Assessment of prehospital tracheal intubation technique using initial direct laryngoscopy during videolaryngoscopy: randomized controlled simulated trial.

BACKGROUND: In critically ill patients, tracheal intubation may be required in the prehospital setting, where airway management presents unique technical and logistical challenges. While videolaryngoscopy has emerged as a potential alternative to direct laryngoscopy by providing a better and easier visualization of the glottis, the improved view of anatomical structures does not necessarily correlate with successful tracheal tube placement. Intubation may be harder because novice providers performing videolaryngoscopy may only look at the screen and only obtain a two-dimensional representation

Variability in oxygen delivery with bag-valve-mask devices: An observational laboratory simulation study.

A bag-valve-mask (BVM) is a portable handheld medical device commonly used in airway management and manual ventilation. Outside of the operating theatre, BVM devices are often used to pre-oxygenate spontaneously breathing patients before intubation to reduce the risk of hypoxaemia. Pre-oxygenation is considered adequate when the end-tidal expiratory fraction of oxygen is greater than 0.85. There are reports that some BVM devices fail to deliver a satisfactory inspired oxygen (FiO2) in spontaneously breathing patients due to variability in design. The primary aim of this study was to evaluate t

Hemodynamic effects of myocardial bridging in patients with hypertrophic cardiomyopathy.

Myocardial bridging (MB) is linked to angina and myocardial ischemia and may lead to sudden cardiac death in patients with hypertrophic cardiomyopathy (HCM). However, it remains unclear how MB affect the coronary blood flow in HCM patients. The aim of this study was to assess the effects of MB on coronary hemodynamics in HCM patients. Fifteen patients with MB (7 HCM and 8 non-HCM controls) in their left anterior descending (LAD) coronary artery were chosen. Transient computational fluid dynamics (CFD) simulations were conducted in anatomically realistic models of diseased (with MB) and virtual

Proton dose calculation with transformer: Transforming spot map to dose.

BACKGROUND: Conventional proton dose calculation methods are either time- and resource-intensive, like Monte Carlo (MC) simulations, or they sacrifice accuracy, as seen with analytical methods. This trade-off between computational efficiency and accuracy highlights the need for improved dose calculation approaches in clinical settings. PURPOSE: This study aims to develop a deep-learning-based model that calculates dose-to-water (DW) and dose-to-medium (DM) using patient anatomy and proton spot map (PSM), achieving approaching MC-level accuracy with significantly reduced computation time. Leveraging Model-Based Simulations to Optimize Extended Dosing of Leuprolide 6-Month Intramuscular Depot Formulation.

BACKGROUND AND OBJECTIVE: A gonadotropin-releasing hormone (GnRH) agonist such as leuprolide is widely used to achieve sustained suppression of testosterone levels, which play a critical role in the treatment of prostate cancer. Recent advances in drug delivery systems have led to the development of long-acting depot formulations, such as the 6-month intramuscular (IM) leuprolide formulation, which aim to simplify dosing and improve convenience for both patients and healthcare providers. Exploring extended dosing intervals for such formulations represents a promising approach to further optimi

Learning hemodynamic scalar fields on coronary artery meshes: A benchmark of geometric deep learning models.

Coronary artery disease involves the narrowing of coronary vessels due to atherosclerosis and is currently the leading cause of death worldwide. The gold standard for its diagnosis is the fractional flow reserve (FFR) examination, which measures the trans-stenotic pressure ratio during maximal vasodilation. However, the invasiveness and cost of this procedure have prompted the development of computer-based virtual FFR (vFFR) computation, which simulates coronary flow using computational fluid dynamics (CFD) techniques. Geometric deep learning algorithms have recently shown the capability to le

Cost-Utility Analysis of Rituximab vs Mycophenolate Mofetil for the Treatment of Pemphigus Vulgaris.

IMPORTANCE: There is an increasing body of literature that supports the use of rituximab as a first-line steroid-sparing agent in pemphigus vulgaris. However, the cost of rituximab is substantial compared with conventional agents, and there are limited health economic data to justify its use. OBJECTIVE: To evaluate the cost-effectiveness of rituximab biosimilars relative to mycophenolate mofetil as a first-line steroid-sparing agent for moderate to severe pemphigus vulgaris. DESIGN, SETTING, AND PARTICIPANTS: A cost-utility analysis over a 24-month time horizon was conducted from the perspec

Retention of cricothyrotomy skills by paramedics using a wire guided technique.

INTRODUCTION: Cricothyrotomy may be necessary for airway management when a patient's airway cannot be maintained through standard techniques such as oral airway placement, blind insertion airway device, or endotracheal intubation. Wire-guided cricothyrotomy is one of many techniques used to perform a cricothyrotomy. Although there is some controversy over which cricothyrotomy technique is superior, there is no published data regarding long term retention rates. The purpose of this study is to determine whether ground based paramedics can be taught and are able to retain the skills necessary to

Engineering inertial flow patterns for signal amplification in disc-based protein assays.

UNLABELLED: Detecting low-abundance proteins is essential in liquid biopsy applications, particularly for extracellular vesicle (EV)-based diagnostics. Here, we introduce RapidEx (Rotationally amplified protein intensity detection of Extracellular vesicles), a disc-fluidic system engineered for highly sensitive protein assays. RapidEx performs two key functions within a single disc cartridge: i) enzymatic chemical reactions for target labeling and ii) efficient washing to enhance signal-to-background contrast. METHODS: The device leverages a unique interplay between disc-specific inertial for

Dysfunctional neural processing of object-related speech with semantically unrelated vs related co-verbal gesture in schizophrenia spectrum disorders.

BACKGROUND: Schizophrenia spectrum disorders (SSD) are accompanied by impaired multimodal communication. This is particularly evident in production and comprehension deficits of both speech and gesture. Previous neuroimaging findings indicate reduced activation in sensorimotor regions, such as the left postcentral gyrus and right insula, during object-related speech processing in SSD (He et al., 2021). However, it remains unclear how semantic congruency of co-verbal gestures influences the neural processing of object-related speech. This study aims to investigate the effect of semantically rel

Cost-effectiveness of DPYD genotyping prior to capecitabine administration for metastatic breast cancer.

PURPOSE: Patients with a DPYD genetic deficiency who receive capecitabine are at increased risk of severe, potentially fatal toxicities due to impaired drug metabolism. Genetic testing for this deficiency allows for proactive dose adjustments to mitigate these risks. We evaluated the cost-effectiveness of DPYD genotyping prior to capecitabine administration, followed by dose modification for patients with metastatic breast cancer. METHODS: We developed a state-transition model to simulate health outcomes and costs for a cohort of 62-year-old women with metastatic breast cancer from the perspe

Association of postoperative delirium with haemodynamic determinants of cerebral perfusion pressure during cardiac surgery: a retrospective cohort study.

BACKGROUND: Delirium is common after cardiac surgery, and an important factor could be cerebral perfusion. As cerebral perfusion pressure (CPP) is determined by the difference between mean arterial pressure (MAP) and central venous pressure (CVP), which are modifiable factors, understanding their relationship with delirium is essential. METHODS: In a retrospective cohort study, patients undergoing cardiac surgery were assessed for delirium using the Confusion Assessment Method for the ICU. Haemodynamic exposures were calculated as time spent in narrow ranges: 14 MAP ranges in increments of 5

A three-dimensional printed laryngoscope holding helmet for continuous laryngoscopy during exercise in a pediatric exercise induced laryngeal obstruction clinic.

INTRODUCTION: Continuous laryngoscopy during exercise (CLE) is increasingly used to diagnose exercise induced laryngeal obstruction (EILO). One barrier to widespread implementation is reliable fixation of the flexible laryngoscope during exercise. This study describes the development and validation of an adjustable, low-cost helmet constructed from commercially available and 3D-printed components to facilitate CLE in a pediatric EILO clinic. MATERIALS AND METHODS: An adjustable CLE helmet was developed using 3D-printed and commercially available materials. Eight otolaryngologists at a tertiar

On the reduction of imaging time-points for dosimetry in radionuclide therapy.

BACKGROUND: The aim was to develop a theoretical framework for how errors in estimated activities propagate to a dispersion in time-integrated activity in radionuclide-therapy dosimetry and how this affects the comparison of radionuclide-therapy dosimetry schemes. METHODS: Formulae for the variance of relative errors of estimated time-integrated activities and relative differences in time-integrated activities between measurement schemes when one or more time-points are removed were derived using the law of propagation of uncertainty for a population of time-activity-curve parameters.
