Comparative safety of atorvastatin 80 mg versus 10 mg derived from analysis of 49 completed trials in 14,236 patients.

Atorvastatin has been shown to reduce coronary events and revascularization procedures in patients with multiple risk factors for coronary heart disease. Recent studies with atorvastatin 80 mg support the overall safety of this dose during long-term treatment. However, physicians appear reluctant to use high doses of statins. A retrospective analysis of pooled data from 49 clinical trials of atorvastatin in 14,236 patients treated for an average period of 2 weeks to 52 months was conducted. The study compared the safety of atorvastatin 10 mg (n = 7,258), atorvastatin 80 mg (n = 4,798), and pla

Malignant lymphoma-associated autoimmune diseases--a descriptive epidemiological study.

Lymphoproliferative disorders and autoimmune diseases have some common aspects in their clinical appearance. We reviewed 940 patient charts with malignant lymphomas to assess the rate of associated autoimmune diseases. Of 421 non-Hodgkin's lymphoma (NHL) patients (230 males, 191 females), 32 (7.6%) had an autoimmune disease (26 females, six males, mean age 48.3 years). The most common diagnosis was Sjögren's syndrome. The other cases were autoimmune skin diseases (5), thyroiditis (3), polymyositis (2), scleroderma (2), other musculoskeletal disorders (2), rheumatoid arthritis (1), vasculitis (

Back pain.

Among the multitude of potential etiologies for low back pain in the elderly, cauda equina syndrome is the only genuine surgical emergency. The consequence of delayed recognition, referral, and decompression may be permanent neurologic deficit. Fortunately, this event is rare. Certain other warning signs and symptoms (as outlined in Fig. 1) should alert the clinician to possible systemic disease and warrant earlier, more aggressive evaluation. Careful history-taking and focused physical examination remain the cornerstones of clinical decision making in managing the geriatric patient with back

[Suspected borreliosis - what's behind it?].

BACKGROUND AND OBJECTIVE: Borreliosis may be associated with unspecific symptoms and thus not only cause difficulties in diagnosis but also lead to overdiagnosis. PATIENTS AND METHODS: Data on 134 patients (mean age 47 [12 - 78] years, 51.5 % male) with suspected borreliosis presenting at the university hospital Regensburg were analyzed retrospectively. RESULTS: The majority of patients had been adequately treated for borreliosis previously. 34 patients (25.4 %) had proven or possible borreliosis, 20 patients (14.9 %) presented for consultation only. Regarding the remaining 80 patients (59.7

A Primer on COL4A1/COL4A2-Related Cerebral Small Vessel Disease for Neurologists.

COL4A1 and COL4A2 encode type IV collagen, a principal structural component of basement membranes, and pathogenic variants in these genes cause a multisystem small vessel vasculopathy with prominent cerebrovascular involvement. Despite increasing recognition with the expansion of genetic testing, COL4A1/2-related cerebral small vessel disease remains underrecognized in clinical practice, in part due to its striking phenotypic heterogeneity. Presentations span a broad clinical spectrum-from asymptomatic individuals or incidental imaging findings to recurrent ischemic events, intracerebral hemor

How are we representing the trunk? A narrative review on marker sets for kinematics analysis of neurological conditions.

BACKGROUND: Trunk movement plays a vital role in daily activities, influencing efficiency, balance, and musculoskeletal health-especially in individuals with neurological conditions. However, three-dimensional (3D) motion analysis of the trunk lacks standardisation, limiting both cross-study comparisons and clinical application. This review synthesises current approaches to trunk modelling, including marker setups, segment definitions, and kinematic outcomes, in studies involving participants with neurological conditions. METHODS: A systematic search of six databases, including MEDLINE and Sc

The gut-Joint axis: investigating functional gastrointestinal disorders as risk factors for prosthetic joint infections a retrospective case-control study.

BACKGROUND: Periprosthetic joint infection (PJI) is a severe complication of total joint arthroplasty (TJA), with traditional risk factors including diabetes and obesity. Emerging evidence suggests functional gastrointestinal disorders (FGIDs) may influence systemic inflammation and infection susceptibility. This study investigated whether preexisting FGIDs are independent risk factors for PJI. METHODS: A retrospective 1:1 matched case-control study analyzed 896 patients (448 PJI vs. 448 non-PJI) undergoing primary TJA (2013-2023). PJI was diagnosed using modified 2018 Musculoskeletal Infecti

Bile acid signaling in health and disease.

Bile acids (BAs), cholesterol-derived amphipathic metabolites which are traditionally regarded as digestive surfactants, are now recognized as pleiotropic signaling molecules that act through ligand-, receptor-, and tissue-specific mechanisms to maintain systemic homeostasis. They exert biological functions via nuclear receptors (FXR, PXR, VDR, CAR) and membrane receptors (TGR5, S1PR2, MRGPRX4). These receptors activate transcriptional and membrane-initiated programs, including FXR-SHP/FGF19 feedback, TGR5-cAMP-PKA signaling, S1PR2-ERK/AKT/RhoA pathways, MRGPRX4-dependent Ca2⁺ signaling, PXR/C

Artificial Intelligence-assisted MRI and CT-based diagnostic anatomical evaluation of experimental rat disease models.

Artificial intelligence (AI) emerged as the transformative technology in biomedical imaging to improve the accuracy, efficiency and reproducibility of disease diagnosis. Integration of multimodal imaging data for the automated anatomical assessment in preclinical disease models remains insufficiently explored. This study aimed to develop and validate an AI-assisted framework integrating the magnetic resonance imaging (MRI) and computed tomography (CT) for comprehensive anatomical evaluation of the experimental rat disease models. Count of 180 Sprague-Dawley rats were allocated into healthy con

Differential diagnosis and management of suspected atypical chronic psychogenic dizziness using the International Classification of Functioning, Disability and Health model: a case report.

BACKGROUND: Dizziness in older adults is a complex, often multifactorial condition that can lead to an increased risk of falls, heightened anxiety, and loss of independence. Its nonspecific presentation complicates accurate diagnosis and treatment planning given the potential involvement of vestibular, musculoskeletal, cardiovascular, or psychological systems. PURPOSE: The purpose of this case report is to describe the differential diagnosis process for a patient with atypical chronic dizziness and highlight the role of the International Classification of Functioning, Disability and Health (I

Inhibition of TWEAK/Fn14 Ameliorates Sarcopenic Obesity by Restoring Mitochondrial Biogenesis via the AMPK/SIRT1/PGC-1α Axis.

PURPOSE: Sarcopenic obesity (SO) represents a critical geriatric syndrome where muscle atrophy and obesity synergistically exacerbate metabolic dysfunction. While the TWEAK/Fn14 pathway is a known regulator of muscle wasting, its specific role and therapeutic potential in the pathogenesis of SO remain unclear. METHODS: To mimic the lipid-rich microenvironment of SO, we utilized a "PrePA-Diff" in vitro model where C2C12 myoblasts were exposed to palmitic acid prior to differentiation. In vivo, SO was established by feeding aged (18-month-old) male C57BL/6 mice a high-fat diet (HFD) for 12 week

A Simple Minimum-Setup Pipeline for Using Leg-Worn Inertial Sensors to Track Knee Flexion: Validation on 10 Movements.

Knee motion is a key biomarker in chronic musculoskeletal diseases, yet conventional in-lab optical motion capture falls short of identifying how knee motion continuously impacts joint health outside the lab. Inertial measurement unit (IMU) provides a clinically attractive approach for continuous real-world motion tracking. Our goal was to establish a clinically practical, minimum-setup pipeline for leg-worn IMUs to estimate knee flexion and determine its concurrent validity to optical motion capture during various knee movements. We recorded thigh and shank-worn IMU data with concurrent marke

Systemic immune responses in patients with spine surgery related infections.

INTRODUCTION: Surgical site infections (SSIs) are significant complications that can occur after spine surgery. Compared with systemic infections such as sepsis, or localised musculoskeletal infections such as periprosthetic joint infection, the immune responses in postoperative infection of the spine are poorly understood. A deeper understanding may provide clinically relevant diagnostic or therapeutic options. METHODS: This study examined changes in immune cell profiles in the blood of patients who developed infections after spinal surgery (Infection group, I, n = 17) compared to control pa

Relationship between knee adduction moment and knee contact forces during walking and running with modified foot progression angles.

Knee osteoarthritis (KOA) is a leading cause of disability, influenced by both systemic and mechanical factors. Gait modifications, such as toe-in (TIN) and toe-out (TOUT) foot progression angles, can alter knee adduction moment (KAM) and knee contact forces (KCF), but the task-specific relationship between these measures in walking and running remains unclear. Twenty healthy adults (age 18-35 years) were randomly assigned to walking or running groups. Participants completed natural and modified gait trials: TIN for walking and TOUT for running. KAM was calculated using a clinically feasible i
