Congenital heart disease in adults: evolving health systems and lifelong outcomes.

Over the past decades, advances in the diagnosis and treatment of congenital heart disease have reshaped its epidemiology. Congenital heart disease is now increasingly a chronic, lifelong disorder, with adult survivors outnumbering affected children in many countries. This demographic transition has shifted the focus from preventing early mortality towards optimizing long-term health and managing comorbidities. Adult congenital heart disease (ACHD) experience acquired cardiovascular and systemic complications that challenge traditional models of care. As the lifelong burden of disease in ACHD

Synthetic Comparative Analysis of Cardiovascular Disease Profiles Using Quantum Information Measures in Indian and Global Populations.

Background/Objectives: Cardiovascular diseases (CVDs) represent the foremost public health challenge of recent days. According to the World Health Organization (WHO), nearly 32% of people die from CVDs each year. This paper introduces a quantum-inspired, information-theoretic framework for comparing CVD risk-factor architectures in modeled Indian and global populations. Methods: Each synthetic population is represented as a population state matrix (ρ) built from discretized epidemiological variables (age, sex, smoking, diabetes, hypertension, and dyslipidemia). Systemic uncertainty is quantifi

Modeling the effects of pandemics on physiology-related fertility decline in society.

Pandemics throughout history have raised challenges in many forms, such as increased mortality rates, economic instability, and political shifts. Recent pandemics, coupled with globally declining fertility rates, pose unprecedented demographic challenges for humanity. A pandemic is inherently connected to a population's fertility in many ways - psychological, economic, and, most importantly, physiological. Nevertheless, this connection is mostly left unintended, blinding us to the effects of the pandemic-caused physiological fertility decline on the population's growth over time. In this study

Prognostic model for pulmonary metastasis in patients with HER2-positive breast cancer: a population-based study.

BACKGROUND: Lung metastasis is an important adverse prognostic factor in patients with HER2-positive breast cancer. However, a lack of an accurate estimation of the prognosis of patients with lung metastasis and the validation of population-based models has been observed. In this study, we aimed to establish a nomogram based on the surveillance, epidemiology, and end results (SEER) database to determine the prognostic factors associated with lung metastasis and evaluate the individualized survival of patients with lung metastasis. METHODS: This study included 613 patients diagnosed with HER2-p

Everything All at Once: On Choosing an Estimand for Multi-component Environmental Exposures.

Many research questions-particularly those in environmental health-do not involve binary exposures. In environmental epidemiology, this includes multivariate exposure mixtures with nondiscrete components. Causal inference estimands and estimators to quantify the relationship between an exposure mixture and an outcome are relatively few. We propose an approach to quantify a relationship between a shift in the exposure mixture and the outcome-either in a single timepoint or longitudinal setting. The shift in the exposure mixture can be defined flexibly in terms of shifting one or more components

Performance of Estimated Kidney Function Equations for Predicting Aminoglycosides Clearance in Thai Population.

Disparities in the performance of various kidney function equations in predicting aminoglycoside clearance (CL) have been identified. However, data specific to the Thai population remains limited. This study aimed to evaluate the performance of kidney function equations in estimating aminoglycoside CL in Thai patients. Data were retrospectively collected from hospitalized Thai patients who received amikacin or gentamicin over a 10-year period. Population pharmacokinetic (PK) analysis was performed using a nonlinear mixed-effects modeling approach. The association between aminoglycoside CL and

Nonlinear expansion, spatial diffusion, and climate-driven dynamics of scorpionism in Brazil: a nationwide time-series and DLNM analysis (2007-2025).

Scorpionism has emerged as a growing public health concern in Brazil, characterized by increasing incidence and geographic expansion. Despite this trend, studies integrating climatic drivers and advanced time-series modeling remain limited. This study aims to analyze the temporal dynamics, spatial diffusion, and climate-disease relationships of scorpion sting incidence in Brazil using a nationwide dataset from 2007 to 2025. Monthly scorpion sting data from SINAN/DATASUS were combined with high-resolution climatic variables from the TerraClimate dataset. Descriptive statistics, seasonal analysi

Predicting the Global Distribution of Vibrio vulnificus Under Changing Climate Scenarios.

Vibrio vulnificus is a Gram-negative bacterium and an emerging pathogen globally distributed in brackish and marine waters. It can be transmitted to humans through contaminated food, which causes primary septicemia, or through wound exposure, causing necrotizing fasciitis. Increased abundance and range expansion have recently been associated with climate change. Further understanding of the spatial epidemiology of V. vulnificus is necessary, and species distribution modeling is a technique to correlate all the factors related to the spread of V. vulnificus. Through meta-analysis aided by Rayya

Effect of Implementing Population-Based Prostate-Specific Antigen Screening on Testing Rates and Prostate Cancer Overdiagnosis in England: A Statistical Modelling Study.

We undertook a statistical modeling study to determine the effect of implementing population-based prostate-specific antigen (PSA) screening in England on overdiagnosis and PSA testing rates in comparison with the current opportunistic testing policy. Our model merged English data on life expectancy, rates of PSA testing and incidence of prostate cancer by stage with epidemiological data on lead time. In the base scenario, introduction of population-based screening led to an approximate 25% reduction in both PSA testing and overdiagnosis in the population compared with the current policy. This

Identifying early-life, environmental, and social stressors as key predictors of U.S. respiratory failure mortality: a machine learning study.

BACKGROUND: Traditional risk factors like smoking and air pollution remain insufficient to explain the persistent geographical variation in respiratory failure mortality across the United States, with significantly higher mortality observed in specific regions such as the Mississippi River Basin. METHODS: We conducted a state-level ecological study using Centers for Disease Control and Prevention Wide-ranging Online Data for Epidemiologic Research (CDC WONDER) mortality data and 68 socio-environmental risk factors from Global Burden of Disease (GBD). From sites to structure to serology: a roadmap for structure-aware molecular evolution of antigenically evolving viruses.

The genomic deluge has pushed viral molecular evolution into a site-resolved era. For antigenically evolving viruses such as influenza and severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), dense genomic sampling now supports mutation-annotated phylogenies and per-site estimates of mutation and substitution processes. These data highlight strong effects of sequence context, genomic region, RNA structure, and protein-level constraints that are blurred by classic uniform substitution models. In parallel, accurate structure prediction and emerging structure-aware phylogenetic and machi

Mechanistic interactions between Plasmodium falciparum and Epstein-Barr virus in endemic Burkitt Lymphoma pathogenesis: a narrative review (2010-2026).

Endemic Burkitt Lymphoma, characterized by an MYC translocation during B-cell activation, is an aggressive, fast-proliferating cancer that predominantly accounts for more than half of all childhood cancers in sub-Saharan Africa. Although a well-established epidemiologic association exists between Epstein-Barr virus infection and Plasmodium falciparum malaria, disentangling their mechanistic interactions in the pathogenesis of endemic Burkitt Lymphoma remains difficult. To address this challenge, a comprehensive narrative review of studies published between 2010 and 2026, was conducted. Evidenc

Seroprevalence, Risk Factors, and Environmental Correlates of Babesia caballi, Toxoplasma gondii, and Coxiella burnetii in Equids from Southwestern Greece.

Equids, primarily horses, are mostly used for recreational purposes, although in some rural areas they also serve as working animals, maintaining close and frequent contact with humans. Their risk of exposure to vector-borne and zoonotic pathogens can be affected by host-related factors, management practices and environmental conditions. This study aimed to investigate the seroprevalence and associated risk factors for infections by Babesia caballi, Toxoplasma gondii, Coxiella burnetii, and Borrelia burgdorferi sensu lato in equids from Southwestern Greece. Unraveling Time-Varying Effects of Risk Factors for Contralateral Invasive Breast Cancer Following Ductal Carcinoma In Situ.

INTRODUCTION: Current studies have not reached a consensus on the factors influencing contralateral invasive breast cancer (CIBC) after ductal carcinoma in situ (DCIS), nor have they examined how these effects vary over time. METHODS: A total of 121,529 women with a first DCIS diagnosis between 2000 and 2020 were identified from the Surveillance, Epidemiology, and End Results (SEER) database. Time-dependent Cox model and landmark analysis with competing risks models assessed factors influencing CIBC and their time-varying effects, illustrated by annual incidence rates. RESULTS: During the 22

Evaluating mobility restrictions through spatiotemporal effective reproduction number analysis in a multi-patch model with complex mobility data.

Understanding the spatial and temporal dynamics of infectious disease transmission is critical for effective epidemic preparedness and response. COVID-19 transmission is influenced by mobility patterns, regional connectivity, and evolving public health interventions, making it challenging to quantify region-specific transmission risks. Our study integrates intervention-driven analysis, real-world data, and high-resolution modeling to establish a robust computational framework for assessing interregional transmission dynamics. We employ a multi-patch model to estimate the time-dependent regiona

Electromagnetic fields and oxidative stress: The link to the development of cancer, neurological diseases, and behavioral disorders.

BACKGROUND: Epidemiological studies suggest an association between exposure to electromagnetic fields (EMFs) and an increased incidence of malignant, cardiovascular, and neurodegenerative diseases. This study aims to elucidate the fundamental principles and plausible mechanisms by which EMFs may influence physiological and pathological processes that lead to disease development. MATERIALS AND METHODS: Published reports of oxidative stress, DNA damage, and disease risk related to EMF exposure were examined. The literature review provided the foundation for building a new conceptual model calle
