Preoperative Prayer and Postoperative Recovery Quality Among Patients Undergoing Thyroidectomy in South Korea.

Cancer is a life-threatening condition often accompanied by psychological distress. Prayer is a longstanding religious and spiritual practice offering psychosocial and meaning-oriented support, yet its role as a resource in perioperative recovery remains underexplored. This non-randomized, preference-based study examined the association between preoperative prayer and postoperative recovery in patients with thyroid cancer undergoing thyroidectomy. The prayer group (n = 36) received physician-led preoperative prayer, while the non-prayer group (n = 36) served as controls. Baseline characteristi

Optimal timing of post-therapeutic [131I]NaI whole-body scintigraphy in predominantly low- and intermediate-risk differentiated thyroid cancer: a secondary analysis of prospectively acquired data and a structured literature review.

BACKGROUND: Post-therapeutic [131I]NaI whole-body scintigraphy (ptWBS) is mandatory after radioiodine therapy (RAIT), but the most suitable acquisition time remains uncertain. We compared image interpretability at early and delayed time points in patients with differentiated thyroid cancer (DTC). METHODS: This retrospective secondary analysis included prospectively acquired ptWBS data from 35 patients. Scans obtained at 6, 24, 48, 72, and 168 h after RAIT were independently ranked by three nuclear medicine physicians and evaluated with a 10-item criteria-based image-quality score. Rankings we

Clinical utility of macroscopic on-site evaluation in EUS-guided tissue acquisition for autoimmune pancreatitis: A retrospective study.

BACKGROUND AND OBJECTIVES: Pathological diagnosis of autoimmune pancreatitis (AIP) is essential for selecting the appropriate treatment strategy. Although macroscopic on-site evaluation (MOSE) can reduce the number of needle passes during EUS-guided tissue acquisition (EUS-TA) in pancreatic neoplasms, its utility in AIP remains unclarified. We investigated the utility of MOSE in diagnosing AIP using EUS-TA-derived specimens. METHODS: We conducted a retrospective study of patients with type 1 AIP who underwent EUS-TA using fine-needle biopsy needles. Macroscopic visible core (MVC) length was m

Large language model-generated clinical practice guideline for appendicitis.

BACKGROUND: Clinical practice guidelines provide important evidence-based recommendations to optimize patient care, but their development is labor-intensive and time-consuming. Large language models have shown promise in supporting academic writing and the development of systematic reviews, but their ability to assist with guideline development has not been explored. In this study, we tested the capacity of LLMs to support each stage of guideline development, using the latest SAGES guideline on the surgical management of appendicitis as a comparison. METHODS: Prompts were engineered to trigge

Assessment of Corneal Biomechanical Changes After Discontinuation of Long-Term Orthokeratology: A 2-Year Prospective Study.

PURPOSE: This study investigated the long-term changes in the corneal biomechanical properties (CBPs) during and shortly after orthokeratology (ortho-k). We further explored the associations between selected CBPs and the effectiveness of ortho-k in mitigating axial elongation in patients with myopia. METHODS: This prospective 25-month follow-up study included children ages 8 to 16 years with low to moderate myopia undergoing ortho-k treatment. They underwent 24 months of ortho-k followed by 1-month discontinuation. Their ocular biometric parameters and CBPs were monitored throughout the study

An Immuno-Fragile Profile Is Associated with Mortality Risk in Patients with Chronic Kidney Disease.

Background/Objectives: Patients with chronic kidney disease (CKD) face higher risks of infections, poor vaccine responses, and cardiovascular diseases, leading to increased morbidity and mortality due to immune dysfunction and frailty. This study aims to evaluate immune status and frailty in CKD patients across different treatments, examine the influence of frailty on immune status, and link these factors to mortality. Methods: A total of 174 participants were included (end-stage renal disease, ESRD n = 40; hemodialysis, HD n = 40; peritoneal dialysis, n = 36; kidney transplant patients, n = 4

Nutritional Supplementation for Obsessive-Compulsive Disorder: Protocol for a Systematic Review of Randomized Controlled Trials.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a chronic mental health condition, and standard treatment often results in incomplete symptom relief. Growing evidence suggests that nutritional supplements, including vitamins, minerals, and amino acids, may serve as potential adjunctive treatments for OCD by influencing neurotransmitter regulation and inflammation. OBJECTIVE: Our systematic review aims to assess the comparative efficacy of nutritional supplements on relevant clinical outcomes in individuals with OCD. METHODS: A protocol was developed and registered for this systematic revi

SAVR and TAVI comparison across the globe based on current regional registry evidence - A meta-analysis of reconstructed time-to-event data.

BACKGROUND: There is debate whether surgical aortic valve replacement (SAVR) or transcatheter implantation (TAVI) provide better results for treatment of aortic valve stenosis. While randomized clinical trials (RCTs) are considered to compare the average treatment effect of two methods in a selected patient population, registry data, although biased, reflect every day clinical practice and provide external validation of RCTs. We evaluated the impact of SAVR or TAVI on long-term survival based on local reports from all available regions in the world. METHODS: We systematically searched three d

Hepatitis C care delivery practices among buprenorphine prescribers and non-prescribers: results from a survey of Washington state primary care providers.

BACKGROUND: Hepatitis C infection (HCV) and opioid use disorder (OUD) are syndemic in the U.S., thus primary care providers (PCPs) who treat OUD by prescribing buprenorphine can play key roles to advance HCV elimination targets. We compared HCV screening and treatment among PCPs who do and do not prescribe buprenorphine in Washington (WA) State. METHODS: This study utilized a cross-sectional survey of PCPs in WA State, designed to characterize HCV care delivery practices and experiences/attitudes toward HCV. In this study, the independent variable was self-reported buprenorphine prescribing,

The impact of standard- versus reduced-dose tacrolimus exposure on clinical outcomes in adult kidney transplant recipients: A systematic review and meta-analysis.

Concerns around calcineurin inhibitor-induced nephrotoxicity after kidney transplantation have led to dose-reduction practices. However, "reduced dose" remains poorly defined, and evidence comparing outcomes for standard- versus reduced-dose tacrolimus trough concentration (C0) above 5 ng/mL is limited. We searched multiple electronic databases (Jan 1, 2000-June 30, 2024) for randomized controlled or observational studies that reported clinical outcomes (acute rejection, nephrotoxicity, graft survival, patient survival, and estimated glomerular filtration rate) directly against tacrolimus C0 a

Physician-Related Gaps in Hospice Referral for Patients with Advanced Dementia.

PURPOSE: Providing essential end-of-life care for patients with advanced dementia depends on physicians' ability to identify eligible individuals and refer them to hospice services. In practice, however, such referrals are often delayed-or not made at all. This study aimed to examine the factors associated with physicians' knowledge and attitudes toward referring patients with advanced dementia to home hospice care, and to compare characteristics of those who refer with those who do not. PATIENTS AND METHODS: This cross-sectional study assessed physicians' knowledge and attitudes regarding ho

Prognostic Nutritional Index as a Biomarker in Metastatic Hormone-Sensitive Prostate Cancer: Impact on Survival and Treatment Optimization.

OBJECTIVE: The Prognostic Nutritional Index (PNI), reflects the nutritional and immunological status of the patient and has been associated with outcomes in various cancers. In this study, the prognostic significance of PNI in metastatic hormone-sensitive prostate cancer (mHSPC) and its potential role in guiding treatment decisions between abiraterone acetate and enzalutamide is investigated. METHODS: Retrospective analysis was performed on 167 mHSPC patients treated between 2019 and 2024. PNI was calculated using the formula: 10 × serum albumin (g/dL) + 0.005 × total lymphocyte count (/mm³).

PSA density as a dynamic prognostic marker of biopsy grade progression during active surveillance.

PURPOSE: The safety and efficacy of active surveillance (AS) for men with prostate cancer (PCa) depend on accurate risk stratification at diagnosis and during follow-up. Initial PSA density (PSAD) has been associated with the risk of AS progression when measured at diagnosis, yet few studies have examined the impact of changes in PSAD during follow-up. We investigated whether serial PSAD measurements during AS were prognostic of subsequent biopsy grade progression. PATIENTS AND METHODS: We queried our institutional AS database to identify men with Grade Group 1 PCa, ≥ 2 prostate biopsies, and

Identification of PLXNC1 as a novel biomarker for consensus molecular subtype 4 in colorectal cancer.

Colorectal cancer (CRC) is a highly heterogeneous malignancy that is divided into four consensus molecular subtypes (CMSs), with CMS4 considered to be the worst type of CRC. The CMS classification has not been translated into clinical practice through the transcriptomic data; thus, the identification of a novel biomarker that can identify CRC subpopulations is imperative. Using bioinformatic analysis and immunohistochemical verification, we found that PLXNC1 mRNA and protein expression were significantly up-regulated in CRC tissues. High PLXNC1 was found to be significantly associated with agg

Three-Dimensional Transjugular Intrahepatic Portosystemic Shunt Geometry Predicts Shunt Dysfunction.

BACKGROUND: Patients with decompensated cirrhosis are at risk of portal hypertension-related complications, such as refractory ascites or variceal bleeding. Transjugular intrahepatic portosystemic shunt (TIPS) insertion is the most effective treatment to reduce portal hypertension. However, patients are at risk for TIPS dysfunction. AIMS: We aimed to investigate the prognostic value of three-dimensional (3D) TIPS geometry in predicting TIPS dysfunction. METHODS: A total of 107 patients who underwent TIPS insertion between 2014 and 2019 and received a computed tomography (CT) scan after TIPS
