Evaluation of genotype-phenotype associations in leber congenital amaurosis.

PURPOSE: To describe the clinical phenotypes associated with various genotypes known to cause Leber congenital amaurosis (LCA). METHODS: One hundred ten LCA patients were screened for various probable disease-causing gene sequence variations. Those patients with a probable disease-causing sequence variation in one of six genotypes were recalled for a follow-up examination. Evaluations included assessment of visual acuity, slit-lamp biomicroscopy, and dilated fundus examination. When possible, Goldmann perimetry was also performed. RESULTS: Of the 37 LCA patients with suspected disease-causin

The visual evoked potential in glaucoma and ocular hypertension: effects of check size, field size, and stimulation rate.

In order to determine the optimum stimulus conditions for the detection of optic nerve damage due to glaucoma and ocular hypertension, checkerboard pattern reversal visual evoked potentials (VEPs) were recorded from 20 glaucoma patients, 20 ocular hypertensive patients, and 20 age-matched normals. Two check sizes (12' and 48'), two field sizes (14 degrees and 28 degrees), and two alternation rates (1.9 and 7.5 alt/sec) were used. All subjects had visual acuities of 20/40 or better in each eye and equal pupils of 2 to 5 mm diameter. The largest number of VEP abnormalities were found with large

Temporal keratoconus in a pediatric patient.

PURPOSE: To report a pediatric patient with bilateral temporal keratoconus. OBSERVATIONS: A 14-year-old male presented with a two-year history of progressively worsening visual acuity in both eyes and suspicion for undiagnosed amblyopia in the right eye. Retinoscopy revealed a scissoring reflex in both eyes and corneal topography demonstrated high keratometry values (Kmax 57.9 D and 46.1 D in the right and left eyes, respectively), with relative temporal steepening approximately coinciding with the thinnest pachymetry in both eyes. Corneal cross-linking was recommended. CONCLUSIONS AND IMPOR

[Combined operation for the foreign body in the eye complicated with traumatic cataract: an analysis of 88 cases].

The foreign body in the eye complicated with traumatic cataract is one of the common causes of blindness. The rate of blindness ranges from 20% to 48.9%. This paper analyses combined operation for 88 cases of foreign body in the eye with traumatic cataract. The visual acuity below 0.05 accounted for 85.2% of the cases before operation and reduced to 27.3% after operation. Postoperatively, the corrected visual acuities were from 0.06 to 1.5 in 73.3% of the cases and 0.6 to 1.5 in 22.7% respectively. The patients should be treated with combined operation earlier. The way of operation depends on

[Pseudotumor cerebri syndrome].

Pseudotumor cerebri is a non-tumor related idiopathic or secondary intracranial hypertension. It is typically associated with papilledema and headache and patients are typically female, young and obese. Diagnosis is established by neuroimaging by magnetic resonance imaging (MRI), cerebral MR venography (MRV) and lumbar puncture. Papilledema is assessed by ophthalmoscopy, ultrasound and optical coherence tomography. Ophthalmologists monitor visual function by measuring visual acuity and visual field testing. Therapy of patients not facing immediate visual loss encompasses weight reduction and a

Blind drunk: alcoholic pancreatitis and loss of vision.

Acute loss of vision and alcoholic intoxication suggests the possibility of methanol poisoning. In this report we describe an alcoholic patient who complained of blindness after recovering from alcoholic pancreatitis and delirium tremens. Visual acuity was severely impaired and fundoscopy revealed florid bilateral cotton wool spots and a cherry red spot at the right macula. Such appearances have previously been reported in cases of post-traumatic visual loss, and may share a common aetiology of fat embolization. The association between alcoholic pancreatitis and visual loss is rare, and fundal

Optic nerve trauma with prolonged blindness followed by visual-evoked potential.

An accidental deep intraorbital penetration of a radio antenna tip damaged the optic nerve and caused immediate amaurosis in a 19-year-old female with normal funduscopic, electroretinographic and computerized tomographic orbital findings. A small visual-evoked potential (VEP) recorded within 2 h of injury and in the following days revealed persistent electrical nerve conduction despite loss of vision and afferent pupillary reaction. The VEP constituted the only anticipating sign for the visual recovery which occurred after 3 days. Further increase of the VEP amplitudes preceded the visual impr

[Ophthalmologic Evaluation of Fitness to Drive].

The ophthalmologist must know the essential principles of the driving fitness assessment. In the case of applications for a driving license renewal, it must always be clarified before the examination whether the fitness to drive is to be checked in accordance with the special regulation for holders of a driving license issued up to December 31, 1998 (see Annex 6 to § 12 of the FeV under 2.2.3 Special regulations for the old German Road Traffic Licensing Regulations). Within the scope of grandfathering, this continues to be valid only for the so-called old holders. A categorization of the manif

Characterising the orientation-specific pattern-onset visual evoked potentials in children with bilateral refractive amblyopia and non-amblyopic controls.

PURPOSE: An orientation-specific visual evoked potential (osVEP) protocol was developed to probe meridional anisotropies in children with refractive amblyopia. The aim was to characterise the osVEP response in children with bilateral refractive amblyopia, evaluate the intra-session repeatability of the main osVEP components (C1, C2 and C3), coefficient of repeatability (CoR) of the response to gratings in different meridians and determine if refractive amblyopes have poorer repeatability as compared with non-amblyopic controls. METHODS: Children aged 4-7 years with newly diagnosed and untreat

Electrophysiological Studies in Thyroid Associated Orbitopathy: A Systematic Review.

Dysthyroid optic neuropathy (DON) is the commonest cause of blindness in thyroid associated orbitopathy (TAO). While diagnosis remains clinical, objective tests for eyes with early or equivocal findings are lacking. Various electrophysiological studies (EPS) have been reported, yet the types and parameters useful for DON remain inconclusive. We performed a systematic literature search in MEDLINE, EMBASE and the Cochrane databases via the OVID platform up to August 20, 2017. 437 records were identified for screening and 16 original studies (1327 eyes, 787 patients) were eligible for review. Multifocal syphilitic chorioretinitis--a case report.

The aim of the paper is to report a case of a 22-year-old male patient with chronic bilateral uveitis and retinitis secondary to syphilis. Until the diagnosis, the patient had been treated symptomatically with ceftriaxone which resulted in visual acuity improvement. The patient was referred to the Sexually Transmitted Disease Clinic for causal treatment after which he did not continue further ophthalmic monitoring. After a year he contacted the Department again due to vision deterioration and a relapse of retinitis and choroiditis was diagnosed. The patient was referred to the Sexually Transmi
