About a case of missed diagnosis of a post-traumatic aneurysm in the ulnar artery. Medical-legal aspects in respect to the professional liability.

Compartment syndrome of the left hand from a late diagnosed post-traumatic ulnar artery pseudoaneurysm. We report the case of 27 years old boy with a tipping and cutting wound on his left wrist, generating an ulnar artery pseudoaneurysm, that was late diagnosed, and therefore complicated by a compartment syndrome in the wrist. Immediately after the trauma the subject went to the emergency room where the severity of the injury was undestimated; in fact, it was sutured and medicated, without further investigation. When he went to the same hospital for the second time, symptoms (pulsatile mass, r

Biomechanical strain of goldsmiths.

The work of the goldsmiths consists in the manufacture of jewelry. The piece, be it an earring, bracelet or necklace, is hand-assembled. This task requires precision, skill, kindness and patience. In this work, we make use of tools such as cuticle clippers and rounded tip, beads or precious stones and also pieces of metal. This type of activity requires a biomechanical stress of hands and wrists. In order to quantify the biomechanical stress, we performed a case study to measure the movements performed by an assembly of pieces of jewelry. As method for research, filming was done during assembl

Role of tissue factor in feto-maternal development: a xiphos.

In this review, the dual role of tissue factor (TF) in pregnancy is described. On the one hand, TF is required for embryonic and placental development in a successful pregnancy, and on the other hand, pathologic expression of TF can lead to serious pregnancy complications in humans and mice. Human studies show increased TF levels in plasma, amniotic fluid and and/or placentas of abnormal pregnancies affected by miscarriages, preterm birth, or pre-eclampsia. Interestingly, using two mouse models, we found that blood-borne TF plays a crucial role in the pathogenesis of pregnancy complications. T

Neuromas of the hand and upper extremity.

The painful neuroma is an often debilitating sequela of nerve injury about the hand. The exact pathophysiology of this condition is poorly understood. After sharp trauma to a peripheral nerve, as nerve ends try to connect with their end organs and "find" the distal nerve stump, fascicular escape and scarring can lead to the development of a painful neuroma. Painful neuromas can even be associated with blunt trauma or retraction of a nerve when the nerve is not actually divided. Green's definition of a neuroma is "the inevitable, unavoidable, and biologic response of the proximal stump after it

Voltage-gated potassium channels in human immunodeficiency virus type-1 (HIV-1)-associated neurocognitive disorders.

Human immunodeficiency virus type-1 (HIV-1)-associated dementia (HAD), a severe form of HIV-associated neurocognitive disorders (HAND), describes the cognitive impairments and behavioral disturbances which afflict many HIV-infected individuals. Although the precise mechanism leading to HAD is incompletely understood, it is commonly accepted its progression involves a critical mass of infected and activated mononuclear phagocytes (brain perivascular macrophages and microglia) releasing immune and viral products in the brain. These cellular and viral products induce neuronal dysfunction and inju

Novel surgical approach to the carpal tunnel: cadaveric feasibility study.

Carpal tunnel syndrome is the most common entrapment neuropathy dealt with by the clinician. Multiple techniques have been used to surgically treat this pathological condition and all of these approach the carpal tunnel from the palmar surface of the hand or wrist. We have developed a novel endoscopic approach to the carpal tunnel utilizing a dorsal approach that necessitates a good appreciation of the anatomy of this region. This approach was carried out in 10 hands. Through a single dorsal incision we were able to transect the flexor retinaculum in all specimens without obvious damage to neu

Acute toxic renal failure.

Acute renal failure (ARF) is a common problem in intensive care medicine. Even modest degrees of ARF not requiring dialysis treatment increase the risk of death approximately fivefold. Despite the widespread appreciation of the role of nephrotoxic drugs in their contribution to ARF, these drugs continue to have an ongoing aetiological role. Potentially nephrotoxic drugs include non-steroidal anti-inflammatory drugs, radiocontrast agents, antimicrobial and anaesthetic agents. Endogenous compounds such as myoglobin and haemoglobin may furthermore cause toxic nephropathy. Tubular injury initiated

[Intramedullary nailing in multiple trauma].

The indication and best time for intramedullary nailing of long bones in the multiple trauma patient is still controversial. While in the Anglo-American literature primary intramedullary nailing is regarded as the method of choice, differentiated management is preferred in German-speaking countries on account of reports about the pathogenesis of trauma shock. On the one hand, it has been shown that severe thoracic injury greatly influences the course after multiple trauma and, in particular, its combination with femoral fracture represents an increased risk. On the other hand, every surgical i

Advanced Percutaneous Endovascular Techniques for the Treatment of Acute Bowel Ischemia-Retrograde Endovascular Recanalization and Lithotripsy-Assisted Angioplasty: Case Report.

Background: Acute bowel ischemia that develops secondarily to thrombotic occlusion of the superior mesenteric artery is a life-threatening abdominal emergency. Although an open surgical repair is still the main treatment modality for this pathology, percutaneous endovascular revascularization is currently recognized as an alternative therapeutic option. However, in some patients, endovascular repair of the occluded superior mesenteric artery is technically very challenging. Case description: We provide technical details of percutaneous endovascular revascularization of the superior mesenteric

Free functional gracilis muscle opponensplasty for thenar reconstruction: Indications, technique, and long-term outcomes.

Thumb opposition is an essential component of human hand function, and loss of the thenar musculature creates substantial disability of the hand. Additionally, first web space contracture as a result of trauma is a substantial limitation in reconstruction of thumb opposition. Reconstruction of thenar function is most commonly performed via opponensplasty tendon transfers. Although beneficial, tendon transfers require cortical retraining and fall short of native function. These may be the only option in the case of median nerve palsy. However, at times, thenar function is lost owing to thenar m

Left-Leg Apraxia and Bilateral Incoordination of the Lower Limb After Left Anterior Cerebral Artery Infarction: A Case Report.

Callosal disconnection syndrome is characterized by impaired interhemispheric communication and motor coordination. Although its effects on upper limbs are well documented, reports on lower-limb function are limited, and their pathological mechanisms remain poorly understood. We describe a case involving a right-handed woman in her early 70s who presented with slurred speech and difficulty moving her feet. Magnetic resonance imaging revealed an atherothrombotic cerebral infarction affecting the splenium, body, and genu of the corpus callosum, as well as the medial region of the left frontal lo

Self-mutilating autocannibalistic onycophagia causing infectious compartment syndrome, osteomyelitis, and sepsis: A case report and review of the literature.

Onycophagia, or nail-biting, is common in adults and children, but self-mutilating onycophagia is almost exclusively associated with nerve injury and psychiatric comorbidities, and may lead to osteomyelitis and flexor tenosynovitis. Cases with autophagy or autocannabalistic components are extremely rare, as are cases requiring proximal amputation. Behavioral interventions are rarely successful, but the literature supports psychiatric medication to decrease the onycophagic impulse. We therefore present a 70-year-old male with history of severe onycophagia, diabetic neuropathy, and vascular dise

[Fatty Density and Air].

Both fat and air have lower attenuation than water on CT images. Excluding ruptured dermoid cysts, a majority of intracranial fats have no clinical significance. In contrast, intracranial air sometimes indicates serious conditions. If CT attenuation of the lesion is apparently lower than that of the orbital fat, it can be regarded as an air bubble. T1-weighted MRI is useful for differentiating fat from air. Air in the subarachnoid space, called pneumocephalus, is a significant sign of serious head injury. In such cases, there may be cerebrospinal fluid leakage, and the risk of meningitis. Iatr

An Update on the Genetic Polymorphism of HLA-B*27 With 213 Alleles Encompassing 160 Subtypes (and Still Counting).

PURPOSE OF REVIEW: This publication updates an earlier review on the ever increasing knowledge about genetic polymorphism of HLA-B*27 and discusses its clinical relevance. RECENT FINDINGS: As of January 1, 2017, there are 213 known alleles of HLA-B*27 at nucleotide sequence level, while at the translated protein level, there are 160 known subtypes based on one or more amino acid sequence differences. Some of these subtypes exhibit differential association with ankylosing spondylitis, and there may even be some level of hierarchy in this regard. On the other hand, HLA-B*27 has a protective eff

Ultrasound-guided supraclavicular brachial plexus block in patient with halo device.

Ultrasound guided regional blocks are on the rise, many institutes are training their staff to master this technique of regional anesthesia. Regional anesthesia in case of an emergency surgery or elective surgery can be the best choice. The case described here is an example - patient with a halo fixation device after motor vehicle accident scheduled for surgery of the extremity. The main aim of management of this case is to achieve a safe anesthesia with minimal interference of the cervical fixation. Supraclavicular brachial plexus block is a good choice for surgeries of the arm and hand and u

Evoked cavernous activity: neuroanatomic implications.

We investigated the autonomic innervation of the penis by using evoked cavernous activity (ECA). We recruited seven men with thoracic spinal cord injury (SCI) and sexual dysfunction, and six men who were scheduled to have pelvic surgery (PS), specifically non-nerve-sparing radical cystoprostatectomy. In the PS patients, ECA was performed both pre- and postoperatively. The left median nerve was electrically stimulated and ECA was recorded with two concentric electromyography needles placed into the right and left cavernous bodies. We simultaneously recorded hand and foot sympathetic skin respon

Hemiplegic shoulder pain: implications for occupational therapy treatment.

BACKGROUND: Hemiplegic shoulder pain is common after stroke causing hemiplegia. It adversely affects the recovery of arm function and independence in activities of daily living. Subluxation, abnormal tone and limited range of motion or capsular constrictions have been reported as potential causes. Other factors such as rotator cuff tears, brachial plexus injury, shoulder-hand syndrome and other pre-existing pathological conditions may also be associated with hemiplegic shoulder pain. The etiology remains unclear, but hemiplegic shoulder pain may result from a combination of the above factors.

Development of computer-based environment for simulating the voluntary upper-limb movements of persons with disability.

Upper-limb orthotic systems have been designed for restoring the upper-limb functions of individuals with disabilities resulting from spinal cord injury (SCI), stroke and muscular dystrophy. These systems employ either functional electrical stimulation or external power. It is proposed that, instead of time-consuming and complicated monitoring using sensors and motion analysis, a software simulator with both angular displacement and acceleration parameters can facilitate the design of a control strategy for an orthosis. Reaching movements of three cervical SCI subjects are used to verify the s

A comparison between control methods for implanted FES hand-grasp systems.

Implanted neuroprostheses employing functional electrical stimulation (FES) provide grasp and release to individuals with tetraplegia. This paper describes and compares three methods of controlling the stimulated hand movement: shoulder position, wrist position and myoelectric activity from the wrist extensors. Three experienced neuroprosthesis users were evaluated with each of the control methods by performing a grasp release test (GRT). A significant improvement was found between each functional electrical stimulation (FES) method and tenodesis without FES. No significant difference in overa

Reduction of angulated metacarpal fractures with a custom fracture-brace.

A thermoplastic fracture-brace was compared to closed reduction with ulnar gutter splinting in the treatment of 38 isolated fractures of the fifth metacarpal with angulation more than 40 degrees at the neck or 25 degrees in the diaphysis. Patient profiles, fracture patterns, compliance rates, and time to clinical union were similar in the two treatment groups. The two treatment methods improved fracture angulation by the same amount, to about one half of the initial angulation. At the time of splint or brace removal, there was more restriction of wrist, metacarpophalangeal, and proximal interp
