Navigating ambulatory perioperative care in patients with obesity and obstructive sleep apnea: a narrative review of ambulatory surgery outcomes and opioid safety.

Obesity and sleep apnea contribute to significant challenges in ambulatory surgical centers which include airway management and perioperative recovery. This narrative review was written following a literature review of articles available on ScienceDirect, PubMed, and Google Scholar from 2009 to 2024. This article highlights that obesity-related anatomical changes, such as increased neck circumference (NC) and reduced cervical extension, may complicate airway management. Successful ambulatory surgery for these high-risk patients depends on detailed attention throughout the perioperative period.

The Real Safety Net Hospitals: High-Medicaid Hospitals Confer Worse Survival for Patients Undergoing Pancreas and Biliary Cancer Surgery.

BACKGROUND: Large-database cancer research often defines hospitals servicing a threshold number of Medicaid patients "safety net"; however, counts are generally with the cancer of interest, and because sufficient rare/complex cases such as hepatobiliary/pancreatic (HPB) cancers tend to be managed at larger institutions, "safety net" may be skewed toward academic centers. Recent data show similar survival for "safety net" HPB care. We examine patterns/outcomes of curative intent surgery (CIS) for HPB cancers by a more comprehensive Medicaid-serving (AID) status utilizing all alimentary tract ca

Evaluating the safety and feasibility of allogeneic NK cell infusion in high-risk lymphoma patients post-autologous stem cell transplantation.

Lymphoma, a cancer with poor prognosis is a growing global health challenge that encompasses two primary types, Hodgkin (HL) and non-Hodgkin lymphoma (NHL), each further divided into various subtypes with distinct biological behaviors. Conventional therapeutic strategies include chemotherapy, radiation, surgery, and autologous hematopoietic stem cell transplantation (auto-HSCT). Natural killer (NK) cells exhibit intrinsic cytotoxicity against tumor cells without the need for prior immunization or activation. In this prospective clinical trial, we evaluated the feasibility of allogeneic NK cell

Efficacy and safety of direct oral anticoagulants (DOACs) for postoperative thromboprophylaxis in patients after bariatric surgery: a systematic review and meta-analysis.

Bariatric surgery is effective for achieving substantial weight loss and improving metabolic comorbidities in patients with severe obesity. However, both obesity and bariatric surgery elevate the risk of thromboembolic events. Traditionally, venous thromboembolism (VTE) prophylaxis in individuals who undergo bariatric surgery has relied on heparins, particularly enoxaparin. Concerns regarding pharmacologic limitations linked to the adipose tissue-related impact on the drug's pharmacodynamics in high-BMI patients have led to increased interest in direct oral anticoagulants (DOACs) as a potentia
