Safe and unsafe urinary catheter use in long-term care homes: a human factors-informed scoping review protocol.

INTRODUCTION: Transurethral catheters are a type of urinary catheters that are commonly used in long-term care (LTC) settings to provide wound healing assistance, manage urinary retention and acute urinary obstruction. However, inappropriate and prolonged catheter use increases the risk of complications such as catheter-associated urinary tract infections (CAUTIs), blockages, leakage, trauma and discomfort, which negatively affects resident outcomes. In the LTC context, CAUTIs lead to increased morbidity, prolonged stay in hospitals and overall higher healthcare costs. Although clinical practi

Olanzapine for the Prevention and Treatment of ICU Delirium: Protocol for a Systematic Review With Meta-Analysis.

BACKGROUND: Intensive care unit (ICU) delirium is a multifactorial acute brain dysfunction associated with prolonged hospitalization, increased morbidity and mortality, and higher healthcare costs. Despite these substantial negative impacts of delirium, the optimal prevention and treatment strategies are unclear. Olanzapine, an atypical antipsychotic drug, is commonly used for delirium management. We will perform an updated systematic review and meta-analysis to assess the benefits and harms of olanzapine for the prevention and treatment of ICU delirium. METHODS: This protocol is developed in

Hemopurifier device in patients with solid tumours who have stable or progressive disease while on a treatment that includes pembrolizumab or nivolumab: protocol for an Australian safety, feasibility and dose-finding study.

BACKGROUND: Immunotherapy with anti-programmed cell death protein 1 (anti-PD-1) inhibitors has revolutionised the treatment of many solid tumours, however, only 30-40% of patients will have a lasting clinical response. Tumour-derived extracellular vesicles (EVs) have been implicated in the spread of solid tumours and resistance to these agents. A lectin-affinity plasmapheresis device called the Hemopurifier (HP) has been developed and shown to remove EVs in vitro and in patients. We hypothesise that the treatment of patients who are not improving on a regimen that includes an anti-PD-1 agent w

Mechanisms of open and hidden placebo in stroke recovery: targeting neuroplasticity insights of the placebo effect - study protocol for a controlled, parallel, randomised clinical trial.

INTRODUCTION: Stroke stands as one of the worldwide leading causes of disability, highlighting the need to develop new therapies. Although placebo effects in motor recovery have been reported to produce clinically meaningful improvements, the neural substrates underlying these effects, particularly across different modalities such as open-label and hidden placebo, remain unclear. This study aims to characterise the neurophysiological signatures of hidden and open placebo during motor-cortex stimulation in chronic stroke survivors and its correlation to motor recovery. METHODS AND ANALYSIS: Th

Efficacy and safety of narlumosbart in combination with stereotactic body radiation therapy followed by first-line chemotherapy combined with immunotherapy in advanced driver gene-negative non-small cell lung cancer patients with bone metastases: a phase II, single-arm, single-centre clinical trial protocol.

INTRODUCTION: Immunotherapy in combination with chemotherapy has been recommended as the first-line treatment of driver gene-negative advanced non-small cell lung cancer (NSCLC), but the efficacy is reduced in NSCLC patients with bone metastases due to the immunosuppressive microenvironment. Both nuclear factor kappa-B ligand (RANKL) inhibitors and stereotactic body radiation therapy (SBRT) have been shown to modulate the tumour immune microenvironment. Therefore, narlumosbart, a monoclonal antibody targeting RANKL, in combination with SBRT, may exert synergistic effects and improve efficacy o

Geographic clustering and population structures of Campylobacter jejuni and Campylobacter coli in South and Southeast Asian poultry systems.

Poultry production is rapidly expanding across South and Southeast Asia to meet rising demand for human consumption. Campylobacter jejuni and Campylobacter coli are the leading causes of human bacterial gastroenteritis worldwide, with poultry serving as a primary reservoir posing significant risks to food safety and public health. This study investigated the genetic diversity of C. jejuni and C. coli isolates from chicken sampled in farms, along with markets (live bird markets and shops) and slaughtering facilities from Bangladesh, India, Sri Lanka and Vietnam. Core-genome multilocus sequence

Impact of different degrees of social engagement interventions (DDSEIs) on cognitive performance and brain activity in community-dwelling adults with subjective cognitive decline in Nanjing, China: a randomised controlled trial protocol.

INTRODUCTION: Social isolation may have adverse effects on incidence and severity of cognitive impairment including subjective cognitive decline (SCD). An increase of environmental variety and social interactions may furthermore reduce progression from SCD to dementia. Research evaluating respective interventions is limited, particularly as regards studies comparing different degrees of social engagement interventions. We thus developed a clinical trial protocol to determine the efficacy of social interventions with different intensities in protecting cognitive function and white matter integr

Molecular Mechanisms of Carbapenem Resistance in Clinical Enterobacterales Isolates: The Role of Carbapenemases and Porin Loss.

Background and objective Carbapenem-resistant Enterobacterales (CRE) have emerged as a major global public health threat, driven by the rapid dissemination of carbapenemase genes that compromise the efficacy of last-line antimicrobial agents. Comprehensive characterization of these mechanisms is essential for informing antimicrobial stewardship and infection-control strategies. The objective of this study was to investigate the molecular mechanisms underlying carbapenem resistance among clinical Enterobacterales isolates, with particular emphasis on carbapenemase genes and outer membrane porin

Contact activation of coagulation in newly inserted central venous catheters (CAS-2): protocol for a randomised controlled study comparing four commercially available catheters.

INTRODUCTION: Central venous catheters (CVCs) are essential in modern healthcare. Insertion of a CVC promotes a hypercoagulable environment, in part through activation of the intrinsic coagulation pathway. We have previously shown that blood obtained from newly inserted CVCs demonstrates immediate coagulation activation, with differences between samples collected directly after insertion and after flushing of the catheter. The aim of this randomised trial is to compare early coagulation activation between four commonly used CVCs and to assess whether the different CVCs demonstrate different de

A systematic review of social media use among rural US adolescents and associated health outcomes.

BACKGROUND: Understanding social media use among rural adolescents is important because they are a geographically and socially isolated population which may impact how they use social media and affect the impacts of use. The goal of this study was to systematically review and evaluate the literature on rural US adolescent social media use. Specifically, what is known about their social media use, amount and type of use, and the associations between social media use and health outcomes. METHODS: A systematic search was conducted on seven databases: PubMed, CINAHL, SCOPUS, PsycINFO (ProQuest), W

Injury prevention content in social media posts by Children's hospitals: a missed opportunity for education and advocacy.

BACKGROUND: Preventable injuries are the leading cause of pediatric death. Most healthcare organizations and the general public engage in social media (SoMe) to disseminate and consume health-related information. It is unknown how frequently pediatric hospitals leverage their SoMe platforms to educate on injury prevention (IP) topics. We sought to better characterize SoMe messaging and IP content by children's hospitals. METHODS: This was a retrospective cross-sectional study of US children's hospitals' primary SoMe Facebook, Twitter/X, and Instagram accounts. Included hospitals were associat

Revision and Expansion of the International Spinal Cord Injury Urinary Tract Infection Basic Data Set.

BACKGROUND: International Spinal Cord Injury (SCI) Data Sets have been developed and disseminated worldwide since 2006 to establish minimum standards for clinical data collection. Basic data sets (BDSs) are intended to ensure a uniform minimum level of information across clinical settings and research projects. In 2013, an International SCI Urinary Tract Infection (UTI) Basic Data Set was published. This BDS defined UTI using the 1992 US National Institute on Disability and Rehabilitation Research criteria: "new onset of symptoms plus bacteriuria, leukocyturia, and positive urine culture." Sin

Electroacupuncture Versus Auricular Acupressure Versus WaitliSt Control for Endocrine Therapy-Related Joint Pain in Patients With Breast Cancer (EASE): Study Protocol for a Pragmatic Multicenter Randomised Controlled Trial.

BackgroundThe incidence of breast cancer has been steadily increasing in recent years. Endocrine therapy, as a first-line treatment, is typically required for 5 to 10 years. However, joint pain is one of the most common adverse effects, affecting 50%-70% of patients and significantly compromising treatment adherence and quality of life. Current management strategies have limited efficacy. Electroacupuncture has been shown to alleviate joint pain intensity and improve related symptoms, but the need for frequent hospital visits imposes a logistical burden on patients and their families, which ma

A Mediation Model of Medication Literacy and Adherence in Type 2 Diabetes During Hospital-Home Transition.

PURPOSE: The hospital-home transition is a high-risk period for medication management in patients with type 2 diabetes mellitus (T2DM). While bivariate relationships among medication literacy, beliefs, self-efficacy, and adherence have been explored, the integrated mediating mechanisms during this phase remain unclear. This study examined these interrelationships, focusing on the serial mediation beliefs and self-efficacy between literacy and adherence. PATIENTS AND METHODS: A cross-sectional study enrolled 315 T2DM patients at discharge from a tertiary hospital in Dongguan, China, between Au

Application of point-of-care ultrasound-guided tip navigation combined with visualised directional high-frequency linear array probe compression to improve the success rate of umbilical venous catheterisation in critical neonates: protocol of a multicentre randomised controlled trial in neonatal units.

INTRODUCTION: Conventional umbilical venous catheterisation (UVC) relies on 'blind' insertion without ultrasound guidance, resulting in low success and high complication rates. While point-of-care ultrasound (POCUS)-based tip navigation and location has improved this scenario by enabling real-time visualisation of the catheter tip during UVC, challenges remain when the catheter is inadvertently inserted into an incorrect vessel. Selecting effective intervention methods to correct the catheter direction has become a key research focus. This study aims to evaluate the safety and efficacy of a no
