An Intracisternal Rabbit Model Reveals Treponema pallidum Egress Across an Intact Blood-Brain Barrier.

BACKGROUND: Treponema pallidum (T. pallidum), the causative agent of syphilis, can invade the central nervous system (CNS) during early infection. However, whether blood-brain barrier (BBB) disruption is required for spirochetal traversal remains unresolved. METHODS: To determine whether T. pallidum can traverse the BBB without disrupting its integrity, we established an intracisternal (i.c.) rabbit model. Systemic serological responses and T. pallidum DNA in peripheral tissues were assessed as evidence of spirochetal egress from the CNS to the periphery. BBB integrity was monitored throughou

A mouse model of human mitofusin-2-related lipodystrophy exhibits adipose-specific mitochondrial stress and reduced leptin secretion.

Mitochondrial dysfunction has been reported in obesity and insulin resistance, but primary genetic mitochondrial dysfunction is generally not associated with these, arguing against a straightforward causal relationship. A rare exception, recently identified in humans, is a syndrome of lower body adipose loss, leptin-deficient severe upper body adipose overgrowth, and insulin resistance caused by the p.Arg707Trp mutation in MFN2, encoding mitofusin 2. How the resulting selective form of mitochondrial dysfunction leads to tissue- and adipose depot-specific growth abnormalities and systemic bioch

The prognostic variables predictive of mortality in patients with an exacerbation of COPD admitted to the ICU: an integrative review.

INTRODUCTION: Chronic Obstructive Pulmonary Disease (COPD) frequently presents with an acute exacerbation (AECOPD). Debate exists as to whether these patients should be admitted to intensive care units (ICUs). An integrative review was performed to determine whether clinical variables available at the time of ICU admission are predictive of the intermediate-term mortality of patients with an AECOPD. METHODS: An integrative review was structured to incorporate a five-stage review framework to facilitate data extraction, analysis and presentation. The quality of the studies contributing to the

Microclot array elastometry for integrated measurement of thrombus formation and clot biomechanics under fluid shear.

Blood clotting at the vascular injury site is a complex process that involves platelet adhesion and clot stiffening/contraction in the milieu of fluid flow. An integrated understanding of the hemodynamics and tissue mechanics regulating this process is currently lacking due to the absence of an experimental system that can simultaneously model clot formation and measure clot mechanics under shear flow. Here we develop a microfluidic-integrated microclot-array-elastometry system (clotMAT) that recapitulates dynamic changes in clot mechanics under physiological shear. Treatments with procoagulan

Scene Construction and Spatial Processing in Post-traumatic Stress Disorder.

INTRODUCTION: Post-traumatic stress disorder (PTSD) is associated with hippocampal system structural and functional impairments. Neurobiological models of PTSD posit that contextual memory for traumatic events is impaired due to hippocampal system dysfunction whilst memory of sensory details is enhanced due to amygdalar impact on sensory cortices. If hippocampal system dysfunction is a core feature of PTSD, then non-traumatic hippocampal-dependent cognitive functions such as scene construction, spatial processing, and memory should also be impaired in individuals with PTSD. METHODS: Forty-six

Neurotransmitter Receptor Interactions Control Astrocyte Ca²⁺ Signal in the Basolateral Amygdala.

INTRODUCTION: The endocannabinoid (eCB) system plays fundamental roles in both physiological and pathological aspects of brain function. Activation of type 1 cannabinoid (CB1) receptors in astrocytes by eCBs elevates their intracellular calcium through a signaling pathway that involves activation of Gq/11 proteins, phospholipase C, and IP3R2 to mobilize calcium. Hippocampal astrocytes have been proposed to integrate synaptic information through nonlinear interactions between glutamate and acetylcholine in eliciting calcium responses. However, whether similar integrative interactions occur amon

Research on dynamic analysis and optimization algorithms for large-scale power systems.

Modern power systems face severe dynamic stability and operational scheduling challenges due to the rapid penetration of renewable energy sources (RES). Aiming at the problems of insufficient dynamic modeling accuracy, low efficiency of stability analysis, and the difficulty in balancing operational safety and economy under high RES integration, this study proposes an integrated theoretical and methodological framework for large-scale power system dynamic analysis and optimal scheduling. First, a sixth-order nonlinear differential equation model is established by integrating the electromechani

MAPPing out the future: Modified autopsy prosection protocol for gross anatomy in medical education.

Contact hour reduction and elimination of dissection in gross anatomy in the increasingly systems-based curricula have reshaped medical education in the United States. While prosection-based anatomical instruction enhances efficiency, regional dissection protocols may no longer be optimally aligned with systems-based curricula. Clinical autopsy protocols may provide an alternative approach that aligns with contemporary medical education needs. The Modified Autopsy Prosection Protocol (MAPP), derived from clinical autopsy procedures, was developed and implemented in a systems-based medical curr

The Oral Microbiome and Systemic Health: Bridging the Gap Between Dentistry and Medicine.

The oral microbiome, consisting of a mixture of bacteria, fungi, and viruses, is an important contributor to oral and systemic health. Microbial balance disruptions are associated with oral pathologies like dental caries and periodontitis as well as systemic diseases such as cardiovascular diseases, adverse pregnancy outcomes, and respiratory diseases. This review explores the mechanistic pathways linking oral dysbiosis to systemic inflammation, endothelial dysfunction, and immune modulation. The roles of key microbial species in health and disease are analyzed, with an emphasis on how hematog

Peripheral anatomy of the dolphin ear and associated nervous structures: Insights from macroscopic dissection, DICE-µCT, histology, and confocal microscopy.

The peripheral auditory system of dolphins comprises specialised bony, fatty, vascular, and neural structures adapted for underwater hearing and diving physiology. These include the external ear canal, acoustic fat bodies, sinuses, and associated neurovascular networks, which together support sound conduction, protection, and possibly sensory functions. Despite advances in gross anatomical description, the detailed integration of these tissues, particularly the innervation, neurovascular organisation, and their functional implications, remains poorly understood. Previous studies have described

RNA-Seq Profiling of Neutrophil-Derived Microvesicles in Alzheimer's Disease Patients Identifies a miRNA Signature That May Impact Blood-Brain Barrier Integrity.

(1) Background: Systemic infection is associated with increased neuroinflammation and accelerated cognitive decline in AD patients. Activated neutrophils produce neutrophil-derived microvesicles (NMV), which are internalised by human brain microvascular endothelial cells and increase their permeability in vitro, suggesting that NMV play a role in blood-brain barrier (BBB) integrity during infection. The current study investigated whether microRNA content of NMV from AD patients is significantly different compared to healthy controls and could impact cerebrovascular integrity. (2) Methods: Neut

Open-Source Anatomical Illustration for Digital Back Pain Documentation: Development and Feasibility Evaluation.

BACKGROUND: Accurate documentation of back pain is essential for effective diagnosis and treatment; however, conventional paper-based methods often lack precision and standardization, limiting their clinical utility. Digital pain drawings can improve consistency and integration; however, the literature continues to report variability in digital pain drawing methods, quantification approaches, body schemas, and implementation. Moreover, open access, anatomically labeled vector assets mapped to clinical terminology remain limited. OBJECTIVE: This study aimed to develop an anatomically accurate,

Nurses' Engagement in Digital Health Clinical Trials Using Unobtrusive Monitoring Technologies: Constructivist Grounded Theory Study.

BACKGROUND: Unobtrusive digital monitoring technologies (UDMTs) enable continuous data collection beyond episodic clinical encounters and are increasingly incorporated into digital health clinical trials. However, their successful use depends on their integration into routine clinical practice. Poor integration can increase hidden nursing workload, disrupt workflows, compromise data quality, and limit the sustainability of digital trials. Although nurses play a central role in coordinating clinical, research, and technological activities, little is known about how they engage with UDMT-enabled

Development of an Equity-Centered Sociotechnical Architecture for Generative AI Integration in Public Health Promotion: Conceptual Framework.

This article is a viewpoint: it presents the authors' perspective, informed by a critical synthesis of the current literature at the intersection of generative AI (GenAI) technologies, public health communication, and digital ethics, rather than original empirical data or analyses. The emergence of GenAI, including large language models (LLMs), represents a profound paradigm shift in digital health communication. By moving beyond traditional information retrieval to dynamic, human-like knowledge generation, GenAI offers unprecedented opportunities for public health promotion. However, the ungu

Patient Outcomes Associated With Continuous Remote Patient Monitoring: A Scoping Review.

BACKGROUND: The COVID-19 pandemic highlighted the need for alternative healthcare delivery models, leading to the development of Continuous Remote Patient Monitoring (CRPM). CRPM allows for real-time monitoring of high-risk patients, reducing the burden on hospital resources. The integration of virtual nursing into CRPM has enhanced remote care capabilities, though it has also introduced new challenges related to patient safety and staffing, that is, nurse-to-patient ratios. OBJECTIVE: This scoping review aims to explore the current evidence on virtual nursing using CRPM and identify challeng

Addressing Clinical Challenges of Platinum Anticancer Drugs through Rational Chemical Design.

Platinum (Pt)-based anticancer drugs have been a cornerstone of chemotherapy for decades, yet their clinical application remains constrained by dose-limiting systemic toxicity and drug resistance. In this Account, we summarize our systematic efforts to address these challenges through two complementary strategies: 1) functionalization of Pt(IV) prodrugs and 2) spatially controlled targeted delivery. The kinetic inertness and octahedral geometry of Pt(IV) complexes offer a versatile platform for axial functionalization, allowing the integration of diverse bioactive ligands that are released upo
