A prospective case-control study of risk factors and perinatal outcomes associated with reduced fetal movements.

BACKGROUND: Maternal perception of reduced fetal movements is a frequent reason for referral to maternity services and may signal risk of adverse outcomes, particularly stillbirth. AIM: To identify contemporary maternal and pregnancy-related risk factors for RFM and to examine associations between RFM and perinatal outcomes. METHODS: A prospective case-control study in a large urban maternity hospital in Ireland was conducted. Women with singleton pregnancies with RFM at ≥24 weeks' gestation between 1 January and 30 September 2020 (cases) were compared with randomly selected women without RF

Pregnancy and glycemic outcomes with hybrid closed-loop insulin delivery versus sensor-augmented pump therapy among women with type 1 diabetes.

OBJECTIVE: Pregnant women with type 1 diabetes (T1D) increasingly utilize hybrid closed-loop (HCL) pumps, but pregnancy-specific algorithms in many countries including the United States are not available. We compared pregnancy and glycemic outcomes between those intending to use off-label automated HCL versus manual sensor-augmented pump therapy (SAPT). METHODS: We conducted a single-center retrospective study from 2022 to 2025. The exposure was intent to use HCL using non-pregnancy algorithms versus SAPT (reference) at the start of pregnancy. Outcomes included pregnancy outcomes (cesarean de

Assessment of Antenatal and Postnatal Prognostic Indicators in the Outcome of Neonatal Congenital Diaphragmatic Hernia: A Prospective Observational Study.

CONTEXT: Despite advances in neonatal intensive care, surgical methods, and anesthesia, congenital diaphragmatic hernia (CDH) is still associated with significant mortality. Predicting which babies will have poorer outcomes is essential to identify the high-risk babies and to give targeted care and accurate prognosis to the parents, especially in a resource crunch set-up. AIMS: The aim of this study is to evaluate the antenatal and postnatal prognostic factors in neonatal CDH that can be used to predict the outcome. SETTINGS AND DESIGN: This was a prospective observational study in a tertiar

Multilocus pathogenic variants in MCM4, RYR1, and G6PD identified by trio-based whole-exome sequencing in a neonate with multisystem symptoms: a case report.

BACKGROUND: Multilocus pathogenic variants are increasingly recognized in neonates with complex phenotypes and have important implications for diagnosis and clinical management. Reporting such cases helps expand the phenotypic spectrum and improve clinical understanding of blended genetic disorders. CASE DESCRIPTION: We describe a rare case of a male neonate born at 38+5 weeks of gestation with a birth weight of 1.73 kg, consistent with small-for-gestational-age status. The infant presented with perinatal asphyxia requiring 37 minutes of resuscitation, followed by hypotonia, poor feeding, a w

Klippel-Feil syndrome at neonatal age presentation: rare case report.

BACKGROUND: Klippel-Feil syndrome (KFS) is a rare, heterogeneous, congenital disorder of the cervical spine due to failure of segmentation during early gestation. We will present a neonate with KFS who had a classic triad. CASE PRESENTATION: A 5-month-old female infant, now in regular orthopedic and pediatric follow-up, was admitted to the neonatal intensive care unit (NICU) immediately after delivery (at the age of 1 hour) with fast breathing and low oxygen saturation. She had a short neck, low hairline, and severely limited neck mobility. A neck X-ray revealed the cervical spine as a single

Diuretic drug utilization in neonates: a French prescription database analysis.

Background: The use of diuretics is extremely common in infants cared for in neonatal wards, despite the lack of proven efficacy for many conditions. The main objective of this study was to assess the rate of diuretics exposure in a multicenter French cohort. The secondary objectives were to describe the evolution of this exposure over time, the indications, the prescription practices, and the exposure rates among centers. Methods: An observational study was conducted in 40 Level 3 French neonatal intensive care units using the same computerized order-entry system. Neonates hospitalized betwee

Study of pain response in neonates during venipuncture with a view to analyse utility of topical anaesthetic agent for alleviating pain.

BACKGROUND: Neonates being nonverbal are unable to express their pain leading to underestimation of their pain and hence insufficient pain relief. Neonatal pain is assessed by pain scales based on the behavioural and physiological changes that occur in response to painful stimuli. This cross sectional study was conducted at a tertiary care centre using Premature Infant Pain Profile (PIPP) score with 4% lidocaine as local anaesthetic agent to produce surface anaesthesia of skin prior to intravenous cannulation. METHODS: Sample size was collected by simple randomisation method. Our study groups

Neonatal outcomes of prenatally diagnosed congenital pulmonary malformations.

BACKGROUND AND OBJECTIVE: Congenital pulmonary malformations (CPM) are mostly recognized on prenatal ultrasound scans. In a minority of cases, they may impair breathing at birth. The factors predictive of neonatal respiratory distress are not well defined, but an understanding of these factors is essential for decisions concerning the need for the delivery to take place in a tertiary care center. The aim of this study was to identify potential predictors of respiratory distress in neonates with CPM. METHODS: We selected cases of prenatal diagnosis of hyperechoic and/or cystic lung lesions fro

Late onset neonatal acute kidney injury: results from the AWAKEN Study.

BACKGROUND: Most studies of neonatal acute kidney injury (AKI) have focused on the first week following birth. Here, we determined the outcomes and risk factors for late AKI (>7d). METHODS: The international AWAKEN study examined AKI in neonates admitted to an intensive care unit. Late AKI was defined as occurring >7 days after birth according to the KDIGO criteria. Models were constructed to assess the association between late AKI and death or length of stay. Unadjusted and adjusted odds for late AKI were calculated for each perinatal factor. RESULTS: Late AKI occurred in 202/2152 (9%) of e

Should Dual Pulse Oximetry Screening Be Used in Neonates? The Insights from the CHOOSE Study.

BACKGROUND: Critical congenital heart disease (CCHD) is a leading cause of neonatal morbidity and mortality. Early detection is crucial to prevent life-threatening complications. Pulse oximetry screening (POS) is a noninvasive method to identify hypoxemia indicative of CCHD, but the optimal timing for screening remains debated. AIM: To evaluate the diagnostic accuracy of dual POS performed at 24-36 h and 48-72 h of life for early detection of CCHD in neonates. METHODS: This prospective observational study was conducted over 20 months at a tertiary care neonatal intensive care unit (NICU) in

Car seat safety for preterm neonates: implementation and testing parameters of the infant car seat challenge.

BACKGROUND: The American Academy of Pediatrics (AAP) recommends that all preterm neonates undergo a period of observation in a car safety seat before discharge, known as the Infant Car Seat Challenge (ICSC), to monitor for respiratory immaturity and the risk of adverse cardiopulmonary events in the upright position. However, no universal guidelines exist to define appropriate cutoffs for failure of the ICSC. We sought to evaluate regional neonatal intensive care unit (NICU) implementation of the ICSC and to evaluate variation in failure criteria. METHODS: We contacted level II and III nurseri

Group versus conventional antenatal care for women.

BACKGROUND: Antenatal care is one of the key preventive health services used around the world. In most Western countries, antenatal care traditionally involves a schedule of one-to-one visits with a care provider. A different way of providing antenatal care involves use of a group model. OBJECTIVES: 1. To compare the effects of group antenatal care versus conventional antenatal care on psychosocial, physiological, labour and birth outcomes for women and their babies.2. To compare the effects of group antenatal care versus conventional antenatal care on care provider satisfaction. SEARCH METH
