Alkaptonuria is a rare inborn error of metabolism with autosomal recessive inheritance with a mutation in homogentisate 1,2-dioxygenase. It results in accumulation of homogentisic acid in connective tissues (ochronosis). Most common ocular manifestations are bluish-black discoloration of the conjunctiva, cornea, and sclera. In this case report, a 39-year-old Indian male patient with additional ocular features in the retina is described.

A 25-year-old man with X-linked retinoschisis presented with multiple giant optic nerve head drusen and multiple retinal drusenoid deposits in one eye. There was an area of coarse pigmentation in the lower half of the retina, with convex upper borders extending on either side of the optic disc, suggestive of a spontaneously resolved retinal detachment. The possible aetiopathogenesis of such occurrence is discussed.

Refraction was measured in eyes of cynomolgus (Macaca irus) monkeys, before and during continuous intravenous infusion of large doses of the carbonic anhydrase (CA) inhibitors acetazolamide and ethoxzolamide. No changes of refraction were seen. Therefore, inhibition of CA in lens, cornea and retina does not appear to be the cause of the transient myopia and associated symptoms, occasionally observed in patients treated with CA inhibitors.

The authors present a case of 34-years-old man with retrobulbar hematoma caused by blunt injury accompanied by partial palsy of the oculomotor nerve and ischemia of the optic nerve and retina. Fractures of the orbit were excluded using computer tomography. Surgical decompression of the orbit was performed. The authors obtained quick and marked improvement with normal visual acuity and limitation of adduction.

Between normal and glaucomatous subjects, a group with ocular hypertension exists which is not homogeneous as 10 p. cent of these will develop glaucoma. The problem is to select early eyes at risk. Three new methods of examination have been proposed: the response to the test of contrast sensitivity, the pupillary reaction test of Marcus Gunn, and photographic exploration of the nerve fiber layer of the retina.

A study is presented of a case of serous detachment of the mucula secondary to congenital pit of the optic disc. We observed during fluorescein angiography a late leakage of dye at the temporal margin of the disc. A lasercoagulation of this lesion gave a favourable result. All these findings support Gass' explanation implicating a mechanical defect in the region of the pit that permits spinal fluid to seep beneath the retina.

The authors describe a patient who suffered traumatic maculopathy following blunt trauma to the eye with commotio retinae, subretinal and preretinal hemorrhage, traumatic macular hole, and outer retinal and retinal pigment epithelium disruption. Serial imaging with commercially available and ultra-high resolution spectral domain optical coherence tomography characterized the evolution of the injury.

This study examines the effect of two local anaesthetics, benzocaine and lidocaine, on propagation, amplitude and duration of the slow potential change of spreading depression. The experiments were performed in isolated chick retina superfused with Ringer's solution. We observed, for both drugs, a dose-related decrease in all parameters analysed. The maximal effect on propagation was blockade of the reaction.

The association of microcephaly and mental retardation with a non-pigmentary retinopathy is described in three siblings of consanguineous parents. The electroretinogram showed the distinctive appearance of markedly attenuated "b" wave but normal "a" wave suggestive of a retinal dystrophy primarily affecting post-receptoral elements in the inner retina. This appears to be an autosomal recessive condition which has not been previously reported.

Comparative electron microscopic, electrophysiological and biochemical studies of thioridazine influence on lipid peroxidation in vivo (rats, rabbits) and in vitro (model systems) were performed. It was shown that thioridazine retinopathy was not followed by an increase in lipid peroxidation and antioxidant (dibunol) failed to protect the retina against the destructive action of thioridazine. Moreover, thioridazine inhibited lipid peroxidation in model system.

Nerve fibre bundle visual field defects are the hallmark of pathology at the optic nerve head or retina. In the present series, six eyes of four patients with large intracranial mass lesions demonstrated such field defects with visual acuity spared in three of the involved eyes. In no cases did visual function in the opposite eye offer a clue to the localization of the lesion.

Two children with abnormally long eyelashes and bushy brows ("trichomegaly") present at birth and associated with pigmentary degeneration of the retina and general growth retardation are described. This combination of findings has been observed previously in three cases. Mental retardation, sparse scalp hair, endocrinologic deficiencies and (in one of our cases) koilonychia may be found in addition, but not necessarily.

The minimum requirements for a reliable study of (acquired) defects of colour vision have been formulated by Verriest. Taking these minimum requirements as a guide, about 200 patients were selected. For the differential diagnosis of disorders of the retina and the optic nerve the determination of the neutral zone and examination with the anomaloscope are important. This is true for both red-green and blue-yellow defects.

A 15-year-old boy was seen initially with decreased vision in his left eye. Ophthalmoscopic examination showed numerous pigmented subretinal tracts with traction lines radiating to the fovea. Peripheral examination disclosed a maggot in the superior nasal quadrant of the vitreous. There was a hemorrhage emerging from the exit wound in the retina. Removal of the maggot was not indicated because there was no inflammation.

A boy with galactosaemia and bilateral cataract developed large intraocular haemorrhages in the left eye which was enucleated. Histologically a persistent hyaloid artery was demonstrated. This combination with galactosaemia seems not to have been described in the literature before. The haemorrhages are most likely secondary to the persistent hyaloid artery system. In addition, some foci of extramedullary haemopoiesis were demonstrated in the retina.

A patient with temporal arteritis developed a variety of ischaemic lesions in the eyes. Infarction of the inner retina and optic nerve head was delineated on presentation by white swelling in the retinal nerve fibre layer. The role of interrupted axoplasmic transport in the production of this sign is discussed. Outer retinal infarction was also noted on presentation and subsequently gave rise to striking pigmented scars.

The eyes of three patients followed clinically for many years with a diagnosis of macular disciform scarring, secondary to ocular histoplasmosis, were studied histopathologically. A fibrovascular scar interposed between the choroid and retina was observed in all cases. Vessels passing from the choroid through defects in Bruch's membrane into the nodule were demonstrated. Theories of the pathogenesis of the lesion are discussed.

A 45-year-old male with von Recklinghausen's disease was noted to have hamartomatous lesions of the retinae similar to those more commonly described in Bourneville's disease and von Hippel-Lindau's disease. These findings would appear to support the concept that the retinal changes in neurofibromatosis occupy an intermediate position between those seen in the other two phakomatoses.

Quantification of retinal ganglion cell (RGC) loss provides a sensitive and reproducible measure of neurodegeneration in mouse models of neurological disease and injury.
