The article discusses 94 patients with pathological dislocation of the hip after hematogenous osteomyelitis who underwent reconstructive-restorative operations on 101 joints. According to the clinical and X-ray signs, all patients were divided into 4 main groups. The types of surgical interventions performed depending on the severity of involvement of the proximal part of the femur and the acetabulum are indicated. Patients with acute hematogenous osteomyelitis of the proximal femur must be subjected to early orthopedic prophylaxis and kept under strict medical control of a specialist. Patholo

Analysis of the long-term outcome of slipped capital femoral epiphysis convincingly demonstrates that early diagnosis is essential. Chronic slipping with significant displacement are associated with a high rate of adverse outcomes including necrosis, chondrolysis, and early osteoarthritis. Orthopedic reduction increases the risk of complications. Inadequate reduction is associated with poor hip function and a high risk of early osteoarthritis. Improvements in overall long-term outcome could be achieved only by reducing the number of chronic forms with major displacement, since prognosis is gen

Treatment of the athlete's shoulder is one of the greatest challenges facing the orthopedic specialist. Surgical procedures, and in particular, the open surgical techniques presented in this article, are always a last resort of treatment. The primary treatment is always nonoperative with a particular emphasis toward physiotherapy. In selected instances in which patients have failed rehabilitation, the techniques that are presented can achieve a relatively high degree of success if particular attention is paid to the specific indications, approaches, and details of each technique. Open surgical

The circumstances of subtrochanteric fractures following the use of multiple pins for fixation of femoral neck fractures were reviewed in four women (age range, 67-94 years). Internal fixation by use of multiple pins in the proximal femur is a common orthopedic procedure. The pins, however, create a significant stress riser in the lateral subtrochanteric cortex. Subtrochanteric fracture has been reported after internal fixation of slipped capital femoral epiphysis with multiple conventional pins and, recently, after internal fixation of femoral neck fractures with large Garden screws.

Traumatic subluxation and dislocation of the extensor digitorum tendons are uncommon in patients without rheumatoid disorders. Management of the acute injury is not well defined in the orthopedic literature. Two cases of traumatic dislocations of the extensor digitorum tendon were seen acutely in young persons without rheumatoid disease. These patients were successfully managed by early closed reduction and immobilization. This treatment had yielded good hand function to date, without recurrence of the dislocation. One of these cases was particularly unusual in that the direction of the tendon

The main use of the penicillins in orthopedic surgery is in the treatment of infections due to Hemophilus influenzae, Staphylococcus aureus, Pseudomonas aeruginosa, Neisseria gonorrhoeae, Escherichia coli, Proteus mirabilis, Streptococcus pneumoniae, and Group D streptococci (enterococci). Penicillins have antimicrobial activity and have a characteristic pharmacodynamic action, including side effects. The tissue penetration characteristics of the penicillins into synovial fluid and human bone are significant. Semisynthetic penicillins, antistaphylococcal penicillins, and the antipseudomonal pe

Slipped capital femoral epiphysis is a common malady which must be managed by virtually every orthopedic surgeon. While each of us has a "foolproof method" of managing the disease, we also have somewhere in our files a patient with a disastrous outcome. The next several instructional cases will focus on the trials and tribulations of this disorder. Each of the discussants has had to formulate his opinions regarding management at each step in the case presentation, without knowledge of the subsequent outcome. We hope through this process we all will learn from our own and others' mistakes.

Rickets secondary to hepatobiliary disease is a rare condition; however, the skeletal manifestations are similar to simple nutritional rickets. These skeletal changes include epiphyseal plate abnormalities, pseudofractures, bowing of long bones, scoliosis, and fractures. One-hundred and twelve charts and radiographs of patients with chronic hepatobiliary disease admitted to Children's Hospital of Michigan from 1968 to 1978 were reviewed. Nineteen, or 17%, of these patients showed roentgenographic evidence of rickets. Orthopedic management is primarily concerned with the treatment of fractures

The ESR, a sensitive measure of the inflammatory response, is elevated in 90% of patients who have serious orthopedic infections, e.g., discitis, septic arthritis, and hematogenous osteomyelitis. In most cases, it is sufficient to distinguish these entities from the less serious disease states, i.e., transient synovitis. After major surgical operations or extensive trauma, the ESR often increases to high levels and, if sepsis does not supervene, it returns to normal within six months. Outpatient determination of the ESR, although useless in the early detection of malignancy, is valuable in det

Diastrophic dwarfism is an unusual variety of short-limb dwarfism characterized by the pathognomic triad of ear calcifications, hitch-hiker thumbs, and severe clubfoot. Other clinical features include joint contractures and subluxations, symphalagism of the fingers, kyphoscoliosis, hip dysplasia, and occasional cleft palate. Radiographic findings include flaring of the metaphyses of tubular bones and flattened epiphyses which appear late. The major orthopedic problem in diastrophic dwarfism is correction of the recalcitrant clubfoot deformity; cervical spine subluxation or kyphoscoliosis may a

INTRODUCTION: Tendon xanthomas are cholesterol-rich deposits associated with disorders of lipid metabolism, most classically familial hypercholesterolemia. Although primarily a dermatological or metabolic finding, they often present to orthopedic surgeons because of functional impairment, cosmetic concerns, or diagnostic uncertainty arising from a mass-like swelling.
CASE REPORT: A 33-year-old woman presented with a 3-year history of progressive, painful swelling over both Achilles tendons, worse on the right, with difficulty walking and wearing shoes. Examination revealed firm-to-hard masses

PURPOSE: This study aimed to evaluate the outcomes of surgical treatment for extremity soft tissue sarcomas in a country without a legislatively designated sarcoma center and to assess the impact of non-centralized surgical care on local recurrence, amputation rates, and patient survival. METHODS: A total of 97 patients treated over a five-year period at our orthopedic oncology unit, with a minimum follow-up of 24 months, were included. Patients were stratified into four groups according to the time point at which they were referred from regional hospitals to our department (before biopsy, aft

OBJECTIVE: The Win Ratio (WR) is a novel approach to analyze composite outcomes that accounts for the relative priority of each outcome. In this study, we evaluated use of the WR for composite outcomes in suicide prevention studies to overcome challenges associated with low base rates of primary outcomes.
METHOD: We illustrate the utility of the WR in suicide prevention studies through two re-analyses: 1) data from the Emergency Department Safety Assessment and Follow-up Evaluation (ED-SAFE) trial, and 2) Medicaid claims data comparing youths with concussions to those with orthopedic injuries

BACKGROUND: Patient-level value analysis (PLVA) demonstrates promise in measuring value, or outcomes achieved per dollar spent, of interventions such as orthopedic sport surgeries. Despite prior applications, PLVA has never been implemented in anterior cruciate ligament (ACL) reconstruction (ACLR). The purpose of this study was to apply PLVA in the setting of an ACLR 2-year episode of care and identify factors that influence value.
MATERIALS AND METHODS: The patient-reported outcome measure (PROM) database of an ambulatory surgical center was queried for ACLR procedures from 2009 to 2022. Any

BackgroundMajor lower extremity amputations (LEA) are frequently performed by vascular, trauma, and orthopedic surgeons, yet comparative outcomes across services and the role of frailty remain unclear.Materials and MethodsWe conducted a single-center retrospective review of adults undergoing major LEA (above-knee, through-knee, and below-knee) from 2015-2022. Frailty was assessed using the 5-factor modified Frailty Index (mFI-5). Outcomes included complications, length of stay (LOS), readmission, re-amputation, mortality, and prosthetic ambulation. We evaluated 30-day return to the operating r

INTRODUCTION: Penicillin allergy is a risk factor for infection in an elective orthopedic surgical procedure, but this has not been studied in orthopedic trauma. We hypothesize there will be an increase in deep surgical site infections in patients sustaining lower extremity fractures with documented penicillin allergy.
METHODS: A retrospective cohort study was performed at an academic, level-one trauma center of patients who underwent surgical fixation of lower extremity fractures. Cohorts were divided based on the documented presence of a penicillin allergy in the medical record. The primary

PURPOSE: Following shoulder dislocation, injuries such as rotator cuff tear (RCT), axillary nerve injury, greater tuberosity (GT) fracture, Hill-Sachs lesion, and Bankart lesion frequently occur. This study aimed to develop and validate a clinical prediction model for the occurrence of RCT after acute anterior shoulder dislocation. METHODS: This study included all patients with acute anterior shoulder dislocation who presented to the emergency orthopedics department of the same institution from July 2021 to March 2025. Lasso regression was used to select independent variables for the model. A

BACKGROUND: Although conventional and metastatic femoral neck fractures (FNF) represent distinct patient populations in routine clinical practice, treatment management is generally similar for both groups. Systematic treatment approaches for FNF are currently used and supported by clinical guidelines; however, patients with metastatic FNF are generally managed according to treatment protocols developed for conventional FNF. The aim of this study was to determine whether the treatment strategy for conventional FNF is effective for patients with metastatic FNF.
METHODS: This retrospective study
