[Surgical treatment of facial palsy].

The results of surgical treatment of 46 cases of facial nerve paralysis from 1974 to 1987 were reported. Among them, facial nerve decompression comprised 36 cases and facial nerve grafting 10 cases. The causes of facial nerve paralysis were head trauma in 13, surgical injury in 22, otitis media in 4, facial neurofibroma in 2 and Bell's palsy in 5. Thirty-six cases were followed up from 0.5 to 6.5 years. Twenty-five cases (69.5%) had totally recovered, 7 cases (19.4%) partially recovered and 4 cases (11.1%) had no recovered. The result of facial nerve decompression was better than that of facia

Epidemiological studies of Streptococcus pneumoniae in infants: antibody to types 3, 6, 14, and 23 in the first two years of life.

Antibodies to pneumococcal types 3, 6, 14, and 23 were measured in sera from 78 infants prospectively studied from birth. Mean levels of antibodies to capsular antigens were 2-4 micrograms/mL, with no overall differences between carriers and noncarriers of given types. Serial serum samples were studied in selected infants to more precisely define the antibody response in relation to specific pneumococcal colonization and infection. Although some infants had little antibody, and made little in response to exposure, others had demonstrable antibody at the onset of acute otitis media. The highest

Acute myelogenous leukemia presenting as atypical mastoiditis with facial paralysis.

Symptomatic otologic involvement by leukemic infiltration is unusual, most often occurring in the already-diagnosed leukemic patient as postauricular mass, acute hemorrhagic otitis media, mastoiditis, cranial neuropathy, vertigo, hearing loss, or leptomeningitis. We think ours is the first reported patient whose leukemia presented as atypical mastoiditis and facial paralysis due to granulocytic sarcoma (chloroma). At mastoidectomy, tan lobulated rubbery tumor filled the mastoid antrum and middle ear cleft. Complete remission and full return of facial nerve function was achieved with 2500 R loc

Tympanometric changes after total maxillary osteotomy.

With the frequency of orthognathic surgery increasing each year, more is being learned about this surgical art. Although techniques are improving, more acute and chronic complications are being reported after orthognathic surgery. In our institution, it has been noticed that approximately 25% to 30% of our patients experience unilateral or bilateral otalgia after total (Le Fort I) maxillary osteotomy. Recently, two patients with maxillary osteotomies. In both patients acute otitis media developed postoperatively and myringotomies for relief of symptoms were required. This paper reports changes

Otogenic Aspergillus Brain Abscess in an SLE teen: Successful case management from Eastern India.

Neuroaspergillosis is a rare, high-mortality complication in Systemic Lupus Erythematosus (SLE). We report an otogenic Aspergillus fumigatus brain abscess in a 17-year-old female with SLE and seizure disorder, managed with immunosuppressants. She presented with altered sensorium and hemiparesis. Despite initial aspiration and combination antifungal therapy (voriconazole and liposomal amphotericin B), clinical worsening demanding radical excision. Culture and sequencing confirmed A. fumigatus, likely spread via chronic otitis media. The patient recovered and was discharged on oral posaconazole,

High-definition otoscopic device for humanitarian mission: how i do it.

BACKGROUND: Despite a high prevalence of chronic otitis media and related complications, many African dispensaries and clinics lack microscopes or fiberoptic equipment for examining external ear ducts and tympanic membranes. METHOD: An alternative, inexpensive, and readily available device designed for ear wax removal is presented as a clinical ear and anterior nasal cavity examination tool. The device connects to Wi-Fi or cellular networks, providing high-definition images of the ear and anterior nasal cavity through a smartphone interface. Its use, utility, strengths, and limitations are di

Formula Otorrhea in Pediatric Patients With Cleft Palate.

The case series details 2 unusual cases of male newborns with cleft lip and palate (CLP) that later developed formula otorrhea. Both patients underwent bilateral myringotomies with the insertion of pressure equalizing (PE) tubes for chronic otitis media with effusion (OME). Chronic otorrhea associated with feeding occurred post-PE tube insertion and the otorrhea was later confirmed to be due to reflux of formula. Patients were treated with antibiotic ear drops, routine ear cleaning, anti-reflux medication, and reflux precautions. After definite cleft palate repair, formula otorrhea completely

Endophthalmitis due to Delftia acidovorans: An unusual ocular pathogen.

Endophthalmitis is a dreaded postoperative complication of cataract surgery. Delftia acidovorans is usually nonpathogenic and an unusual ocular pathogen. Isolated reports of delftia-associated sepsis, otitis media, endocarditis, keratitis, etc. exist in literature. We report a rare and unique case of delftia-related endophthalmitis in a 67-year-old male diagnosed 2 weeks after uneventful cataract surgery. He was treated successfully with core vitrectomy and intravitreal antibiotics. Microbiological evaluation of vitreous sample identified the causative organism as Delftia acidovorans. Post-vit

The biofilm-associated bacterial infections unrelated to indwelling devices.

Biofilms are microbial communities established in the self-produced extracellular substances that include up to 80% of associated microbial infections. During biofilm formation, bacterial cells shift from the planktonic forms to aggregated forms surrounded by an extracellular polymeric substance. The bacterial biofilm shows resistance against immune reactions as well as antibiotics and is potentially able to cause disorders by both device-related and nondevice-related infections. The nondevice-related bacterial biofilm infections include dental plaque, urinary tract infections, cystic fibrosis

Chronically Discharging Ears: Evalution with High Resolution Computed Tomography.

Chronic suppurative otitis media (CSOM) refers to the middle ear inflammation which is clinically characterized by the discharging ear, hearing deficit, fever and otalgia. Although a clinical diagnosis, imaging is imperative to rule out associated complications which apart from causing hearing deficit, may prove fatal at times. Both high resolution computed tomography (HRCT) and MRI are helpful in evaluating middle ear pathologies, usage being indication specific. Due to its excellent spatial resolution, HRCT is invaluable in assessment of chronically discharging ears, especially to look for b

Tuberculous Otomastoiditis.

Tuberculous otitis media and mastoiditis, or tuberculous otomastoiditis, is a rare but well-described infectious process occasionally affecting individuals in the United States but more frequently seen in countries where tuberculosis is endemic. Infection may be primary and occur through mucus aspirated through the Eustachian tube. Alternatively, organisms may secondarily infect the nasopharynx when expectorated from the lungs and, less frequently, may be hematogenously spread. Chronic otorrhea and hearing loss are common symptoms, and extensive perforation of the tympanic membranes and facial

Interim results of an ecological experiment - Conjugate vaccination against the pneumococcus and serotype replacement.

Streptococcus pneumoniae has more than 95 serotypes, each of which presumably can cause sepsis, meningitis, pneumonia, and acute otitis media. Pneumococcal conjugate vaccines (PCV) targeted against a limited number of serotypes have nonetheless revealed an impressive impact on each manifestation of pneumococcal disease. At the same time, growing evidence of significant non-vaccine type (NVT) replacement disease following implementation of infant PCV programs has raised questions about the long-term viability of PCV immunization strategies and how to optimize PCV formulations. We discuss here t

Community-acquired Pseudomonas meningitis causes acute obstructive hydrocephalus.

INTRODUCTION: Pseudomonas aeruginosa (PS) infection is serious in children and can cause malignant external otitis, endophthalmitis, endocarditis, meningitis, pneumonia, and septicemia (Huang et al. Pediatr Infect Dis J 1). The treatment of Pseudomonas infection requires prompt medical evaluation and appropriate antibiotic treatment. CASE REPORT: We report the case of a 6-month-old boy with an unusual presentation of acute obstructive hydrocephalus owing to Pseudomonas meningitis. Treatment with optimal antibiotic begun immediately after the pathogen was recognized and continued for 4 weeks.

[Surgical treatment of exostosis in the external auditory canal].

Exostoses of the external auditory canal are benign bony tumours very common in individuals who frequently participate in aquatic activities. Although most of the cases are asymptomatic, patients with more severe exostoses have recurrent episodes of external otitis and related conductive hearing loss. In the great majority of these cases, a medical treatment (aspiration and antibiotic drops) resolves the symptoms. Patients with more severe canal stenosis, resistant to medical treatment, are candidates for surgical removal of the exostoses. This report reviews our surgical experience with 45 pa
