Botulinum toxin A compared with stretching casts in the treatment of spastic equinus: a randomised prospective trial.

Conservative therapies for equinus in cerebral palsy may help to postpone calf surgery in younger children. This study reports a prospective randomised trial of intramuscular botulinum toxin A (BtA) as an alternative to serial casting in 20 children with a dynamic component to calf equinus. Outcome was assessed in the short term to show the effect of one treatment cycle. Assessments were by clinical examination, video gait analysis, and three-dimensional gait analysis. BtA was of efficacy similar to that of serial casting. Tone reduction in the BtA group allowed a more prolonged improvement in

Estimating and interpreting how humans prioritize multiple movement goals during walking.

BACKGROUND: Walking requires managing competing movement goals shaped by the demands of everyday activity. Several important goals during overground walking include maintaining a desired walking speed, achieving an intended foot placement, preserving postural and locomotor balance, and reducing energy expenditure. Biomechanical simulation and motor control-based methods use goal-related cost functions or structured models to identify people’s movement priorities but are often restricted to simple movement scenarios and are currently impractical for broad deployment. METHODS: Here, we systemati

Vestibular Function Drives Gaze Stability in Locomoting Macaques.

Stable gaze is essential for effective navigation, yet during locomotion each stride generates phasic head movements that can destabilize the visual scene. The vestibular system is well positioned to counter these disturbances by sensing head motion and rapidly generating reflexive compensatory eye movements, but its specific contribution to gaze stabilization during locomotion is not well understood. To directly test the vestibular system's contribution during walking, we compared gait and gaze kinematics in male and female rhesus macaques with normal vestibular function and those with chroni

Effects of proprioceptive neuromuscular facilitation therapy combined with orthotic insole intervention on lower limb function improvement in children with foot drop and inversion deformity.

OBJECTIVE: To evaluate whether combining proprioceptive neuromuscular facilitation (PNF) therapy with customized orthotic insoles leads to greater improvements in gait parameters, ankle range of motion, and lower-limb function than either intervention alone in pediatric foot drop and inversion deformity. METHODS: Ninety-seven children (aged 5-14 years) with foot drop and inversion deformity were randomized into three groups: (1) PNF plus insole, (2) insole only, and (3) PNF only. All interventions lasted 12 weeks. Gait parameters, the Gross Motor Function Measure (GMFM), ankle range of motion

Feasibility of Smartphone-Based Exercise Training Integrated with Functional Electrical Stimulation After Stroke (SETS): A Preliminary Study.

One emerging method in home stroke rehabilitation is digital technology. However, existing approaches typically target one domain (e.g., upper limb). Moreover, existing interventions do not cater to older adults with stroke (OAwS), especially those with high motor impairment, who require adjunct therapeutic agents to independently perform challenging exercises. We examined the feasibility of Smartphone-based Exercise Training after Stroke (SETS) with Functional Electrical Stimulation (FES). A total of 12 participants (67 ± 5 years) with stroke (onset > 6 months) exhibiting moderate-to-high mot

Reorganization of spinal neural connectivity following recovery after thoracic spinal cord injury: insights from computational modelling.

Rats exhibit significant recovery of locomotor function following incomplete spinal cord injuries, albeit with altered gait expression and reduced speed and stepping frequency. These changes likely result from and give insight into the reorganization within spared and injured spinal circuitry. Previously, we developed computational models of the mouse spinal locomotor circuitry controlling speed-dependent gait expression (Danner et al. 2017; Zhang et al. 2022). Here, we adapted these models to the rat and used the adapted model to explore potential circuit-level changes underlying altered gait

Real-World Wrist-Derived Digital Mobility Outcomes in People with Multiple Long-Term Conditions: A Comparison of Algorithms.

Digital Mobility outcomes can serve as objective biomarkers of health, but their validation in populations with multiple long-term conditions (MLTCs) based on wrist-worn devices remains unexplored. We refined, improved, and introduced novel algorithms, specifically tailored and adapted for (i) gait sequence detection, (ii) initial contact identification, and (iii) stride length estimation from a single wrist-worn device. Validation was performed using data from 28 participants with co-occurring MLTCs performing a 2.5 h real-world monitoring session. Reference data from an established multi-sen

Associations between less knee kinematic variability and worse patient-reported outcomes following anterior cruciate ligament reconstruction.

Individuals with anterior cruciate ligament reconstruction (ACLR) exhibit less knee kinematic variability while walking than uninjured controls, associated with deleterious changes in cartilage composition linked to an increased risk for early knee osteoarthritis (KOA). It is unknown whether less knee kinematic variability is also associated with worse knee-related patient-reported outcomes (PROs) consistent with KOA development. This study examined associations between kinematic variability during gait and PROs in individuals post-ACLR. Gait kinematics and the Knee Injury and Osteoarthritis O

Walking on common ground: a cross-disciplinary scoping review on the clinical utility of digital mobility outcomes.

Physical mobility is essential to health, and patients often rate it as a high-priority clinical outcome. Digital mobility outcomes (DMOs), such as real-world gait speed or step count, show promise as clinical measures in many medical conditions. However, current research is nascent and fragmented by discipline. This scoping review maps existing evidence on the clinical utility of DMOs, identifying commonalities across traditional disciplinary divides. In November 2019, 11 databases were searched for records investigating the validity and responsiveness of 34 DMOs in four diverse medical condi

Short-term effect of a close-fitting type of walking assistive device on spinal cord reciprocal inhibition.

One of the major problems with walking encountered by patients with spastic hemiplegia is diminished toe clearance due to spasticity of their leg muscles. To improve their walking, a specialized robot assist for ankle movements (RE-Gait) has been utilized. The present study examined the neurophysiological effects whether spinal cord reciprocal Ia inhibition (RI) in the leg was altered by using RE-Gait. Sixteen patients with a clinical diagnosis of stroke were divided into the two groups, RE-Gait walking group (Group R) and normal (controlled) walking group (Group C). In each group, they walked

Functional effects of botulinum toxin type-A treatment and subsequent stretching of spastic calf muscles: a study in patients with hereditary spastic paraplegia.

OBJECTIVE: Although calf muscle spasticity is often treated with botulinum toxin type-A, the effects on balance and gait are ambiguous. Hereditary spastic paraplegia is characterized by progressive spasticity and relatively mild muscle weakness of the lower limbs. It is therefore a good model to evaluate the functional effects of botulinum toxin type-A. DESIGN: Explorative pre-post intervention study. SUBJECTS: Fifteen subjects with pure hereditary spastic paraplegia. METHODS: Patients with symptomatic calf muscle spasticity and preserved calf muscle strength received botulinum toxin type-A

Transplantation of embryonic motor neurons into peripheral nerve combined with functional electrical stimulation restores functional muscle activity in the rat sciatic nerve transection model.

Reinnervation of denervated muscle by motor neurons transplanted into the peripheral nerve may provide the potential to excite muscles artificially with functional electrical stimulation (FES). Here we investigated whether transplantation of embryonic motor neurons into peripheral nerve combined with FES restored functional muscle activity in adult Fischer 344 rats after transection of the sciatic nerve. One week after sciatic nerve transection, cell culture medium containing (cell transplantation group, n = 6) or lacking (surgical control group, n = 6) dissociated embryonic spinal neurons was
