A major pitfall to avoid: retroclival hematoma due to odontoid fracture.

BACKGROUND: Retroclival hematoma (RCH) is a rare occurrence. The hemorrhage is usually small and hidden and can be easily missed on CT scan. Here, we report the association of a RCH with an odontoid fracture. METHODS: Case report and review of the literature. RESULTS: We describe a case of a 75-year-old man with a history of squamous cell carcinoma of the tongue base, treated with chemo-radiation. He was on warfarin for atrial fibrillation. He presented to the hospital 6 weeks after falling from standing height, with headache, neck pain, and stiffness. Clinical examination did not show any f

Usefulness of the retromandibular transparotid approach for condylar neck and condylar base fractures.

Condyle fractures represent 20% to 30% of all mandibular fractures and are thus among the most common facial fractures. The fracture pattern can vary greatly and may occur anywhere along the line from the sigmoid notch to the mandibular angle. The main problems are access, difficulty in repositioning the extremely slender fragments, and fixation of the condyle.Eighty-seven patients were diagnosed with condylar neck or condylar base fractures from January 2007 to December 2009 in the Department of Oral & Maxillofacial Surgery of Kyung Hee University Dental Hospital. In this study, we included 3

Closed antegrade intramedullary pinning for reduction and fixation of metatarsal fractures.

The purpose of this study was to present the results of the metatarsal fractures treated with a closed antegrade intramedullary pinning technique. The records of 35 consecutive patients with metatarsal fractures operated on from August 2005 to June 2010 by the authors were retrospectively reviewed. Four patients were not contactable and 1 patient refused to participate. Accordingly, the study cohort was composed of 30 patients (24 male, 6 female) with 46 metatarsal head, neck, or shaft fractures. Inclusion criteria were metatarsal head, neck, or shaft fractures with a displacement of more than

Quantitative measurements of the coronoid in healthy adult patients.

PURPOSE: We investigated the hypothesis that a quantitative 3-dimensional computed tomography (CT) modeling technique that can measure size, shape, and proximal articular surface area can be used to develop formulas that could predict the volume and proximal surface area of the intact coronoid based on anatomical and demographic data available in patients with fracture of the coronoid process. METHODS: We used a consecutive series of 50 CT scans with a slice thickness of 1.25 mm or less obtained in patients with fracture of the distal humerus, but no injury to the coronoid, to create 3-dimens

A non-recurrent inferior laryngeal nerve in a man undergoing thyroidectomy: a case report.

INTRODUCTION: A non-recurrent variant of the inferior laryngeal nerve has been seldom reported. These reports are mostly based on cadaveric dissection studies or large chart review studies in which the emphasis is placed on the determination of the frequency of the variation, and not on the clinical appearance of this variant. We graphically describe the intraoperative identification of a non-recurrent inferior laryngeal nerve. CASE PRESENTATION: A 44-year old Caucasian man was referred to the Head and Neck Surgery Outpatient Clinic with the diagnosis of a nodular mass in his left thyroid lob

Anastomoses between the cervical branch of the facial nerve and the transverse cervical cutaneous nerve.

Head and neck surgeons must possess a thorough knowledge of facial nerve anatomy to avoid inducing iatrogenic injury during surgery. Anastomoses of the cervical branch (CB) of the facial nerve and the transverse cervical cutaneous nerve (TCCN) are poorly documented in our field. Knowledge of these anastomoses and their positions allows for (1) preservation of high CBs that contribute to lower lip depressor function, and (2) identification of the facial nerve in retrograde dissections. Our objective was to improve understanding of facial nerve anatomy by describing these anastomoses. The commun

[Four cases of phrenic nerve paralysis following surgery of thoracic esophageal carcinoma].

Four cases of phrenic nerve paralysis (two cases in right side, two cases in bilateral side) were experienced after surgery of thoracic esophageal carcinoma with extended lymph node dissection in neck and upper mediastinum. In all of the four patients, phrenic nerve paralysis was indicated by difficulties in weaning the patients from respirator, and respiratory management was requiring so many effort that the periods under respirator were prolonged from 12 to 41 days, but all patients recovered from phrenic nerve paralysis with conservative therapy and discharged. Vital capacity of the four pa

Traumatic atlanto-occipital dislocation in children.

BACKGROUND: Traumatic atlanto-occipital dislocation in children and adolescents is a rare and often fatal injury. Although historically most reported cases have been fatal, the advent of modern prehospital care has led to an increase in survival following this injury. As a consequence, some patients may achieve or maintain satisfactory neurologic function following early intervention, stabilization, and definitive management. We analyzed the data on children and adolescents in whom traumatic atlanto-occipital dislocation had been treated with modern resuscitation techniques at our institution.

Fractured Truths: A Collected Review of Modern Insights on Femoral Neck Fracture Management.

Femoral neck fractures (FNFs) remain one of the most devastating injuries in adult patients, associated with a high morbidity, mortality and economic burden worldwide. The global incidence of hip fractures exceeds 1.6 million annually and is projected to rise to between 4.5 and 6.5 million by 2050. FNFs account for approximately 50% of all hip fractures and are strongly associated with aging, osteoporosis and frailty. Mortality rates remain high, approaching 10% within the first month and up to 36% within one year of injury. While displaced FNFs are widely managed with arthroplasty, the role o

Feasibility of "Awake" Extracorporeal Membrane Oxygenation Cannulation Before Intubation in Children.

When the risk of cardiopulmonary arrest during intubation is high, "awake" extracorporeal membrane oxygenation (ECMO) cannulation with sedation and a natural airway may provide hemodynamic stability and gas exchange to mitigate the hazards of intubation. This retrospective case series characterizes patient selection, clinical periprocedural management, and frequency of "awake" pediatric ECMO cannulations in nonintubated patients at a quaternary pediatric hospital. Between 2014 and 2024, 58 patients (7-18 years) with nonpostoperative cardiac indications underwent ECMO support, with six (10%) "a

High tackle headache: implications of referee agreement for tackle height law change.

OBJECTIVES: Rugby Union has a relatively high risk of injury. Early evidence suggests a benefit of lowering tackle height to reduce head and neck injuries, although concerns persist among stakeholders regarding implementation challenges. This study aimed to understand whether referees can reach the same conclusion regarding tackle height in a controlled environment (ie, video) and whether priming influenced these decisions. METHODS: Forty-eight active referees completed a questionnaire based on high-tackle decision-making guidelines after watching tackles. Participants were randomly assigned
