Risk of preeclampsia in second-trimester triploid pregnancies.

OBJECTIVE: To determine the magnitude of the risk and the predictive clinical characteristics for development of preeclampsia when triploidy is diagnosed in the second trimester. METHODS: A retrospective analysis of databases maintained by the cytogenetics laboratories at the University of Iowa and University of North Carolina was performed to identify all cases of triploidy. We examined the karyotype, maternal serum screening (particularly the hCG level), ultrasound results, and evidence of maternal hypertensive disease. RESULTS: Seventeen cases of triploidy were identified between 1987 and

Transfer of paracetamol across the placenta and fetal blood-brain barriers and its safety for use in pregnancy.

Paracetamol (acetaminophen) is the most widely used analgesic and antipyretic during pregnancy and has long been considered safe at recommended doses by national health authorities. However, recent preclinical and epidemiological studies have prompted renewed scrutiny of its pharmacology and safety profile in the context of fetal development. This review summarizes current evidence on the pharmacokinetics of paracetamol in pregnancy, placental transfer and permeability across the fetal brain barriers. We also examine findings from both experimental and population-based studies investigating sh

Metabolism and homeostasis of energy in trophoblast cells of the placenta: from development to disease†.

The placenta is a highly metabolic organ essential for fetal growth by mediating nutrient transport, hormone production, and immunological regulation. These functions depend on continuous and efficient adenosine triphosphate (ATP) supply, primarily generated through glycolysis and oxidative phosphorylation. However, due to high turnover of ATP and multi-step de novo synthesis, these pathways may not always meet the rapid and localized energy demands of trophoblast cells. The phosphagen system, comprising creatine kinase (CK), creatine (Cr), and phosphocreatine (PCr), provides a rapid ATP-buffe

A multiparameter model combining ultrasound and maternal serum biomarkers accurately predicts late-onset fetal growth restriction.

OBJECTIVES: Late-onset fetal growth restriction (FGR) in pregnancy is frequently encountered and is linked to adverse perinatal results. This investigation aimed to evaluate the clinical use of a multiparameter model combining ultrasound features and maternal serum biomarkers for late-onset FGR. METHODS: A retrospective investigation was conducted on 350 pregnant women in their third trimester who received standard prenatal care between January 2021 and December 2025. Participants were divided into a late-onset FGR group (n = 175) and a normal fetal development group (n = 175) according to th

Fetal heart rate variability in relation to maternal physical activity and metabolic health.

Physical activity (PA) during pregnancy may have a positive effect on the fetal cardiac maturation which is reflected in a decreasing resting heart rate and increasing heart rate variability (HRV). Different types of PA, for example during leisure or work time, have differential effects on HRV; however, this relationship has not yet been investigated in pregnancy. In our work, we related different types of PA during pregnancy with maternal and fetal HRV. We assessed the levels of PA in 95 pregnant women between 28 and 32 weeks of gestational age using the Baecke Physical Activity Questionnaire

Fetal heart and surroundings: umbilical cord traction triggers sympathetic heart rate surge in fetal lambs in the artificial womb.

OBJECTIVE: To investigate whether umbilical cord traction triggers a unique umbilico-neuro-cardio reflex via sympathetic autonomic nervous system (ANS) activation, potentially explaining fetal heart rate (FHR) increases such as the double mountain peak sign and unexplained tachycardia outside labor. DESIGN: Experimental physiological study using the EXTrauterine Environment for Neonatal Development (EXTEND) system. SETTING: Controlled laboratory environment at The Children's Hospital of Philadelphia with fetal lambs supported by the EXTEND system. POPULATION OR SAMPLE: Twenty-nine experimen

EHDPP impairs intestinal microbiota homeostasis and induces placental injury through choline mediated gut-placenta axis.

2-Ethylhexyl-diphenyl phosphate (EHDPP) is an organophosphate ester (OPE) with roles of flame retardant and plasticizer. It is widely used in various applications, detected in environmental matrices and human body, threatening ecological environment and human health. Some OPEs have been reported to disturb the gut microbiota, the gut microbiota mediates placental function. Our previous study showed EHDPP causes placental toxicity and fetal weight loss, it is unknown that whether EHDPP affects fetal development through the gut-placenta axis and whether it is feasible to fight against EHDPP indu

Mid-late gestation leptin infusion induces placental mitochondrial and endoplasmic reticulum unfolded protein responses in a mouse model of preeclampsia.

INTRODUCTION: Preeclamptic patients, both lean and obese, present with elevated leptin levels which are associated with the development of maternal endothelial dysfunction and adverse fetal outcomes, such as growth restriction, leading to low birth weight. Recent studies in pregnant mice demonstrate that mid-late gestation leptin infusion induces clinical characteristics of preeclampsia, including elevated maternal blood pressure, maternal endothelial dysfunction and fetal growth restriction. However, whether leptin triggers placental stress responses that contribute to adverse fetal outcomes

Umbilical artery thrombosis diagnosed by fetal ultrasound.

OBJECTIVES: Umbilical artery thrombosis (UAT) is a rare and severe condition associated with grave perinatal outcomes, including intrauterine fetal death. This case report presents the case of a 38-year-old woman (gravida 3, para 1) of Japanese ethnicity, with a history of one spontaneous miscarriage, who conceived through micro-insemination and blastocyst transfer. CASE PRESENTATION: Initial patient screening at 30 weeks and 6 days of gestation revealed normal fetal development, with two umbilical arteries and one umbilical vein. However, at 34 weeks and 5 days of gestation, we observed redu

BEAN: Brain Extraction and Alignment Network for 3D Fetal Neurosonography.

Brain extraction (masking of extra-cerebral tissues) and alignment are fundamental first steps of most neuroimage analysis pipelines. The lack of automated solutions for 3D ultrasound (US) has therefore limited its potential as a neuroimaging modality for studying fetal brain development using routinely acquired scans. In this work, we propose a convolutional neural network (CNN) that accurately and consistently aligns and extracts the fetal brain from minimally pre-processed 3D US scans. Our multi-task CNN, Brain Extraction and Alignment Network (BEAN), consists of two independent branches: 1

Type 1 Diabetes in Pregnancy: A Review of Complications and Management.

BACKGROUND: Pre-gestational diabetes can pose significant risk to the mother and infant, thus requiring careful counselling and management. Since Saint Vincent's declaration in 1989, adverse maternal and fetal outcomes, such as preeclampsia, perinatal mortality, congenital anomalies, and macrosomia, continue to be associated with type 1 diabetes. Although pregnancy is not considered an independent risk factor for the development of new onset microvascular complications, it is known to exacerbate pre-existing microvascular disease. Strict glycaemic control is the optimal management for pre-exis

Fetal Omphalocele: Review of Predictive Factors Important for Antenatal Counseling?

IMPORTANCE: Congenital omphalocele is among the most common antenatally diagnosed anomalies. As additional abnormalities are found in majority of cases, antenatal investigations target the search for additional structural anomalies and genetic disorders, including aneuploidy. Antenatal management focuses on the assessment of fetal well-being. Unfortunately, antenatal prediction of postnatal and long-term outcomes represents 2 less well-documented but crucial facets of this pathology. A large part of the prognosis relies on aspects that are difficult to predict such as quality of life, neurolog

Fetal growth restriction and a single umbilical artery are independent predictors of hypospadias during pregnancy.

INTRODUCTION: Little is known about the association between hypospadias and small fetuses, as well as the pathological implications of fetal growth restriction (FGR). Thus, we aimed to investigate the association between hypospadias and small fetuses using a database of fetal ultrasound and obstetric events. METHODS: A cohort of male singleton infants delivered after 22 weeks of gestation at Keio University Hospital between 2013 and 2019 was retrospectively reviewed. FGR was defined according to the Delphi criteria. Logistic regression analysis was performed to identify the significant predic

Egg cylinder development during in vitro extended embryo culture predicts the post transfer developmental potential of mouse blastocysts.

PURPOSE: To establish parameters during mouse extended embryo culture that accurately predict fetal developmental potential of a blastocyst without performing embryo transfer. METHODS: Embryos of three varying qualities were produced: poor quality embryos produced from in vitro matured oocytes (IVM), intermediate quality embryos produced from in vivo matured oocytes followed by in vitro fertilization and embryo culture (IVF); high quality embryos developed in vivo (VIVO). Embryonic day (E) 3.5 embryos from each group with similar morphologies were used for surgical embryo transfer to assess i

Differential Long-Chain Polyunsaturated Fatty Acids Status and Placental Transport in Adolescent Pregnancies.

Adolescent pregnancy increases risk of adverse perinatal outcomes. Placental delivery of long-chain polyunsaturated fatty acids (LCPUFA) is essential for fetal growth and development. In this pilot study, we aimed to assess maternal and fetal status of fatty acids (FA) measured at birth and the expression of key genes involved in FA uptake, transport and metabolism in the placenta of fifteen adolescents and fifteen adults. FA were quantified by gas-liquid chromatography. Placental expression of FA transporters was assessed by quantitative real-time polymerase chain reaction (qRT-PCR) and perox

Vitamin D: effects on pregnancy, maternal, fetal and postnatal outcomes.

A high prevalence of vitamin D deficiency and its negative consequences for health is identified as area of primary concern for scientists and clinicians worldwide. Vitamin D deficiency affects not only bone health but many socially significant acute and chronic diseases. Observational studies support that pregnant and lactating women, children and teenagers represent the high risk groups for developing vitamin D deficiency. Current evidence highlights a crucial role of vitamin D in providing the fetal life-support system and fetus development, including implantation, placental formation, intr

A combined opiate agonist and antagonist treatment reduces prolactin secreting pituitary tumor growth.

Prolactin secreting pituitary adenomas (prolactinomas) is the most common pituitary tumors in humans. Animal studies have identified aggressive prolactinoma development in fetal alcohol exposed rats. We have recently identified a combination treatment of a μ opioid receptor antagonist naltrexone and a δ opioid receptor agonist D-Ala2-,N-Me-Phe4,Gly-ol Enkephalin (DPDPE) increases innate immune function. In this study, we tested whether naltrexone and DPDPE combination therapy is useful to control pituitary tumor growth. Fetal alcohol exposed and control Fischer 344 female rats at 60 days of ag
