Redesigning the care of fragility fracture patients to improve osteoporosis management: a health care improvement project.

OBJECTIVE: To develop new processes that assure more reliable, population-based care of fragility fracture patients. METHODS: A 4-year clinical improvement project was performed in a multispecialty, community practice health system using evidence-based guidelines and rapid cycle process improvement methods (plan-do-study-act cycles). RESULTS: Prior to this project, appropriate osteoporosis care was provided to only 5% of our 1999 hip fracture patients. In 2001, primary physicians were provided prompts about appropriate care (cycle 1), which resulted in improved care for only 20% of patients.

Artificial intelligence for early sepsis detection and dynamic prognostication in onco-critical care.

Critically ill cancer patients have a unique physiological profile marked by severe immunosuppression, frailty, and multimorbidity, making traditional tools like Acute Physiology and Chronic Health Evaluation II or Sequential Organ Failure Assessment often inadequate for accurate risk assessment. This review explores artificial intelligence's potential to transform onco-critical care from reactive to predictive management. We will synthesize literature on two key applications: Early sepsis detection in critically ill cancer patients and refining mortality prediction models to guide ethical car

Associations of body mass index and waist circumference with incidence of overall and of 27 site-specific cancers: a population-based retrospective cohort study.

BackgroundOverweight and obesity are known risk factors for cancer. The aim of this study was to investigate associations of body mass index (BMI) and waist circumference (WC) with incidence of 27 site-specific cancers stratified by sex and menopausal status accounting for non-linearity.MethodsWe performed a population-based retrospective cohort study using the Korean National Health Insurance Service (KNHIS 2009-2020) database. We included 3,986,155 participants (aged ≥ 20 years), and the mean follow-up duration was 9.0 ± 1.6 years. The primary outcome was the incidence of cancer.ResultsThere

A model of prevention of mother-to-child transmission and health management team for improving adverse outcomes of pregnancy syphilis in Ningxia, China.

Regional variations exist in the implementation of Syphilis Mother-to-Child Transmission Prevention (PMTCT). Thus, it is crucial to assess the effectiveness of this model in the Ningxia region and explore the supplementary role of Health Management Teams (HMT). This study established the PMTCT + HMT model and examined its impact on adverse outcomes in pregnant women with syphilis infection. The majority of participants were urban residents, married, had a minimum high school education, and held public positions; 36.7% and 26.7% were from minority ethnic groups. The PMTCT + HMT model enhanced p

Knowledge, Perceptions, and Use of Psychedelics among Individuals with Fibromyalgia.

Fibromyalgia (FM) is a difficult to treat chronic pain condition for which there is strong interest in alternative treatments. There is growing interest in the potential of psychedelic substances (e.g., psilocybin) in conjunction with psychotherapy to treat chronic pain. Via a cross-sectional, anonymous, online survey, we aimed to characterize knowledge, perceptions, and past use of serotonergic ("classic") and non-serotonergic psychedelics among a population of individuals with FM, and to investigate interest in psychedelic-based FM treatments. Among a North American population of 354 partici

Morbidity and mortality in orthostatic hypotension.

This review summarizes the current literature on the epidemiology of orthostatic hypotension (OH) in the elderly and in patients with autonomic impairment also known as neurogenic OH (nOH); these two conditions have distinct pathophysiologies and affect different patient populations. The prevalence of OH in the elderly varies depending on the study population. In community dwellers, OH prevalence is estimated at 16%, whereas in institutionalized patients, it may be as high as 60%. The prevalence of OH increases exponentially with age, particularly in those 75 years and older. Multiple epidemio

Rapidly assessing the risks of infectious diseases to wildlife species.

Predicting the likelihood of rare events is increasingly demanded by risk managers. A key challenge is dealing with different types of uncertainty, including epistemic uncertainties (lack of knowledge), stochasticity (inherent randomness) and natural variation. One potentially catastrophic event which is impacted by high levels of all three of these uncertainty types is the transmission of livestock pathogens to wildlife, particularly for endangered species. There is often a lack of basic information, e.g. about a given pathogen's presence in local livestock populations or the susceptibility o

Radiation risk of individual multifactorial diseases in offspring of the atomic-bomb survivors: a clinical health study.

There is no convincing evidence regarding radiation-induced heritable risks of adult-onset multifactorial diseases in humans, although it is important from the standpoint of protection and management of populations exposed to radiation. The objective of the present study was to examine whether parental exposure to atomic-bomb (A-bomb) radiation led to an increased risk of common polygenic, multifactorial diseases-hypertension, hypercholesterolaemia, diabetes mellitus, angina pectoris, myocardial infarction or stroke-in the first-generation (F1) offspring of A-bomb survivors. Insecticide resistance in vector mosquitoes in China.

Because of their special behaviour, physiology and close relationship with humans, mosquitoes act as one of the most important vectors of human diseases, such as filariasis, Japanese encephalitis, dengue and malaria. The major vector mosquitoes are members of the Culex, Aedes and Anopheles genera. Insecticides play important roles in agricultural production and public health, especially in a country with a huge human population, like China. Large quantities of four classes of insecticides, organochlorines, organophosphates, carbamates and pyrethroids, are applied annually to fields or indoors

Precision Prevention for Health: The Global Alliance Framework for the Promotion of Physical Activity.

This conceptual, implementation-oriented paper proposes a framework for precision prevention centred on physical activity promotion and early risk mitigation, aiming to support a shift from reactive disease management toward proactive prevention. The proposed framework focuses on the early identification of risk trajectories by integrating biological, environmental, and social determinants of health through systems thinking, multi-omics technologies, digital health, and behavioural science. It aims to inform tailored, context-sensitive interventions that will help to mitigate chronic non-commu

Equity by design: integrating a Deprivation Index into digital platforms for breast cancer and chronic disease prevention-lessons from the ELISAH project.

BACKGROUND: Socio-economic inequalities are a major determinant of chronic disease risk and access to preventive services. Deprivation indices can quantify these disparities, but very few studies have considered them when examining the relationship between breast cancer incidence, prognosis, and particulate matter as a potential effect modifier. Moreover, these indices have rarely been used as active tools in service delivery. Digital health technologies offer an opportunity to embed equity into chronic disease prevention through the real-time integration of socio-economic metrics. The ELISAH

Aboriginal and non-aboriginal Australian former prisoners' patterns of morbidity and risk of hospitalisation.

BACKGROUND: People who have been in custody are more likely to experience multiple, long standing health issues. They are at high risk of illness and injury post release and experience poor access to health services both of which contribute to high rates of recidivism. The study was conducted to examine Aboriginal and non-Aboriginal former prisoners' risk of hospitalisation and rehospitalisation in the five years post release from custody and identified the common reasons for hospitalisations. METHODS: Common reasons for hospital admission were identified by conducting descriptive analysis of

Effects of a culturally tailored intervention on medication adherence in Chinese and Vietnamese Americans with hepatitis B: a randomized controlled trial.

INTRODUCTION: Asian Americans are disproportionately affected by chronic hepatitis B (CHB), which is caused by infection with hepatitis B virus (HBV). While adherence to antiviral medication is an effective practical approach to managing CHB and preventing liver cancer, medication adherence rates among Chinese and Vietnamese Americans with CHB, two vulnerable yet understudied populations, remain largely unknown. METHODS: We designed and implemented a randomized controlled clinical trial to investigate the potential improvement of long-term adherence to HBV medication in Asian American populat

The interrelationship between periodontitis and pancreatic cancer: a retrospective cohort study.

BACKGROUND: Periodontitis and pancreatic cancer are major public health concerns with increasing prevalence worldwide, including in Taiwan. Although previous studies have suggested a potential association between periodontitis and pancreatic cancer, findings remain inconclusive, particularly in Taiwanese populations. This study aimed to investigate the relationship between periodontitis and pancreatic cancer. METHODS: The study conducted a retrospective cohort study using data from Taiwan's National Health Insurance Research Database (NHIRD) between 2000 and 2019. Patients diagnosed with peri

Enhanced slime mould algorithm with chaotic and orthogonal optimization-based learning for improved severity prediction accuracy in malaria patient outcomes.

Malaria remains a critical health challenge in developing countries, particularly in Africa, where it disproportionately affects vulnerable populations. Accurate malaria severity prediction is important for proper treatment and improved patient survival rates. This study proposes an improved Slime Mould Algorithm (iSMA) to address the limitations of the standard SMA, including slow convergence, poor initialization, local optima entrapment, and imbalance between exploration and exploitation. The iSMA incorporates four key innovations: a Chaotic Initialization Strategy (CIS) to generate a divers

Relationship between multimorbidity, SARS-COV-2 infection and long COVID: a cross-sectional population-based French survey.

BACKGROUND: Understanding the risks of COVID-19-related consequences for vulnerable groups such as people with multimorbidity is crucial to better tailor health care and public health measures. The main objective of this study was to explore the association between multimorbidity and WHO-defined post-COVID condition (PCC), while also considering the association with SARS-COV-2 infection given that the infection is a prerequisite of PCC. METHODS: This population-representative cross-sectional study was conducted in the general adult population in mainland France between 29 August and 31 Decemb

Accuracy augmentation of body composition measurement by bioelectrical impedance analyzer in elderly population.

Bioelectrical impedance analysis (BIA) is currently the most commonly used method in clinical practice to measure body composition. However, the bioelectrical impedance analyzer is not designed according to different countries, races, and elderly populations. Because different races may have different body compositions, a prediction model for the elderly population in Taiwan should be developed to avoid population bias, thereby improving the accuracy of community evaluation surveys.Dual energy X-ray absorptiometry (DXA) was used as a standard method for comparison, and impedance analysis was u

Co-designing adult weight management services: a qualitative study exploring barriers, facilitators, and considerations for future commissioning.

BACKGROUND: Weight management services have not always benefitted everyone equally. People who live in more deprived areas, racially minoritised communities, those with complex additional needs (e.g., a physical or mental disability), and men are less likely to take part in weight management services. This can subsequently widen health inequalities. One way to counter this is to co-design services with under-served groups to better meet their needs. Using a case study approach, we explored how co-designed adult weight management services were developed, the barriers and facilitators to co-desi

Wildland fire management and air quality in the southern Sierra Nevada: using the Lion Fire as a case study with a multi-year perspective on PM(2.5) impacts and fire policy.

Management of fire is an important and controversial policy issue. Active fire suppression has led to a backlog of fuels, limited the ecological benefits of fire, and reduced short-term smoke impacts likely delaying these emissions to future generations over a larger spatial extent. Smoke impacts can be expected to increase as fire size and intensity increase and the fuel backlog is consumed; whether through reintroduction of fire under desirable conditions or through stand replacing fire. Land Management Agencies would like to increase the use of naturally ignited fires to burn during favorab

Rural disparities in emergency department utilization for migraine care.

OBJECTIVE: To evaluate differences in emergency department (ED) utilization and quality of care for migraine in patients with rural and nonrural residences. BACKGROUND: Migraine is a significant problem in the United States with direct health-care utilization cost amounting to US $4.2 billion annually. A considerable portion of this cost is attributed to more than 4 million annual ED visits for migraine and headache. Previous research has documented health disparities among rural populations in other disease states, which can be influenced by factors such as socioeconomic status and health-ca
