Prediction of Thyroid Malignancies by Thyroid Auto Antibodies.

Thyroid cancer is the most frequent cancer among endocrine tumors, and account for approximately 1% of all malignancies. Recent literature has suggested an association between autoimmune thyroiditis and papillary thyroid cancer. The aim of the study was to choose the role of preoperative thyroid auto-antibodies as a predictive marker that could distinguish benign and malignant thyroid nodules and any other occult malignancy. This cross sectional study was done from July 2012 to June 2014 in department of Surgery Sir Salimullah Medical College & Mitford Hospital (SSMC&H) in collaboration with d

Exploring the Connection Between Thyroid Health and Psychiatric Disorders: A Comprehensive Review With a Focus on Schizophrenia and Bipolar Disorder.

The interplay between thyroid dysfunction and major psychiatric disorders, particularly schizophrenia and bipolar disorder, represents a critical area of research with significant implications for diagnosis and management. Thyroid dysfunction manifests as either hyperthyroidism or hypothyroidism, each exerting distinct effects on psychiatric conditions. This bidirectional relationship underscores the need for a comprehensive understanding to enhance clinical outcomes. This comprehensive review explores the underlying mechanisms linking thyroid dysfunction and psychiatric disorders, including n

Biochemical and ultrasonographic parameters influencing thyroid nodules elasticity.

Elastography is a method of tissue stiffness assessment. It has already been demonstrated that thyroid cancers are less elastic than benign lesions. However, little is known about other factors, which might influence the stiffness of thyroid nodules and disrupt the prediction of malignancy using this technique. The aim of this study was to conduct the first systematic assessment of factors potentially affecting the elasticity of thyroid lesions. One hundred and twenty-two patients with thyroid nodular disease admitted for thyroidectomy underwent preoperative ultrasonography and sonoelastograph

Primary thyroid disorders in endogenous Cushing's syndrome.

OBJECTIVE: To study the prevalence of primary thyroid disorders in patients who underwent endogenous hypercortisolism. DESIGN: Retrospective evaluation of 59 patients with Cushing's syndrome (CS) who had, at least, a record of thyroid palpation by expert endocrinologists and basal measurements of TSH by second generation assays. When available, tri-iodothyronine and thyroxine serum levels, TRH-TSH tests and anti-thyroid antibodies were also analyzed. There were two age- and gender-matched control groups. The 'goiter control group' comprised 118 healthy subjects who underwent thyroid palpation

Genistein in autoimmune diseases: from experimental evidence to emerging clinical data.

Autoimmune diseases are chronic inflammatory disorders characterized by loss of immune tolerance, activation of autoreactive immune cells, and progressive tissue injury. Despite advances in immunosuppressive and biologic therapies, treatment responses remain heterogeneous and long-term therapy is frequently associated with adverse effects and economic burden. These limitations have stimulated increasing interest in complementary immunomodulatory strategies, including naturally occurring bioactive compounds. Genistein, a soy-derived isoflavone belonging to the phytoestrogen family, has emerged

Adverse childhood experiences, mental distress, and autoimmune disease in adult women: findings from two large cohort studies.

BACKGROUND: Adverse childhood experiences (ACEs) have been associated with increased risks of autoimmune diseases. However, data are scarce on the role of specific ACEs as well as the potential mediating role of adverse mental health symptoms in this association. METHODS: A cohort study using the nationwide Icelandic Stress-And-Gene-Analysis (SAGA, 22,423 women) cohort and the UK Biobank (UKB, 86,492 women) was conducted. Participants self-reported on five ACEs. Twelve autoimmune diseases were self-reported in SAGA and identified via hospital records in UKB. Poisson regression was used to ass

Autoimmune polyendocrine syndrome type 1: an Italian survey on 158 patients.

BACKGROUND: Autoimmune Polyglandular Syndrome type 1 (APS-1) is a rare recessive inherited disease, caused by AutoImmune Regulator (AIRE) gene mutations and characterized by three major manifestations: chronic mucocutaneous candidiasis (CMC), chronic hypoparathyroidism (CH) and Addison's disease (AD). METHODS: Autoimmune conditions and associated autoantibodies (Abs) were analyzed in 158 Italian patients (103 females and 55 males; F/M 1.9/1) at the onset and during a follow-up of 23.7 ± 15.1 years. AIRE mutations were determined. RESULTS: The prevalence of APS-1 was 2.6 cases/million (range

Ocular manifestations and immunological profiles of thyroid eye disease with lacrimal gland enlargement.

INTRODUCTION: Thyroid eye disease (TED) is a prevalent autoimmune inflammatory condition in patients diagnosed with Graves' disease (GD). The lacrimal glands are implicated as an additional target organ of autoimmune reactions in TED, alongside orbital connective tissues. However, the underlying pathogenesis of lacrimal gland enlargement in TED and its associated symptoms and effective therapeutic strategies remain to be elucidated. This study aimed to investigate the ocular manifestations and immunological profiles of TED with lacrimal gland enlargement. METHODS: This study collected patient

Children and Adolescents Diagnosed With Inflammatory Bowel Disease Are at Increased Risk of Developing Diseases With a Possible Autoimmune Pathogenesis.

BACKGROUND: The development of diseases with a possible autoimmune pathogenesis is common in adults with inflammatory bowel disease (IBD). In early onset IBD, it may differ but the evidence is sparse. We aimed to investigate the risk and time span from IBD diagnosis to outcomes with different associated disorders with possible autoimmune pathogenesis. METHODS: A register-based study included all Danish patients with early onset of IBD (≤18 years) between 1980 and 2021 and 50 matched references without IBD for each case. We examined the risk of type 1 and type 2 diabetes, celiac disease, thyro

Neonatal TSH as a marker of iodine nutrition status. Effect of maternal ioduria and thyroid function on neonatal TSH.

INTRODUCTION: Neonatal thyroid stimulating hormone (nTSH) is a marker of iodine nutrition status in the population. The WHO considers a prevalence of less than 3% of nTSH levels greater than 5 mIU/L in samples obtained within 72h from birth indicative of iodine sufficiency. The aim of this study was to determine the prevalence of nTSH levels greater than 5 mIU/L in an iodine-sufficient population and its association with maternal, neonatal and obstetric factors. MATERIALS AND METHODS: A total of 243 pregnant women were recruited between May and June 2017 in our health area. A questionnaire of

Epidemiology of subtypes of hypothyroidism in Denmark.

OBJECTIVE: Studies of hypothyroidism are often based on referred patients, and limited information is available on the incidence rates of subtypes of hypothyroidism in the general population. We therefore studied incidences of subtypes of primary, overt hypothyroidism in a Danish population cohort and compared incidences in two subcohorts with different levels of iodine intake. DESIGN: A prospective population-based study, monitoring a well-defined cohort representative of the Danish population. METHODS: The Danish Investigation of Iodine Intake and Thyroid Diseases registry of hyper- and hy

Musculoskeletal Disorders in Chronic Obstructive Airway Diseases: A Neglected Clinical Entity.

Chronic obstructive pulmonary disease (COPD) is a lung disease that can affect various extra-pulmonary organs; one being the musculoskeletal system. Skeletal muscle dysfunction and osteoporosis are two important musculoskeletal disorders that have an impact on the quality of life in COPD patients in terms of morbidity and mortality. Treatment related adverse effects of COPD such as steroid-induced myopathy and osteoporosis are well recognised. Other comorbidities like sarcopenia, cardiovascular disease, metabolic diseases (diabetes mellitus, obesity, and thyroid diseases), chronic kidney disea

Hashimoto encephalopathy: a case report and a short revision of current literature.

Hashimoto encephalopathy (HE) is a rare but controversial entity encompassing a variety of neuropsychological presentations in the setting of autoimmune thyroid disease. HE, mostly described in adults, with a female‑to‑male ratio of 4:1, is a relatively rare entity in the pediatric population and probably under recognized as a cause of acute encephalopathy in children and adolescents. A number of pathogenetic mechanisms have been suggested. Female prevalence, presence of autoantibodies, fluctuating course, and response to immunomodulatory therapy suggest the autoimmune nature of the disease. E

Dermatitis herpetiformis in siblings.

Two Caucasian sisters, XZ and YZ, suffered from DH. However, the clinical course of their diseases was different; patient XZ, contrary to her sister YZ, suffered besides dermatitis herpetiformis (DH) also from coeliac disease (CD) and an autoimmune thyroid disease. The sisters were ordered to adhere to gluten-free diet and dapsone was prescribed, however, patient XZ developed a hypersensitivity to dapsone. The HLA typing disclosed that they were homozygous and that they shared HLA alleles DQB1*0201. Our results confirm the known association of DH to other autoimmune disorders and its well esta

[Autoimmune hypothyroidism nonresponsive to high doses of levothyroxine and severe hypocalcemia].

The factors that make difficult the normalization of TSH in hypothyroidism need special attention because some patients on thyroxine replacement do not maintain a normal TSH. We report a 50 year-old woman with autoimmune hypothyroidism of difficult compensation, associated with anemia, hypocalcemia with a previous episode of tetany, hypomagnesemia, psychologic alterations and important weight loss. After compensation of the hypothyroidism with doses of L-thyroxine as high as 325 microg/day, the hypothesis of a malabsorptive syndrome was raised. Celiac disease was confirmed by elevated serum an

Coeliac disease.

Increased awareness of non-classical presentations and more reliable screening tests have led to higher detection rates for coeliac disease in elderly adults. Clinical presentations are influenced largely by the long-standing course of the subclinical disease before diagnosis. In the majority of elderly patients, weight loss, diarrhoea and iron deficiency anaemia are present. With a delay in diagnosis, there are increased risks of associated autoimmune diseases, of neoplasms (mostly small bowel lymphoma) and of metabolic bone diseases. Thyroid disease is the most common autoimmune disease. Lym

[The general internal medicine department].

The Department of General Internal Medicine is devoted to the evaluation of patients with autoimmune systemic diseases, multiorganic disorders or presenting non specific symptoms such as chronic fatigue, unexplained weight loss or fever of unknown origin. The interest in salt and water metabolisms had led to original contributions in the treatment of hyponatremia in man and to the understanding of the osmotic demyelinating syndrome in a rat model of hyponatremia. The study of ageing in man and rodents had contributed to better understand lymphocyte and thyroid function in the elderly. The care

[Simultaneous occurrence of acute myasthenia gravis and acute polymyositis in 3 patients and in 2 patients also associated with a thymoma].

In the Slovak Centre for Neuromuscular Diseases 768 patients with myasthenia gravis are on the records. 112 myasthenics (14.5%) have another associated autoimmune disease. Most frequently a combination with an autoimmune disease of the thyroid gland or rheumatoid arthritis is involved. Extremely rare is the concurrent manifestation of the acute form of mysthenia gravis and acute polymyositis, observed by the authors in three patients, incl. two associated with a thymoma. In all three patients the disease had a serious clinical picture which called for controlled ventilation and a nasogastric tu

Graves' ophthalmopathy.

The Graves' ophthalmopathy consists of all the pathological changes in orbital tissues, preceding, accompanying of following hyperthyroidism. Although ophthalmopathy has its clinical expression only in 50% of patients with Graves disease, the ultrasound exploration and the CT scan of the orbital area, as well as the values of intraocular pressure showed that occult changes exist in all patients. The etiopathological autoimmune mechanism responsible of ophthalmopathy doesn't imply the same autoantibodies, though ophthalmopathy is associated with thyrotoxicosis, and the degree of ocular changes
