The hemodynamic effects of sacubitril/valsartan in the first 24 h of initiation in pediatric cardiac intensive care unit patients: insights using high-fidelity data.

Sacubitril/valsartan is increasingly used in pediatric cardiac patients, though its acute physiologic effects in critically ill children remain incompletely characterized. We sought to define the early hemodynamic and tissue-level response to sacubitril/valsartan using high-fidelity physiologic data. This was a retrospective study of children in a cardiac intensive care unit with continuous physiologic monitoring. High-resolution data were temporally aligned to the time of sacubitril/valsartan initiation. Locally weighted regression was used to characterize percent change in physiologic variab

Real-world experience using multiplex polymerase chain reaction in intensive care unit patients with hospital-acquired and ventilator-associated pneumonia in South Korea.

BACKGROUND: Hospital-acquired pneumonia (HAP) and ventilator-associated pneumonia (VAP) are common among critically ill patients, leading to increased morbidity and mortality rates. Conventional culture-based diagnostics require 48-72 hours, which delays pathogen identification and prolongs the use of broad-spectrum antibiotics. Multiplex polymerase chain reaction (mPCR) enables the rapid detection of pathogens and resistance genes, but its effects on real-world antibiotic decision-making remain unclear. METHODS: This retrospective study included patients in the intensive care unit who were d

Analysis of the knowledge, attitudes, and practices of intensive care unit staff regarding Candida auris: a mixed-methods study in a private general hospital in Hanoi, Vietnam.

BACKGROUND: Candida auris, recently renamed Candidozyma auris, is an invasive fungal pathogen that is on the priority list on the World Health Organization (WHO) for future research. Infection poses major diagnostic and treatment challenges due to its phenotypic similarity to other Candida spp. and multiple antifungal resistance. Despite its emergence in Vietnam, the level of understanding among healthcare professionals is unknown. This study explores the knowledge, attitudes, and practices toward C. auris of registered physicians and nurses in a Vietnamese hospital intensive care unit (ICU).

Alternating pressure air mattresses in the intensive care unit as a cost-effective strategy for preventing stage III-IV pressure injuries: a retrospective cohort study.

BACKGROUND: Pressure Injuries (PIs) are frequent, preventable complications in Intensive Care Units (ICUs), leading to morbidity, mortality, and increased healthcare costs. Alternating Pressure Air Mattresses (APAMs) have been proposed to reduce PI incidence, but real-world data remain scarce. OBJECTIVES: To assess the effectiveness and cost-efficiency of APAMs in preventing PIs among critically ill patients. METHODS: This retrospective cohort study was conducted in a 10-bed tertiary ICU, comparing patients before and after APAM implementation. Multivariate logistic regression identified ind…

Volume removal intolerance during net ultrafiltration in patients with acute kidney injury in the intensive care unit: protocol for a prospective multicentre observational study (VINKO).

INTRODUCTION: Fluid overload in critically ill patients with acute kidney injury (AKI) is independently associated with increased mortality, prolonged mechanical ventilation and impaired renal recovery. Net ultrafiltration (UFNET) during continuous renal replacement therapy (CRRT) is a cornerstone de-resuscitation strategy; however, tolerance to fluid removal is highly variable and difficult to predict. Ultrafiltration intolerance, encompassing haemodynamic instability, peripheral perfusion deterioration and tissue hypoperfusion during UFNET, lacks a standardised definition and has not been pr

Determinants of intensive care unit-acquired acute brain injury in acute respiratory distress syndrome: a retrospective cohort study of ventilator settings and patient factors.

BACKGROUND: Acute respiratory distress syndrome (ARDS) is associated with neurological complications, but whether mechanical ventilation parameters or patient factors predominantly contribute to intensive care unit (ICU)-acquired acute brain injury (ABI) remains unclear. We aimed to determine whether ICU-acquired ABI in ARDS is independently associated with positive end-expiratory pressure (PEEP) and driving pressure delivered early after intubation or, instead, with patient-level factors. METHODS: We conducted an exploratory, single-center retrospective cohort study of mechanically ventilate

Occurrence of antimicrobial-resistant, extended-spectrum beta-lactamase and carbapenemase-producing bacterial isolates from intensive care unit environment in two hospitals of Ethiopia.

BACKGROUND: Intensive care unit environments are a critical reservoir of antimicrobial-resistant bacterial species and facilitate the transmission of nosocomial infections. This unit harbors multidrug-resistant organisms, thereby complicating the management of infectious diseases within clinical settings, particularly in resource-limited countries. This study, therefore, aimed to determine the occurrence and antimicrobial resistance profiles of bacteria, including Extended-spectrum beta-lactamase- and Carbapenemase-producing isolates, in the intensive care units of two hospitals in Addis Ababa…

Doxycycline versus azithromycin as combination therapy in the empirical treatment of community-acquired pneumonia in an intensive care unit - A retrospective propensity score-matched cohort study.

INTRODUCTION: Community-acquired pneumonia constitutes a significant burden to healthcare systems. While international guidelines recommend beta-lactam plus macrolide in treating severe community-acquired pneumonia, doxycycline instead of macrolide provides antimicrobial activity to macrolide-resistant organisms that are highly prevalent in East Asia and avoids its side effects. Yet, doxycycline's immunomodulatory effect is not as well studied. We investigated the comparative efficacy of doxycycline with azithromycin in the treatment of severe community-acquired pneumonia. METHODS: From 1 Jan

Intensive Care Unit Liberation After Complete Left Anterior Descending Artery Occlusion: Unexpected Neurologic Recovery After a 53-Minute Cardiac Arrest.

Out-of-hospital cardiac arrest (OHCA) is associated with low survival and neurologic recovery rates, especially when return of spontaneous circulation exceeds 45 minutes. Additionally, neurologic prognostic indicators, such as absent brainstem reflexes and presence of alpha coma electroencephalogram (EEG) patterns, are typically associated with poor outcomes. This report outlines the case of a 48-year-old woman with a history of hypertension, dyslipidemia, and tobacco use who suffered an OHCA due to an acute left anterior descending artery occlusion. She experienced a total downtime of 53 minu
