Post-traumatic tuberculosis of Zygoma - An uncommon presentation.

Tubercular osteomyelitis of mid-facial bones is extremely rare because of its immense vascular supply. Due to rare incidence, myriad presentation, and lack of specific symptoms, this condition presents a challenge in diagnosis and calls for acute clinical awareness. This article presents a case report of a 31-year-old martial artist with complaints of trauma to his right malar region since one month and a gradually increasing swelling below his right eye for 3 weeks without any discharge. A diagnosis of primary Tuberculosis of right zygomatic bone was made with the help of multiple diagnostic

A Case Report: The Challenging Diagnosis of Spontaneous Cervical Epidural Hematoma.

INTRODUCTION: We present the case of a patient with a spontaneous cervical epidural hematoma that presented with neck pain and mild, left arm parasthesia. CASE REPORT: A 59-year old man presented with sudden onset of severe neck pain, without history of injury or trauma. The patient also complained of associated left arm parasthesias that progressed to left arm and leg weakness while in the emergency department. Multiple diagnoses were considered and worked up; eventually the correct diagnosis was made with magnetic resonance imaging of the cervical spine. CONCLUSION: Spontaneous cervical ep

A case of thoracoabdominal splenosis.

We describe a case of a 38-year-old male with a remote history of motor vehicle trauma who presented to the emergency department with 1-week history of progressively worsening abdominal pain localized to the epigastric region. Patient history included splenectomy. Computerized tomography demonstrated multiple masses in the left pleural space as well as masses continuous with the diaphragm and abdominal wall in the left upper quadrant. In addition, a lobulated mass was identified in the right upper quadrant along the anterior right hepatic lobe. A diaphragmatic defect was noted containing splen

Insufficiency fractures: A rare cause of foot and ankle pain in three patients with rheumatoid arthritis.

Insufficiency fractures are recognized but rare complications in patients with longstanding rheumatoid arthritis. Such fractures are typically solitary and are rarely seen to affect the foot and ankle. We describe 3 women with longstanding rheumatoid arthritis, treated with one, or a combination of, corticosteroids, DMARDs and anti-TNF, presenting with foot and ankle pain with no prior history of trauma. MRIs showed rare multiple florid insufficiency fractures of the foot and ankle, in 2 cases bilaterally, which were managed conservatively. These cases highlight the importance of considering i

Extending the range of differential diagnosis of chronic traumatic encephalopathy of the boxer: Insights from a case report.

Sports activities associated with repetitive cranial trauma have become a fad and are popular in gyms and even among children. It is important to consistently characterize the consequences of such sports activities in order to better advise society on the real risks to the central nervous system. We present the case of a former boxer reporting cognitive and behavioral symptoms that began six years after his retirement as a boxer, evolving progressively with parkinsonian and cerebellar features suggestive of probable chronic traumatic encephalopathy (CTE). Using our case as a paradigm, we exten

Sickle Cell Beta-Plus Thalassemia with Subcapsular Hematoma of the Spleen.

While splenic complications like hypersplenism, sequestration crisis, and infarction are commonly reported in sickle cell variants like sickle cell beta-plus thalassemia, splenic rupture with hematoma is rare. We present a case of a 32-year-old young male who presented with dull left upper quadrant pain who was found to have multiple subcapsular splenic lacerations and hematoma on abdominal imaging. Hemoglobin electrophoresis confirmed sickle cell beta-plus thalassemia in the patient. There was no history of trauma, and rest of the workup for possible cause of spontaneous rupture of spleen was

[TRAUMATIC RUPTURE OF THE AORTIC ISTHMUS: MODERN PERSPECTIVES].

Traumatic aortic rupture of the thoracic aorta (usually at the isthmus) is frequently associated with concomitant life-threatening injuries. Historically, the conventional care consisted of surgical repair of the lesion performed as soon as possible. However, in spite of constant technical improvements the morbi-mortality remains high because of these associated lesions. In addition, their management can have priority and delay aortic surgery. The endovascular approach has been shown to be a feasible and efficient technique and currently represents a valuable alternative to open surgery for pa

Intrahepatic splenosis mimicking liver cancer: report of a case and review of literature.

Intrahepatic splenosis is a rare situation occurred after traumatic splenic rupture or splenectomy. We report a 33 year old man with multiple isolated liver masses indistinguishable from primary and metastatic liver tumor by standard evaluation. The man underwent a splenectomy due to trauma at the age of three so that the hepatic splenosis was suspected. The final fine-needle biopsy pathological examination proved the diagnosis of intrahepatic splenosis. The importance of suspicion for this diagnosis should be highlighted when tumor-like lesions disclosed on imaging occurs in a patient with a

Lhermitte's Sign: The Current Status.

Lhermitte's sign was described by Marie and Chatelin and named after Jean Lhermitte. This sign is mostly described as an electric shock like condition by some patients of multiple sclerosis. This sensation occurs when the neck is moved in a wrong way or rather flexed. It can also travel down to the spine, arms, and legs, and sometimes the trunk. Demyelination and hyperexcitability are the main pathophysiological reasons depicted for the Lhermitte's sign. Other causes for Lhermitte's sign include transverse myelitis, behçet's disease, trauma, etc. This article reviews the Lhermitte's sign, its

[Successful repair of left ventricular laceration due to traumatic broken ribs].

A 27-year-old woman was hospitalized by emergency due to multiple blunt trauma by traffic accident. Computed tomography (CT) showed pericardial effusion. Because hematothorax and pericardial effusion appeared alternatively, thoracocentesis and pericardiocentesis were performed. Finally, the diagnosis of cardiac rupture was confirmed, and surgery was performed through median sternotomy. The left ventricular free wall was penetrated by the edge of a broken rib through the hole of the pericardium and the pleura. Broken ribs were considered to have penetrated the pericardium and caused cardiac rup

[Internuclear ophthalmoplegia--causes, symptoms and management].

Internuclear ophthalmoplegia (INO) is a disorder of eye movements caused by a lesion in an area of the brain called the medial longitudinal fasciculus (MLF). The most common causes of INO are multiple sclerosis and brainstem infarction. Other causes include head trauma, brainstem and fourth ventricular tumors, Arnold-Chiari malformation, infection, hydrocephalus, and lupus erythematosus. Internuclear ophthalmoplegia is clinically characterized by total or partial failure to adduct one eye in lateral gaze and a monocular nystagmus of the abducting eye. It may be unilateral and bilateral. The me

Chyle leaks: consensus on management?

Lymphatic injury leading to leakage of chyle is a potential complication that may arise from trauma or surgery in the chest, abdomen, or neck. Although the incidence of chyle leak post surgery is low (1%-4%), this complication can present significant challenges. Multiple approaches to the treatment of chyle leak have emerged, including nutrition, surgical, and pharmacological therapies. Although there are strong feelings among clinicians about the use of bowel rest, parenteral nutrition, or a low-fat enteral formula for the treatment of chyle leak, definitive evidence supporting one nutrition

Recurrent pleural effusions in a neonate.

Difficulty breathing is a common presenting complaint for infants in the emergency department (ED). Periodic breathing, respiratory or systemic infection, and congenital heart disease are common diagnoses in this age group. We report the case of a male neonate presenting to the ED on multiple occasions with respiratory distress and recurrent pleural effusions of unique origin. Unlike adult pleural effusions, pediatric effusions are most commonly exudative and of infectious origin. In neonates, acquired chylothorax secondary to surgery or trauma is the most common cause of symptomatic pleural e

Salvage options after stent collapse in the thoracic aorta.

The endograft was originally developed to repair aneurismal disease of the infra-renal aorta and has since realised many other applications, including the treatment of arterial trauma. Traumatic transection of the thoracic aorta is a condition associated with a high mortality and affected patients often have multiple injuries. Endovascular repair of thoracic transection is an attractive option in those patients for whom open surgical repair would be highly dangerous and other groups have reported early technical success. However, we report 3 cases of young patients with traumatic thoracic aort

Videofluoroscopy and swallowing studies for neurologic disease: a primer.

Many patients with neurologic impairment due to stroke, multiple sclerosis, trauma, bulbar palsy, and other disorders have difficulty swallowing. Videofluoroscopy can provide important information on patterns of impairment of the swallowing mechanism, allowing important changes in patient treatment. The detailed videofluoroscopic evaluation required to provide this information is now seldom taught and is practiced by relatively few radiologists. The aim of this article is to (a) describe the indications for videofluoroscopic swallowing studies in the evaluation of patients with neurologic cond

[Complication of tracheal intubation: severe cervical cellulitis].

A 40-year-old man, victim of a traffic accident has been hospitalized for a severe head trauma. His trachea has been intubated under general anaesthesia with an 8.0 mm ID tube (Vygon). The cerebral scan revealed a surgical subdural haematoma. In the postoperative period, the patient was admitted in surgical intensive care, under sedation and mechanically ventilation. At day 12 the patient developed a cervical cellulitis complicated of a septic shock. The cervical scan showed an important dilatation of the trachea in the site of the tube cuff. The surgical exploration discovered a complete dest

Brain abscesses complicating Staphylococcus aureus sepsis in a premature infant.

Brain abscess is a rare complication of staphylococcal bacteremia in infants. Here we present a case of a premature infant who developed multiple brain abscesses 12 weeks following an episode of inadequately treated Staphylococcus aureus sepsis. The abscess developed in the absence of trauma, prior surgery, cyanotic heart disease, or immune defect. The initial staphylococcal isolate exhibited identical pulsed-field gel electrophoresis pattern with that of the isolate cultured from abscess aspirate. The infant was successfully treated by surgical drainage and administration of antibiotics for 1

High-resolution ultrasonography in the diagnosis of scrotal pathology: I. Normal scrotum and benign disease.

Ultrasonography is a readily accessible imaging modality. It is relatively inexpensive, is noninvasive, and requires no ionizing radiation or previous preparation. For these reasons, ultrasonography is the examination of choice in the study of the scrotum and its contents. We present a review of the pathology of the scrotum and its contents. For that, we cite multiple sources of the current medical literature and illustrate them with cases performed at the University of California Davis Medical Center during the last 2.5 years. Due to the extent of the pathology in consideration, we have divid

Leiomyosarcoma is an uncommon tumor that rarely occurred in extremity. Multiple leiomyosarcomas in different extremities is even rarer. We report a 90 year-old male veteran who was found to have four leiomyosarcomas, one in right arm and the other three in right thigh. The patient sustained gunshot injuries over right thigh and right arm about 60 years ago in a war with severe wound sepsis due to open communited fracture of the femur. These tumors happened to distribute in the vicinity of the old scars. It was speculated that these multicentric tumors were due to decreased immune surveillance

[Complex disorders of radius and ulna in childhood].

Treatment of congenital or acquired disturbances of the radius or ulna always requires recognition of the complex biomechanics of the forearm. In particular trauma, seemingly restricted to one of the bones, should caution against concomitant lesions to the other bone. The typical example is unfortunately the often missed dislocation of the capitulum radii in Monteggia-type lesions. Additional examples are deranged growth of the distal epiphysis of the radius after a fracture of the ulna, in association with multiple osteochondromas and of the proximal epiphysis of the radius in presumptively c
