Can Artificial Intelligence Chatbots Plan Therapeutic Ketogenic Diets for Children With Epilepsy?

RATIONALE: Ketogenic Diet Therapy (KDT) is an effective but complex treatment for paediatric drug-resistant epilepsy. Access to trained dietitians limits the global use of KDT. The increasing use of artificial intelligence (AI) chatbots for health and dietary advice presents both opportunities and risks. This exploratory study evaluates the accuracy and feasibility of AI-generated KDT plans for paediatric epilepsy. METHODS: We conducted a comparative analysis of three major AI chatbots: ChatGPT, Microsoft Copilot, and Gemini. Three clinical vignettes, one for each KDT type, were created to re

VINE-seq and MultiVINE-seq for single-nucleus and multiome profiling of the brain vasculature.

The human cerebrovasculature is a critical yet historically understudied component of neurological health. Dysfunction of the diverse endothelial, mural, and perivascular cells that comprise cerebral vessels is central to diseases ranging from stroke to Alzheimer's disease. However, characterizing these cell populations at a molecular level has proven exceptionally challenging. Encased within a robust basement membrane, vascular cells resist standard dissociation methods, leading to their systematic depletion and underrepresentation in existing single-nucleus genomic atlases. This has created

Physiological Optimization of Digital Self-Disimpaction Using a Step Stool: A Case Report.

Digital disimpaction, which is often required for cases of severe fecal impaction, carries significant risks of mucosal injury and vasovagal reflex. Evidence-based guidance for safe home-care disimpaction is limited. Moreover, detailed physiological self-assessments of medically trained individuals during such events have seldom been reported. Herein, we report a rare case of self-disimpaction performed by a gastroenterological surgeon, providing a detailed clinical analysis of the physiological mechanisms involved. A 49-year-old male gastroenterologist presented with recurrent distal fecal im

Structural imaging predictors of ketamine response in treatment-resistant depression: a machine learning approach.

Ketamine has demonstrated rapid antidepressant efficacy in treatment-resistant depression (TRD), but clinical decision-making is challenging due to variability in individual response. Current trial-and-error prescribing practices may expose patients to ineffective treatment and avoidable adverse effects, underscoring the need for reliable predictive tools to optimize treatment selection and support personalized, evidence-based care. We developed a machine-learning model (support vector classifier) to predict antidepressant response to ketamine using pre-treatment structural MRI data. The model

Comparison of Suprahyoid Muscle Activation During Inspiratory Muscle Training and the Shaker Exercise: An Exploratory Pilot Study.

Inspiratory muscle training (IMT) has been discussed in the context of swallowing rehabilitation, yet its neuromuscular characteristics relative to established swallowing exercises remain unclear. This study aimed to compare profiles of muscle activation amplitude during IMT and the Shaker exercise, with a focus on task-specific recruitment of the suprahyoid, cervical, and respiratory muscles. In a single-center, prospective crossover study, healthy young adults performed IMT using a flow-resistive device and the Shaker exercise in randomized order. Surface electromyography (sEMG) was recorded

AI lesion tracking in PET/CT imaging: a proposal for a Siamese-based CNN pipeline applied to PSMA PET/CT scans.

PURPOSE: Assessing tumor response to systemic therapies is one of the main applications of PET/CT. Routinely, only a small subset of index lesions out of multiple lesions is analyzed. However, this operator dependent selection may bias the results due to possible significant inter-metastatic heterogeneity of response to therapy. Automated, AI-based approaches for lesion tracking hold promise in enabling the analysis of many more lesions and thus providing a better assessment of tumor response. This work introduces a Siamese CNN approach for lesion tracking between PET/CT scans. METHODS: Our a

Role of macrophages reprogramming in pathogenesis of gestational diabetes mellitus.

Gestational diabetes mellitus (GDM) is a pregnancy-associated condition resulting from glucose intolerance affecting approximately 14 % of pregnant women worldwide and contributing to both maternal and neonatal complications. Macrophages, essential components of the innate immune system, exist in three main polarization states: M0 (resting), M1 (pro-inflammatory), and M2 (anti-inflammatory). In normal pregnancy, a proper balance between M1 and M2 phenotypes is critical for successful placentation and fetal development. Although this classification provides a useful framework, emerging evidence

C57 and DBA mouse strains express distinct cocaine avoidance phenotypes in an operant runway independent of differences in striatal dynorphin-enkephalin balance.

RATIONALE: Human self-report data and rodent models indicate cocaine can induce negative affect, which may impact future cocaine use severity. Despite this, understanding of the neurobiology driving cocaine avoidance is limited. Within the striatum, the opioid peptides enkephalin and dynorphin are associated with cocaine reward and aversion, respectively. Additionally, striatal dynorphin signaling resulting from stress or cocaine withdrawal acts as a negative reinforcer to increase cocaine seeking. OBJECTIVES: We validated the use of the cocaine self-administration runway in mice to measure t

Nasal trigeminal projections and medullary dorsal horn neuronal activation during voluntary diving in rats.

The diving response, an autonomic reflex characterized by apnea, bradycardia and increased peripheral vascular resistance, is initiated when animals submerge underwater. Neurons located within the trigeminal nucleus caudalis, specifically the ventral medullary dorsal horn (MDH), become activated and produce Fos after repetitive diving. It is assumed nerves innervating the nasal passages are important for activating these neurons during diving. The present study investigated the anatomical route by which nasal stimulation during diving can produce activation of MDH neurons. In rats trained to r

Dynamic current-clamp unveiling the indispensable interplay between Nav1.7 and Nav1.8 for shaping action potential trajectory and retaining neuroexcitation of visceral sensory neurons.

Tetrodotoxin (TTX)-sensitive/Nav1.7- and TTX-resistive/Nav1.8 Na+ channels contribute to neuroexcitation in the sensory neurons of the nodose ganglion (NG); however, their specific roles remain debatable. Therefore, we aimed to study the action potential (AP) elicited from NG neurons isolated from adult rats and simulated by a dynamic current clamp (DCC) using gNa0/Nav1.7 and/or gNa1/Nav1.8 injection. We trained and tuned the voltage-dependent profiles of the Na+ current generated from the DCC using the Hodgkin-Huxley/Vandenberg models to match the AP parameters elicited by a brief pulse. A- o

Prevalence and Associated Factors for Purchasing Antibiotics Without a Prescription Among Patients in Rural South Africa: Implications for Addressing Antimicrobial Resistance.

Background: Antimicrobial resistance (AMR) is now a critical issue in South Africa, enhanced by considerable inappropriate prescribing of antibiotics. There is currently variable dispensing of antibiotics without a prescription. Where this occurs, it is principally for urinary tract infections (UTIs) and sexually transmitted infections (STIs). Consequently, there is a need to comprehensively evaluate antibiotic dispensing patterns and factors influencing this to reduce AMR. Methods: A previously piloted questionnaire was administered to patients exiting three different categories of community
