Proportion and distribution of upper extremity musculoskeletal disorders and its association with disability in type 2 diabetes mellitus.

STUDY DESIGN: Cross-sectional INTRODUCTION: Globally, diabetes is a leading cause of disability with an increased prevalence rate in the past three decades. Chronic diabetes has been shown to affect collagenous tissue which often leads to subsequent musculoskeletal complications. Despite increasing prevalence of musculoskeletal disorders, the proportion and distribution of types of upper extremity musculoskeletal disorders resulting in disabilities is poorly understood. PURPOSE OF THE STUDY: This cross-sectional study aims to gather data on the prevalence, proportion and distribution of upper

A Hierarchical Three-Stage Feature Selection Strategy for Efficient and Generalizable sEMG-Based Joint Torque Estimation in Athletic Training and Rehabilitation.

Estimating joint torque in athletes provides important information for understanding neuromuscular performance and optimizing high-intensity training. While surface electromyography (sEMG) offers a non-invasive measure of muscle activation, existing methods often struggle to balance prediction accuracy, computational efficiency, and generalizability across different movement conditions. This study proposes a Hierarchical Three-Stage Feature Selection (H3FS) strategy for efficient and stable sEMG-based joint torque estimation. H3FS integrates domain knowledge, statistical and model-driven scree

The Impact of Lower Back Pain on Hip Extension and Lumbosacral Lateral Flexion in Junior Gymnasts during Back-Bending.

We sought to compare the spine and lower limb alignment using inertial measurement units (IMUs) in athletes with and without low back pain (LBP). Fifty junior gymnasts were divided into two groups: those with (n = 12) and without LBP (n = 38). IMU sensors were placed throughout the body. Participants were instructed to perform back-bending. The movements of shoulder, thoracolumbar, lumbosacral, hip, and knee joints were assessed. Additionally, differences between thoracolumbar and lumbosacral joints were evaluated. In the sagittal plane, lumbosacral extension was greater in the no-LBP group (3

Population Study of Hand and Wrist Manifestations of Congenital Insensitivity to Pain.

Background: Congenital insensitivity to pain is a rare autosomal recessive condition characterized by insensitivity to painful stimuli due to absence of sensory and sympathetic post ganglionic neurons in the skin and skeletal system leading to lack of protective sensation and altered joint propioception. This study was performed to assess hand and wrist manifestations of patients with congenital insensitivity to pain in the Maltese Islands. Methods: Records of public and private hospitals were reviewed to identify patients suffering from this condition. A review of notes, patients, and imaging

Intra- and interpersonal movement coordination in jointly moving a rocking board.

In this study, we investigate how two persons (dyads) coordinate their movements when performing cyclical motion patterns on a rocking board. In keeping with the Leading Joint Hypothesis (Dounskaia, 2005), the movement dynamics of the collaborating participants were expected to display features of a prime mover with low movement variability. Fourteen subject pairs performed the task in nine amplitude-frequency combinations that were presented in the form of a to-be-tracked stimulus on a computer display. Participants were asked to track the stimulus by jointly rocking the Board sideways while

[Hypermobile syndrome: functional and aesthetic damages. Observations on 50 cases].

BACKGROUND: Type III Ehlers-Danlos syndrome (EDS III) is a collagen defect and manifests as hypermobile joints, which cause dislocations, and fragile skin, which causes scars. There are three clinical-ultrasound (US) stages. OBJECTIVES: Diagnostic techniques are described that permit assessment of cutaneous and phalangeal impairment, and job fitness of patients with EDS III. METHODS: The Beighton scoring system (Bss) and the ultrasound test (US) of the skin and phalanxes (with Sonora Logic 400 MD and 7.5-10 MHz probes) were used in the diagnosis of 50 patients. Dislocations and scars were co

Postdysenteric reactive arthritis. A clinical and immunogenetic study following an outbreak of salmonellosis.

Following an outbreak of foodborne gastroenteritis caused by Salmonella typhimurium, questionnaires were sent to affected individuals and then to the family physicians of any who experienced extra-enteric complications. Of 260 individuals infected with S typhimurium for whom adequate data were obtained, 19 patients developed joint disease (7.3%). All were men; the mean age +/- SD was 39.3 +/- 1.6 years. Among the 16 patients for whom this information was available, the interval from the onset of diarrhea to the onset of joint pain was less than 7 days in 7, 8-21 days in 2, and greater than 21

[Symptomatic and evolutive characteristics of articular destruction noted in chondrocalcinosis].

Referring to 17 personal observations, the authors endeavour to clarify the main clinical and radiological traits of the destructive arthropathies occuring in patients suffering from diffuse, articular chondrocalcinosis. These arthropathies appear to be relatively frequent and older, obese women suffering from demineralization are more readily affected. The knees, coxo-femoral joints and the shoulders are principally concerned, and to a lesser extent the wrists, the trapezo-metacarpal joints and even the spine. The lesions can be polyarticular and symmetrical, be grouped in more or less random

[Arthroscopic anterior capsular release in medial contracture of the shoulder secondary to brachial plexus birth palsy. Preliminary results].

PURPOSE OF THE STUDY: Retraction of the shoulder in internal rotation is observed in 25% of children with brachial plexus birth palsy (C5, C6 +/- C7). Early bone and joint deformities affecting the glenohumeral joint are the consequences. The stiff internal rotation requires surgical release which can involve the capsule and ligaments, muscles, or both. Internal release can be combined with muscle transfer to improve active external rotation. We report the results obtained with arthroscopic anterior capsular release combined with latissimus dorsi transfer. MATERIAL AND METHODS: From 1999 thro

Intraoperative decision making in the treatment of shoulder instability.

Between April 1990 and April 1994, 100 patients with a preoperative diagnosis of anterior instability underwent a diagnostic arthroscopy of the shoulder. Patients with isolated SLAP lesions were excluded from the study. Patients with multidirectional instability, bony Bankart lesions, and large Hill-Sachs lesions were also excluded. Football players and dominant arm throwing athletes were also excluded. Thirty patients remained who had post-traumatic, unidirectional, anterior instability and a repairable Bankart lesion and did not have any exclusions as noted above. All of these patients under

[Extraskeletal calcification in chronic renal failure during hemodialysis and after renal transplantation (author's transl)].

The frequency of calcifications in vessels and soft tissue was 39% in 184 patients with chronic renal failure. Non-dialyzed patients and dialyzed patients at the start of dialysis treatment had most frequently calcifications of the aorta and larger vessels. Renal transplant recipients, who developed osteonecrosis or spontaneous fractures after transplantation, showed at the time of transplantation a higher frequency of calcifications compared to renal transplant recipients, who did not develop these complications. The frequency of calcifications increased with increasing age not only in the ao

Tang's method is an effective new treatment for anterior shoulder dislocation.

The principal aim of this study was to present a novel approach to manipulative repositioning for the management of anterior shoulder dislocation. This was evaluated in comparison with the established repositioning method to ascertain its clinical efficacy. Seventy-six patients with anterior dislocation of the shoulder joint were randomly assigned to one of two groups. Each group underwent reducing using either Tang's method or Hippocrates' method, respectively. The patients were then monitored for six months. The researchers analyzed and compared the general condition, reduction time, reducti

Changes in Glenohumeral Musculoskeletal Development Following Brachial Plexus Birth Injury.

Brachial plexus birth injury (BPBI), one of the most common nerve injuries in children, often leads to impaired shoulder development, resulting in sustained postural and bone deformity and muscle weakness. Despite the substantial long-term consequences, clinical consensus is lacking for what BPBI treatments are optimal in terms of timing and approach, primarily because BPBI sequelae are complex, involving stunted muscle growth, muscle denervation, and limb disuse that can disrupt glenohumeral joint development. Injury can occur as nerve rupture (postganglionic injury) or nerve avulsion (pregan

Continuous Suprascapular Catheter and Axillary Nerve Block for Analgesia for Reverse Total Shoulder Arthroplasty: A Case Report.

Reverse total shoulder arthroplasty (RTSA) is a widely employed surgical intervention for managing advanced shoulder arthritis. Postoperatively, patients frequently experience intense pain, particularly within the first 48 hours. Effective pain management through regional analgesia not only facilitates a quicker hospital discharge but also minimizes the reliance on opioids. One such regional analgesic approach is the combined suprascapular and axillary nerve block, which targets the glenohumeral joint, rotator cuff muscles, and the shoulder's lateral region for effective pain alleviation. Does desflurane need more irrigating-pump pressure for the visibility in arthroscopic shoulder surgery than sevoflurane?

BACKGROUND: In arthroscopic shoulder surgery, a mechanical fluid-irrigation system is used to wash out blood from the joint. If used at high pressure, it can cause side effects such as fluid extravasation, leading to airway obstruction after surgery. Desflurane is prone to increasing sympathetic nerve activity and plasma catecholamine release more than another inhalation anesthetics. The objective of this study was to determine whether desflurane could increase in the irrigation pump pressure than sevoflurane in shoulder arthroscopy. METHODS: Patients were randomized into a sevoflurane group

A simple method to directly evaluate the lateral extension of the acromion: an anatomic study of 128 cadaveric scapulae.

BACKGROUND: The lateral extension of the acromion from the glenohumeral joint is the critical variable that both the acromial index and critical shoulder angle reflect. The purpose of this study was to establish a simple and reproducible method to directly measure the lateral extension of the acromion that will be independent of patient demographic characteristics, scapular rotation, or other morphologic features of the shoulder. METHODS: This study used 128 unpaired cadaveric scapulae with a mean age of 69.4 ± 11.1 years (66 right and 62 left scapulae, 65 female and 63 male cadaveric specime

An All-Anterior Approach for Quadruple Nerve Transfer for Upper Trunk Brachial Plexus Injuries.

BACKGROUND: The most commonly performed nerve transfers in upper trunk (UT) or partial brachial plexus injuries (BPIs) include the spinal accessory nerve to suprascapular nerve, Oberlin, and, lately, radial nerve (RN) (branch to triceps) to axillary nerve (AN) transfers. Routinely, the former 3 procedures are performed through an anterior approach (supraclavicular plus infraclavicular), while the triceps branch of the RN-AN transfer has been performed through a posterior approach with the patient in either the prone or semilateral position, which requires a separate incision in the posterior a
