Local drug delivery in the treatment of periodontal diseases: assessing the clinical significance of the results.

BACKGROUND: Local drug delivery can be used in the management of periodontal patients. However, statistically significant responses to therapy recorded in clinical trials may not be clinically significant. METHODS: Controlled clinical trials were selected that assessed the capability of local drug delivery to improve periodontal health. RESULTS: Several local drug delivery systems employed as monotherapies improved periodontal health and provided results that were not statistically significantly different than attained with scaling and root planing (SRP) alone. In contrast, many local drug d

Autoantibodies directed to extracellular matrix components in patients with different clinical forms of periodontitis.

BACKGROUND: Periodontal disease occurs in different clinical forms, from mild and easily controllable to more aggressive inflammatory manifestations, with difficult clinical or surgical control. There is evidence that a local autoimmune reaction may participate in the onset and persistence of the aggressive forms of periodontal disease. As the underlying mechanism in this process is not fully understood, we decided to investigate whether patients bearing this form of disease presented higher levels of antibodies directed to extracellular matrix (ECM) components such as type I collagen, fibrone

Development of tetracycline-serratiopeptidase-containing periodontal gel: formulation and preliminary clinical study.

The purpose of this research was to reduce the polymer concentration and to obtain reasonable viscosity at a lower concentration of pluronic by the addition of a viscosity modifier. A 20% wt/wt pluronic gel was prepared on a weight basis using the cold method. The effect of the amount of tetracycline and Aerosil on gel properties was studied. The gel was evaluated using different parameters: polarizing microscopy, gelation, gel melting, bioadhesivity, viscosity, drug release, and stability of enzyme. An in vivo study was performed to evaluate the clinical efficiency of the liquid crystalline g

The effects of periodontal instrument handle design on hand muscle load and pinch force.

BACKGROUND: In comparison with people in other occupations, dentists and dental hygienists are at increased risk of developing work-related musculoskeletal disorders, including carpal tunnel syndrome. An important risk factor in dental practice is forceful pinching, which occurs during dental scaling. Ergonomically designed dental instruments may help reduce the prevalence of MSDs among dental practitioners. METHODS: In the authors' study, 24 dentists and dental hygienists used 10 custom-designed dental scaling instruments with different handle diameters and weights to perform a simulated sca

Gingival fibroblasts are better at inhibiting osteoclast formation than periodontal ligament fibroblasts.

Various studies indicate that periodontal ligament fibroblasts (PLF) have some similarities to osteoblasts, for example they have the capacity to induce the formation of osteoclast-like cells. Here, we investigated whether a second population of tooth-associated fibroblasts, gingival fibroblasts (GF), has similar osteoclastogenesis properties. PLF and GF were co-cultured with peripheral blood mononuclear cells (PBMC) in the presence and absence of dexamethasone and 1alpha,25dihydroxycholecalciferol (dex + vit D(3)) on plastic and on cortical bone slices. Inflammation-induced bone remodeling in periodontal disease and the influence of post-menopausal osteoporosis.

During physiological conditions, the skeleton is remodeled in so-called bone multi-cellular units. Such units have been estimated to exist at 1-2 x 10(6) sites in the adult skeleton. The number and activities of these units are regulated by a variety of hormones and cytokines. In post-menopausal osteoporosis, lack of estrogen leads to increased numbers of bone multi-cellular units and to uncoupling of bone formation and bone resorption, resulting in too little bone laid down by osteoblasts compared with the amount of bone resorbed by osteoclasts. Inflammatory processes in the vicinity of the s

Identification of candidate periodontal pathogens and beneficial species by quantitative 16S clonal analysis.

Most studies of the bacterial etiology of periodontitis have used either culture-based or targeted DNA approaches, and so it is likely that pathogens remain undiscovered. The purpose of this study was to use culture-independent, quantitative analysis of biofilms associated with chronic periodontitis and periodontal health to identify pathogens and beneficial species. Samples from subjects with periodontitis and controls were analyzed using ribosomal 16S cloning and sequencing. Several genera, many of them uncultivated, were associated with periodontitis, the most numerous of which were gram po

Using a Delphi panel to survey criteria for successful periodontal therapy in posterior teeth.

BACKGROUND: Health care providers have focused on outcome measures to determine the success or failure of treatment. In periodontics, there is no consistent view regarding what outcome measure(s) is(are) important for successful treatment. This study used a Delphi surveying technique to determine which outcome measures were clinically relevant to an expert panel of periodontists. METHODS: The Delphi surveyed 35 periodontists from the United States to determine the level of importance of attachment level, probing depth, furcation involvement, mobility, plaque, inflammation, esthetics, pain, an

Quantitative measures of aggressive periodontitis show substantial heritability and consistency with traditional diagnoses.

BACKGROUND: Aggressive periodontitis (AgP) research nearly always classifies subjects into traditional discrete categories of localized or generalized, based upon degree of attachment loss (AL) and types of affected teeth. Since AL is continuous and quantitative, however, useful information is lost. We developed quantitative measures of AgP, compared these to traditional methods, and estimated heritabilities in families. METHODS: We examined 237 healthy, 169 localized AgP, and 204 generalized AgP subjects. We used the site of maximum AL of each tooth to calculate means for each subject for di

Molecules contributing to the maintenance of periodontal tissues. Their possible association with orthodontic tooth movement.

This review is aimed at providing a depiction of molecules and their topography which characterize native gingiva and PDL fibroblasts, to describe their function in tissue maintenance, and to discuss their possible modulation due to orthodontic tooth movement. Maintenance of the human periodontium requires the balance of proliferation and differentiation in the respective tissues' cells. Moreover, the cells must synthesize the extracellular matrix molecules and receptors that facilitate adhesion. To describe the molecules that contribute to periodontal tissue maintenance, we illustrate the loc
