Off-the-Shelf Third-Party Virus-Specific T Cell Therapy to Treat JC Polyomavirus Infection in Hematopoietic Stem Cell Transplantation Recipients.

Progressive multifocal leukoencephalopathy (PML) is a progressive and generally fatal demyelinating neurologic disease that occurs in profoundly immunocompromised patients due to infection with the human polyomavirus JC virus (JCPyV). Treatment options are limited and are largely focused on restoring T cell immunity, and outcomes are historically poor. Control of JCPyV in the setting of an immunocompromised patient by adoptive transfer of third-party virus specific T cells (VSTs) has been described in a small number of cases. To investigate treatment response and outcomes in recipients of hema

Third dose of anti-SARS-CoV-2 vaccine for patients with cancer: Should humoral responses be monitored? A position article.

Taking into account higher risk of severe coronavirus disease 2019 or death among patients with cancer, as well as impaired immunogenicity after anti-SARS-CoV-2 vaccines, in addition to waning immunity, booster dosing appears mandatory in this patient population. This review sought to provide reasonable evidence so as to assist oncologists in their daily practice, helping them decide when an anti-SARS-Cov2 antibody (Ab) dosage should be scheduled after a full two-dose vaccination and, if necessary, propose an early third dose (D3). Such D3 could apply to non-responder patients with anti-Spike

Pathogen-Specific T Cells Beyond CMV, EBV and Adenovirus.

PURPOSE OF REVIEW: Infectious diseases contribute significantly to morbidity and mortality in recipients of allogeneic haematopoietic stem cell transplantation (aHSCT), particularly in the era of highly immunosuppressive transplant regimens and alternate donor transplants. Delayed cellular immune recovery is a major mechanism for the increased risk in these patients. Adoptive cell therapy with ex vivo manipulated pathogen-specific T cells (PSTs) is increasingly taking its place as a treatment strategy using donor-derived or third party-banked cells. RECENT FINDINGS: The majority of clinical t

Early Use of Etanercept for Graft-Versus-Host Disease After Liver Transplant: the Importance of Broad Spectrum Infective Prophylaxis.

Graft-versus-host-disease after orthotopic liver transplant is a rare and life-threatening complication. The diagnosis is challenging and usually confirmed by chimerism and skin biopsies. The most common cause of death is sepsis (60%), and broad-spectrum antibiotics and antifungal prophylaxis are strongly recommended. We present a case of a 61-year-old man with hepatocellular carcinoma and a previous history of metabolic and alcoholic cirrhosis who underwent orthotopic liver transplant. The immunosuppression regimen consisted of corticosteroids, calcineurin inhibitor, and mammalian target of r

Toxoplasmosis: A history of clinical observations.

It has been 100 years since Toxoplasma gondii was initially described in Tunis by Nicolle and Manceaux (1908) in the tissues of the gundi (Ctenodoactylus gundi) and in Brazil by Splendore (1908) in the tissues of a rabbit. Toxoplasma gondii is a ubiquitous, Apicomplexan parasite of warm-blooded animals that can cause several clinical syndromes including encephalitis, chorioretinitis, congenital infection and neonatal mortality. Fifteen years after the description of T. gondii by Nicolle and Manceaux a fatal case of toxoplasmosis in a child was reported by Janků. In 1939 Wolf, Cowen and Paige w

Successful non-surgical treatment of disseminated polymicrobial fungal infection in a patient with pancytopenia and graft-versus-host disease.

Invasive fungal infections after bone marrow transplantation have an extremely poor prognosis. Surgical excision in combination with antifungal therapy is considered necessary for treatment, especially for central nervous system (CNS) infection. We describe successful medical management with lipid complex amphotericin B (ABLC) and itraconazole, without surgical excision, of disseminated fungal infection involving the lungs and CNS in a patient with pancytopenia and graft-versus-host disease.

Disseminated Bartonella washoensis infection in a kidney transplant recipient diagnosed by 16S rRNA next-generation sequencing: A case report.

Bartonella species are increasingly recognized as causes of invasive infections in immunocompromised hosts. We report the first case in Colombia of disseminated Bartonella washoensis infection in a kidney transplant recipient. A 43-year-old woman receiving maintenance immunosuppression presented with prolonged fever, constitutional symptoms, abdominal pain, multiple splenic lesions, and vertebral osteomyelitis. Extensive microbiological, serological, and molecular investigations were initially negative. Due to persistent fever and diagnostic uncertainty, a diagnostic splenectomy was performed,

[Metagenomic analysis for central nervous system infections: clinical utility and future directions].

Encephalitis and meningitis are neurological emergencies in which delayed diagnosis may lead to severe neurological sequelae, necessitating accurate and rapid etiological identification. In recent years, metagenomic next-generation sequencing (mNGS), which enables comprehensive analysis of microbial genomes without prespecified hypotheses, has attracted increasing attention. Its clinical application in neuroinfectious diseases has contributed to improved diagnostic yield and the detection of rare pathogens. In particular, mNGS has been shown to be useful in clinically challenging situations su

Two Cases of Catheter-Related Bloodstream Infection: A Previously Unreported Complication of Long-Term Ritodrine Tocolysis.

Prolonged use of ritodrine hydrochloride (RT) for threatened preterm labor is discouraged in Western countries due to a lack of proven efficacy and severe maternal risks; however, it remains common in Japan. We report two rare cases of catheter-related bloodstream infections (CRBSI) in pregnant women undergoing long-term peripheral-line RT infusion. The first case involved Stenotrophomonas maltophilia bacteremia, a multidrug-resistant pathogen rarely reported in pregnancy. Despite limited antibiotic options, the patient recovered and delivered a healthy infant. The second case involved Achromo

Case Report: Rare Community-Acquired Pneumonia with Concurrent Methicillin-Resistant Staphylococcus aureus and Cryptococcus neoformans Infection in an Immunocompetent Young Male.

Cryptococcal pneumonia predominantly occurs in immunocompromised patients, but its incidence among immunocompetent individuals has been increasing in recent years. However, cases of co-infection with Cryptococcus neoformans and community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) in immunocompetent hosts remain exceedingly rare. This study reports on a previously healthy 30-year-old male who presented with 20-days cough and 1-day fever. Through nanopore targeted sequencing (NTS) of bronchoalveolar lavage fluid (BLAF) and traditional microbial culture, he was ultimately diag

Polyarticular Septic Arthritis in an Immunocompromised Adult: A Case Report.

Septic arthritis is an inflammatory process affecting one or more joints due to infection, most commonly of bacterial origin. Polyarticular involvement is a rare but severe form of the disease, frequently associated with preexisting joint pathology, involving an average of 3.5 joints and carrying increased morbidity and mortality. We report the case of a 61-year-old woman with rheumatoid arthritis and long-term immunosuppression who was admitted with invasive methicillin-sensitive Staphylococcus aureus infection, complicated by polyarticular septic arthritis involving five major joints (left h

Monomorphic Epitheliotropic Intestinal T-Cell Lymphoma Concomitant with Nontuberculous Mycobacteria Infection: A Case Report.

Monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL) is a rare non-Hodgkin lymphoma originating from mature T lymphocytes within the intestinal epithelium. It is a distinctly heterogeneous malignant tumor with a highly aggressive nature and poor prognosis. Nontuberculous mycobacteria (NTM) are ubiquitous and can infect immunocompromised hosts. NTM infections pose an increased risk of morbidity and mortality in patients with hematological malignancies, with a reported prevalence of 1.4% in those with lymphoma. Here we present a case of a 47-year-old patient, who experienced diarrhea f

Invasive Listeriosis in an Otherwise Immunocompetent Elderly Patient.

Listeria monocytogenes is a foodborne, intracellular Gram-positive bacillus that typically causes mild gastrointestinal illness or flu-like symptoms in healthy hosts but can lead to invasive disease, including bacteremia and central nervous system infection. Although classically associated with immunocompromised states, advanced age has emerged as an independent risk factor for invasive listeriosis. We report a case of an otherwise healthy elderly male patient who presented with nonspecific systemic symptoms and was subsequently found to have Listeria monocytogenes bacteremia and meningitis. I

An Atypical Presentation of Culture-Negative Cryptococcal Meningitis in an Immunocompetent Host With Prior Stroke: A Case Report.

Cryptococcal meningitis commonly presents with acute symptoms such as fever and signs of raised intracranial pressure and usually occurs in immunocompromised hosts. A 55-year-old woman presented with worsening cognitive decline, gait disturbance and recurrent falls. Magnetic resonance imaging (MRI) of the brain revealed a prior stroke and ventriculomegaly. A lumbar puncture (LP) showed normal opening pressure. Cerebrospinal fluid (CSF) analysis revealed pleocytosis with raised protein and reduced glucose. Cryptococcal antigen in the CSF was positive; however, Cryptococcus was not cultured. The

Case Report: Rare invasive aspergillosis with brain abscess in a non-classically immunosuppressed patient, and pooled analysis of individual patient data (2000-2024).

Intracranial aspergillosis is uncommon but often lethal, especially in classically immunocompromised hosts. We report a 71-year-old man with poorly controlled diabetes (a non-classical risk factor) who developed bilateral frontal abscesses due to Aspergillus fumigatus. After an initial craniotomy with negative cultures and galactomannan, recurrent disease was confirmed by stereotactic biopsy with next-generation sequencing (NGS). Targeted azole therapy (voriconazole, isavuconazole) and multidisciplinary care led to marked clinical and radiographic improvement. We also pooled 343 published case

Disseminated Musculoskeletal Coccidioidomycosis in an Immunocompetent Host: A Rare Case Requiring Extensive Spinal and Pelvic Surgery.

Coccidioidomycosis, also known as "Valley Fever," is an endemic fungal infection caused by Coccidioides species, typically presenting as a respiratory illness. Disseminated disease, particularly with extensive skeletal involvement, is rare and typically seen in immunocompromised individuals. We report a unique and severe case of disseminated musculoskeletal coccidioidomycosis in a previously healthy immunocompetent 33-year-old African American man, involving multiple spinal levels, pelvic bones, and the right chest wall. The patient required multiple orthopedic surgeries, including anterior an

Diagnosis of Central Nervous System Strongyloidiasis in a Patient with Advanced Human Immunodeficiency Infection with Quantitative Polymerase Chain Reaction: Improving Detection and Clinical Management.

Strongyloides stercoralis is a soil-transmitted nematode that leads to asymptomatic or mild infections in immunocompetent hosts; however, in immunocompromised individuals, it can lead to hyperinfection and disseminated strongyloidiasis that can be fatal. Central nervous system (CNS) involvement is rare but can result in severe complications, such as meningitis. Diagnosis is challenging because of the low sensitivity of conventional microscopy. Molecular techniques, such as quantitative polymerase chain reaction (qPCR), are more sensitive, especially for detecting S. stercoralis in cerebrospina

A Fatal Case of Multidrug-Resistant Pleural Nocardiosis by Nocardia otitidiscaviarum in an Immunosuppressed Patient: A Case Report and Literature Review.

Nocardiosis is known as an opportunistic infection in immunocompromised hosts. We present to you a case of pleural nocardiosis in a 38-year-old male patient who was a chronic smoker and presented with a left-sided pleural effusion. He was a known case of thrombocytopenia due to immune thrombocytopenia (ITP) and was on steroid therapy. On admission, he was found to be positive for HIV. Pleural fluid was sent to microbiology, where acid-fast staining with 1% sulfuric acid (H2SO4)showed acid-fast branching filamentous rods and cultures grew Nocardia, which was resistant to ampicillin, ceftriaxone

Disseminated Nocardia cyriacigeorgica Infection Disguised as a Metastatic Adrenal Gland Malignancy in a Healthy Patient.

Nocardia, typically recognized as an uncommon opportunistic pathogen affecting immunocompromised individuals, has also been documented in various case reports involving infections in immunocompetent hosts. Transmission occurs through inhalation or inoculation into compromised skin. Subsequently, it can lead to disseminated infection via hematogenous spread, affecting nearly any organ with a particular affinity for the central nervous system. Dissemination to the adrenal glands is extremely rare, with only a few cases reported. In this report, we present a rare case of disseminated Nocardia cyr
