Perilymph fistulae have been described for over 30 years and yet there are no universally accepted diagnostic criteria or treatment regimes for them. This paper describes an unusual case of a presumed traumatic perilymph fistula and discusses its treatment and prognosis in relation to previous studies.

PURPOSE: The objectives of this study were to determine the cold water exposure necessary to produce exostoses of the external auditory canal in individuals who predominantly surf along the Oregon and/or the northern California coastline and to ascertain from surfers' otologic histories symptoms attributable to exostoses.
MATERIALS AND METHODS: Free ear examinations were provided at two surf shops in northern Oregon. The ear canals were examined with an otoscope, and each surfer was given a summary score indicating the percentage of both canals that was obstructed by exostoses. Surfers were t

This paper reports findings in a pair of temporal bones from a patient with clinical evidence of Lermoyez's syndrome, a rare variant of Meniere's disease. Endolymphatic hydrops is limited to the basal turn of the cochlea and saccule in Lermoyez's syndrome, but more generalized in Meniere's disease.

An 18-year follow-up of a case of bilateral Lermoyez's syndrome is presented. The left ear having reached a stabilized hearing loss about 9 years after the onset of the disease, the right ear, apart from some isolated early periods of hearing loss, started to show the full extent of clinical symptoms after about 16 years. Electrocochleographic observations are presented. Studies were performed twice in the right ear during a period of strongly fluctuating hearing thresholds, once in an impaired and once in a relatively good condition. Electrocochleography of the stabilized left ear was perform

Otosclerosis begins in the second and third decades of life but usually does not produce a hearing loss until after the fourth decade. Chalky white plaques in the tympanic membrane may be benign, but when associated with hearing loss may point to tympanosclerosis. Presbycusis is the most common cause of hearing loss. Unilateral hearing loss raises the specter of acoustic neuroma.

There is a disturbing variation in the reported success rate in Endolymphatic Sac Surgery in the literature. Relatively long-term (24 months to 10 years) follow-ups are given in a series of 66 patients in this paper. Results reveal only a 55% long-term relief of vertigo in these patients. Only 30% of this series of cases had any improvement in cochlear function. There was a definite trend towards recurrence of vertigo in the longer follow-up cases. It is concluded that Endolymphatic Sac Surgery still is important in the treatment of idiopathic endolymphatic hydrops, but is of questionable valu

The equilibrium troubles occuring in connection with cervical locomotoric diseases and their otorhino-laryngologic relations have been studied in the ORL Department of National Institute of Rheumatism and Medical Hydrology. It has been found that: a) there is no connection between the seriousness of roentgenologic deformations and equilibrium troubles; b) the damage of the periphery is not characteristic; c) the complaints and symptoms are explained by the hypoxia of vestibular kern-areas as well as by damages of receptors in cervical muscles, bands and joints; d) hypotension and other central

BACKGROUND: Ménière's disease is an incapacitating disease in which recurrent attacks of vertigo are accompanied by hearing loss, tinnitus and/or aural fullness, all of which are discontinuous and variable in intensity. A number of different therapies have been identified for patients with this disease, ranging from dietary measures (e.g. a low-salt diet) and medication (e.g. betahistine (Serc®), diuretics) to extensive surgery (e.g. endolymphatic sac surgery). The Meniett® low-pressure pulse generator (Medtronic ENT, 1999) is a device that is designed to generate a computer-controlled sequenc

Hyperacusis is marked by an abnormal sensitivity to everyday sounds, causing considerable distress and hindrance in daily life. It is frequently linked with other hearing disorders like tinnitus and may occur alongside neurological and psychiatric conditions such as autism, migraines, and depression. Recent research into hyperacusis treatment has explored various sound therapy methods, including counselling and the use of therapeutic sounds. Consequently, the current systematic review investigated whether sound-based therapy programs could alleviate the effects of hyperacusis. A total of 149 a

BACKGROUND: Compression of the upper segment (J3) of the internal jugular vein (IJV) between the C1 transverse process (C1TP) and styloid process (SP) is an increasingly recognized condition. No previous case of J3-IJV compression by the internal carotid artery (ICA) has been reported. The authors present the case of a patient with J3-IJV compression treated with arterial transposition.
OBSERVATIONS: A 49-year-old male presented with a multiyear history of intractable pulsatile tinnitus, headaches, visual disturbances, cognitive disturbance, and head pressure. The patient had previously under

BACKGROUND: Ceruminous adenomas (CA) are rare neoplasms of the external auditory canal (EAC), and their nonspecific symptoms frequently contribute to diagnostic challenges in clinical practice. By examining the clinical features, diagnostic considerations, and therapeutic approaches for these tumors, the findings aim to improve understanding of these tumors and improve clinical outcomes.
CASE PRESENTATION: A retrospective analysis was conducted on six individuals diagnosed with CAs of the EAC at a single institution between January 2010 and January 2023. Clinical data, histopathological chara

BACKGROUND: Arachnoid granulations are extensions of the subarachnoid space, an important component of the complex circulation of brain cerebrospinal fluid. While these structures primarily transmit cerebrospinal fluid into the dural venous sinuses, they also may serve as a conduit for brain tissue herniation. Such occurrences have been referred to in the literature as brain herniations into arachnoid granulations (BHAGs), which are considered incidental and asymptomatic but can be associated with nonspecific neurologic symptoms such as headache, tinnitus, vertigo, and seizure. BHAGs can be vi

The authors developed a new disease-specific patient-reported outcome measure to capture the multifaceted impacts of vestibular schwannoma on daily life, the Vestibular Schwannoma Quality of Life (VSQOL) Index, which contains 40 items grouped into eight categories: hearing problems; dizziness and imbalance; pain, discomfort, and tinnitus; problems with face or eyes; impact on physical, emotional, and social well-being; difficulty with thinking and memory; satisfaction or regret; and impact on employment.1 The VSQOL Index was developed in English, although the availability of this instrument in

BACKGROUND: Ménière's disease is a chronic inner ear disorder characterized by recurrent vertigo, tinnitus, aural fullness, and fluctuating sensorineural hearing loss. For patients with debilitating symptoms unresponsive to conservative treatment, intratympanic gentamicin is a therapeutic option. However, its efficacy and safety remain uncertain.
AIMS/OBJECTIVES: This systematic review aims to evaluate the effectiveness and safety of intratympanic gentamicin in Ménière's disease, focusing exclusively on randomized controlled trials (RCTs).

PURPOSE: This article addresses the complex clinical scenario where patients present to otolaryngologists with symptoms typically ascribed to chronic rhinosinusitis (CRS) or migraines which may in fact stem from elevations in intracranial pressure. We aim to clarify the diagnostic challenges and emphasize the importance of considering elevated intracranial pressure (eICP) as its symptoms overlap with both CRS and migraines.
METHODS: This narrative review synthesizes clinical experiences and literature to discuss the differential diagnoses involving CRS, facial pain/pressure, migraines, and eI

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is the most common cause of peripheral vertigo. BPPV is usually idiopathic but can also result from trauma, viral labyrinthitis, or Meniere's disease. We present a case of BPPV caused by earbuds.Case Descripton:This is the case of a 43-year-old male, a health care management and emergency medicine specialist, with no history of inner- or middle ear disease, vertigo and dizziness, or other medical conditions. The subject began using earbuds while driving and during routine exercise. He reported suffering from nausea and temporary vertigo a
