[165 bilateral atherosclerotic carotid occlusive lesions. Surgical and secondary risk].

Operative treatment was applied in 139 patients with hemodynamically significant bilateral stenosis of the carotid arteries due to atherosclerosis. Three groups could be distinguished: group I--stenosis greater than 75% on one side and between 50 and 75% on the other side; group II--stenosis greater than 75% on both sides; group III--obstruction on one side and stenosis greater than 50% on the other side. The risk of clamping did not appear abnormally high, while the peroperative neurologic risk was slightly enhanced due to the possibility of accidents in the region which lies contralateral to

Prevention and treatment of complications and sequelae of tracheal resection anastomosis.

Deaths following tracheal resection anastomosis for tumoral or iatrogenic disease have become fairly rare (four deaths in 122 cases in our experience). Complications can usually be prevented or treated successfully. The most common of these were: infectious lesions of the operated area (6 cases), infection of the tracheal anastomosis with subsequent disruption (1 case), erosion of the innominate artery (no case), granulomatous lesions and recurrence of stenosis (3 cases). The risk of complications can be greatly reduced by meticulous preparation, both anti-infectious and anti-inflammatory, whi

Ureteric complications following treatment of gynaecological cancer.

The incidence, development and diagnosis of ureteric injuries secondary to radical operative or radiological treatment of uterine malignancy is described. Special attention is paid to the importance of early detection and management. Immediate peroperative repair of surgical ureteric damage gives most favourable results. Also in postoperatively diagnosed ureteric lesions the definitive treatment has to be carried out without delay. Ureteroneocystostomy, psoas-hitch procedure or transureteroureterostomy are the most preferable methods. In the late ureteric strictures secondary to radiotherapy t

Diagnostic Accuracy and Clinical Outcomes of the Intraoperative Intelligent Knife (REIMS) in Adult Oncologic Surgery: A Systematic Review.

Rapid intraoperative distinction between malignant and non-malignant tissue may improve resection completeness and reduce reoperation in cancer surgery. The intelligent knife (iKnife) integrates rapid evaporative ionisation mass spectrometry with electrosurgery to provide real-time tissue classification during surgery. This systematic review assessed the diagnostic accuracy and reported clinical outcomes of intraoperative iKnife use in adults undergoing oncological surgery. The review was prospectively registered in PROSPERO (CRD420251049162) and written according to PRISMA 2020 and PRISMA-DTA

Effectiveness of free pericardial fat pad reinforcement for air leak control in pulmonary lobectomy.

OBJECTIVE: Intraoperative management of air leak is essential for favorable recovery after lung cancer resection. We present our approach using a free pericardial fat pad (FPFP) for injured lung parenchyma and analyze the postoperative course and radiological changes over time to evaluate the usefulness of this method. METHODS: From January 2020 to December 2023, 345 pulmonary lobectomies were performed for primary lung cancer. Postoperative course was compared between patients who received an FPFP for intraoperative air leak management (FPFP group, n = 63) and those managed without an FPFP (

Transcatheter aortic valve implantation-related acute ascending aortic dissection diagnosed intraoperatively using transoesophageal echocardiography: a case series.

BACKGROUND: Transcatheter aortic valve implantation (TAVI) has been widely adopted as a less invasive treatment for aortic stenosis (AS). Although rare, acute ascending aortic dissection associated with TAVI is a life-threatening complication that necessitates prompt intraoperative diagnosis. This case series reports two cases of acute ascending aortic dissection occurring during TAVI performed under general anaesthesia. CASE SUMMARY: Cases 1 and 2 involved an 87-year-old woman and a 90-year-old man, respectively, both of whom underwent transfemoral TAVI for severe AS. In both cases, procedur

Clinical significance of postoperative cardiothoracic ratio for postoperative pleural effusion following hepatectomy.

BACKGROUND: Although several preoperative risk factors for postoperative pleural effusion (POPE) after hepatectomy have been reported, few postoperative predictors have been investigated. We aimed to examine risk factors for clinically relevant POPE (CR-POPE) and whether cardiothoracic ratio (CTR) could be a predictive factor. METHODS: Participants in this retrospective investigation comprised 382 patients who underwent hepatectomy between January 2012 and December 2021. Perioperative characteristics that were considered potential risk factors for CR-POPE were evaluated. CR-POPE was defined a

The capsular vein as a novel surgical landmark for safe access to the temporomandibular joint: a retrospective cohort study.

BACKGROUND: Temporomandibular joint (TMJ) surgery carries a risk of facial nerve injury and intraoperative bleeding, especially in patients with anatomical distortion due to ankylosis or inflammation. This study introduces a novel anatomic landmark-the capsular vein-and evaluates a preauricular approach utilizing this vein to improve surgical safety. METHODS: A retrospective cohort study was performed on 104 patients (109 TMJs) who underwent TMJ surgery between 2014 and 2022. During the approach, a vein consistently found at the superior aspect of the glenoid fossa (designated the capsular ve

Pitfalls of Intraoperative Parathyroid Hormone Monitoring in Achieving Complete Surgical Resection of Ectopic Mediastinal Parathyroid Adenoma: A Case Report and Literature Review.

Ectopic mediastinal parathyroid adenoma is rare and is generally managed by surgical resection as a definitive treatment. Intraoperative parathyroid hormone (ioPTH) monitoring is valuable for ensuring the complete removal of a target lesion. However, there is no consensus criteria regarding the utilization of ioPTH for complete resection in patients with ectopic mediastinal parathyroid adenomas. A 65-year-old woman presented with asymptomatic hypercalcemia, and was subsequently diagnosed as having hyperparathyroidism. Radiological imaging displayed a solid mediastinal tumor, suspected to be ec

Modified Nasoseptal Rescue Flap Technique for Pituitary Adenoma Resection via Endoscopic Endonasal Approach.

Nasoseptal rescue flap (NSRF), which preserves the pedicle of the flap and is harvested as a nasoseptal flap (NSF) when intraoperative leakage of cerebrospinal fluid (CSF) occurs, is an alternative strategy for skull base reconstruction in patients with pituitary adenoma resection via an endoscopic endonasal approach. However, in practice, the original NSRF technique cannot meet the needs during operation. Therefore, the authors aimed to describe a modified NSRF technique for the resection of pituitary adenoma via endoscopic endonasal approach and to examine its utility and outcomes. The autho

A novel method for intraoperative osseomechanical strength measurements: a biomechanical ex vivo evaluation on proximal femora.

INTRODUCTION: The increasing number of geriatric traumatology cases has intensified the need to reliably and objectively evaluate local bone quality, the latter poses a decisive factor for the choice of an optimal approach to treat osteoporotic fractures. Osteodensitometry imaging techniques are not routinely available in acute operative settings, nor do they provide objective information on local bone properties specifically needed for the prognosis of implant stability. MATERIALS AND METHODS: This study sought to verify ex vivo the feasibility and sensitivity of a novel method for the deter

Effect of preoperative balloon dilation on treatment outcomes of laparoscopic Heller-Dor surgery for achalasia: a propensity score matched study.

PURPOSE: There is some debate about whether preoperative balloon dilation influences the outcomes of laparoscopic Heller-Dor surgery (LHD), with no consensus opinion as yet. Thus, we investigated if preoperative dilation influences the treatment outcomes of LHD for achalasia. METHODS: The subjects of this study were 526 patients with achalasia who underwent LHD as an initial treatment between August 1994 and February 2017. The patients were roughly classified by the status of preoperative balloon dilation and matched with propensity scores for age, sex, BMI, morphologic type, and maximum esop
