Therapeutic strategy with indirect spinal manipulations in C2-C3 segments for long-term treatment of cervicogenic headache.

Cervicogenic Headache (CGH) is a secondary type headache, associated with dysfunction in upper cervical segments (C1-C2-C3) and manifested by specific clinical features. It is unilateral, starting from one side of the posterior head and neck, migrating to the front, sometimes associated with ipsilateral arm discomfort and, in addition, neuralgia with ipsilateral conjunctival inection. Our research is based on 29 patients (with average age of 49.78 years ± 10.41 (34-73)) with headache: 19 females and 10 males. We examined the active range of motion (AROM). We used palpatory maneuvers, finding l

Bugbears in the Waiting Room: Revisiting Arber and Sawyer's Classic Study of GP Reception Work Using Ethnography in Eight English General Practices.

In 1985, Arber and Sawyer described the discretionary rationing power of general practice receptionists. Our paper revisits this territory. Much has changed in the intervening decades. Digitalisation has altered reception work. Increasing multimorbidity and rising chronic illness, combined with a dwindling workforce, restricted funding and systemic pressures on public services, have fuelled the 'crisis' in general practice. 'Unacceptable' delays in getting a general practitioner (GP) appointment are seen as evidence of this. Our focused ethnography in eight English NHS general practices highli

Open Challenges and Opportunities in Piezoelectricity for Tissue Regeneration.

Emerging piezoelectric biomaterials have demonstrated their huge potential in diverse medical applications, including ultrasound diagnosis and tissue regeneration. Human body possesses inherent piezoelectricity, producing electrical signal under endogenous load or external pressure to modulate cellular behaviors. Tissue defects caused by traumatic injury will disrupt the electrophysiological microenvironment of the injured area, resulting in unenviable self-healing. Triggered by physical activities or external stimulation, piezoelectric biomaterials exhibit a unique capability to generate elec

Aerial urination suggests undescribed sensory modality and social function in river dolphins.

Scent marking through urine spraying is known to aid mate selection, territory marking and chemical communication in terrestrial, but not in aquatic mammals. We quantify an unusual aerial urination behaviour in botos (Inia geoffrensis) and discuss its potential functions. Between 2014 and 2018, we conducted land-based behavioural surveys on wild botos in central Brazil, recording the sequence, duration and social context of aerial urination. Aerial urination starts with a boto slowly positioning itself upside down, exposing its penis above water, and ejecting a stream of urine into the air. Wh

Dual Vascular Insult of Fibrocartilaginous Embolism and Intercostal Artery Dissection Causing Anterior Spinal Artery Infarction in a 14-Year-Old.

Acute spinal cord infarction (SCI) is a rare and devastating cause of myelopathy in adolescents. We report the case of a 14-year-old boy who developed acute paraplegia approximately 24 hours after a minor fall with axial loading. The clinical picture presented as the anterior spinal artery (ASA) syndrome, characterized by profound lower limb weakness and the pathognomonic finding of dissociated sensory loss (loss of pain and temperature with preserved proprioception). Magnetic resonance imaging (MRI) confirmed spinal cord ischemia with T2 hyperintensity restricted to the anterior cord. A subse

Spontaneous Internal Carotid Dissection Presenting With Pulsatile Tinnitus and Hearing Loss.

Internal carotid artery (ICA) dissection often presents with headache and neck pain, along with symptoms related to the anterior circulation if stroke occurs. Less commonly, it may cause Horner's syndrome and involvement of the lower cranial nerves (IX, X, XI, and XII). However, it is rare for ICA dissection to present with pulsatile tinnitus and hearing loss, which is typically linked to vertebral artery dissection. We report a rare case of a man in his 50s who initially presented to the Otolaryngology Department with a brief episode of high-pitched pulsatile tinnitus and hearing loss, preced

[Direct Bypass for Moyamoya Disease].

We performed direct bypass in adult moyamoya disease and combined direct and indirect bypasses in pediatric cases. Our surgical approach was based on techniques learned from Dr. Hiroyasu Kamiyama, which we refined over time. This paper provides an overview of our method, including its technical modifications and rationale. We performed multiple direct bypasses using the superficial temporal artery(STA) in the anterior and middle cerebral artery territories. Bypass of the anterior cerebral artery is routinely performed in pediatric patients to improve cognitive function. Because secondary bypas

Bilateral lower-extremity paresis as an atypical presentation of subarachnoid hemorrhage: A case report.

BACKGROUND: Subarachnoid hemorrhage (SAH) is a neurological emergency typically presenting with a sudden, severe headache. Focal neurological deficits are generally unilateral and correspond to the location of the aneurysm or hematoma. Bilateral lower limb paresis as an initial manifestation is extremely rare and may delay diagnosis. This report highlights a unique case and explores potential mechanisms underlying this atypical presentation. CASE DESCRIPTION: A 62-year-old woman presented with acute bilateral leg weakness and headache. Imaging revealed diffuse SAH from a ruptured aneurysm at

A rapid LASV detection method based on CRISPR-Cas13a and recombinase aided amplification with special lateral-flow test strips.

Lassa virus (LASV) is a high-risk pathogen associated with severe viral hemorrhagic fever in both humans and animals. Owing to its significant treatment challenges and high infectivity, LASV is classified as a biosafety level 4 (BSL-4) pathogen. It is essential to establish a rapid LASV detection method to prevent and control the disease. To address the biosecurity threats caused by LASV, in this study, we developed a new test method for LASV detection by combining the recombinase-mediated isothermal amplification (RAA) and CRISPR-Cas13a detection technology. The detection efficiency of this m

A Review of the Role of Caveolin-1 in Acetaminophen-Induced Liver Injury.

BACKGROUND: Acetaminophen (APAP) is commonly used as an antipyretic and analgesic agent. Excessive APAP can induce liver toxicity, known as APAP-induced liver injury (ALI). The metabolism and pathogenesis of APAP have been extensively studied in recent years, and many cellular processes such as autophagy, mitochondrial oxidative stress, mitochondrial dysfunction, and liver regeneration have been identified to be involved in the pathogenesis of ALI. Caveolin-1 (CAV-1) as a scaffold protein has also been shown to be involved in the development of various diseases, especially liver disease and tu

The conformational landscape of human transthyretin revealed by cryo-EM.

Transthyretin (TTR) is a natively tetrameric thyroxine transporter found in blood and cerebrospinal fluid whose misfolding and aggregation causes transthyretin amyloidosis. A rational drug design campaign identified the small molecule tafamidis (Vyndaqel/Vyndamax) as an effective stabilizer of the native TTR fold, and this aggregation inhibitor is regulatory agency-approved for the treatment of TTR amyloidosis. Despite 50 years of structural studies on TTR and this triumph of structure-based drug design, there remains a notable dearth of structural information available to understand ligand bi

Case Report: Treatment of Akinetic Mutism after Unilateral Anterior Cerebral Artery Infarction with Methylphenidate and Levodopa/Benserazide.

INTRODUCTION: Akinetic mutism is a severe state of impaired volition that can result from a stroke. Its therapeutic evidence relies on single case reports that used atomoxetine or levodopa with variable latency and efficacy. CASE PRESENTATION: We present the case of a 54-year-old woman who developed akinetic mutism after infarction in the territory of the right anterior cerebral artery, successfully treated with methylphenidate and levodopa/benserazide. Oxygen extraction fraction change in M1-M6 brain regions of patients with unilateral or bilateral middle cerebral artery occlusion.

Cerebral blood flow (CBF) and oxygen extraction fraction (OEF) can be measured using arterial spin labeling (ASL) and quantitative susceptibility mapping (QSM) sequences, respectively. ASL and QSM sequences were performed on 13 healthy participants and 46 patients with unilateral or bilateral Middle cerebral artery (MCA) occlusion. M1-M3 and M4-M6 correspond to anterior, lateral, and posterior MCA territories within the insular ribbon and centrum semiovale, respectively. In patients with unilateral MCA occlusion, significant decreases in CBF were observed in the lesions in M1, M3, M5 and M6 re

Drug utilisation research and medicine access in Mozambique: An overview.

Access to medicine is fundamental to the provision of equitable health systems. However, availability of affordable quality-assured medicines continues to be poor, especially in Low- and Middle-Income Countries (LMICs). Targeted efforts associated with transparency and accountability around medicine utilisation are required in LMICs to address this problem. With the increased global focus on improved access to medicines, there is a heightened imperative to understand the current landscape of medicine prescribing in these territories. In Sub-Saharan Africa, drug utilisation research is generall

Sargassum Inundations and the Risk of Hypertension Disorders Among Pregnant Women Living in the French Caribbean Island of Martinique.

Since 2011, Caribbean territories have experienced massive and repeated sargassum seaweed inundations. Once on shore, sargassum degradation through anaerobic metabolism elicits the release of many noxious molecules, including hydrogen sulfide (H2S) and ammonia (NH3). H2S has been long recognized as a malodorous and highly toxic gas, while chronic exposure has not been extensively explored. Our objective was to assess whether pregnant women exposed to sargassum emissions would be more prone to developing hypertensive disorders compared to unexposed women. We conducted a retrospective study incl

Middle cerebral artery fenestration with a contralateral early branching investigated through magnetic resonance angiography: an embryological view on an incidental finding.

PURPOSE: Middle cerebral artery (MCA) anomalies are a small group of congenital variants, including fenestration, duplication and Twig-like MCA. Some other variants, i.e. in the branching pattern, have been described independently from the previous ones, but their association might raise an embryological reasoning. METHODS: We are presenting an incidental finding in a patient undergoing brain Magnetic Resonance Imaging (MRI) and Angiography (MRA) for episodic headache, i.e. right MCA fenestration and left MCA very early branching. The patient was investigated only by MRA, being the finding in

Polytrauma Caused by a Bear Attacking a Human with a Benign Outcome.

Injuries to humans caused by wild animals, particularly bears, are rarely mentioned in the literature. Such injuries are frequent in Siberia, which is a territory surrounded by dense forests inhabited by brown bears. In the last 4 months alone (September-December 2023), four bear attacks on humans were registered in Khakassia, Russia. This article presents a clinical case of rehabilitating a patient after a bear attack, who suffered multiple fragmentary fractures of the facial skeleton with displaced bone fragments, subcutaneous emphysema of the soft tissues of the face, damage to the parietal
