Addressing the dual challenge: Managing smoking cessation in patients with diabetes.

Smoking increases complications and mortality in people with diabetes. Quitting delivers major benefits, but physiological (weight gain, insulin sensitivity shifts) and psychological barriers make cessation harder in this group. Our review article examined strategies tailored to people with diabetes. All smokers with diabetes should receive clear, personalized advice to quit, embedded in routine care. Among the available medications, varenicline has the strongest evidence in this population. Nicotine replacement therapy and bupropion may help but have limited diabetes-specific data, so they sh

Carvedilol-loaded self-nanoemulsifying drug delivery system target diabetic nephropathy: preclinical evidence of antioxidant and antifibrotic effects.

Diabetic nephropathy (DN) is a significant complication of diabetes, leading to chronic kidney failure and end-stage renal disease. While traditional treatments exist, innovative strategies are being investigated to enhance DN management. This study examines a novel nanosized formulation of carvedilol (CVL), a drug recognized for its β-blocker and α-1-adrenoreceptor antagonist properties. We developed a self-nanoemulsifying drug delivery system (SNEDS) to assess its potential in improving DN outcomes. DN was induced in rats using streptozotocin (STZ), and the animals received oral administrati

Maternal Gut Microbiota in Gestational Diabetes Mellitus and Fetal Macrosomia: Is There an Association?

Background/Objectives: Gestational diabetes mellitus (GDM) is associated with altered maternal gut microbiota and increased risk of large-for-gestational age (LGA) births. The contribution of gut microbiota to fetal overgrowth in GDM, independent of glycemic control, remains unclear. Methods: In this pilot longitudinal study, the gut microbiota of 18 women with GDM was followed from the second (2T) to the third trimester (3T). Maternal fecal samples were analyzed by 16S rRNA gene sequencing, and associations between microbial profiles and infant birth weight were examined. In addition, these a

Glucagon-like peptide-1 receptor agonists and muscle health: potential role in sarcopenia prevention and treatment.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have emerged as a cornerstone therapy for weight loss and glycemic control in type 2 diabetes mellitus and obesity. Moreover, robust cardiometabolic benefits and favorable safety profiles have positioned GLP-1RAs at the forefront of modern obesity pharmacotherapy. However, findings from large-scale trials (eg, SUSTAIN, STEP, SURPASS) have raised concerns that a significant proportion of the weight loss achieved with GLP-1RA treatment may derive from lean body mass-particularly skeletal muscle-which could be detrimental in populations already

Optimizing Physical Activity Strategies for Older Adults with Diabetes.

The increasing prevalence of diabetes among older adults has emerged as a major socioeconomic burden. This population is highly heterogeneous, ranging from functionally independent to severely impaired individuals, making it difficult to establish standardized recommendations. Physical activity (PA) is a cornerstone of diabetes management; however, current exercise guidelines do not adequately address the wide spectrum of functional capacities observed in older adults. For those with physical limitations, relatively simple activities such as walking, breaking up sedentary time, incorporating m

Small Intestine-Targeted Long-Acting Oral Insulin Formulation Based on Engineered Milk Protein Nanoparticles.

Insulin therapies remain essential for glycemic control in diabetes mellitus, yet conventional subcutaneous injection is associated with poor patient compliance, risk of hypoglycemia, and other adverse effects. Oral insulin formulations offer a promising alternative by improving patient adherence and mimicking the endogenous insulin pathway. However, their clinical trials, mostly in enteric capsules and tablets, have yielded limited efficacy due to low insulin bioavailability. In this study, we prepared an oral insulin formulation by coencapsulating insulin and the permeation enhancer sodium c

The potential role of GLP-1 receptor agonists in the management of psoriatic disease: a scoping review.

BACKGROUND: Psoriatic disease (PsD) is a chronic systemic inflammatory condition associated with significant cardiometabolic comorbidities, including obesity, type 2 diabetes mellitus (T2DM), and cardiovascular (CV) disease. These comorbidities are interlinked via shared immunopathogenic mechanisms, notably chronic low-grade inflammation driven by Th1/Th17 cytokines such as TNF, IL-6, and IL-17. Obesity, in particular, exacerbates PsD severity and treatment resistance, underscoring the need for integrated therapeutic strategies. This scoping review investigates the biological rationale and evi

Gut‒heart axis: emerging therapies targeting trimethylamine N-oxide production.

Trimethylamine N-oxide (TMAO) has garnered considerable attention because of its role in the pathophysiology and pathogenesis of various disorders, particularly heart and kidney disease. Gut microbes produce trimethylamine (TMA) moieties from common dietary precursors, such as choline or carnitine, which are subsequently metabolized by the liver into TMAO. Circulating TMAO then exerts various effects, influencing dyslipidemia, metabolic syndrome, endothelial dysfunction, and inflammation, as well as other detrimental processes. Many existing medications have been shown to decrease TMAO levels
