Surgical ductal stent implantation in total anomalous pulmonary venous connection to vena porta with right pulmonary sequestration in a mature newborn.

In many cases of total anomalous pulmonary venous connection (TAPVC), the four pulmonary veins (PV) join together behind the left atrium, where they form a collector. This collector can drain into the right atrium directly through the innominate vein into the superior vena cava (SVC), into the coronary sinus (CS), or through the diaphragm to the venous structures of the abdomen. In our case, a mature newborn had TAPVC draining into the vena porta along with severe pulmonary hypertension. Additionally, there were right pulmonary sequestration, dextrocardia, transposition of the great arteries,

Estimating right atrial pressure using upright computed tomography in patients with heart failure.

OBJECTIVES: Upright computed tomography (CT) can detect slight changes particularly in the superior vena cava (SVC) volume in healthy volunteers under the influence of gravity. This study aimed to evaluate whether upright CT-based measurements of the SVC area are useful for assessing mean right atrial pressure (mRAP) in patients with heart failure. METHODS: We performed CT in both standing and supine positions to evaluate the SVC (directly below the junction of the bilateral brachiocephalic veins) and inferior vena cava (IVC; at the height of the diaphragm) areas and analyzed their relationsh

Echoscopy in scanning abdominal diseases in a critical care setting.

BACKGROUND AND AIMS: Targeted ultrasound examinations with portable ultrasound device (handheld ultrasound system [HHUS]) have been defined as "echoscopy" by the European Federation of Societies of Ultrasound in Medicine and Biology (EFSUMB). For abdominal diseases it has been shown that echoscopy is sensitive and specific. The aim of this study is to show that the use of HHUS for abdominal ultrasonography is possible under the conditions prevailing in emergency and intensive care medicine and that it is not inferior to high-end devices (high-end ultrasound systems [HEUS]). METHODS: Examinati

A Case Report of Left Atrial Isomerism in a Syndromic Context.

The objective of our paper is to underline the importance of assessing microarray genetic analysis for the detection of chromosomal abnormalities in rare cases such as left atrial isomerism, mostly in the context of antenatally detected syndromes. We present the case of a 26-year-old primipara, at 26 weeks of gestation, with prior first trimester normal anomaly scan, who presented in our department accusing lower abdominal pain. An anomaly ultrasound examination of the fetus revealed cardiomegaly with increased size of the right atrium, non-visualization of the atrial septum or the foramen ova

Enoxaparin treatment in high-risk trauma patients limits the utility of surveillance venous duplex scanning.

OBJECTIVE: The value of surveillance venous duplex scanning for detecting unsuspected deep venous thrombosis (DVT) in trauma patients who are receiving enoxaparin prophylaxis is open to question. This study was undertaken to determine whether enoxaparin reduced the clinical utility of surveillance scanning and whether management of these patients was altered by findings of the scans. METHODS: The medical records of trauma patients who met defined criteria for high DVT risk, admitted during 30 consecutive months, were reviewed. These patients received enoxaparin 30 mg every 12 hours for the du

A volumetric index for the quantification of deep venous thrombosis.

PURPOSE: The evaluation of treatment strategies for deep venous thrombosis (DVT) is assessed through the use of a reliable method of quantifying the extent of the thrombotic process. Previous indices of thrombus burden have suffered from various limitations, including lack of clinical relevance, poor correlation with actual thrombus mass, and failure to include important venous segments in the methodology. The use of a novel scheme of quantifying venous thrombus was evaluated as an alternative method that would avoid some of the drawbacks of existing indices. METHODS: The volumes of 14 venous

Hepatic venous blood and the development of pulmonary arteriovenous malformations in congenital heart disease.

BACKGROUND: Pulmonary arteriovenous malformations (PAVMs) are a known complication after some types of cavopulmonary anastomoses (CVPAs). Their cause is unknown, but they may be related to the absence of pulsatile flow or the presence or absence of circulating factors. These PAVMs are diffuse and are presumed to be progressive and irreversible. METHODS AND RESULTS: All patients with congenital heart disease (CHD) seen at Children's Hospital, Boston, Mass, between 1970 and 1993 were reviewed. We report on the 10 patients with CHD who were found to have developed PAVMs, as diagnosed by cardiac

An unusual cause of fatal pulmonary embolism: A case report.

Adrenal tumours invading the inferior vena cava (IVC) are rare but can cause life-threatening complications such as pulmonary embolism. Early recognition, rapid diagnosis, and prompt imaging-including point-of-care ultrasonography (POCUS), echocardiography, and advanced modalities like CT and PET-CT-are essential for optimal management. We present a patient with pulmonary adenocarcinoma and secondary adrenal tumour that extended into IVC and caused fatal tumour thrombus pulmonary embolism.

A Cause of Right-Sided Flank Pain: Retrocaval Ureter.

BACKGROUND: The retrocaval ureter represents a rare congenital anomaly that occurs in 0.1% of the population and causes progressive ureterohydronephrosis, most of which are asymptomatic. CASE REPORT: In this case report, the retrocaval ureter detected in an adult male complaining of right-sided colicky flank pain is discussed. CONCLUSION: This condition, whose main cause is an anomaly in the development of the vena cava, is easily diagnosed with imaging methods, and its symptoms can be completely corrected with surgical treatment.

A missed diagnosis of retrocaval ureter; case report and review of the literature.

The retrocaval ureter is an uncommon congenital anomaly due to an abnormal development of the inferior vena cava. Our case describes an 8 year-old boy who was referred to our center as a case of ureteropelvic junction obstruction with persistent hydronephrosis after pyeloplasty. Retrograde pyelogram showed features of retrocaval ureter which was managed surgically with constructive repair of the ureter. The low clinical incidence may be due to a number of asymptomatic cases that are not diagnosed in the patient's lifetime.

[Bronchogenic Cyst with Rapid Increase and Emergency Operation;Report of a Case].

A 21-year-old man was admitted with fever and back pain. Chest computed tomography(CT) showed a cystic mass of 7×6 cm in the right middle mediastinum. After 3 days, symptoms worsen, and chest X-ray revealed the enlargement of the cyst, and an emergent operation was performed. Since the tumor was severely adhered to the superior vena cava and the azygous vein, the cystic mass was partially resected. The pathological diagnosis was bronchogenic cyst.

Systemic Venous Rerouting Through the Coronary Sinus for ccTGA With Bilateral SVCs.

The double-switch operation for congenitally corrected transposition of the great arteries, in which the morphologic left ventricle is restored to the systemic circulation, is an effective surgical option. This case report describes an atrial switch technique without using supplemental material during the double-switch operation for congenitally corrected transposition of the great arteries associated with persistent left superior vena cava, with the systemic venous blood flow rerouted through the enlarged coronary sinus.

Percutanous management of tricuspid regurgitation: The "Achilles tendon" of transcatheter valve interventions.

Moderate-to-severe tricuspid insufficiency affects 1.6 million patients in the USA, resulting in increased morbidity and mortality and without safe and effective medical or surgical treatment options. The unique tricuspid valve anatomy, the presence of immediate adjacent anatomic landmarks at risk and the decreased blood pressure of flow complicate the development of effective transcatheter strategies. The HOVER trial will assess the safety and effectiveness of the CAVI approach with implantation of the Edwards, Sapien XT in the inferior vena cava.

A case of mediastinal fibrosis due to radiotherapy and 'downhill' esophageal varices: a rare cause of upper gastrointestinal bleeding.

'Downhill' varices are located in the proximal part of the esophagus. Their etiology differs from the distal types, with most of them usually being related to superior vena cava obstruction. Although bleeding due to 'downhill' varices is very rare, it can be life-threatening. Here, we present a case of upper gastrointestinal bleeding due to mediastinal fibrosis associated with chest radiotherapy for seminoma metastasis sixteen years previously, which was successfully treated conservatively.

Right heart thrombus causing syncope in an elderly patient.

Pulmonary embolism and thromboembolic disease carry a high mortality if not recognised and managed appropriately. Herein we illustrate the case of a dehydrated elderly female patient with recurrent syncope who proved to have high risk pulmonary embolism and a free floating right heart thrombus. The echocardiographic findings of right heart thrombus and possible thrombi 'in transit' within a low flow inferior vena cava, guided a life-saving treatment in this frail elderly patient.

Systemic-to-pulmonary venous shunt in a patient with non-Hodgkin lymphoma: A case report and review of the literature.

We describe a case of a systemic-to-pulmonary venous shunt secondary to superior vena cava obstruction in a patient with newly diagnosed non-Hodgkin lymphoma. This rare condition manifested with symptoms of dyspnea and hypoxemia that were out of proportion to the pleural effusion diagnosed on chest imaging. Standard treatment of such rare collateral plexuses is observation. However, it is important for clinicians to be cognizant that in rare cases such plexuses can lead to right-to-left shunt complications such as embolism.

[Lipomatous hypertrophy of the interatrial septum is a rare, benign cause of atrial arrhythmia].

Lipomatous hypertrophy of the interatrial septum is a pathological thickening of the interatrial septum characterized by a benign infiltration of fat. It usually occurs in older, obese persons and in most cases the diagnosis is made incidentally. Atrial arrhythmias may occur and in rare cases also superior vena cava syndrome caused by obstruction of venous inflow to the right atrium. We present a case of lipomatous hypertrophy of the interatrial septum in an 81-year-old woman with atrial arrhythmia.

Primary adrenal melanoma with inferior vena caval thrombus.

Primary adrenal melanoma is a rare tumor. Most lesions are confined to the adrenal gland, without extension to surrounding structures. We report a primary adrenal melanoma discovered during an evaluation for pulmonary embolism. Tumor thrombus was present in the inferior vena cava extending into the right atrium. A complete resection was obtained using cardiopulmonary bypass and profound hypothermic circulatory arrest. Long-term survival with melanoma is still limited, regardless of the organ of origin.
