Are sugars involved in the binding of rhodopsin-membranes by the retinal pigment epithelium?

The binding by the retinal pigment epithelium (RPE) cells of the embryonic chick, in culture, of recombinant membranes composed of phosphatidylcholine and bovine rhodopsin (rhodopsin-liposomes [RL]) was investigated to explore the influence that the carbohydrate groups of rhodopsin might have on this process. The reaction was quantitated by measuring the amount of RL associated with the cells using a radioimmunoassay for rhodopsin. The process was saturated at about 8 microM RL (calculated as rhodopsin-equivalents), and a two- to threefold greater rate and extent of binding was observed at 37

LINC00632 combines with ERα to target VISTA expression, ameliorating endometriosis progression.

Endometriosis, a prevalent gynecological disease affecting women's health, involves altered immune function. This study investigates the immune checkpoint protein, V-domain immunoglobulin suppressor of T cell activation (VISTA) as a therapeutic target. In patient lesions, elevated VISTA expression correlated with reduced CD8+ T cell abundance. Using a 3D primary cell model, the research linked this immune suppression to a high-estrogen microenvironment and mast cell infiltration. Mechanistically, mast cell-derived LINC00632 cooperated with ERα to drive VISTA overexpression. A liposomal drug de

First human pleural infection caused by Naganishia globosa (formerly Cryptococcus saitoi).

Naganishia globosa (formerly Cryptococcus saitoi) is an environmental basidiomycetous yeast rarely associated with human disease. We report the first case worldwide of N. globosa isolated from pleural fluid in a patient with T-cell acute lymphoblastic leukemia (T-ALL), an immunocompromised host, hospitalized in Türkiye. The isolate was identified by MALDI-TOF MS and confirmed by molecular sequencing. The patient received antifungal therapy following diagnosis. Liposomal amphotericin B, isavuconazole, fluconazole, and voriconazole were used in the patient's treatment and were modified according

Use of Expanded Access Fosmanogepix in Two Pediatric Patients With Rhino-Orbital-Cerebral Mucormycosis.

Mucormycosis is a severe fungal infection with high morbidity and mortality. Pharmacotherapeutic options are limited, often used in combination, and associated with significant adverse effects. Fosmanogepix is an investigational antifungal with a novel mechanism of action and has shown broad in vitro activity. Early clinical trials suggest a manageable adverse effect profile, making it an attractive treatment option. Two pediatric patients presented in diabetic ketoacidosis and were found to have extensive rhino-orbital-cerebral mucormycosis. Treatment with standard of care, including liposoma

Disseminated blastomycosis in pregnancy presenting with arthritis and cutaneous lesions.

Blastomycosis is a rare fungal infection, and disseminated disease in pregnancy is exceedingly uncommon. We report a 26-year-old primigravida at 28 weeks' gestation who presented with progressive knee arthritis and erythematous nodular skin lesions. Skin biopsy and synovial fluid culture confirmed blastomyces dermatitidis. She was treated initially with liposomal amphotericin B but transitioned to itraconazole due to electrolyte abnormalities. Despite complications of severe knee pain and hypokalemia, she improved clinically. At 39 weeks, she underwent induction of labor and cesarean delivery

Injectable PROTAC-loaded hydrogel remodels the tumor microenvironment to potentiate radio-immunotherapy.

Radiotherapy (RT) is a key component of comprehensive cancer treatment regimens; nevertheless, its concomitant immunosuppression may diminish therapeutic efficacy. In this study, we developed an injectable hydrogel system for the local delivery of PROteolysis TArgeting Chimeras (PROTACs), achieved by loading tumor cell membrane-fused liposome nanoparticles to enhance the anti-tumor effect. The system targeted Bromodomain-containing protein 4 (BRD4), and combined treatment with RT promoted DNA damage, reduced DNA repair and decreased tumor cell proliferation and survival. Additionally, this str

Breaking the mold: a case of invasive Medicopsis romeroi pulmonary infection with innovative granulocyte treatment strategy.

BACKGROUND: Medicopsis romeroi only rarely causes infection in immunocompromised individuals. We report a rare case of invasive infection in a specific primary immunodeficiency not arising from primary cutaneous infection. CASE SUMMARY: A 16-year-old male with chronic granulomatous disease (CGD) developed progressively enlarging diffuse pulmonary nodules in the setting of admission with obstructive hydrocephalus. A transthoracic biopsy grew a dematiaceous mold, identified as Medicopsis romeroi through DNA sequencing of the internal transcribed spacer (ITS) region of ribosomal DNA (rDNA). The

Rare Cutaneous Presentation of Disseminated Cryptococcus gattii Infection in an Immunocompetent Male: A Case Report.

Cryptococcosis is a systemic mycosis caused by Cryptococcus neoformans and Cryptococcus gattii, affecting mainly immunocompromised and immunocompetent hosts, respectively. Cutaneous involvement may indicate secondary hematogenous dissemination from systemic disease or, less frequently, primary cutaneous cryptococcosis resulting from direct inoculation without systemic infection. Although cutaneous cryptococcosis can present with diverse morphologies, cutaneous involvement is uncommon in C. gattii infection and is more frequently associated with C. neoformans. We present the case of a 56-year-o

A Budding Pancreatic Mass: A Review of Disseminated Blastomycosis.

A 67-year-old male patient presented with anorexia, weight loss, skin masses, and flank pain over the past three months. Imaging revealed a lung mass and a cystic lesion in the pancreatic head. After initial diagnostic work-up and clinical deterioration, broad based budding, seen on pathology, led to a diagnosis of disseminated blastomycosis. After receiving a 14-day course of liposomal amphotericin B followed by itraconazole, the patient showed significant clinical improvement. We reviewed all disseminated blastomycosis cases in the medical literature involving gastrointestinal organs. Althou

Deep Sternal Wound Infection Caused by Rhizopus Species After Coronary Artery Bypass Graft.

Deep sternal wound infection is a rare complication of cardiac surgery that is typically caused by skin resident flora, such as species of Staphylococcus and Streptococcus. Infections caused by fungi are less common and are generally caused by Candida species. Regardless of etiology, these infections are associated with significant morbidity and mortality. We present a case of postoperative mediastinitis that occurred following a 5-vessel coronary artery bypass graft and was caused by a filamentous fungus of the Rhizopus genus. The patient was treated with serial debridement, liposomal amphote

Navigating Diagnosis and Treatment Dilemmas in Visceral Leishmaniasis Reactivation in a HIV-Positive Patient.

Reactivation of visceral leishmaniasis (VL) in human immunodeficiency virus (HIV)-positive patients poses complex management challenges, requiring tailored treatment approaches and robust follow-up strategies. There are very few case reports of visceral leishmaniasis (VL) published in Sri Lanka as of today. Here, we present a case of a 38-year-old, HIV-positive male who was treated for a reactivation of VL. He presented with a prolonged febrile illness without an identifiable infection focus. The evaluation confirmed VL reactivation. Using current evidence-based guidelines, our patient was tre

Case Report: A Case of Sporotrichoid Cutaneous Leishmaniasis Caused by Leishmania (Viannia) peruviana.

Leishmaniasis is caused by an obligate intracellular protozoa of the genus Leishmania. Its clinical manifestations include cutaneous, mucocutaneous, and visceral forms. Sporotrichoid cutaneous leishmaniasis (SCL) is an atypical and rare form of cutaneous leishmaniasis (CL) reported mainly in the Old World. This case report describes SCL in a Japanese man infected with Leishmania (Viannia) peruviana in Peru. His lesions occurred on both feet, with the left foot lesion being a simple CL that resolved spontaneously. However, the lesion on the right foot did not cure by itself; instead, it progres

[A case of Candida meningitis in a patient with CARD9 deficiency: an autopsy report].

Here we present the case of a 23-year-old female with a history of onychomycosis and oral thrush since childhood. She presented with a gradual onset of headache, and cerebrospinal fluid (CSF) analysis on admission revealed an elevated mononuclear cell count. Hydrocephalus was observed on brain MRI. Candida albicans (C. albicans) was detected in the CSF, and antifungal treatment was initiated to diagnose of Candida meningitis. Due to an insufficient therapeutic response, intraventricular administration of liposomal amphotericin B initiated; however, the lesions persisted. Subsequently, the pati

Survival of Hemophagocytic Syndrome Secondary to Fungal and Bacterial Infection in a Pediatric Patient with HIV: A Case Report.

HIV-associated hemophagocytic lymphohistiocytosis (HLH) is mainly due to infections caused by viruses, fungi, and, to a lesser extent, bacteria, often with fatal results. Case presentation: A 15-year-old pediatric patient from another institution was admitted to our hospital with a fever of unknown origin (FUO). Clinical analysis and laboratory studies diagnosed HIV infection. The approach to an FUO in a patient with AIDS is much more complex due to the search for common etiologies and opportunistic infections. In this case, disseminated histoplasmosis, pulmonary tuberculosis, pneumocystosis,

Rhino-Orbital Mucormycosis in a COVID-19 Patient: A Dreadful Challenge.

Coronavirus disease 2019 (COVID-19) is a pandemic disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-COV-2). COVID-19 infections may be associated with a wide range of bacterial and fungal co-infections. Recent studies are reporting invasive fungal infection associated with severe COVID-19. Herein, we report a case of COVID-19 rhino-orbital mucormycosis infection caused by Rhizopus sps in a 32 year old diabetic patient who was successfully managed with early aggressive debridement of infected tissue endoscopically with extended ethmoidectomy by modified Denker's approach a

Donor-derived renal allograft mucormycosis in a combined liver and kidney transplantation: Case report and review of the literature.

Mucormycosis infrequently may present with isolated renal involvement. Among solid organ transplant recipients, renal allograft mucormycosis has been most often associated with medical tourism or transplantation outside of the western world. We report a case of an HIV/HCV co-infected woman who underwent simultaneous liver and kidney transplantation with a Public Health Service increased risk donor organ. 16 days after transplant, she developed massive hematuria and was found to have renal allograft Rhizopus spp. involvement, we surmise to have been from donor-derived infection. Potential benefit of combination antifungal therapy in Aspergillus endocarditis.

Aspergillus endocarditis (AE) is a rare condition with a mortality rate greater than 60%. While it is generally accepted that both antifungal therapy and surgery are necessary for survival, the optimal antifungal regimen is unclear. A 62-year-old man was diagnosed with AE of a prosthetic aortic valve, complicated by cerebral emboli. He underwent debridement of the aortic valve abscess and valve replacement, and was managed with a combination of liposomal amphotericin B and voriconazole for 7 weeks followed by long-term suppressive azole therapy. He remained well at follow-up 18 months later. D

Leishmania in a Patient with Small Lymphocytic Lymphoma/Chronic Lymphocytic Leukemia.

BACKGROUND Leishmaniasis is a parasitic infection spread by the bite of infected sand flies that are usually present in the Middle East, Africa, and some parts of Asia and Europe. Leishmaniasis manifests in 3 different forms: Visceral (also known as Kala Azar), which is the most serious type; cutaneous, which is the most common type; and mucocutaneous. The symptoms of this infection range from a silent infection to fever, enlargement of the liver and spleen, weight loss, and pancytopenia. CASE REPORT In this case report, we discuss a 73-year-old man known to have chronic lymphocytic leukemia (
