NELL-1 positive membranous nephropathy in association with antisynthetase syndrome.

Membranous nephropathy (MN) is an important cause of nephrotic syndrome and is associated with significant adverse health outcomes including progression to end-stage kidney disease, complications relating to volume overload and increased risk of venous thromboembolism. Primary MN is frequently linked to antibodies against the phospholipase A2 receptor, although a broader range of target autoantigens are emerging. We report a case of a man in his mid-60s who had been recently diagnosed with antisynthetase syndrome presenting with nephrotic syndrome. A kidney biopsy revealed findings in keeping

Helicobacter pylori outer membrane protein families and related pathogenesis.

Helicobacter pylori infection is one of the most common bacterial infections worldwide and a major cause of chronic gastritis, peptic ulcers, and gastric cancer. The outer membrane proteins (OMPs) of H. pylori play crucial roles in its pathogenesis, acting as key mediators of bacterial adhesion, immune evasion, and interactions with the host. This review aims to provide a comprehensive overview of the various OMPs involved in H. pylori pathogenesis, including their roles as adhesins, porins, and in biofilm formation. Additionally, this review explores the involvement of OMPs in triggering proi

Revealing the Intersection: Scleroderma Renal Crisis Complicating Membranous Nephropathy.

An 82-year-old woman with systemic sclerosis (anti-topoisomerase I (anti-Scl-70) positive) and a history of membranous nephropathy presented with a prolonged illness marked by worsening oedema, reduced urine output, and severe hypertension. Laboratory investigations revealed nephrotic-range proteinuria, acute kidney injury (AKI), and a marked decline in renal function. Initial treatment with diuretics and antihypertensive therapy yielded limited improvement. Renal biopsy demonstrated dual pathology: primary membranous nephropathy and acute vascular changes indicative of scleroderma renal crisi

Structural basis for membrane remodeling by the AP5-SPG11-SPG15 complex.

The human spastizin (spastic paraplegia 15, SPG15) and spatacsin (spastic paraplegia 11, SPG11) complex is involved in the formation of lysosomes, and mutations in these two proteins are linked with hereditary autosomal-recessive spastic paraplegia. SPG11-SPG15 can cooperate with the fifth adaptor protein complex (AP5) involved in membrane sorting of late endosomes. We employed cryogenic-electron microscopy and in silico predictions to investigate the structural assemblies of the SPG11-SPG15 and AP5-SPG11-SPG15 complexes. The W-shaped SPG11-SPG15 intertwined in a head-to-head fashion, and the

A Comprehensive Review of Left Atrial Venoarterial Extracorporeal Membrane Oxygenation.

Cardiogenic shock (CS) in patients with structural heart disease (SHD) and severe valvular abnormalities poses unique challenges to traditional mechanical circulatory support (MCS) strategies, such as intra-aortic balloon pumps and ventricular assist devices. These devices may fail to address the complex interplay between ventricular unloading and systemic perfusion. Left atrial venoarterial extracorporeal membrane oxygenation (LAVA-ECMO) incorporating left atrial (LA) drainage into the ECMO circuit to reduce left ventricular (LV) preload, mitigate pulmonary congestion, and maintain systemic p

Anatomical and functional changes after internal limiting membrane peeling.

Internal limiting membrane (ILM) peeling has been an acceptable step in vitrectomy surgeries for various retinal diseases such as macular hole, chronic macular edema following epiretinal membrane (ERM), and vitreoretinal traction. Despite all the benefits, this procedure has some side effects, which may lead to structural damage and functional vision loss. Light and dye toxicity may induce reversible and irreversible retina damage, which will be observed in postoperative optical coherence tomography scans. Retinal nerve fiber layer damage is attributed to ganglion cell degeneration and axonal

Engineering a membrane protein chaperone to ameliorate the proteotoxicity of mutant huntingtin.

Toxic protein aggregates are associated with various neurodegenerative diseases, including Huntington's disease (HD). Since no current treatment delays the progression of HD, we develop a mechanistic approach to prevent mutant huntingtin (mHttex1) aggregation. Here, we engineer the ATP-independent cytosolic chaperone PEX19, which targets peroxisomal membrane proteins to peroxisomes, to remove mHttex1 aggregates. Using yeast toxicity-based screening with a random mutant library, we identify two yeast PEX19 variants and engineer equivalent mutations into human PEX19 (hsPEX19). These variants eff

Acoustical Effects of Tympanostomy Tube Attachment to Human Tympanic Membrane.

PURPOSE: Several therapeutic approaches for hearing disorders involve attaching medical devices to the tympanic membrane. The attachment of these devices can change the mechanical and acoustical properties of the middle ear, affecting the middle-ear vibrations. The alteration of passive mechanical properties results from the mass, stiffness, and geometry of the attached device. Additionally, procedures like tympanostomy tube attachment create perforations on the tympanic membrane, altering both the mechanical and acoustical properties of the middle ear. This study examined the acoustical effec

Conformational dynamics and membrane insertion mechanism of B4GALNT1 in ganglioside synthesis.

Glycosphingolipids (GSLs) are crucial membrane components involved in essential cellular pathways. Complex GSLs, known as gangliosides, are synthesised by glycosyltransferase enzymes and imbalances in GSL metabolism cause severe neurological diseases. B4GALNT1 synthesises the precursors to the major brain gangliosides. Loss of B4GALNT1 function causes hereditary spastic paraplegia, while its overexpression is linked to cancers including childhood neuroblastoma. Here, we present crystal structures of the human homodimeric B4GALNT1 enzyme demonstrating dynamic remodelling of the substrate bindin

Unraveling viral protein-host membrane interaction for dengue and Zika.

Mosquito-borne flaviviruses, including dengue virus (DENV) and Zika virus (ZIKV), constitute a significant and escalating public health threat. Elucidating the mechanisms by which these flaviviruses subvert cellular processes through viral protein-host cell interactions provides critical insights into their replication and pathogenicity. Here, we present an analysis based on anisotropy calculation across pixels in raster images to investigate differential protein interactions during nucleocytoplasmic shuttling. This methodology highlights regions of high and low anisotropy, suggesting differen

Structural insights into outer membrane protein biogenesis in pathogenic Neisseria.

N. gonorrhoeae (Ngo) causes the sexually transmitted infection gonorrhea with ∼106 million infections worldwide annually. Ngo infections can result in an increased risk of acquiring HIV, infertility, and blindness. To combat Ngo infections, we report the cryoelectron microscopy (cryo-EM) structure of the Ngo β-barrel assembly machinery (NgBAM), which is responsible for the biogenesis of β-barrel outer membrane proteins (OMPs). NgBAM was observed in an inward-open state; however, the polypeptide transport-associated (POTRA) domains more closely match those found in the outward-open state in E.

Tiny packages, big potential: bacterial membrane vesicles in vaccinology.

Bacterial membrane vesicles (BMVs) are nanoscale, bilayered proteolipid structures secreted by both Gram-negative and Gram-positive bacteria. Initially considered cellular debris, BMVs are now recognized as evolutionarily conserved entities with critical roles in bacterial communication, immune modulation, virulence factor delivery, and horizontal gene transfer. Their structural and functional resemblance to eukaryotic extracellular vesicles has fueled growing interest in their use as versatile vaccine platforms. Licensed meningococcal OMV vaccines established proof-of-concept for their safety

Amniotic membrane transplantation in the treatment of macular hole secondary to neuroretinitis.

This case report aims to demonstrate the use of amniotic membrane transplantation (AMT) as a treatment for a refractory full-thickness macular hole (FTMH) that developed secondary to neuroretinitis. A 14-year-old girl presented to our hospital with a complaint of blurred vision persisting for one week. The patient was diagnosed with neuroretinitis due to cat scratch disease with a FTMH that developed secondarily. A pars plana vitrectomy, internal limiting membrane (ILM) peel, and ILM transplantation were performed. The FTMH did not close completely. The third surgery was performed using the AM

A Pathogen's Whisper: Reactivation of Quiescent Membranous Nephropathy by Disseminated Tuberculosis.

Primary membranous nephropathy (MN) is an autoimmune glomerular disease commonly associated with anti-PLA2R antibodies, with relapses typically attributed to spontaneous immune reactivation. We report the first documented case of a relapse of primary, PLA2R-positive MN that was temporally and immunologically linked to disseminated tuberculosis (TB) infection. A 42-year-old man, previously in complete remission, developed severe nephrotic syndrome and acute kidney injury unresponsive to standard immunosuppressive regimens. Concomitant diagnosis of miliary TB was confirmed by culture and imaging

Electrospun Nanofiber Membrane for Cultured Corneal Endothelial Cell Transplantation.

The corneal endothelium, comprising densely packed corneal endothelial cells (CECs) adhering to Descemet's membrane (DM), plays a critical role in maintaining corneal transparency by regulating water and ion movement. CECs have limited regenerative capacity within the body, and globally, there is a shortage of donor corneas to replace damaged corneal endothelia. The development of a carrier for cultured CECs may address this worldwide clinical need. In this study we successfully manufactured a gelatin nanofiber membrane (gelNF membrane) using electrospinning, followed by crosslinking with glut
