Forecasting ICU Acute Kidney Injury with Actionable Lead Time Using Interpretable Machine Learning: Development and Multi-Center Validation.

Background: Acute kidney injury (AKI) is common in intensive care and is frequently recognized late. We aimed to develop an interpretable, dynamic early warning score (EWS) to predict incident AKI within the next 24 h in ICU adult patients and to test its transportability temporally and geographically. Methods: We performed a retrospective cohort multi-center study using hospitalized ICU patients from MIMIC-IV and eICU-CRD databases. The outcome was incident AKI (KDIGO stage 1-3). Gradient-boosted trees (XGBoost) were trained with 10-fold cross-validation. Predictors were prespecified from the

Novel variants of the KLF1 gene associated with the In(Lu) phenotype lead to alterations in protein structure.

BACKGROUND: The rare erythrocyte Lutheran inhibitor (In(Lu)) blood group phenotype is characterised as a Lumod phenotype and primarily caused by heterozygous variants in the erythroid-specific Krüeppel-like factor 1 (KLF1) gene. OBJECTIVES: This study aimed to analyse the genetic background of the In(Lu) phenotype in the Chinese population. METHODS: Four apparent Lunull samples were identified among 3466 Chinese blood donors using flow cytometry. These samples were typed for Lua and Lub antigens using the gel card method. Targeted next-generation sequencing (NGS) was performed to identify po

Does more generous health insurance reimbursement lead to moral hazard? Evidence from large-scale administrative data in China.

This paper exploits the discrete increase in reimbursement rates at the statutory retirement age as a quasi-natural experiment to examine ex post moral hazard in inpatient care in China. Using nationally representative administrative data covering more than 450,000 hospitalization records across 29 provinces, we implement a cohort difference-in-differences design to identify the causal effect of insurance generosity on healthcare utilization. We find that a 5-percentage-point increase in reimbursement rates increases length of stay by 1.73 days and total inpatient expenditures by RMB 709.98. T

Speciation-dependent bioaccessibility of lead in e-waste dismantling soils: Synchrotron spectroscopy reveals molecular controls across physiological compartments.

Lead (Pb) pollution is prominent in electronic waste (e-waste) disposal sites. While Pb bioaccessibility depends critically on its molecular speciation, the quantitative relationship between Pb speciation and bioaccessibility across different physiological compartments remains poorly understood. This study characterized soil Pb speciation and bioaccessibility in soil samples from an abandoned e-waste dismantling site through integrated assessment of synchrotron X-ray absorption near-edge structure (XANES) spectroscopy, compartment-specific bioaccessibility testing, sequential chemical extracti

Pacemaker Lead-Associated Innominate Vein Thrombosis Despite Non-vitamin K Oral Anticoagulant (NOAC) Therapy: An Incidental CT Finding.

Venous thrombosis occurring despite therapeutic non-vitamin K oral anticoagulant (NOAC) therapy represents a diagnostic and management challenge, particularly in patients with indwelling cardiac implantable electronic devices. Pacemaker-associated central venous thrombosis is a recognized but often underdiagnosed finding due to its frequently asymptomatic presentation. We report the case of an older adult with a permanent pacemaker implanted approximately 10 months earlier who was undergoing evaluation for unexplained weight loss, in whom cross-sectional imaging incidentally revealed left inno

High-performance photoelectrochemical biosensor with Bi2S3/RuO2 heterojunction cascade multi-tetrahedral DNA nanonet for ultrasensitive lead ions detection.

Herein, a novel signal-off photoelectrochemical (PEC) biosensor was rationally constructed by integrating a Bi2S3/RuO2 heterojunction with the multiple tetrahedral DNA nanonet (mTDN) for ultrasensitive and accurate lead ions detection. This photoactive Bi2S3/RuO2 composite was synthesized through a simple ultrasonic-assisted mixing method, in which the RuO2 not only functioned as an efficient electron conductor and a protective layer for Bi2S3, but also formed a typical p-n heterojunction with Bi2S3, effectively promoting the separation of photogenerated electron-hole pairs for achieving an ex

Early-life Co-exposure to fluoride and lead modifies the tissue distribution of essential and toxic trace elements.

Exposure to toxic metals such as lead (Pb) during early development is known to induce long-lasting physiological and neurological effects. However, little is known about how co-exposure to fluoride (F)-a common environmental and dietary element-modulates Pb toxicokinetics and the homeostasis of other trace elements in soft and mineralized tissues. This study aimed to evaluate the effects of early-life exposure to Pb (30 µg/L) and/or F (50 µg/L) on the distribution of nine trace elements (Al, As, Ba, Cd, Cu, Mn, Pb, Sr, and Zn) in soft organs (brain, heart, kidney, liver) and bone of Wistar ra

Ventricular lead positioning within the left conduction system in permanent cardiac pacing: a systematic review and meta-analysis.

BACKGROUND: Left bundle branch pacing (LBBP) and left bundle fascicular pacing (LBFP) are conduction system pacing techniques that preserve physiological ventricular activation. Whether proximal or distal capture yields superior electrical or echocardiographic outcomes is uncertain. OBJECTIVE: To systematically evaluate and compare LBFP versus left bundle branch trunk pacing (LBBP) in terms of electrical synchrony and echocardiographic characteristics in patients requiring permanent cardiac pacing. METHODS: We searched PubMed, Embase, and Cochrane through May 21, 2025, for randomized and obser

Computational strategies for unraveling insights from known inhibitors for further lead optimization: A case study on Celecoxib analogues.

Inhibition of Cyclooxygenase-2 (COX-2) represents a well-established and promising strategy in the development of anti-inflammatory drugs, given its pivotal role in mediating inflammation. Selective inhibition of COX-2 over COX-1 is critical for reducing the gastrointestinal side effects commonly associated with traditional nonsteroidal anti-inflammatory drugs (NSAIDs). Celecoxib is a widely recognized selective COX-2 inhibitor that has been extensively studied for its therapeutic efficacy. Its structural analogues differ in their biological activities despite their close structural resemblanc

Gas Sensing Performance of Lead-Free Perovskite Cs3Sb2Cl9 for Monitoring Typical Electrolyte (DMC) Leakage in Lithium-Ion Batteries.

Dimethyl carbonate (DMC), the primary component of lithium-ion battery (LIBs) electrolytes, is volatile at room temperature and can serve as a key indicator for detecting electrolyte leakage. In this work, leaf-like Cs3Sb2Cl9 was synthesized via a single solvent solubility difference method, with its surface exhibiting strong orientation on the (110) crystal plane. The sensor demonstrated excellent gas-sensing performance toward detecting DMC (50 ppm of DMC of 3.94), with relatively short response and recovery times of 104 and 66 s, respectively (when tested 25 ppm of DMC), and a low detection

Nano-Enabled Potentiation of a Lead Mono-Carbonyl Curcumin Analogue via PEGylated Graphene Oxide for Enhanced Glycemic Control.

Background: The global healthcare system faces a significant challenge due to the escalating prevalence of type 2 diabetes, affecting over 10% of the world's population. Suppression of postprandial hyperglycemia through inhibition of carbohydrate-hydrolyzing enzymes is an effective therapeutic strategy. Although curcumin effectively inhibits α-amylase and α-glucosidase activities, its lower solubility and bioavailability restrict its clinical application. In this study, five mono-carbonyl curcumin analogues (CA1-CA5) were synthesized and evaluated for their antidiabetic potential following sel

Evaluation of Lead exposure's link to autism-like behavior and Shank 3 gene expression in C57BL/6J mice.

Autism spectrum disorder (ASD) affects about 1 in 100 people worldwide, yet its causes remain poorly understood. Environmental factors, such as perinatal lead (Pb) exposure, have been linked to neurodevelopmental disruption, especially during periods when the blood-brain barrier is immature. This study aimed to assess ASD-like behavioral outcomes in offspring following maternal Pb exposure during neurodevelopment using a C57BL/6J mouse model. Pregnant dams received 100 ppm Pb acetate in their drinking water from the second week of gestation until weaning; controls were given deionised distille

Deep Learning-Based Virtual Screening Identifies Potential Small-Molecule Inhibitors of ASK1: Natural Product Lead Discovery for MASH.

Apoptosis signal-regulating kinase 1 (ASK1) represents a critical therapeutic target for metabolic dysfunction-associated steatohepatitis (MASH). Natural products, owing to their unique chemical diversity, constitute a rich reservoir for discovering novel ASK1 inhibitors. The emergence of artificial intelligence-assisted drug discovery (AIDD) has opened new avenues for exploring small-molecule inhibitors. Through virtual screening, molecular docking, interaction profiling, molecular dynamics simulations, and MM-GBSA binding free energy calculations, we systematically evaluated the binding mode

Blood Lead Levels and Associated Sociodemographic and Occupational Factors in Children from Two Mexican Communities: A Comparative Study.

Lead is a neurotoxic metal. No safe blood lead level (BLL) has been identified in children; the CDC reference value was lowered to 3.5 µg/dL in 2021, while Mexico's regulatory threshold remains at 10 µg/dL. Lead-glazed earthenware is the main documented source of pediatric exposure in Mexico. We studied children 0-15 years insured by the Mexican Social Security Institute (IMSS) in two Jalisco municipalities: Tonalá (n = 500), historically a ceramic-producing center, and Zapopan (n = 374), an urban area without this tradition. BLL was measured by graphite furnace atomic absorption spectrophotom
