Helicobacter pylori and benign upper digestive disease.

Acute infection with Helicobacter pylori causes hypochlorhydria and gastrointestinal upset. As the infection persists, patients develop chronic antral-predominant or pangastritis. Gastric and duodenal ulcers arise from chronic mucosal inflammation and disordered acid secretion in the stomach. With successful eradication of H. pylori, non-NSAID-related gastric and duodenal ulcers heal even without long-term acid suppression. More importantly, peptic ulcers and their complications rarely recur. Clearing H. pylori infection also reduces the risk of mucosal injury in NSAID and aspirin users; the p

Analysis of antral biopsy specimens for evidence of acquired mucosa-associated lymphoid tissue in HIV1-seropositive and HIV1-negative patients.

BACKGROUND: There is conflicting evidence concerning the prevalence of Helicobacter pylori gastritis in HIV1-infected patients. Furthermore, a possible influence of immunodeficiency on the acquisition of mucosa-associated lymphoid tissue (MALT) in the antral mucosa remains to be elucidated. METHODS: Seventy-seven consecutive HIV1-infected patients (mean age, 40.2 years) were compared in a prospective study with 77 HIV1-negative age-matched patients, using immunohistochemical stainings. RESULTS: In HIV1-infected patients the prevalence of H. pylori gastritis was lower and the inflammatory rea

Region-specific genomic signatures of multidrug-resistant Helicobacter pylori isolated from East and South India.

Helicobacter pylori is a ubiquitous bacterium and contributes significantly to the burden of chronic gastritis, peptic ulcers, and gastric cancer across the world. Adaptive phenotypes and virulence factors in H. pylori are heterogeneous and dynamic. However, limited information is available about the molecular nature of antimicrobial resistance phenotypes and virulence factors of H. pylori strains circulating in India. In the present study, we analyzed the whole genome sequences of 143 H. pylori strains, of which 32 are isolated from two different regions (eastern and southern) of India. Genom

Helicobacter pylori therapy: a paradigm shift.

Helicobacter pylori (H. Pylori) is a leading cause of gastroduodenal disease, including gastric cancer. H. pylori eradication therapies and their efficacy are summarized. A number of current treatment regimens will reliably yield >90% or 95% cure rates with susceptible strains. None has proven to be superior. We show how to predict the efficacy of a regimen in any population provided one knows the prevalence of antibiotic resistance. As with other infectious diseases, therapy should always be susceptibility-based. Susceptibility testing should be demanded. We provide recommendations for empiri

Elevated carbon dioxide in exhaled breath induces false-positive DOB results during Helicobacter pylori detection with ¹³C-urea breath test using infrared spectroscopy.

BACKGROUND: The urea breath test (UBT) detects Helicobacter pylori (H. pylori) by measuring 13CO₂ in exhaled air after the ingestion of a 13C-urea metabolized by bacterial urease. However, the influence of CO₂ concentrations in exhaled breath on UBT results remains unclear. This study aimed to evaluate the effect of varying exhaled CO₂ levels on the outcomes of infrared spectroscopy-based ¹³C-UBT. METHODS: Six volunteers confirmed to be H. pylori-negative via ¹³C-UBT and stool antigen assay were enrolled. All participants completed overnight fasting and oral rinsing before specimen collection

Helicobacter pylori eradication status is associated with lower motor severity despite similar levodopa pharmacokinetics in Parkinson's disease.

BACKGROUND: Helicobacter pylori (H. pylori) infection has been associated with impaired levodopa absorption and worse motor symptoms in Parkinson's disease (PD). However, the relationships among H. pylori infection, eradication history, levodopa pharmacokinetics, and motor severity remain unclear. METHODS: We retrospectively analyzed 40 patients with PD who underwent standardized levodopa/carbidopa pharmacokinetic testing. Participants were classified as HP-positive, never infected, or prior eradication. Plasma levodopa and carbidopa concentrations were measured serially after administration

Screening and functional validation of key genes in Helicobacter Pylori-induced macrophage M1 polarization: role in migraine-associated functional dyspepsia.

BACKGROUND: Migraine, a prevalent and debilitating neurovascular disorder, frequently co-occurs with functional dyspepsia. Emerging evidence suggests that Helicobacter pylori (H. pylori) infection may contribute to both conditions via neuroimmune pathways, though the molecular basis for this association has yet to be fully elucidated. METHODS: H. pylori-related datasets from GEO were analyzed to identify differentially expressed genes (DEGs), followed by functional enrichment analyses including GO, KEGG, GSEA, and GSVA. Immune infiltration patterns were assessed, and CGRP-related hub genes we

Helicobacter pylori-Induced Inflammatory Cancer Transformation Microenvironment Drives Gastric Carcinogenesis.

BACKGROUND & AIMS: Helicobacter pylori (H pylori) eradication significantly reduces gastric cancer risk but offers limited benefit for patients with advanced atrophic gastritis. After eradication, a persistent inflammatory-cancer-transformation microenvironment (ICTM) drives gastric carcinogenesis, warranting further investigation. METHODS: We constructed a single-cell atlas of 22 gastric antral mucosae across disease stages, including normal controls, H pylori-infected non-atrophic gastritis, H pylori-infected chronic atrophic gastritis, and post-eradication chronic atrophic gastritis (strat

Early population-level impact of Helicobacter pylori eradication on gastric cancer mortality in Japan: a counterfactual analysis of short-term divergence.

BACKGROUND: Gastric cancer has historically been driven by long‑standing Helicobacter pylori infection. The nationwide expansion of H. pylori eradication therapy beginning in 2013 created a unique opportunity to evaluate its population‑level impact on gastric cancer mortality. However, short‑term mortality trends following eradication are difficult to interpret because they reflect overlapping influences of ageing, cohort replacement, and cumulative infection history. This study aimed to provide a model‑based, population‑level assessment of the early impact of eradication during the first deca

Effects of Herbal and Natural Product Interventions on Gut Microbiota and Clinical Outcomes in Patients Receiving PPI-Containing Therapy: A Systematic Review and Meta-Analysis.

Proton pump inhibitor (PPI)-containing regimens, including bismuth quadruple therapy, may perturb gut microbiota through combined exposure to acid suppression, antibiotics, bismuth, and underlying disease context. Herbal medicines and natural products have been proposed as adjunctive interventions to mitigate treatment-related microbiota perturbations; however, systematic synthesis of the clinical evidence remains limited. This systematic review and meta-analysis evaluated the effects of herbal and natural product interventions on gut microbiota and clinical outcomes in patients receiving PPI-

Systemic serological signatures and novel biomarker CST4 link CagA-positive Helicobacter pylori to broad systemic alterations.

OBJECTIVE: Helicobacter pylori (H. pylori) virulence factors, particularly CagA, determine pathogenic outcomes, yet their systemic serological impact remains poorly characterized. This study aimed to delineate systemic serological signatures associated with CagA-positive H. pylori infection, evaluate the novel biomarker CST4 as an exploratory pathophysiological indicator of host response to virulent infection, and develop predictive nomograms integrating host and bacterial factors for gastric diseases. METHODS: This retrospective study enrolled 565 participants who underwent bidirectional end

The association between Helicobacter pylori seropositivity and acne vulgaris among adolescent schoolchildren in Sudan: a case-control study.

Acne vulgaris affects an estimated 9.4% of the world's population and is the eighth most prevalent disease globally. Helicobacter pylori has been implicated in several inflammatory dermatoses, but the evidence linking it to acne rests on a small number of clinic-based studies, and no data have been reported from Africa. We examined this association in a school-based case-control study of adolescents aged 10-19 years attending public schools in Almatamah Locality, northern Sudan, between October and November 2022. Acne was diagnosed and graded by trained physicians using the Global Acne Grading

Gastric Microbiota from Populations with High Risk of Gastric Cancer in Nariño, Colombia.

Gastric cancer is a major cause of cancer mortality in Colombia, particularly in the Andean region of Nariño. Helicobacter pylori is the main etiological factor, but its oncogenic potential depends on bacterial variability, host susceptibility, and interactions with the gastric microbiota. This study evaluated the composition and diversity of the gastric microbiota in patients with preneoplastic lesions. Gastric biopsies from 30 patients (24 with non-atrophic gastritis (NAG); 6 with atrophic gastritis and intestinal metaplasia (AGIM)) were analyzed by full-length 16S rRNA gene sequencing using

Development and Evaluation of a Multi-Epitope Vaccine Based on P22 Virus-Like Particles Targeting Helicobacter pylori.

BACKGROUND: The current first-line treatment for Helicobacter pylori (H. pylori) infection is bismuth-based quadruple therapy. However, the widespread use of antibiotics has contributed to the emergence of antibiotic-resistant strains, thereby increasing the likelihood of treatment failure. This study aimed to develop a multi-epitope H. pylori vaccine, designated P22-C3, based on P22 Virus-like Particles, with the objective of reducing infection rates and preventing transmission.
