Screening duplex ultrasonography in neurosurgery patients does not correlate with a reduction in pulmonary embolism rate or decreased mortality.

OBJECTIVE: Deep vein thrombosis (DVT) is a major focus of patient safety indicators and a common cause of morbidity and mortality. Many practices have employed lower-extremity screening ultrasonography in addition to chemoprophylaxis and the use of sequential compression devices in an effort to reduce poor outcomes. However, the role of screening in directly decreasing pulmonary emboli (PEs) and mortality is unclear. At the University of Mississippi Medical Center, a policy change provided the opportunity to compare independent groups: patients treated under a prior paradigm of weekly screenin

Screening nasal swabs for methicillin resistant Staphylococcus aureus: A regional burn center's experience.

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) is a pathogen that can result in substantial morbidity and mortality. Early detection of MRSA colonization by screening nasal swabs may be important in the management of burn-injured patients. However, studies examining its use in this population are limited. The aim of this study was to study the utility of admission MRSA screening nasal swabs and determine if being positive for MRSA on admission impacted outcomes. MATERIALS AND METHODS: A retrospective review was conducted of burn patients who presented to a single regional burn

Incidental renal tumours on low-dose CT lung cancer screening exams.

Introduction Renal cancer incidence has increased markedly in the United States in recent decades, largely due to incidentally detected tumours from computed tomography imaging. Here, we analyze the potential for low-dose computed tomography lung cancer screening to detect renal cancer. Methods The National Lung Screening Trial randomized subjects to three annual screens with either low-dose computed tomography or chest X-ray. Eligibility criteria included 30 + pack-years, current smoking or quit within 15 years, and age 55-74. Subjects were followed for seven years. Low-dose computed tomograp

Screening for harmful substance use in emergency departments: a systematic review.

BACKGROUND: Substance use-related emergency department (ED) visits have increased substantially in North America. Screening for substance use in EDs is recommended; best approaches are unclear. This systematic review synthesizes evidence on diagnostic accuracy of ED screening tools to detect harmful substance use. METHODS: We included derivation or validation studies, with or without comparator, that included adult (≥ 18 years) ED patients and evaluated screening tools to identify general or specific substance use disorders or harmful use. Our search strategy combined concepts Emergency Depar

A protocol for developing screening criteria for degenerative cervical myelopathy: AO Spine Myelopathy.org RECODE-DCM.

STUDY DESIGN: Protocol OBJECTIVES: Degenerative cervical myelopathy (DCM) is caused by degenerative changes of the spinal column that lead to progressive spinal cord compression. Early identification of DCM is paramount to ensure appropriate specialist referral, timely intervention, and optimal treatment outcomes. Unfortunately, diagnosis of DCM is often delayed and may be missed entirely. A lack of screening or referral criteria has been identified as a major contributor of diagnostic delay. The objective of this study is to outline the methodology that will be used to develop screening crite

[Imported Schistosomiasis among unaccompanied minor migrants in Nice (France)].

INTRODUCTION: The prevalence of imported schistosomiasis among unaccompanied minor migrants (UMMs) in France is not well understood. METHODOLOGY: From December 2023 to February 2024, we screened UMMs for schistosomiasis, in addition to tuberculosis screening, at our Alpes-Maritimes Tuberculosis Control Center. We offered serological screening (Western Blot) to all UMMs from sub-Saharan Africa (SSA). If the result was positive, we offered a parasitological examination of urine and stool samples. RESULTS: During these three months, we screened 187 UMMs from SSA. Serology was positive for 116 o

Necrotizing Enterocolitis Complicated by Hepatic Abscesses in a Neonate: Diagnostic Utility of an Abdominal Ultrasound.

We report a case of multiple liver abscesses after necrotizing enterocolitis (NEC), which were successfully detected by ultrasound screening and treated successfully. This baby was not premature but had truncus arteriosus and was at risk of NEC. NEC developed on day 7, and abdominal drainage, bowel resection, and enterostomy were performed on day 12. The postoperative clinical course and laboratory data were uneventful. However, abdominal ultrasound screening, which was performed to check for any postoperative complications, revealed multiple liver abscesses on postoperative day 14. Contrast-e

Early Screening of Autism in Toddlers via Express-Needs-With-Pointing Protocol.

The incidence of autism spectrum disorders (ASD), a neurodevelopmental condition associated with challenges in social communication, has witnessed a remarkable surge in recent years, with adverse effects on individuals, families, and society at large. Early screening for autism ensures timely access to interventions, yet screening lacks systematic and methodical approaches for objectively quantifying social behaviors. In response to this, we propose a protocol for early assistive screening, termed the Express-Needs-with-Pointing (ENP), which employs a multi-sensor platform to quantify the one

Development of a Cystic Fibrosis Primary Palliative Care Intervention: Qualitative Analysis of Patient and Family Caregiver Preferences.

To prevent or mitigate chronic illness burden, people with cystic fibrosis (pwCF) and their family caregivers need primary (generalist-level) palliative care from the time of diagnosis forward. We used qualitative methods to explore their preferences about a screening-and-triage model ("Improving Life with CF") developed to standardize this care. We purposively sampled and interviewed 14 pwCF and caregivers from 5 Improving Life with CF study sites. Thematic analysis was guided by a priori codes using the National Consensus Project's Guidelines for Quality Palliative Care. Participants include

Medical stakeholder perspectives on implementing a computerized battery to identify neurocognitive impairments among youth in Botswana.

HIV infection and in utero exposure, common in Sub-Saharan Africa, are associated with pediatric neurocognitive impairment. Cognitive screening can identify impairments, but it is rarely used in this setting. The Penn Computerized Neurocognitive Battery (PennCNB), an evidence-based cognitive screening tool, was adapted for use in Botswana. To facilitate future implementation, 20 semi-structured interviews were conducted to elicit key stakeholders' perspectives on factors likely to be related to successful uptake of the PennCNB in clinical settings. An integrated analytic approach combining con

Differences in clinical practice regarding screening and treatment of infections associated with spontaneous preterm birth: An international survey.

OBJECTIVE: An association between infections in pregnancy and increased risk of preterm birth (PTB) is described in the literature. We anticipated that differences may exist in screening and treatment approaches for infections associated with PTB, within and between European countries. The aim of this study was to examine and analyse these differences in clinical practice in greater detail. STUDY DESIGN: We created a descriptive survey examining the screening and treatment of infections in pregnancy. The survey was sent to European representatives of the International Spontaneous Preterm Birt

Comparison of digital imaging screening and indirect ophthalmoscopy for retinopathy of prematurity.

The aims of this study were to determine the incidence and severity of retinopathy of prematurity (ROP) using digital imaging screening, confirm findings by indirect opthalmoscopy, and document risk factors of ROP in the neonatal intensive care unit (NICU) of a large tertiary hospital in a developing country. This prospective cohort study included infants with gestational age (GA) ≤ 32 wk, birth weight (BW) ≤ 1,500 g, or older and heavier neonates who were critically ill. Two hundred twenty two eyes (111 infants) were screened with digital imaging (Ret-Cam) and indirect ophthalmoscopy until re

Familial pancreatic cancer--current knowledge.

Familial pancreatic cancer (FPC) describes families with at least two first-degree relatives with confirmed exocrine pancreatic cancer that do not fulfil the criteria of other inherited tumour syndromes with increased risks of pancreatic cancer, such as Peutz-Jeghers syndrome, hereditary pancreatitis, and hereditary breast and ovarian cancer. The inheritance of FPC is mostly autosomal dominant and with a heterogeneous phenotype. The major gene defect is yet to be identified, although germline mutations in BRCA2, PALB2 and ATM are causative in some FPC families. Expert consensus conferences con

PCR-based screening of targeted mutants for the fast and simultaneous identification of bacterial virulence factors.

Understanding the strategies used by pathogens to infect, survive, and proliferate in their hosts requires the identification of virulence factors. We developed PCR-based screening of targeted mutants to facilitate quick, simultaneous detection of multiple novel bacterial virulence genes. Based on direct PCR screening of pooled targeted mutants, this approach provides a fast and sensitive measure of virulence attenuation while significantly reducing the number of animals and time required. We demonstrate that the careful design of gene-specific primers allows the direct relative quantification

The case for routine HIV screening and impact of managed care.

An estimated 230,000 people in the United States are unaware that they are infected with the human immunodeficiency virus (HIV). Targeted testing strategies are likely to miss a significant proportion of the HIV-infected population, and routine screening could help identify individuals who are likely to experience complications or transmit the virus. Recent advances in treatment, including the introduction of highly active antiretroviral therapy, have dramatically extended the life expectancy of HIV-infected individuals and allowed many patients to have their condition managed in a manner simi

A genetic history questionnaire-based system in primary prenatal care to screen for selected fetal disorders.

DNA (and other) diagnostic tests are now available for a number of serious, but uncommon, fetal disorders. We designed and evaluated a screening system for this purpose in primary care, coupled with targeted information for practitioners and patients. We developed a 15-question family history form for completion by office staff or patients, addressing conditions for which definitive diagnosis was available, linked to secondary questionnaires to follow up on "yes" answers. Guidelines for assessing risk, follow-up recommendations, and information resources were also linked. Following pilot testi

The detection of colorectal cancer at an asymptomatic stage by screening is useful.

BACKGROUND/AIMS: The validity of mass screening using fecal occult blood testing remains controversial. In addition, no controlled clinical study has yet been performed to show the usefulness of sigmoidoscopy. The purpose of the present study was to compare the surgical results achieved in asymptomatic patients with colorectal cancer detected by screening with those in symptomatic individuals. METHODOLOGY: A total of 285 patients underwent a surgical resection of colorectal cancer between 1991 and 1997 at our institution. Among them, 233 patients had complaints related to cancer at the time o

A breast cancer screening educational intervention targeting medical office staff.

There is persistent evidence that breast cancer screening techniques remain under-utilized. While physicians cite lack of time as a barrier to the provision of preventive services, nurses and other medical office staff are in an ideal position to educate women and motivate adherence to screening recommendations. This paper describes the design, implementation and process evaluation of a breast cancer screening educational program targeting primary care medical office staff. This intervention was conducted in two Washington State counties as part of a larger community organization study. The PR
