The effect of cytotoxic lesions of the hippocampus on recognition memory in the rat: effects of stimulus size.

Rats with excitotoxic hippocampal lesions were trained on delayed nonmatching-to-sample (DNMS) with small goal boxes, containing complex objects, presented on a pseudo trial-unique schedule. A series of experiments then tested performance on repeated presentation of either the small object or large empty goal boxes. All rats acquired the nonmatching rule, but hippocampal-lesioned rats performed less well than controls on choice accuracy for the final 2 blocks of acquisition. In the study's main phase, the lesions impaired choice accuracy when the large empty boxes were used as stimuli. This de

Thoracoscopic surgery for pneumothorax in older patients.

BACKGROUND: Secondary pneumothorax is more common in older pneumothorax patients. Therefore, we set out to investigate the usefulness of thoracoscopic surgery in this group. METHODS: A total of 59 operations were performed on patients aged > or = 50 years, with the goal of excising only the lesions responsible for air leaks by using the minimum degree of dissection. If the patients were found to have numerous bullae, the lesions responsible were identified by pleurography. The duration of chest tube drainage, hospital stay, and recurrence were assessed. RESULTS: The air leaks stopped and all

Guided tissue regeneration update.

The ultimate goal of periodontal therapy is to restore periodontal tissues lost through disease or trauma. The most predictable way to accomplish this goal is by guided tissue regeneration (GTR). The principle of GTR is to give preference to certain cells to repopulate the wound area to form a new attachment apparatus. Clinically this is accomplished by placing a barrier over the defect thereby excluding gingival tissues from the wound during early healing. The first generation of GTR barriers were non-resorbable which implies that they have to be removed in a second surgical procedure. Resorb

Progress toward poliomyelitis eradication--Bangladesh, 1995-1997.

As a member of the World Health Assembly, in 1988 Bangladesh adopted the goal of poliomyelitis eradication by 2000. To achieve this goal, Bangladesh has implemented the following strategies recommended by the World Health Organization (WHO): 1) achieving high routine coverage with at least three doses of oral poliovirus vaccine (OPV3) among infants aged <12 months; 2) conducting National Immunization Days (NIDs) to interrupt widespread circulation of poliovirus; 3) establishing sensitive systems for surveillance of polio cases and poliovirus that rely on acute flaccid paralysis (AFP) reporting

Coil embolization of ruptured vertebral dissection in acute stage with interlocking detachable coils.

BACKGROUND: Although dissecting aneurysm of vertebral artery is known as one of the causes of subarachnoid hemorrhage (SAH) in the posterior circulation, the best surgical treatment method remains controversial. METHOD AND RESULT: This 64-year-old woman was admitted to our service with headache due to SAH caused by a ruptured vertebral dissecting aneurysm in the distal portion of the posterior inferior cerebellar artery. After confirming tolerance of parent artery occlusion by temporary balloon occlusion, both the dissection site and the proximal portion of the parent artery were occluded com

Arthroscopy assisted operative management of tibial plateau fractures.

In recent years the treatment of fractures has evolved from extensive open reduction and internal fixation to minimally invasive surgery and biologically benign internal fixation. The ultimate goal of treatment for tibial plateau fractures is to prevent the late development of degenerative osteoarthritis. Surgery is often required to restore joint congruity and alignment, to stabilize the knee, and to allow early joint motion. In most cases, these goals can be achieved through arthroscopic reduction and internal fixation. Arthroscopic reduction and internal fixation offers the advantage of dir

[Questions regarding demarcation in occupational therapy].

Work therapy (Arbeitstherapie) has become an intermediate link between medical and vocational rehabilitation. It is distinct from insurable employment primarily in that it is guided by a medical treatment plan directed at the rehabilitative needs of the patient. Delimitation problems as against insurable employment mostly occur due to the requirement that the work therapy setting should be realistic. This entails a risk of abuse. Where work therapy is provided in practice, greater note should be taken of today's degree of differentiation in vocational rehabilitation, which has made available a

Intra-abdominal sepsis: controversies and choices in management.

Contamination of the peritoneal cavity may lead to generalized peritonitis or abscess formation. Both aerobic and anaerobic bacteria participate in the infectious process. The concentration and the particular mix of aerobes and anaerobes depends on the site of perforation, with the highest concentrations and greatest predominance of anaerobes in the colon. A number of factors contribute to the risk of infection, and a formula has been devised for calculating a given patient's risk for infection following intestinal perforation secondary to penetrating abdominal trauma. The goal of therapy is t

Insulin therapy in diabetes. The importance of good glucose control.

Good blood glucose control is desirable to prevent or retard the chronic complications of diabetes mellitus. In diabetics who have a nearly normal hemoglobin A1c (glycohemoglobin) concentration and who can adjust their diet and activities to avoid hypoglycemia, one insulin injection daily may be adequate. In most insulin-dependent diabetics, however, a more complicated insulin regimen, tailored to the patient's needs, is required. The best treatment regimen is one that causes minimal disruption in the patient's life-style, costs little, yet produces good results. Glucose levels should be asses

Departmental analysis of robotic cadaver surgical training outcomes at Tottori university robotic surgery training and medical device innovation center.

Cadaver surgical training (CST) plays an essential role in surgical education and innovation by enabling hands-on practice under realistic anatomical conditions without compromising patient safety. In Japan, CST has expanded rapidly following the establishment of ethical and legal frameworks, and has contributed not only to technical training but also to anatomical discovery and surgical procedure development. However, interdepartmental perceptions of multidisciplinary robotic CST remain poorly characterized. We conducted a descriptive analysis of post-course questionnaires completed by partic

"I Was Told I Needed Both Knees Surgically Replaced, but I Walked the Camino Trail Instead." A Case Report of A 66-year-old Male with Knee Osteoarthritis.

BACKGROUND: This case report describes the lived experience of a 66-year-old male (PN) with bilateral knee osteoarthritis whose engagement with best practice conservative management resulted in a positive outcome of unanticipated scope/magnitude. PN had long-term symptomatic knee osteoarthritis and was advised that bilateral arthroplasty was required. He chose to avoid surgery, instead participating in a clinical trial evaluating the effect of embedding pain science education (PSE) into an individualized, graded exercise program (EPIPHA-KNEE trial). METHODS: The intervention involved four wee

Optimizing Approaches in Advance Care Planning in Dementia as Perceived by General Practitioners.

CONTEXT: Feasible, person-centered advance care planning (ACP) approaches for persons with dementia and their care partners are needed, and the optimal approach may differ depending on the situation. Two contrasting approaches involve a highly scripted medical order-setting approach to decide on specific treatments in advance, vs a flexible goal-eliciting, more psychosocially-oriented coping-based approach. OBJECTIVES: To distinguish situations in which either approach is preferred in dementia from the perspective of general practitioners. METHODS: We interviewed 13 practitioners participati

Leveraging sociological systems theory: integrating transformational leadership and social capital to enhance quality of care - an exploratory longitudinal study of German hospitals.

BACKGROUND: Numerous studies have examined ways to improve care quality and patient safety. Further progress could be achieved by leveraging underexplored sociological concepts. To date, transformational leadership and social capital have been examined primarily as isolated factors in healthcare research to explain variations in hospital care quality. Yet, these concepts have rarely been integrated into a broader theoretical framework. Drawing on Parsons' AGIL framework, we identified two key social prerequisites for effective healthcare organizations: goal attainment and integration. We hypot

A Treat-to-Target approach in hereditary angioedema: expert consensus from a European committee.

BACKGROUND: Hereditary angioedema (HAE) is a rare, unpredictable disease that imposes a substantial and multifaceted burden on patients' daily lives, with many not achieving the World Allergy Organization (WAO)/European Academy of Allergy and Clinical Immunology (EAACI) recommended treatment goal of normalisation of life. METHODS: In March 2025, a European expert committee convened to discuss how to effectively implement WAO/EAACI guidelines in clinical practice, identifying a Treat-to-Target (T2T) approach to support consistent adoption of guideline recommendations into routine care. Demonstration of a Training Protocol for Individuals with Tetraplegia and High Paraplegia Using a Self-Balancing Exoskeleton.

First-generation personal and home-use exoskeletal-assisted walking devices for individuals with spinal cord injury (SCI) require the use of forearm crutches or a walker and sufficient trunk control for balance and mobility, limiting their use among individuals with tetraplegia and high paraplegia. In addition, these devices typically permit only forward walking, which may restrict maneuverability in indoor environments. A self-balancing exoskeleton that provides trunk stability, enables standing and walking without an assistive walking device, permits hands-free activity, and allows forward,

How to Improve Pancreatic Cancer Network Care Using a Human-Centered Design Sprint.

Pancreatic cancer is considered a complex cancer requiring specific expertise in diagnostic workup and multimodality treatment. Often, multiple health care providers in different hospitals are involved during patient care. This fragmentation of care challenges health care providers in the network to deliver efficient, coherent, and continuous network care. We performed a human-centered design (HCD) sprint in order to find means to improve network care for patients diagnosed with pancreatic cancer. The sprint comprised 5 days with different goals: empathize, define, ideate, prototype, and test.

Diagnostic accuracy of screening questionnaires for obstructive sleep apnea in psychiatric patients.

BACKGROUND: Obstructive sleep apnea (OSA) is prevalent among psychiatric patients and can exacerbate psychiatric conditions. Screening for OSA in this population is challenging due to overlapping symptoms and adherence issues. This study evaluated the diagnostic accuracy of the STOP-Bang, BOAH, GOAL, NoSAS, and No-Apnea screening questionnaires in psychiatric patients. METHODS: In this observational cohort study, consecutive psychiatric patients admitted to a single tertiary care center between June 1, 2016 and December 31, 2022 were screened using the STOP-Bang questionnaire and additional d

The Role of Community Engagement in Successful Recruitment of Research Participants During the COVID-19 Pandemic.

BACKGROUND: Our research team was in the process of recruiting American Indian and Alaska Native (AIAN) women for a community-based intervention to prevent alcohol-exposed pregnancy when the COVID-19 pandemic began. Safety measures adopted at the tribal, state, and national level required us to rethink and revise study protocols. We followed the principles of community-based participatory research, especially community engagement. The goal of this article is to report the recommendations from local AIAN field staff and the community advisory board that enabled us to exceed our prepandemic recr

Impairment of arbitration between model-based and model-free reinforcement learning in obsessive-compulsive disorder.

INTRODUCTION: Obsessive-compulsive disorder (OCD) is characterized by an imbalance between goal-directed and habitual learning systems in behavioral control, but it is unclear whether these impairments are due to a single system abnormality of the goal-directed system or due to an impairment in a separate arbitration mechanism that selects which system controls behavior at each point in time. METHODS: A total of 30 OCD patients and 120 healthy controls performed a 2-choice, 3-stage Markov decision-making paradigm. Reinforcement learning models were used to estimate goal-directed learning (as
