Estimating Chronic Kidney Disease Stage Transitions from Irregular Electronic Health Record Data Using an Expectation-Maximization Framework.

OBJECTIVE: To estimate chronic kidney disease (CKD) stage transition probabilities in patients with small renal masses (SRMs) using irregularly observed electronic health record (EHR) data, addressing challenges of interval censoring and irregular measurement intervals in real-world clinical practice. DATA SOURCES: We used EHR data from the institutional Small Renal Mass registry (2006-January 2026), capturing outpatient renal function data prior to any definitive treatment for SRM. CKD stages were defined using estimated glomerular filtration rate (eGFR) thresholds based on KDIGO guidelines.

Understanding the lived experience of student-recorded video assessments in undergraduate nursing education during the COVID-19 pandemic: A pilot study.

PURPOSE: In-person delivery of courses requiring the assessment of physical psychomotor skills was severely impacted during the COVID-19 pandemic. To compensate, nursing educators implemented emergency procedures to rapidly transition from a traditional, in-person delivery to a remote format. The purpose of this pilot study was to better understand the perspective of undergraduate nursing students and their instructors' lived experiences regarding student-recorded video assessments during the COVID-19 pandemic. METHODS: Six participants (four students, two instructors) were interviewed using

First record of the forensically important genus Protopiophila Duda (Diptera: Piophilidae) from South Korea and an integrative framework for forensic identification.

The family Piophilidae is of significant forensic importance due to its association with late-stage decomposition; however, the Korean piophilid fauna remains poorly studied, lacking comprehensive morphological and molecular data. This study reports the first record of the genus Protopiophila Duda from South Korea. Five male specimens were collected and identified as Protopiophila contecta (Walker) based on morphological characters. We provide the first detailed illustrations and descriptions of the previously poorly documented male internal genitalia and a new identification key to the South

Time trends in stroke incidence and modifiable risk factor prevalence in England (2018-2024): A record linkage population-based study.

BACKGROUND: The incidence of stroke at younger ages appears to be increasing, whilst it is falling at older ages. We aimed to determine the current trend and how it might have changed during and after the COVID-19 pandemic. METHODS AND FINDINGS: We identified first-ever stroke events in adults in England between 1st April 2018 and 31st March 2024 through record-linkage with multiple overlapping sources including primary care, hospital admission, death certificate, and records from a stroke specific national audit. Standardised age-specific (age < 55 versus ≥ 55 years) stroke incidence was cal

Suvorexant Prescription-Record Rates After a Palliative Care Pocket Manual Revision: A Single-Centre Interrupted Time Series Study.

To examine whether a palliative care pocket manual revision suggesting suvorexant for nocturnal awakening was associated with changes in suvorexant prescription-record rates. We conducted a single-center retrospective interrupted time series study at a Toyama University Hospital using weekly prescription records for suvorexant (15 mg and 20 mg tablets combined). Prescription records, not unique patients, were analyzed with segmented negative binomial regression using an offset for days per week. The manual was updated on 16 May 2025; the first post-implementation week was 19 May 2025. The prim

Liquid-derived, solvent-free vapor-mediated dimensional reconstruction yields a record fill factor in inverted perovskite solar cells.

Despite recent advances, the fill factor (FF) of perovskite solar cells remains limited, largely owing to defect-related recombination. Paradoxically, most defect passivation approaches still depend on solvents, which deteriorate stability and pose challenges for large-scale fabrication. Here, we introduce a vapor-phase deposited from a liquid triethylammonium pentafluoropropionate (TEA-PFP) layer on top of perovskite. During deposition, TEA⁺ reacts with residual PbI2 to generate a one-dimensional TEAPbI3 interfacial phase, promoting continuous electronic coupling and facilitating charge extra

Stable Isotopes in Eye Lenses Record Patterns and Variation in Resource-Use Ontogeny of Three New Zealand Kelp Forest Fishes.

Fishes can undergo dramatic social and morphological changes throughout development that drive ontogenetic shifts in diet and habitat association. Measurements of trophic ontogeny at the individual level often complement population-level assessments, detailing foraging strategies that have underpinned long-term growth and survival. Using stable isotope measurements from muscle and eye lenses, we modeled size-based patterns in basal resource use and trophic position across multiple levels of organization for three New Zealand reef fishes (Notolabrus fucicola, Odax pullus, and Parapercis colias)

Clinical features, diagnosis, and referral of women with breast abnormalities in primary health care settings in rural Ethiopia: a retrospective record analysis.

BACKGROUND: Breast cancer is the most commonly diagnosed cancer among women worldwide. At the primary health care level, understanding the magnitude of different breast abnormalities and current referral practices can improve patient-centred health care services, enhance patient education and awareness, and encourage interdisciplinary collaboration to enhance breast cancer outcomes, in line with the World Health Organization Breast Cancer Global Initiative (GBCI) Pillar One Goal. Thus, this study aims to explore the burden of breast-related abnormalities and their clinical features during pres

Real-world outcomes of anthracycline and taxane-based perioperative breast cancer therapy using the Japanese electronic medical record database.

BACKGROUND: Anthracycline (A) and taxane (T)-based therapies improve breast cancer survival, with guidelines strongly recommending these regimens and dose-dense approaches. However, the real-world maintenance of optimal dose intensity, a critical prognostic factor, remains unclear. We aimed to clarify the current treatment situation regarding relative dose intensity (RDI), with a secondary focus on safety. METHODS: In this retrospective observational study, we analyzed big data from the DATuM IDEA® electronic medical record database of the Japan Medical Association Medical Information Managem

Impact of Dedicated Nurse Time on Electronic Health Record (EHR)-Based Diabetes Quality Measures: A Pre-Post Observational Study.

BACKGROUND AND OBJECTIVE: Despite major advances in diabetes therapies, only about 60% of U.S. patients achieve recommended glycemic targets in primary care. Inconsistent electronic health records (EHR) complicate diabetes care delivery and highlight the need for strategies that support accurate documentation and patient engagement. The objective of this project is to have 50% of patients with diabetes meet six quality measures: hemoglobin A1c, blood pressure, statin use, aspirin use, tobacco status, and urine microalbumin.  Methods: In April 2019, a registered nurse (RN) was tasked with overs

Multimorbidity patterns and phenotype transitions in patients with clinician-coded long COVID: a multicenter US electronic health record cohort study.

BACKGROUND: Long COVID is clinically heterogeneous, but longitudinal changes in documented chronic disease burden and transitions in multimorbidity phenotypes after infection are not well characterized in routine care. METHODS: We considered 425,614 patients with clinician-coded long COVID (ICD-10-CM U09.9) and a definable COVID-19 index date between October 1, 2021 and September 16, 2024 using deidentified electronic health record data from Epic Cosmos, a multicenter US network. Pre-index and post-index windows were defined as days - 365 to - 1 and days 91 to 455 relative to infection, respe

A Novel Digital Phenotype for Burn Sepsis: Leveraging Electronic Health Record Data and Natural Language Processing to Improve Case Definition.

BACKGROUND: Sepsis remains a leading cause of death for burn patients, yet the condition is hard to spot early. Hospitals generally rely on the International Classification of Diseases (ICD) codes for surveillance, but these codes are assigned late and often miss active cases. OBJECTIVES: We developed and validated a scalable, electronic health record (EHR)-based digital phenotype that improves identification of burn-related sepsis compared with ICD codes alone. METHODS: We performed a retrospective cohort study of adult burn inpatients (n = 1,371) admitted to Loyola University Medical Cente

Prevalence of multiple morbidities and cancers in individuals with Down syndrome: A matched descriptive study using linked electronic health record data.

BACKGROUND: Down syndrome is associated with the development of multiple morbidities throughout the life course, yet comprehensive data on its relative burden remains limited. This descriptive, matched, retrospective cohort study aimed to assess the 20-year period prevalence of morbidities and cancers in adults and children with Down syndrome. METHODS: We analysed electronic health record data from January 1998 to December 2017, matching individuals with Down syndrome to up to five matched controls. Period prevalence and odds ratios (OR) were calculated for 30 morbidities and 24 cancers. RES

Identification of acute kidney injury after non-cardiac surgery by machine learning leveraging a large electronic health record database.

Post-operative acute kidney injury (PO-AKI) is a frequent complication after non-cardiac surgery, substantially increasing patients' mortality and morbidity rates. Early identification of patients at high risk of PO-AKI could facilitate improved patient care. We investigated a large, single-center database of 13,890 patients undergoing non-cardiac surgery, of whom 1,718 (12.37%) developed PO-AKI. Based on 62 pre-, intra- and postoperative patient characteristics, including time-series mean arterial pressure measurements, we trained and validated eight different machine learning (ML) algorithms

Interstitial lung disease in mixed connective tissue disease: Insights from an electronic health record-based retrospective cohort study.

OBJECTIVE: Pulmonary complications, including interstitial lung disease (ILD), are common and contribute to morbidity and mortality in mixed connective tissue disease (MCTD). However, risk factors for ILD in MCTD are poorly understood, which can hinder early detection of this fatal complication. The purpose of this study was to identify predictors of ILD presence in MCTD using structured electronic health record (EHR) data. METHODS: We performed a retrospective EHR-based investigation of adults with MCTD evaluated at a large urban academic medical center between 2014 and 2026. Patients were i

First record of Sigmodon minor (Rodentia) in the early Blancan of central Mexico: Asymmetrical dispersal from the Great Plains and paleoecology inferences.

In this study, we report the first occurrence of Sigmodon minor (Cricetidae, Rodentia) in Mexico, from the early Blancan of the Pliocene in the San Miguel de Allende Basin, Guanajuato. This record represents an early and rapid dispersal of these rodents from the Great Plains to lower latitudes, likely driven by climatic fluctuations during the late Neogene that favored the expansion of grassland biomes. The fossils described here are among the most complete for this species, including well-preserved mandibles and maxillary elements, found in association with megafauna and supported by radiomet
