Thyroid storm precipitated by trauma.

A 21-year-old woman presented to the emergency department complaining of head, neck, and abdominal pain after being assaulted. She denied a previous history or significant symptoms of hyperthyroidism. The physical examination was remarkable for lethargy, low-grade fever, tachycardia, facial abrasions, swelling of the anterior neck, and a tremor. A diagnosis of thyroid storm was made. An extensive work up excluded other causes of the patient's tachycardia, altered mental status, and neck swelling. A review of the clinical features and management of thyroid storm is presented. Relatively minor t

Parkinson's disease.

The prevalence of Parkinson's disease rises with age. The clinical features can be the typical ones or those associated with the "late-onset" type, with more gait and balance problems and a mild progressive dementia. Other disorders, such as stroke and essential tremor, may be mistaken for Parkinson's disease, or multiple conditions may be present which modify the response to treatment. The treatment of choice is a low dose of one of the L-dopa-decarboxylase inhibitor drugs. Other agents have a high incidence of side effects, particularly confusion, which also occurs with L-dopa in higher dose

Four cases of neuroleptic malignant syndrome.

The neuroleptic malignant syndrome (NMS) is a relatively uncommon, sometimes fatal, idiosyncratic drug reaction usually associated with the administration of neuroleptic drugs. The syndrome is characterized by generalized rigidity, fever, altered consciousness or mutism, tremor, autonomic dysfunction, tachypnea, elevated serum creatinine phosphokinase, leukocytosis and myoglobinuria. We report four cases of NMS; one following levodopa/bromocriptine withdrawal, two related to neuroleptic administration and one following heroin use. There are no previous reports linking heroin to NMS. The syndro

An autopsy case of late infantile and juvenile neuroaxonal dystrophy with diffuse Lewy bodies and neurofibrillary tangles.

The clinical and pathological features of a sporadic case of juvenile neuroaxonal dystrophy beginning at the age of 10 and leading to death at the age of 26 are described. Clinical manifestation began with cerebellar symptoms. The subject subsequently developed dementia, pes cavus (Friedreich's feet), epilepsy, myoclonus, and Parkinsonian syndrome, but demonstrated neither tremor nor choreoathetoid movement. Pathological examination showed typical generalized axonal dystrophy throughout the central nervous system (Seitelberger's disease). Iron-positive pigmentation was seen in the pallidonigra

[Atrium-related hepatitis. Report of four cases].

Four patients developed acute hepatitis after receiving Atrium, an association of phenobarbital, febarbamate and difebarbomate, for the treatment of tremor or for the prevention of alcohol withdrawal symptoms. Hepatitis occurred 1 to 3 months after treatment. Asthenia was the unique clinical manifestation. Marked increase in serum aminotransferases and gamma-glutamyltranspeptidase levels were the main biological features. Histological examination showed liver cell necrosis in two cases, prominent in the centrolobular area in one case. There was no case of hepatic failure. Atrium withdrawal was

Cyclosporin therapy for severe scleritis.

To ascertain the efficacy of systemic cyclosporin therapy in the management of scleritis we performed an open, uncontrolled study of the use of this drug in severe refractory disease. Five of seven patients whose disease had previously been poorly controlled with a combination of corticosteroids and immunosuppressive drugs responded to cyclosporin therapy (10 mg/kg/day). Systemic side effects occurred in all but one patient, with tremor, hirsutism, hypertension, and raised serum creatinine being common. Recurrence of disease activity on decreasing the dosage of cyclosporin was frequent. The re

[Pulmonary function tests in asthmatic children in treatment with tulobuterol].

The objective of the present study was to evaluate the effectiveness and tolerance of a new beta-2 agonist, tulobuterol, in children. Fifteen patients with acute asthma were studied, during a period of 21 days. Significant increase in forced vital capacity (FVC), forced expiratory volume in the first second (FEV-1), and forced expiratory flow 25-75% (FEF 25-75%) were observed after treatment with tulobuterol (p less than 0.01). Clinical improvement was evident as measured by a clinical-scale and a functional scale. The only side effect attributable to the drug was a tremor which was observed i

Carbon disulfide-induced neuropsychiatric changes in grain storage workers.

The clinical and neuropsychiatric features of seven workers from the grain storage industry are described. All were exposed to fumigants, especially carbon disulfide and malathion. Clinical manifestations included cogwheel rigidity, resting and action tremor, peripheral neuropathy, pyramidal tract signs, an EEG pattern of sleep apnea, and characteristic measurable neurobehavioral dysfunction. Similar signs and symptoms due to carbon disulfide exposure in the viscose rayon industry strongly supported the conclusion that these grain storage workers were suffering from carbon disulfide poisoning.

[Benzodiazepine physical dependence. 6 cases (author's transl)].

The symptoms include, by ascending order of severity: nightmares and insomnia, nausea and vomiting, muscular weakness or tremor, postural hypotension, hyperthermia, muscle twitching, convulsions, confusional state or psychosis. Prominent features are the late onset of these symptoms, several days after treatment has been discontinued, and the sometimes difficult diagnosis, since patients are usually unaware of their dependence on these drugs. Reinstituting benzodiazepine treatment, then withdrawing it progressively are the best curative measures. Prevention is easy if treatment is gradually ra

X-linked spinal and bulbar muscular atrophy of late onset. A separate type of motor neuron disease?

The clinical, electrophysiological and muscle biopsy findings of 7 cases with a peculiar form of X-linked hereditary degenerative motor neuron disease are presented. It is suggested that the disease might be a separate clinical entity with the following characteristics: (1) sex-linked recessive inheritance, (2) unusual but not invariable late onset, (3) slow progression, (4) facial-bulbar and proximal spinal muscle involvement, (5) consistent fasciculations, sometimes massive and more pronounced about the lips, chin and tongue, (6) fine tremor of the hands, (7) muscle cramps usually preceding

Chapter 33: the history of movement disorders.

UNLABELLED: Role of basal ganglia: Vesalius and Piccolomini distinguished subcortical nuclei from cortex and white matter in the 16th century. Willis' mistaken concept in the late 17th century that the corpus striatum was the seat of motor power persisted for 200 years and formed the basis of mid-19th-century localizations of movement disorders to the striatum (chorea by Broadbent and Jackson, and athetosis by Hammond). By the late 19th century, many movement disorders were described but for most no pathologic correlate was known. Tremor: Descriptions of tremors progressed from Galen's definit

Management of Lithium-associated Hyperparathyroidism in an Elderly Patient.

Lithium-associated hyperparathyroidism is one of the most common causes of hypercalcemia in patients taking lithium. We present an elderly patient who developed hypercalcemia secondary to chronic lithium therapy-induced hyperparathyroidism. He presented with tremors and myoclonus. It was precipitated by a concomitant intake of diuretics, leading to dehydration. Extracorporeal therapy for lithium removal was not given even though he had neurological symptoms, and he was managed with intravenous fluids. He promptly recovered and lithium was stopped. Lithium-related endocrinological manifestation

Generation of an ITPR1+/- and isogenic control induced pluripotent stem cell line for SCA15/16 model development.

Spinocerebellar ataxia type 15/16 (SCA15/16) is a rare neurodegenerative disorder caused by heterozygous deletions of ITPR1, leading to haploinsufficiency of the encoded endoplasmic reticulum membrane calcium channel. Patients present with progressive gait disturbances, abnormal eye movements, difficulties with speech and swallowing, and tremors associated with atrophy of the cerebellum. Using CRISPR/Cas9 technology, we generated ITPR1+/- and isogenic control induced pluripotent stem cell (iPSC) lines from PGP1 iPSCs for SCA15/16 model development. The clones were genotyped, karyotyped, and as

A Rare Case of Pyrethroid Poisoning Presenting as Auditory Hallucination.

Pyrethroid is an insecticide which is widely used for household pests. Even though they are relatively less toxic to humans, reports of accidental poisoning are not uncommon. Cases of anosmia, tremors and arrhythmias after pyrethroid exposure have been reported; however auditory hallucinations have not been presented yet. Herein, we present a case of auditory hallucination in the form of ringing in the ear after exposure to airborne pyrethrin in a 69-year-old male. The significance of this case report is related to the wide use of insecticides containing pyrethrin and highlights the need for p

FDG PET/CT Aiding in Early Diagnosis of Central Pontine Myelinolysis.

We report a 70-year-old man with central pontine myelinolysis who presented in the emergency department with recent onset altered sensorium and tremors (Glasgow Coma Scale score, 13). Laboratory findings revealed hyponatremia and hypokalemia. MRI brain was unremarkable. Subsequently, patient was referred for FDG PET/CT to rule out malignancy or paraneoplastic syndrome. FDG PET revealed focal radiotracer uptake in the pons without any underlying CT abnormality consistent with central pontine myelinolysis, thus aiding in early diagnosis where conventional imaging modality was unremarkable.

Management of Postural Orthostatic Tachycardia Syndrome in the Absence of Randomized Controlled Trials.

Postural orthostatic tachycardia syndrome (POTS) is a clinical syndrome causing patients to experience light-headedness, palpitations, tremors, and breathlessness upon assuming an upright posture. Despite the absence of available long-term, multicenter, randomized controlled trial data, this literature review aims to concisely present the nonpharmacological and pharmacological interventions that have been used in the treatment of POTS reported to date by cross-sectional studies, cohort studies, and retrospective studies. We attempt to classify treatments as first-, second-, and third-line ther

Acute dysphagia in an octogenarian: an unusual case of tetanus.

Tetanus is a major worldwide health problem, and its global incidence has been estimated to be approximately 1 million cases per year. In particular, tetanus is more frequent in the elderly as compared with adults. We report a case of an octogenarian who presented with dysphagia and tremors as the only clinical symptoms. During hospitalization, the patient's clinical conditions worsened rapidly, and, although in absence of the classic clinical presentation (trisma, nuchal rigidity, and opisthotonus), a diagnosis of tetanus was suspected. Thus, the patient underwent a tetanus immunoglobulin imm

Sarcocystis neurona-associated meningoencephalitis and description of intramuscular sarcocysts in a fisher (Martes pennanti).

A free-ranging juvenile fisher (Martes pennanti) with ataxia, lethargy, stupor, and intermittent, whole-body tremors was examined postmortem. Microscopically, the fisher had protozoal meningoencephalitis caused by Sarcocystis neurona, which was confirmed by immunohistochemistry, polymerase chain reaction (PCR) and restriction fragment length polymorphism testing, and genetic sequencing. Sarcocysts found in the skeletal muscle of the fisher were negative for S. neurona by PCR, but were morphologically similar to previous light and electron microscopy descriptions of S. neurona. This is the firs

[Importance of alcoholism markers in the diagnosis of alcoholic heart lesions].

A study was made of opportunities for the use of some signs of alcoholic intoxication in the verification of alcoholic etiology of heart lesion. Clinical signs like hyperemia of the face with telangiectasia, venous plethora of the eyeballs, tremor of the lips, tongue, limbs, Dupuytren's contracture, enlarged liver size combined with a positive macrocytosis test and, to a lesser degree, with a higher activity of gamma-glutamyl transpeptidase, and the detection of fatty hepatosis in liver puncture biopsy were shown to suggest alcoholic intoxication, and excluding other cases of heart lesion they
