From mutation to symptoms: a multi-center study on HNF1B-related nephropathy in Chinese children.

BACKGROUND: Hepatocyte nuclear factor 1β (HNF1B) pathogenic variants constitute a major genetic contributor to congenital anomalies of the kidney and urinary tract (CAKUT), with patients simultaneously exhibiting distinct extrarenal features. Among these clinical manifestations, renal disease progression is crucial for long-term outcomes, needing comprehensive evaluation. METHODS: Using the Chinese Children Genetic Kidney Disease Database (2017-2024), we analyzed 26 pediatric HNF1B cases to characterize renal phenotypes and genotype correlations. RESULTS: All patients exhibited abnormal rena

Neurodevelopmental Progression and Functional Outcomes in a Child With Joubert Syndrome: A Case Study.

Joubert syndrome (JS) is a rare autosomal recessive neurodevelopmental disorder characterized by malformations of the cerebellum and brainstem, most notably the pathognomonic "molar tooth sign" on magnetic resonance imaging (MRI). Clinical manifestations are heterogeneous and include dysmorphic features, motor and ocular abnormalities, and, in most patients, intellectual developmental disorder (IDD). We describe the case of a nine-year-old boy with prenatal suspicion of ventricular asymmetry and postnatal findings of macrocephaly and facial dysmorphisms. In infancy, he exhibited nystagmus, ocu

Identification of a novel single nucleotide deletion in the NHS causing Nance-Horan syndrome.

BACKGROUND: Nance-Horan syndrome (NHS) is a rare X-linked dominant disorder caused by pathogenic variants in the NHS gene on chromosome Xp22.2-Xp22.13. Clinical manifestations consist of congenital cataracts, along with dysmorphic facial features and dental anomalies and, in certain instances, intellectual disability. This study aimed to identify the genetic cause responsible for NHS in a Chinese family with four individuals primarily presenting with congenital cataracts. METHODS: Genomic DNA was collected from six family members, including four affected individuals (three females and one mal

Recurrent Spontaneous Pneumothorax as a Manifestation of the Birt-Hogg-Dubé Syndrome: A Case Report.

Birt-Hogg-Dubé syndrome (BHDS) is a rare autosomal dominant disorder caused by mutations in the folliculin (FLCN) gene, characterized by a clinical triad of pulmonary cysts with spontaneous pneumothorax, cutaneous fibrofolliculomas, and renal tumors. Pulmonary manifestations may occur in isolation, making early diagnosis challenging. This report describes the case of a 36-year-old woman with a history of right-sided spontaneous pneumothorax, managed with video-assisted thoracoscopic surgery (VATS) and pleurodesis, who re-presented seven years later with a left-sided pneumothorax. Conservative

A Guide for Implementing DPYD Genotyping for Systemic Fluoropyrimidines into Clinical Practice.

The safety of systemic fluoropyrimidines (e.g., 5-fluorouracil, capecitabine) is impacted by germline genetic variants in DPYD, which encodes the dihydropyrimidine dehydrogenase (DPD) enzyme that functions as the rate-limiting step in the catabolism of this drug class. Genetic testing to identify those with DPD deficiency can help mitigate the risk of severe and life-threatening fluoropyrimidine-induced toxicities. Globally, the integration of DPYD genetic testing into patient care has varied greatly, ranging from being required as the standard of care in some countries to limited clinical use

Gingival enlargement following levetiracetam treatment: A clinical case analysis.

This report describes a 21-year-old female with a history of epilepsy, diagnosed at age 17, who developed gingival enlargement after being treated with levetiracetam. Her dosage was titrated to 750 mg twice daily, achieving satisfactory seizure control. Regular neurological evaluations and laboratory tests remained within normal limits for 3 years. However, the patient subsequently reported increased gingival size and a perception that her teeth appeared smaller. Examination confirmed gingival enlargement, with no other medications or underlying conditions contributing to this side effect. Gen

Pyrimidine-5'-Nucleotidase Deficiency: a New Homozygous NT5C3A Mutation (c.693+1G>A variant).

BACKGROUND: Erythrocytes have an average lifespan of 120 days, after which they are typically removed by macrophages in the reticuloendothelial system. Hemolytic anemia can shorten erythrocyte lifespan, leading to varying clinical presentations depending on whether hemolysis occurs intravascularly or extravascularly. Among intrinsic causes of hemolysis, pyrimidine 5'-nucleotidase (P5N) deficiency is a notable condition, often presenting as nonspherocytic hemolytic anemia. METHODS: We report a case of a 65-year-old female patient with systemic lupus erythematosus and a history of splenectomy,

Germline variant analysis from a cohort of patients with severe hypertriglyceridemia in Brazil.

Hypertriglyceridemia (HTG) is a common dyslipidemia associated with an increased risk of cardiovascular disease and pancreatitis. It is well stablished that the severe cases of disease often present with an underlying genetic cause. In this study, we determined the frequency and variation spectrum of genes involved in the triglyceride metabolism in a series of Brazilian patients with severe HTG. A total of 212 patients with very high HTG, defined with fasting triglycerides (TG) ≥ 880 mg/ dL, that underwent a multi-gene panel testing were included in this research. Germline deleterious variants

Preimplantation genetic testing for Cockayne syndrome with a novel ERCC6 variant in a Chinese family.

BACKGROUND: Cockayne syndrome (CS) is a rare, multisystem, autosomal recessive disorder characterized by cachectic dwarfism, nervous system abnormalities, and premature aging. Mutations in the ERCC6 and ERCC8 genes are the predominant causes of Cockayne syndrome, with ERCC6 gene mutations present in approximately 75% of cases. METHODS: Trio-based whole-exome sequencing (trio-WES) was employed to identify potential pathogenic variants associated with CS. Preimplantation genetic testing for monogenic disorders (PGT-M) was conducted to prevent the transmission of the pathogenic variant. RESULTS

A Rare Genetic Intersection: Down Syndrome With Coexisting Spinal Muscular Atrophy.

Down syndrome (DS), characterized by trisomy of chromosome 21, and spinal muscular atrophy (SMA), an autosomal recessive neuromuscular disorder, are individually recognized distinct entities. Their co-occurrence in clinical practice is rare and has not been extensively reported. We present a case of a three-month-old, female child who presented with respiratory failure necessitating intubation. Due to typical facial features and congenital heart disease, DS was confirmed with chromosomal analysis. However, subsequent recurrent chest and bloodstream infections, failure to extubate, and laborato

Whole-exome sequencing and electrophysiological study reveal a novel loss-of-function mutation of KCNA10 in epinephrine provoked long QT syndrome with familial history of sudden cardiac death.

Congenital long QT syndrome (LQTS) is one type of inherited fatal cardiac arrhythmia that may lead to sudden cardiac death (SCD). Mutations in more than 16 genes have been reported to be associated with LQTS, whereas the genetic causes of about 20% of cases remain unknown. In the present study, we investigated a four-generation pedigree with familial history of syncope and SCD. The proband was a 33-year-old young woman who experienced 3 episodes of syncope when walking at night. The electrocardiogram revealed a markedly epinephrine-provoked prolonged QT interval (QT = 468 ms, QTc = 651 ms) but

Mutation screening in autosomal dominant congenital cataract families from North India.

Congenital cataract an opacity of the eye lens is present at birth and results in visual impairment during early childhood. If left untreated, it can lead to permanent blindness. Its prevalence is ten times higher in developing countries like India. Thus, we aimed to investigate the underlying genetic defects in three autosomal dominant congenital cataract (ADCC) families from North India. Detailed family histories were collected, pedigrees drawn followed by slit-lamp examination and lens photography. Mutation screening was performed in the candidate genes for crystallins, connexins, and membr

High Risk of Gestational Trophoblastic Neoplasia Development in Recurrent Hydatidiform Moles with NLRP7 Pathogenic Variations.

OBJECTIVE: Pathogenic variations of the NLRP7 and KHDC3L genes are responsible for familial recurrent hydatidiform moles, a rare autosomal recessive phenomenon that can lead to severe comorbidities. Little is known about the diversity of genetic defects or the natural course of disease progression among recurrent hydatidiform mole cases from distinct ethnicities. In this study, we aimed to investigate the mutation profile and pregnancy outcomes in patients with multiple molar pregnancies. MATERIAL AND METHODS: Three unrelated cases with recurrent molar pregnancies are included in this study.
