Neoplasms arising at the CIED pocket: a hybrid study combining a case report, scoping review, and clinical survey.

Cardiac implantable electronic device (CIED) implantation is a minor surgical procedure, with a low complication incidence. Neoplasm occurrence at the CIED pocket is a rare event, and sporadic cases are reported without a comprehensive evaluation. This hybrid study aims to fill this gap through a three-part approach: a new case report, a scoping review with individual patient data (IPD) analysis, and a clinical survey. In March 2024, a male patient with a history of high-grade urothelial cancer referred to our Department with a mass at the CIED pocket. Incisional biopsy confirmed the diagnosis

Adult-type epithelial neoplasms in children and adolescents-A retrospective analysis on a subgroup of very rare tumors.

BACKGROUND: Epithelial malignancies, which constitute the majority of adult cancers, are relatively rare in the pediatric population, accounting for only 2% of all childhood tumors. In contrast to common childhood cancers-which typically arise from embryonal or mesenchymal tissues-epithelial malignancies originate from the epithelial lining of glands and include carcinomas of the lung, breast, colon, and genitourinary tract. Understanding the occurrence, behavior, and treatment outcomes of adult-type epithelial malignancies in children is essential for developing age-appropriate management str

Label-free screening and grading of follicular thyroid neoplasms enabled by Fourier transform infrared microspectroscopy and machine learning.

Follicular thyroid neoplasms represent a common subtype of thyroid tumor that has become the most prevalent endocrine neoplasms in recent decades. Accurate diagnosis and grading of these tumors are critical for clinical management of follicular thyroid neoplasms, yet remain challenging due to the limitations of conventional imaging modalities without histopathological molecular information. There is growing interest in analytical techniques that can provide metabolic and molecular insights without the need for exogenous reagents. In such a context, Fourier transform infrared (FTIR) microspectr

Optimal timing of endoscopic submucosal dissection for gastric epithelial neoplasms: impact of preoperative waiting time on clinical outcomes.

BACKGROUND: Endoscopic submucosal dissection (ESD) is considered the standard treatment for gastric epithelial neoplasms (GEN). However, the waiting time between the diagnosis of GEN and undergoing ESD varies among patients, and its effect on clinical outcomes remains to be elucidated. This study aimed to assess the impact of waiting time on clinical outcomes in patients undergoing ESD for GEN. METHODS: This retrospective study included 797 patients with 994 lesions who underwent ESD for GEN between April 2013 and March 2018. Waiting time was defined as the period between endoscopic diagnosis

Cancer Risk After Initial Surveillance of Intraductal Papillary Mucinous Neoplasms (IPMNs): Are We Missing the Window for Prevention?

BACKGROUND: The management of intraductal papillary mucinous neoplasms (IPMNs) has evolved through successive International Association of Pancreatology guidelines, aiming to refine surgical indications and improve cancer prevention. With broader adoption of surveillance, the oncologic outcomes of patients resected after follow-up-and the effectiveness of current strategies in preventing malignancy-remain unclear. This study examined how evolving management has influenced surgical selection and cancer prevention, particularly by comparing patients resected at diagnosis versus after surveillanc

Distinct ALK Expression Patterns Are Associated with Canonical and Noncanonical STRN::ALK Transcript Architectures in Oncocytic Thyroid Neoplasms.

Oncocytic thyroid carcinomas are characterized by a unique chromosomal landscape and frequent resistance to radioactive iodine therapy, underscoring the need for improved molecular characterization. Although ALK rearrangements, particularly STRN::ALK fusions, have been described in thyroid carcinomas, their biological and clinical significance in oncocytic thyroid neoplasms remains unclear. This study investigated STRN exon 3-ALK exon 20-derived transcripts in 56 oncocytic thyroid neoplasms using RT-PCR, followed by cloning and Sanger sequencing of positive cases. Transcript-positive cases wer

AI-powered hierarchical classification of ampullary neoplasms: a deep learning approach using white-light and narrow-band imaging.

BACKGROUND: Endoscopic diagnosis of Ampulla of Vater (AoV) lesions remains challenging owing to complex morphology and limited representative images, particularly for high-risk dysplastic lesions. This study aimed to develop a hierarchical deep learning framework for the stepwise classification of ampullary lesions using white-light (WL) and narrow-band endoscopic images (NBI). METHODS: The framework employs three sequential binary classifications: (1) normal vs. abnormal, (2) adenoma vs. cancer, and (3) high-grade dysplasia (HGD) vs. low-grade dysplasia (LGD) within adenomas. Each stage uses

True Incidence of Appendiceal Neoplasms in Patients Presenting with Complicated Appendicitis: A Call for a Revised Management Algorithm.

BACKGROUND: Prior studies suggest an association between complicated appendicitis and underlying appendiceal neoplasms, but the reported incidence varies widely. This study aimed to determine the incidence and predictors of appendiceal neoplasms among patients with radiographically diagnosed complicated appendicitis. METHODS: This study analyzed adult patients with radiologic diagnoses of complicated appendicitis (perforation, abscess, phlegmon, gangrene, or mucocele) at a tertiary center (2010-2024). To ensure diagnostic accuracy, an expert surgeon reviewed radiologic images with uncertain f

Validation of the Reticulin Algorithm for Adrenocortical Neoplasms: Assessing Diagnostic Accuracy Across All Age Groups and Histological Subtypes.

Adrenocortical carcinoma is a rare and clinically heterogeneous malignancy, often posing difficulty in accurate prognostication. Existing clinicopathological scoring systems, the Weiss, AFIP/Wieneke, and Lin-Weiss-Bisceglia (LWB) criteria, are constrained by their age/subtype-specific applicability and are not suitable for evaluating the rare myxoid and sarcomatoid morphologies or needle biopsies. The Reticulin Algorithm (RA) is a simplified approach for the histopathological assessment of adrenocortical neoplasms, but its prognostic utility requires further validation. We evaluated the progno

Key H&E morphologic features associated with preoperative misdiagnosis of presacral neoplasms: a retrospective slide re-review study.

Presacral neoplasms are rare retrorectal tumors with highly heterogeneous histogenesis and obscure clinical manifestations. Due to their deep anatomical location and non-specific perianal symptoms, they are frequently misdiagnosed as perianal mimics (benign inflammatory anorectal lesions that mimic presacral tumors clinically), leading to delayed intervention, increased malignant progression risk, and compromised oncological prognosis. The present study aimed to identify core H&E-stained morphological features associated with preoperative misdiagnosis of presacral neoplasms, evaluate their val
