Accuracy of the fetal echocardiogram in double-outlet right ventricle.

OBJECTIVE: Double-outlet right ventricle (DORV) is a complex congenital heart defect with heterogeneous anatomy. Patients require a variety of surgical interventions, and the long-term prognosis is variable. Therefore, accurate prenatal diagnosis is important in providing parents with appropriate counseling. DESIGN: Medical records were reviewed in patients with a diagnosis of DORV who had fetal echocardiography at our institution from 1998 to 2004. Pre- and postnatal diagnoses were compared, anticipated surgical procedure was compared with surgery performed, and neonatal outcome was assessed

Ventricular inversion and tricuspid atresia (VITA complex): long survival without surgical treatment.

Five patients with ventricular inversion, corrected transposition of the great arteries, and atresia of the left-sided atrioventricular (tricuspid) valve have been observed. Three patients died in infancy, two had large interatrial communications, one with an early stage of arterial-type hypertensive pulmonary vascular disease, and the third patient had a very small interatrial communication and severe cyanosis. The other two patients survived into adulthood; both had small interatrial communications, and one had severe venous-type hypertensive pulmonary vascular disease. The longer survival o

Main systolic blood flow patterns in the left and right ventricular outflow tracts determined by Doppler echocardiography.

The main blood flow velocity patterns in the LVOT and RVOT were recorded by pulsed Doppler echocardiography in 28 normal healthy cases, in two athletes, and in 85 patients with atrial septal defects, pulmonary regurgitation, tetralogy of Fallot, aortic regurgitation, mitral stenosis, aortic stenosis, mitral regurgitation, hypertrophic cardiomyopathy, ischemic heart disease, and pulmonary hypertension. Blood flow velocities were displayed using a graphic system to form a real time sonogram, using Fast Fourier Transformation. In the normal group, the blood flow velocity was 1.69 KHz in LVOT, and

Evaluation of systolic murmurs by Doppler ultrasonography.

Non-invasive continuous and pulsed wave Doppler ultrasonography was performed in 102 consecutive patients with clinically ill defined systolic murmurs to differentiate between flow murmurs, mitral regurgitation, aortic stenosis, and ventricular septal defect, as well as to assess the severity of aortic stenosis. Diagnoses with the Doppler method were based on velocity, direction, and duration of flow signals and were subsequently verified by cardiac catheterisation in all patients. Multiple evaluations were made in 31 patients. Sensitivity and specificity were 0.87 and 0.77 in mitral regurgita

Applications of diagnostic ultrasound and radionuclides to cardiovascular diagnosis. Part II. Cardiovascular disease in the young.

Echocardiography (ECHO) and radionuclide cardiography have had a significant impact on pediatric cardiology because they have proved to be sensitive enough to permit early diagnosis of many forms of heart disease and in some cases to estimate its severity and to provide information concerning ventricular performance. An overview of the anatomic and functional information that can be obtained from these two methods will be presented first, followed by details concerning the indications for their use and their relative clinical value in various acquired and congenital heart diseases. Coarctoplasty in a case of ventricular septal defect, patent ductus arteriosus with coarctation of aorta and Eisenmenger's syndrome: Is it safe?

Endovascular Coarctoplasty (CoAP) with stenting is the treatment of choice in Coarctation of Aorta (CoA). Literature on the harms or benefits of CoAP in Eisenmenger's syndrome (ES) is scarce. There is the hypothetical possibility of an increase in right-to-left intracardiac shunting once the coarctation is relieved. We report a 17-year-old male adolescent who presented with secondary hypertension, cyanosis, and lower limb claudication. He was diagnosed as a case of ventricular septal defect (VSD), patent ductus arteriosus (PDA), and CoA with associated ES. He underwent successful balloon CoAP

Early postoperative treprostinil in for severe pulmonary arterial hypertension in a high-risk pediatric patient with complete atrioventricular septal defect: a case report.

BACKGROUND: Perioperative management of pediatric patients with congenital heart disease (CHD) and severe pulmonary arterial hypertension (PAH) remains challenging because of limited evidence-based guidelines. CASE PRESENTATION: We present the case of a 14-year-old female with a complete atrioventricular septal defect (Rastelli type A) and a single atrium with a biventricular connection. The preoperative pulmonary vascular resistance (PVR) was 6.3 Wood units after oxygen inhalation. Following a multidisciplinary evaluation, fenestrated patch closure was performed to reduce the right ventricul

Identification and pathological significance of variants in the promoter region of ACTC1 gene in congenital ventricular septal defect.

Ventricular septal defect (VSD) is one of the most common congenital heart diseases, but the role of variants at the ACTC1 gene promoter region in VSD is unclear. We investigated variants in the promoter region of ACTC1 gene in 627 subjects (309 sporadic VSD patients and 318 healthy controls) by Sanger sequencing and 6 variants including 1 novel heterozygous variant [g.5163 T > A] were identified. Of the 6 variants, 3 were found only in VSD patients. In mouse cardiomyocytes (HL-1), the transcriptional activity of the ACTC1 gene promoter was significantly changed by the variants (p < 0.05). Aortic atresia with complex aortic arch collateralization through the subclavian arteries.

A term newborn boy was diagnosed with aortic atresia and ventricular septal defect with duplicated subclavian arteries bilaterally. Computed tomography angiography demonstrated highly unusual aortic arch vasculature. A diminutive aortic arch and complex head vessel anatomy precluded standard surgical palliation with a Damus-Kaye-Stansel, arch reconstruction, and systemic to pulmonary artery shunt. Instead, the patient underwent placement of bilateral pulmonary artery bands and stenting of the patent arterial duct (PDA). At 2 months of age, the patient underwent PDA stent dilation. At 4 months

Transthoracic echocardiography and its limitations in the diagnosis of congenital supernumerary aortic valve in a Thoroughbred.

Aortic valve malformation is a common congenital abnormality reported in human medicine. The malformation is characterised by an increased or decreased number of cusps. Anatomical variations of the aortic valve that have been documented in humans include unicuspid, bicuspid, quadricuspid and quinticuspid valves. Two reports described a quadricuspid aortic valve in horses associated with either a ventricular septal defect (VSD) or tetralogy of Fallot. In this case report we describe the clinical and echocardiographic findings of a horse with a quadricuspid aortic valve as single congenital abno

Heterotaxy Syndrome with Increased Nuchal Translucency and Normal Karyotype Associated with Complex Systemic Venous Return. Ultrasound Diagnosis with Autopsy Correlation.

Background: Prenatal ultrasound (US) detection of heterotaxy syndrome can be challenging, especially in identifying cardiovascular and associated anomalies. We present a new case of heterotaxy syndrome with anomalous systemic venous return (ASVR) fully displayed at autopsy. Case report: Left heterotaxy syndrome was diagnosed in a 19 weeks' of gestation fetus with right-sided stomach. The heart showed both ventricles with left morphology, a large ventricular septal defect, persistent left superior vena cava draining into the coronary sinus, ASVR with interrupted inferior vena cava (IVC) and azy

Congenitally corrected transposition of great vessels with infundibular pulmonary stenosis with ventricular septal defect, presenting with atypical atrioventricular nodal reentrant tachycardia: a rare association.

Congenitally corrected transposition of great vessels (CCTGV) is a rare congenital heart disease (CHD) accounting for <1% of CHDs. CCTGV with infundibular pulmonary stenosis (PS) with ventricular septal defect (VSD) is part of Fallot's physiology. It is known to be associated with bradyarrhythmias like atrioventricular (AV) blocks, and acquired complete AV block occurs at a rate of 2% per year. Patients can have left-sided accessory pathways, which may cause atrioventricular reentrant tachycardia (AVRT). Tachyarrhythmias like atrioventricular nodal reentrant tachycardia (AVNRT) are very rare i

Decompressive hemicraniectomy in a 2-year-old girl with a left middle cerebral artery infarct.

INTRODUCTION: We report on the case of a 2-year 5-month-old girl with congenital heart disease who developed left middle cerebral artery occlusion and cerebral hemorrhagic infarction a day after ventricular septal defect patch closure. RESULTS: Cranial computed tomography scan revealed an acute hemorrhagic infarct over the left middle cerebral artery territory with midline shift to the right. Since medical treatment failed, decompressive hemicraniectomy with duraplasty was performed, successfully reversing herniation. Decompressive surgery allows extracranial expansion of the swollen brain an

Patterns of developmental dysfunction after surgery during infancy to correct transposition of the great arteries.

A randomized clinical trial was conducted to compare the early development of children with d-transposition of the great arteries (N = 171) repaired by an arterial switch operation that used either predominantly total circulatory arrest or predominantly continuous low-flow cardiopulmonary bypass. The children assigned to the circulatory arrest group scored lower than those assigned to low-flow bypass on the Bayley Scales of Infant Development at 1 year of age. Responses to parental questionnaires completed when the children were 2.5 years old indicated that the children in the circulatory arre

Right pulmonary artery arising from ascending aorta: report of a successfully treated case.

A nine-day-old neonate presented with signs of congestive heart failure during the first week of life. The echocardiogram showed an early bifurcating great vessel originating from the left ventricle. In addition, there was another vessel originating from the right ventricle directly connected to the descending aorta without early branching into the bilateral pulmonary arteries. A muscular ventricular septal defect (VSD) was also found. Cardiac catheterization and angiography showed that: 1) the anomalous origin of the right pulmonary artery arose from the ascending aorta; 2) a large patent duc

[Total anomalous pulmonary venous connection associated with Klippel-Feil syndrome: a case report].

Klippel-Feil syndrome is characterized by congenital fusion of the cervical vertebrae. Anomalies in other organs sometimes combine. Many kinds of congenital cardiac anomalies, most frequently ventricular septal defect, considerably associate with this syndrome. Few cases of surgical corrections of cardiac anomalies with Klippel-Feil syndrome have been reported up to today. Therefore surgical problems with this syndrome have not been solved yet, completely. We have experienced a very rare case of a 18-year-old female of total anomalous pulmonary venous connection (Darling Ia type) associated wi
