extended zones of sharply lowered EEG-synchrony dividing the local and isolated areas in frontal and occipital regions, mainly of normal or sometimes raised synchrony. Also, there were significant correlations of synchrony estimates with memory and attention. T

Changes in N400 topography following intensive speech language therapy for individuals with aphasia.

Our goal was to characterize the effects of intensive aphasia therapy on the N400, an electrophysiological index of lexical-semantic processing. Immediately before and after 4 weeks of intensive speech-language therapy, people with aphasia performed a task in which they had to determine whether spoken words were a 'match' or a 'mismatch' to pictures of objects. Pre-therapy, people with aphasia exhibited an N400 mismatch effect that started over right hemisphere electrodes. Post-therapy, gains were seen in clinical measures of language ability, and the onset of the N400 was left-lateralized. No

Central apnea at complex partial seizure onset.

Sudden Unexpected Death in Epilepsy (SUDEP) is the most common cause of epilepsy related mortality in treatment resistant epilepsy. Most SUDEPs occur after one or more seizure(s) during sleep. Nocturnal seizures may go unrecognized. Respiratory depression in the peri-ictal period is one of the primary potential causes of SUDEP. Ictal and postictal apnea is often overlooked because it is not routinely assessed, but appears common and has been a recent focus of SUDEP research. We report a 37 year-old man who had central apnea as the initial manifestation of partial complex seizures associated wi

From migraine to epilepsy: a threshold mechanism?

A 67-year-old man complained of a transient blurring of vision in his right visual field lasting 30 min followed by headache. Two weeks later, the visual disturbance changed its pattern; it was described as the occurrence of brown round-shaped images in the right visual field spinning and turning for few seconds. This evolution from visual aura to visual seizures, with video-EEG correlation, supports the hypothesis of modification in threshold of cortical hyperexcitability from migraine to epilepsy.

Risk-taking and other effects of sleep loss on brain function and behaviour.

The sleep-deprivation paradigm remains a powerful approach in the study of the functions of sleep. When combined with the assessment of novel dependent measures or integration of multiple standard variables new insights may be obtained. This issue of the Journal of Sleep Research contains several studies that shed some new light on the effects of sleep deprivation and sleepiness. In addition, several papers emphasize the need to better characterize and understand the consequences of insomnia.

[Mechanism of anesthesia: view from the EEG during anesthesia].

It is widely known that electroencephalogram (EEG) shows dramatic changes with increase of the concentration of anesthetic. It is considered that volatile anesthetics (i. e. isoflurane, sevoflurane), barbiturates, propofol show anesthetic effect by potentiating GABAA receptor. Changing patterns of EEG by these anesthetics are quite similar. In light anesthetic level, high frequency with low amplitude waves are dominant. With increase of anesthetic concentration, waves in alpha range (8-13 Hz) become dominant. In deeper levels, powers in alpha range then become smaller and theta or delta powers

[Interictal epileptiform discharges and cognitive impairment in children].

INTRODUCTION: Learning disorders are very frequent among children with epilepsy. The etiology is multifactorial, being affected by the type of epileptic syndrome, the cause of epilepsy, a high frequency of epileptic seizures, a previous history of status epilepticus, the age of onset of epilepsy, the antiepileptic treatment being selected, and the role of interictal epileptiform discharges. Several studies have sought to analyze to what extent cognitive impairment can be attributed to interictal epileptiform discharges among the other epilepsy factors. AIM: To review the existing evidence on

Metabolic correlatives of brain activity in a FOS epilepsy patient.

The correlation and the interactions between neuronal activity and underlying metabolic dynamics are still a matter of debate, especially in pathological conditions. This study reports findings obtained on a subject suffering from fixation-off sensitivity (FOS) epilepsy, exploited as a model system of triggerable anomalous electrical activity. Functional Magnetic Resonance Spectroscopy was used to investigate the metabolic response to visual spike-inducing stimuli in a single voxel placed in the temporo-occipital lobe of a FOS epilepsy patient. MRS measurements were additionally performed on a
