Ultrasonographic evaluation of intraoperative optic nerve sheath diameter changes in patients undergoing laparoscopic surgery.

BACKGROUND: Laparoscopic surgery offers several advantages such as reduced postoperative pain and faster recovery; however, the establishment of pneumoperitoneum has the potential to transiently increase intracranial pressure (ICP). Ultrasonographic measurement of the optic nerve sheath diameter (ONSD) provides a reliable noninvasive indicator of ICP changes. This study aimed to evaluate ONSD measurements, which may reflect intraoperative ICP changes during laparoscopic upper abdominal surgery performed in the reverse Trendelenburg position, and to assess the specific effects of pneumoperitone

MRI Based Radiomics as an Imaging Biomarker for Locally Advanced Carcinoma Rectum: Predicting Tumor Response Following Neoadjuvant Chemoradiotherapy as an Organ Preservation Strategy.

BACKGROUND: Patients with locally advanced rectal cancer show variable responses to neoadjuvant chemoradiotherapy (NACTRT), making early prediction of treatment response clinically important. Radiomics and machine learning may offer a potential non-invasive imaging biomarker to identify patients who are more likely to respond to treatment. AIM: We investigated a machine learning-based radiomic approach to predict tumor response (TR) to NACTRT in patients with adenocarcinoma of the rectum from baseline and post-NACTRT magnetic resonance imaging (MRI). METHODS: One hundred patients with locall

Combined Assessment of Gastrointestinal Hormones and Metabolomic Profiling Following Mixed-Meal Tolerance Tests in Patients at Increased Risk of Refeeding Syndrome: Study Protocol.

Introduction: Refeeding syndrome (RS) is a life-threatening metabolic complication of nutritional support. Prolonged fasting, frequently observed in malnourished patients referred for percutaneous endoscopic gastrostomy (PEG), may induce histological and ultrastructural changes in the intestinal mucosa, potentially influencing metabolic adaptation during nutritional reintroduction. Mixed-Meal Tolerance Tests (MMTT) combined with targeted metabolic profiling allow dynamic assessment of serum glucose, gastrointestinal hormones and metabolites, which may help to elucidate the metabolic adaptation

Pre-induction nociception indices are associated with post-induction hypotension during general anesthesia in older patients: a prospective observational study.

We investigated whether pre-induction High Frequency Variability Index (HFVI)/Analgesia Nociception Index (ANI) and Nociception Level (NOL) values could predict hypotension during general anesthesia induction in older patients. This prospective observational study included patients aged ≥ 65 years undergoing elective surgery under general anesthesia. After recording HFVI/ANI and NOL values, remifentanil was administered at 0.2 µg/kg/min for 2 min, followed by a rapid intravenous bolus administration of propofol at 2 mg/kg. Noninvasive blood pressure was measured at 1-min intervals over 5 min.

Untreated mild-moderate mitral regurgitation after isolated aortic valve replacement for aortic stenosis.

BACKGROUND: Postoperative persistent/worsening mitral regurgitation (MR) is reported to be a prognostic marker in patients undergoing isolated aortic valve replacement (AVR). However, there is little evidence regarding the surgical indication for coexisting mild-to-moderate MR in patients undergoing isolated surgical AVR. This study evaluates the relationship between untreated mild-to-moderate MR and postoperative significant MR after surgical AVR. METHODS: Between 2016 and 2021, 430 patients with severe aortic stenosis underwent isolated surgical AVR with untreated mild-to-moderate MR. The p

Pain processing and pain assessment in Huntington's disease: Study protocol of the Huntington's disease - PAIN study.

BACKGROUND: Huntington's disease (HD) is an autosomal dominant neurodegenerative disease that causes motor (e.g., chorea) and non-motor symptoms (e.g., neurocognitive, neuropsychiatric and autonomic disturbances). Pain is a frequently reported non-motor symptom in HD, with a prevalence of around 40% in the manifest stage. Despite its high prevalence, patients with HD seem to be at risk for undertreatment of pain, as reflected by the lower analgesic use in HD in the advanced stages. The relatively low use of analgesics may result from under-recognition and inadequate assessment of pain, particu

Development and application of a structured preclinical protocol for MRI-guided endomyocardial biopsy: a multimodal imaging approach using a hybrid vessel phantom and porcine in vivo models.

BACKGROUND: MRI-guided cardiovascular procedures provide high-resolution, radiation-free imaging, but clinical translation is limited due to the lack of MR Conditional medical devices. This study presents a structured preclinical evaluation protocol for MRI-guided endomyocardial biopsy (EMB) interventions, demonstrating its feasibility for both instrument performance and procedural workflow. METHODS: An MR-compatible prototype EMB forceps was evaluated using the three-stage preclinical protocol. First, device visibility was assessed using fluoroscopy and two MRI systems (Magnetom Sola 1.5T an

Prognostic impact of tumor location after abdominoperineal resection of rectal cancer: A single-center propensity score matching analysis based on 1806 cases.

BACKGROUND: Randomized controlled trials have revealed that abdominoperineal resection leads to inferior oncological outcomes compared with low anterior resection, especially regarding local recurrence rates (LRRs). While neoadjuvant chemoradiotherapy can lower LRRs, it is linked to potential short- and long-term radiation-induced adverse effects. Consequently, meticulous patient selection for neoadjuvant chemoradiotherapy is imperative to balance benefits and risks. METHODS: This research encompassed individuals with rectal cancer (RC) who underwent abdominoperineal resection (APR) from Janu

Evaluation of prothymosin alpha, trimethylamine-N-oxide, and ischemia-modified albumin in type 2 diabetes mellitus patients with dysregulated lipid profile.

Background: Prothymosin alpha (PTMα) is a small acidic polypeptide from the thymosin family with immune activity and protective properties against oxidative stress induced by reactive oxygen species trimethylamine-N-oxide (TMAO), produced in the liver from gut bacterial metabolite trimethylamine and associated with increased cardiovascular disease risk and higher all-cause mortality. Ischemia-modified albumin (IMA) is a significant oxidative stress biomarker, particularly in ischemia-reperfusion conditions. This study investigates PTMα, TMAO, and IMA levels in type 2 diabetes mellitus (T2DM) p

Intubation of obese patients in the operating room with or without bag-mask ventilation: study protocol for a randomized multicenter trial (VENT OR NOT).

BACKGROUND: Almost two-thirds of the adults in Europe are overweight or obese. Obese patients are at higher risk of complications, especially during intubation. Therefore, the management of general anesthesia is crucial. Regarding intubation, there is no consensus among anesthesiologists between the two strategies: standard sequence intubation (i.e., with bag-mask ventilation between induction and intubation) vs. rapid sequence intubation (i.e., without bag-mask ventilation between induction and intubation) assuming that these patients are at high risk of inhalation and desaturation during apn
