Posterior listhesis of a lumbar vertebra in spinal tuberculosis.

The management of spinal tuberculosis, especially in children, is controversial. In children, vertebral destruction is more severe than adults because of the cartilaginous nature of their bone. Modern chemotherapy has significantly decreased mortality in spinal tuberculosis, but morbidity remains high. Without early surgery, patients can develop severe kyphosis leading to respiratory insufficiency, painful costopelvic impingement and paraplegia. Lumbar kyphosis results in early degenerative lumbar canal stenosis and is cosmetically unacceptable. We report a paediatric case of atypical spinal t

[Mid-term results of 360-degree lumbar spondylodesis with the use of a tantalum implant for disc replacement].

PURPOSE OF THE STUDY: The study presents the mid-term results in patients treated with circumferential, i.e., 360-degreee, fusion of the lumbar spine carried out by posterolateral instrumented spondylodesis and anterior intersomatic fusion using a tantalum implant for replacement of an intervertebral disc. The aim of the study was to verify tantalum implant quality, to evaluate segment fusion and to assess the outcome of this method by patients' subjective findings. MATERIAL AND METHODS: The prospective non-randomised study included the implantation of 47 tantalum cages in 40 patients by the

Short segment pedicle screw fixation for unstable T11-L2 fractures: with or without fusion? A three-year follow-up study.

In unstable thoracolumbar fractures T11-L2, exaggerated kyphosis at the end of treatment may predispose to late back pain and poor functional outcome. Short-segment (SS) (3 vertebrae) pedicle instrumentation has become a popular method of treatment. However the question to add a fusion or not is still under debate. The authors retrospectively evaluated the radiological and functional results in 74 patients who had undergone an SS pedicle screw fixation. They were divided into two groups: group 1 (39 patients) was the non-fusion group; group 2 (35 patients) was the fusion group. In the non-fusi

Open reduction of C1-C2 subluxation with the use of C1 lateral mass and C2 translaminar screws.

OBJECTIVE: Spinal cord compression secondary to a subluxation of one vertebral body over another can be achieved with reduction of the translational deformity. Intraoperative reduction of C1-C2 subluxations can be technically challenging when one uses traditional techniques (e.g., wiring and transarticular screw fixation). The popularization of C1 lateral mass and C2 pedicle screws has allowed surgeons to achieve a more complex realignment at this region of the spine. Control of both C1 and C2 with independent fixation can be used to obtain reduction. In certain instances, placement of C2 pedi

[Anterior cervical corpectomy and fusion with preserved posterior vertebral wall for multi-level cervical spondylosis: a prospective random study of 84 cases].

OBJECTIVE: To compare the results of anterior cervical corpectomy and fusion with preserved posterior vertebral wall (PWCF) with the classical anterior cervical corpectomy with fusion (ACF). METHODS: Eighty-four patients with multi-level cervical spondylosis were randomized to ACF (n = 42) or PWCF (n = 42). Postoperative JOA score were evaluated by an independent observer. Fusion rate, segmental lordosis, and disc height were assessed by radiographs at postoperative 3 months, 6 months, and 12 months. Fusion evidence was observed by CT scan. RESULTS: JOA score improvement was similar for both

Adjacent instability after instrumented lumbar fusion.

The invention of pedicle screw instrumentation has greatly improved outcomes of spinal fusion, which has become the treatment of choice for lumbar spondylolisthesis. As researchers accumulate experience, both theoretical and clinical advances are continually being reported. A review of the literature and the experience of the authors show that the development of adjacent instability, as in the breakdown of a neighboring unfixed motion segment, is a common consequence of an instrumented lumbar spine. This article reviews the risk factors and surgical treatment of adjacent instability. The autho

Lateral approach for resection of the C3 corpus: technical case report.

OBJECTIVE AND IMPORTANCE: The C3 level is the transition zone between the upper and lower cervical spine. Because of its high position and anatomic relationships to significant structures, exposing C3 is challenging, and the surgical approach is controversial. CLINICAL PRESENTATION: A 16-year-old girl was admitted to our institution with a 3-year history of neck pain and progressive quadriparesis. Neuroradiological examination revealed severe spinal cord compression from kyphosis at the C3 level. TECHNIQUE: We used the lateral approach to resect the C3 corpus and realign the cervical spine.

[Patient with skeletal metastases. Mobile despite destroyed bones].

Bone metastases endanger both the mobility of the cancer patient and his ability to profit from medical/nursing care. Treatment aims to prevent fractures of the long bones and vertebrae and to avoid neurological defects caused by compression of the spinal cord. Treatment is indicated when the following criteria are met: for the long bones a defect measuring more than 2.5 cm in length or affecting more than 50% of the circumference; for the vertebrae destruction of more than 60%. Almost all patients require radiotherapy, with a minority also needing surgical treatment in the form of intralesion

[The surgical treatment of traumatic dislocations of the cervical spine].

Thirty patients with injury-induced complete and incomplete dislocations and of the cervical vertebral column were operated on. New and old dislocations (up to 28 days) were managed by the open reduction and stabilization of an injured segment by a two-vane monolithic corundum ceramic endoprosthesis. If there was a significant stability of the injured segment and if temporary external fixation could not be applied, C-shaped braces providing effects of thermomechanical shape memory were used. In old injury, corporectomy was performed by means of anterior wedging corporodesis with an autogenous

Cervical amyloidoma of C2. Case report and review of the literature.

STUDY DESIGN: Second published report of a patient with amyloidoma of the upper cervical spine. OBJECTIVES: To describe a patient with rare radiculopathy to alert other physicians to consider amyloid tumor as a differential diagnosis of locally destructive spine lesions. SUMMARY OF BACKGROUND DATA: Localized amyloid tumor of the bone is a rare disease. Only seven cases of spine involvement have been reported. Appropriate tissue sampling is required to establish the diagnosis. Histopathologic examination shows pathognomonic apple-green birefringence under polarized light. When bone is involve

Osteoid osteoma of the lateral mass of C5. Should excision be combined with fusion?

A 10-year-old girl presented with a 1-year history of pain and stiffness in her neck associated with left shoulder and arm pain. This was found to be caused by an osteoid osteoma of the lateral mass of C5. Surgical excision of the tumour was performed through a posterior approach. Following surgery, the patient's pre-operative pain resolved. However, 3 months later she developed a recurrence of neck pain secondary to cervical instability. Further investigation revealed a grade II spondylolisthesis at the C5/6 level. A combined anterior and posterior fusion was performed and the patient's insta

[Biomechanical prerequisites for examining the stability of osteosynthesis procedures of the spine].

Basic reflections on biomechanical tests of spinal implants are reviewed. Four different pedicle screw systems are compared during loading in extension, flexion, side-bending, and rotation. The thoracolumbar and the lumbar spine were tested. Stability against rotation forces was lowest in all systems. Because of this, a cross-connector is recommended in highly unstable situations. All tests were done in cadaver spines with normal bone mineral density (measured before the tests). The first series of tests was done without destroying the cadavers. A second series was done with maximal load until

Whiplash injury and surgically treated cervical disc disease.

The incidence of a previous whiplash injury in 215 unselected patients who underwent an anterior cervical discectomy and fusion between 1988 and 1991 was found to be twice that of a control population of 800 general orthopaedic outpatients (P < 0.05). The mean age at which the whiplash injury occurred in the surgical group was 37 years and in the control group 36 years. The mean age at operation of those patients with a previous whiplash injury (45 +/- 12 years) was significantly less than those patients without a previous whiplash injury (55 +/- 14 years, P < 0.001). This study provides furth

Operative management of bilateral facet dislocation.

Fifty-two patients with acute traumatic bilateral locked facets were treated at one trauma center during a 3 1/2-year period (July, 1987, to December, 1990). The patients presented with complete motor quadriplegia (34 cases), incomplete myelopathy (13 cases), or intact long-tract function (five cases). The injuries occurred at C2-3 (one patient, with intact function), C4-5 (12 patients), C5-6 (16 patients), C6-7 (19 patients), and C7-T1 (four patients). Immediate traction (with increasing weight and serial x-ray studies) and/or induction of general anesthesia and muscle relaxation reduced the

[Trends and results of treatment of spinal fractures].

As long as simple and efficient operative techniques for stabilizing of injured segments of the vertebral column were missing naturally conservative methods dominated. The fixateur interne and the transpedicular spongiosa bone graft, however, provide operative techniques of high efficiency and technical simplicity, which make a revision of old indicational rules- operation versus conservative treatment - necessary. The example of a stable spine fracture points out that the calculated final result has to be clearly better if risk and effort of an operation should be advantageous for the patient
